[Objective] To investigate the endoscope feature of gastric signet-ring carcinoma and evaluate the methods of diagnosis and treatment. [Methods] To retrospectively analysis 36 cases who were confirm diagnosed.Their clinical feature, endoscope feature, pathologic types and curative effect of therapy were analyzed. [Rusult]Most carcinoma site located at anterior or posterior wall at lesser curvature of mid-upper part of gastric body. Endoscope feature: mucosa plica were tumefactional, small node formated, erosion, it was easy to bleeding. Biopsy: Mojority carcinoma type was BorrmannⅡ and Ⅱb and Ⅱc type. More topical biopsy and deep biopsy were necessary for improving biopsy positive rate. [Conclusion] Endoscope and biopsy are effective methods for early diagnosis of gastric signet-ring carcinima.

Objective To explore the treatment and diagnostic value of combined detection of high sensitivity C reactive protein(hs-CRP) and D-dimer (DD) levels for carotid artery plaque (CAS) in patients with acute cerebral infarction(ACI).Methods 48 patients with ACI were selected as the observation group,while 48 healthy people who excluded ACI were selected as the control group.The vascular ultrasonography and serum levels of hs-CRP and DD were detected within 3 days,and the observation group was classified according to TOAST.Results The hs-CRP and DD levels in the observation group were (7.88 ±2.54)mg/L and (1 286.2 ±233.4) μg/L,respectively,which were significantly higher than (1.14 ± 0.32) mg/L and (462.8 ± 147.2) μg/L in the control group (t =15.53,20.67,all P ＜ 0.05).The differences of hs-CRP and DD levels in different TOAST subtypes ACI patients were not statistically significant(P ＞0.05).The hs-CRP level was positively correlated with the number of instability carotid plaque(r =0.465,P =0.000),and DD level had no significant correlation with the number of instability carotid plaque.Conclusion Elevated serum hs-CRP and DD levels in ACI patients confirmed its participation in the acute inflammatory response,and hs-CRP can better reflect the instability of ACI patients with CAS.

Objective To measure the collagen content in the granulation tissue of skin wound surface in rats with depression in order to explore the possible mechanism about depression to wound healing.Methods Totally 126 Sprague-Dawley rats were randomly and equally allocated to 3 groups: simple wound group,depression plus wound group,wound plus depression group.Unpredictable stimuli,including fasting,electrocution,water deprivation,swimming in cold water and others,were performed for 18 d to establish depression model of rats.A full-thickness dermal wound(2.2 cm in diameter) was made on the back of rats to build wound model.Rats from depression plus wound group received 18 days' stimuli and then wound cutting.For those of wound plus depression,stimuli were carried out from the next day of wounding.The basal tissue in the center of the wound was taken out from 6 rats of every group in 3,5,7,10,14,18 and 21 d after wounding.Hydroxyproline reagent kit was used to detect the content of hydroxyproline in collected tissue.Wound collagen depositions in every time point were stained by nitroxanthic acid sirius red and then observed by polarization microscopy.Results Hydroxyproline content and collagen deposition in wound granulation tissue of depression plus wound group rats were lower than those in the other 2 groups.From 18 d after wounding,rats of depression plus wound group had a tendency to the same level as wound plus depression group.Conclusion Depression plus wound significantly decreases hydroxyproline synthesis,reduces collagen deposition in wound,and then delays the wound healing.

Objective: To investigate the childrens congenital and acquired nutritional status and the physical development among the different nationalities in Guizhou province; to find out the factors that greatly influence the childrens physical development. Methods: Investigations were carried out on the diet, height and the weight of the mothers during the early and late stage of their pregnancy and after the delivery of the babies. Results: The living conditions in the groups of Miao and Buyi were very poor. For the pregnant women among Han, Miao and Buyi groups, during their middle and late stage of the pregnancy, the intakes of protein, calcium and VB 2 did not reach 80% RDA. For the 2-4 years old children, the intake of protein was below 80% RDA. For those in Miao and Buyi groups, the chief source of protein was from cereals. There was a great insufficient intake of calcium and VB 2 in different age groups of the children mentioned above. With the increase of age, the infants of Buyi and Miao groups were gradually diverting from the standard criterion from 3rd or 4th month. The greatest difference was in Miao group, i.e. at the age of 6, the difference in height was 9.8 cm(male), 8.3 cm(female); the difference in weight was 1.64 kg(male), 1.92 kg(female). Conclusion: The physical development of the infants in Buyi and Miao groups is inferior to those in Han group. Besides the environmental influence, the quality and quantity of the protein in diet play an important role in physical development.

