1.Clinical observation of ropivacaine mesylate in combined spinal epiduaral anesthesia of hypogastric operations of elder patient

Objective To investigate the effect and safety of Ropivacaine mesylate in combined spinal-epid uaral anesthesia of hypogastric operations of elder patient.Methods 90 cases patients scheduled for the lower abdominal surgery were randomly divided into two groups(appendectomy and Oblique inguinal hernia neoplasty).group Ropivacaine mesylate(group A,n=45),group upivacaine hydrochloride(group B,n=45),A,B groups,punctures were selected between L2~L3 ,Hyperbaric solution of Ropivacaine mesylate were injected intrathecaly respectively in two groups,(compounding:group A:0.894% Ropivacaine hydrochloride 1.5ml+10% Glucose lml)and group B;(0.75% Bupivacaine hydrochloride 1.5ml+10% Glucose lml).The two groups after the subarachnoid medication Indwelling catheter to the cephalic in the epidural space.The time of anaesthetic emergence,maintainance,and the anaesthetic level were observed;the analgetic effect was assessed;movement signs of the lower limbs were observed;blood pressure,respiration and SpO2 .postoperative complications were observed.Results There was apparent differentiation between group A which Systob'c blood pressure declined 8.9% ,heart rate decreased 6.7% ,intraoperative nausea came up to 6.7% after anaesthesia and group B which was 28.9% ,20% ,17.8% ,respectively(P < 0.05).The effective time of anaesthetic emergence was(52.0 ±21.5)s and(13.2 ±3.3)min in group A longer than group B was(41.2±18.2)s and(11.3 ±3.6)min(P<0.01).There was no difference in Sp02,Bromage Score about extreme movement blockage degree(2.3 ±0.7;2.2 ±0.5),the incidence of postoprative headache(0% ;2.2%)between the group A and group B(P > 0.05).Conclusion Ropivacaine mesylate can be used effectively and safetly in combined spinal-epiduaral anesthesia applied in hypogastric operations of elder patients,for cardiovascular stability is superior to bupivacaine.
2.Discussion on Errors of Conformal Plumbic Blocks Made with Manual Cutter and Automatic Cutter

Objective To discuss errors of conformal plumbic blocks made with manual cutter and automatic cutter.Methods According to beam files of Treatment Planning System,the conformal plumbic blocks were made with manual cutter and automatic cutter respectively.Then their errors were measured.Results With manual cutter,the errors of conformal plumbic blocks were 2.355 mm.With automatic cutter,the errors of conformal plumbic blocks were 1.425 mm.Conclusion The errors of conformal plumbic blocks made with automatic cutter are less than the errors of conformal plumbic blocks made with manual cutter,so automatic cutter should be a necessary tool in making conformal plumbic blocks.
3.JNK signal pathway in acute necrotizing pancreatitis associated lung injury in rats

Objective To investigate the effect of JNK signal pathway in acute necrotizing pancreatitis (ANP) associated lung injury.Methods A total of 24 SD rats were randomly divided into sham group and ANP group,ANP rats were induced by retrograde injection of 5% sodium taurocholate into common bile duct.The rats were sacrificed 12 and 24h later,and the pancreas and lung tissue were resected and underwent routine pathologic examination,ELISA method was used to detect the level of TNF-α and IL-1β in lung tissue.Expression of JNK mRNA was detected by real time PCR,and the expression of JNK protein were evaluated by Western blotting.Results There was bleeding,necrosis,large amount of inflammatory cells infiltration in pancreatic tissue; and there was edema,pulmonary consolidation,interstitial edema,inflammatory cells infihration in lung tissue in ANP group.The levels of TNF-α and IL-1β,JNK mRNA,JNK protein,phosphorylation JNK were ( 374.3 ± 124.0) pg/ml,(649.0 ± 114.9) pg/ml,2.57 ± 0.76,1.40 ± 0.81,0.81 ± 0.20 in ANP group,which were significantly higher than those in with sham group[( 218.2 ± 68.4)pg/ml,(524.3±58.4)pg/ml,1.03±0.11,0.32±0.11,0.32±0.11,P<0.05].Conclusions JNK signal pathway plays an important role in experimental ANP associated lung injury in rats.
4.Sampling survey on the prevalence of schistosomiasis among floating population in Zhejiang province