Rat models of diabetes were established by injecting streptozocin intraperitoneally.According to random number table,three groups were divided:normal group,diabetic group,and fisetin-treated group.After 24 weeks,all rats were sacrificed.Biochemical parameters of blood and urine samples were tested.The pathological changes were observed by paraffin sections staining with HE.The expression of extracellular matrix proteins was analyzed via PAS and Masson staining.Location of p300 protein expression was analyzed by immunohistochemistry.The protein expressions of p300 and MMP-2 were determined by Western blotting.The mRNA expressions of MMP-2 were analyzed via real-time PCR.The biochemical parameters and kidney pathological images in fisetin-treated group were better than those in diabetic group.The expression of extraeellular matrix proteins was lower than that in diabetic group.Immunohistochemistry analysis showed that among three groups the expression of p300 was mainly in glomeruli,and was also expressed in cell nucleus and cytoplasm and the coloration of fisetin-treated group was weakened as compared with diabetic group.Western blotting analysis showed that the expression of p300 protein in fisetin-treated group was lower than that in diabetic group(P＜O.05).The expressions of MMP-2 mRNA and MMP-2 protein were higher than those in diabetic group (P ＜ 0.05).It is suggested that fisetin may attenuate diabetes associated abnormalities in the kidney of rats,owing probably to inhibiting the expressions of p300 and enhancing the expressions of MMP-2.

Objective To develop a portable automatic tourniquet.Methods Using microcomputer and pressure sensor,the pressure and hemostatic time of current pressurized tourniquet were controlled.Results Portable automatic tourniquet was composed of gasbag pressurized bandage,electric micro-pump,pressure sensor,preamplifier,impact damper,A/D converter,single-chip micro-computer controller,data memory,keyboard and its interface circuit,display and its interface circuit,photoelectronic control circuit,etc.Conclusion Portable automatic tourniquet is small,light,safe and convenient.Besides,rapid hemostasia and automatically controlled & stable pressure enable it to be used both in the war and at peace time.[Chinese Medical Equipment Journal,2008,29(2):21-23]

Objective To explore and compare the clinical effects of pregnant women with liver cirrhosis after hepatitis in cesarean section between general anesthesia and single spinal anesthesia.Methods Clinical data of pregnant women of 35,including compensated cirrhosis 26 cases,decompensated cirrhosis 9 cases,aged 24-45 years,weighing 55-98 kg,falling into ASA Ⅱ or Ⅲ,were divided into group A (general anesthesia group,n=15) and B (single spinal group,n=20).In patients of group A,propofol were infused at 1.5 mg/kg till the time for fetal delivery,after intubation,followed by infusion of remifentanil at a rate of 0.2 μg·kg-1·min-1 TCL combined with propofol until the end of operation.In group B,the puncture was performed at L3-4 interspace,following by intrathecal injection of ropivacaine 10~15 mg and within 10 seconds.With liver cirrhosis after hepatitis were collected and analyzed,retrospectively.Preoperative general status,blood loss and time of operation,Apgar score of neonates compared and analyzed in groups A and B.And the changes of hepatic function and coagulation function of the body under different anesthesia methods were also studied.The influential factors of postoperative elevation of hepatic function were analyzed by non-conditional logistic regression models.Results Blood loss and time of operation,Apgar score of neonates and postoperative hospital stay had no significant differences between the two groups.Postoperative albumin of group A was higher than that of preoperative,with a statistical significance (P<0.05).Alanine aminotransferase,aspartate aminotransferase of postoperative of group B were increased,with a statistical significance (P<0.01).Logistic analysis showed that the type of anesthesia was the possible influencing factor of postoperative elevation of total bilirubin (OR=12.04,95%CI 1.14-127.45).Conclusion The negative effect of single spinal anesthesia on hepatic function more than that of general anesthesia in pregnant women with liver cirrhosis in cesarean section.

Objective To explore the best way for clinical screening of primary aldosteronism (PA).Methods Three hundred and three suspected cases of PA were collected and divided into groups of primary aldosteronism group, essential hypertension group, and nonsecreting cortical adrenal tumor group.The plasma aldosterone concentration/plasma renin concentration ratio ( ARR) was used to draw the receiver operating characteristic ( ROC) curve and obtain the best cut-off point.Furthermore, the current screening schemes for PA were compared.Results Upright ARR yield had larger areas under the ROC curve than plasma aldosterone concentration or plasma renin concentration under all conditions of testing. The best cut-off point of upright ARR[(pg/ml)/(μIU/ml)] for the diagnosis of PA was 43.45.During the two postural stimulation tests,the two upright ARR exceeded 43.45 with the highest diagnostic sensitivity of PA reaching 0.94.During the two upright tests ARR was less than 43.45, with a sensitivity of 0.74, and a specificity of 0.94.Conclusion To screen for PA in high-risk populations, twice postural stimulation test is recommended.As long as the upright ARR is above 43.45, PA may be considered and further confirmation is needed to prevent misdiagnosis.

This study was aimed to reveal current achievements and the future trend about Chinese materia medica (CMM) processing, which pointed future research direction for scholars.“CMM processing” was used as the subject term in document retrieval to the Web of Science (WoS) and CNKI. The CitespaceII and Ucinet & Netdraw were used to build mapping knowledge domains to acceptable both English and Chinese literature data. The results showed that the mapping knowledge for“CMM processing” for co-citation, source journal, author, key words and so on were established. It was concluded that the rapid present period of Chinese literature on CMM processing was earlier for around 15 years than English literature. And the total amount of English documents was much less than Chinese documents. Combing the developing process of CMM, it revealed its current achievements and future trend, which pointed future research direction for scholars, and moreover it put forward new thinking and method to summarize or expand traditional medicine and related research fields.