Objective To survey on the prevalence of schistosomiasis among floating population in Zhejiang province. Methods A survey on prevalence of schistosomiasis among floating population was conducted from September to November 2008, and the stratified cluster sampling method was adopted in the survey. Totally 129 villages of 19 counties or districts were selected as survey sites, and 100 samples of migrants aged 6 to 65 from schistosomiasis-endemic areas were taken in each selected village. All selected individuals were surveyed by questionnaire and underwent serum indirect hemagglutination (IHA) test. For individuals with positive serum IHA testing the fecal examination was carried out to detect the eggs by nylon sedimentation method. SPSS 13.0 software was used for data processing. Results The number of migrants in survey sites was 3 357 420, among whom 303 219 were from schistosomiasis-endemic areas (9.03%).The positive rate in serum IHA test was 2.06% (286/13 898), 276 IHA-positives individuals received fecal examination, and 7 cases were positive (2.52%). Based on above data it was estimated that there would be potentially about 33 500 serum IHA-positive cases and 845 egg-positive cases among floating population in Zhejiang province. Conclusion The risk of schistosomiasis transmission still exists in Zhejiang province due to the infected migrants from endemic areas, and a surveillance system and quick response are required for prevention of re-emergence of the disease.
5.Mollusicidal effect and toxicity to fish with META-Li in field

The mollusicidal effect and toxicity to fish with META-Li in field were tested in mountain areas in Zhejiang Province. The snail control test, which had 3 groups including a META-Li group, niclosamide group and control group, were performed by the spraying method. The adjusted mortalities of snails in the META-Li group on the 3rd, 7th and 15th day were 89.76% , 87.98% and 94.10% , respectively, in the niclosamide group were 89.70% , 83.22% and 94.72% , respectively .and there was no significant difference between the two groups ( P > 0.05), but they were decreased obviously compared with the control group, which were 9.24% , 9.50% and 15.21% , respectively(P <0.05). The toxicity test to fish with META-Li included 3 groups, namely a high concentration META-Li group, low concentration META-Li group and control group, and none fish was dead. It is suggested META-Li has low toxicity to fish, and is suitable to snail control in aquiculture areas.
6.The diagnosis and treatment of acute myocardial infarction complicated with left ventricular wall rupture:a report of three cases

Objective To investigate the diagnosis and treatment in patients with acute myocardial infarction (AMI) and complicated left ventricular wall rupture (LVWR). Methods A retrospective analysis was made on the clinical features, diagnosis and successful treatment in three AMI patients with LVWR from December 2015 to April 2016. Results Three cases were included in this study. Case 1, the mesh like cardiac rupture after AMI was diagnosed by ultrasonic Doppler. Emergency revascularization was performed due to the combined cardiac shock, and the infarct related artery was opened. The vasoactive drugs were used after revascularization to reduce ventricular pressure load and volume load in the haemodynamic monitoring, and anticoagulation, antiplatelet agents were less used or discontinued to promote local thrombus healing of ventricular rupture. Case 2 was a recurrent myocardial infarction patient. LVWR was diagnosed by ultrasonic Doppler one day after emergency operation. The ruptured ventricular wall was encapsulated by thrombus. The drug therapy was effective in hemodynamic monitoring. LVWR was further confirmed by cardiac CT after clinical stabilization. Case 3 was diagnosed LVWR by ultrasonic Doppler four days after AMI. Because the ruptured ventricular wall was limited by incompletely organized thrombus, and the haemodynamic condition was stable, selective surgical repair of rupture after coronary angiography was performed. Conclusion The effective drug therapy combined with percutaneous coronary intervention and surgical repair can reduce the risk of death in patients with LVWR after AMI.

Objective To find out current distribution of Oncomelania snails and Schistosoma infection in snails in Zhejiang Province, so as to improve the project of schistosomiasis control. Methods Investigation spots were selected by stratified cluster sampling method, 100 villages of 34 counties were selected from 7 106 villages of 55 counties as survey spots. Synchronously systematic and environmental samplings were used for the survey. Snails were dissected to determine the infection status and spots with sham snails were set to assess the quality of the survey. Results The result showed that snails were found in 223 strips, 1 572 frames and 73 300 m 2 area in 32 villages of 21 counties. Snails were found in an area covering 72 640 m 2 in 29 villages of 18 counties in hilly region, which accounted for 99.1% of total snail habitats. Objective To investigate the value of endoscopic ultrasound ( EUS)-guided drainage via upper gastrointestinal tract for peripancreatic fluid collection. Methods The clinical data of 103 patients with peripancreatic fluid collection undergoing EUS-guided drainage via upper gastrointestinal tract from May 2006 to July 2017 in Nanjing Drum Tower Hospital were retrospectively analyzed, the treatment and postoperative complications were summarized. Results Among the 103 patients, 101 ( 98. 1%) were punctured successfully, and patients were punctured mostly through the gastric wall (97. 1%, 100/103). There were 29 cases of metal stent placement, and 66 plastic stent placement. Ten patients underwent ERCP with pancreatic stent placement. After operation, 8 patients underwent percutaneous drainage, and 1 surgical drainage. Complications occurred in 14 ( 13. 6%) cases, including stent displacement in 5 cases, and postoperative infection in 9 cases. There was no perforation, bleeding, pancreatic fistula, other serious complications or death. Conclusion EUS-guided drainage is safe and reliable for peripancreatic fluid collection.
9.Value of lumen-apposing metal stent in the treatment of 27 cases with peripancreatic fluid accumulation in acute pancreatitis

Objective To assess the application value of lumen-apposing,fully covered,self-expandable metal stent (LAMS) in the treatment of peripancreatic fluid collection (PFC).Methods From July 2015 to July 2017,at Drum Tower Hospital Affiliated to Medical School of Nanjing University,the patients with PFC who received LAMS under endoscopic ultrasound were enrolled.The inclusion criteria was the first treatment,age ≥ 18 years old,PFC diameter > 6 cm and clinical manifestations of compression,or increased diameter of PFC after continuous observation,or signs of infection,course >four weeks,and the distance between the cyst wall and the digestive gastrointestinal wall less than 1 cm;exclude other pancreatic cystic lesions,such as serous cystic neoplasm,mucinous cystic neoplasm and solid pseudopapillary neoplasm.The clinical data of enrolled patients were retrospectively analyzed,and the efficacy and postoperative complication were summarized.Results A total of 27 patients with PFC were enrolled.All patients were successfully punctured and placed with LAMS.The percentage of complete drainage rate of PFC was 96.3% (26/27).The median LAMS maintenance time was 26 days,with a range of four to 135 days.Seventeen patients received debridement treatment through LAMS,and the number of debridement treatment was (2.5± 1.3) times.Two patients underwent endoscopic retrograde cholangiopancreatography (ERCP) and were placed with pancreatic stent,five patients received percutaneous drainage,and one patient underwent surgical drainage.Among 27 patients with PFC,eight patients (29.6%) coexisted with complications,including stent displacement in two cases,postoperative infection in three cases,perforation in two cases and bleeding in one case.They all improved after conservative treatment and none of them died.Conclusion LAMS is effective and relatively safe in the treatment of PFC,and may change the clinical treatment strategy of PFC.
10.Efficacy and outcomes analysis of endoscopic ultrasound-guided early removal of lumen-apposing metal stent in the treatment of pancreatic walled-off necrosis

Objective:To evaluate the efficacy and outcomes of early removal of lumen-apposing metal stent (LAMS) in the treatment of pancreatic wall-off necrosis (WON).Methods:A retrospective analysis was performed on 51 patients with WON who underwent endoscopic ultrasound (EUS)-guided transluminal drainage (ETD) and direct endoscopic necrosectomy (DEN) using LAMS at Nanjing Drum Tower Hospital from January 2018 to December 2022. Patients were divided into the early removal group (within two weeks, n=24) and the traditional removal group (after two weeks, n=27) based on the timing of LAMS removal. The short-term effects, safety and long-term outcomes of WON were compared between the two groups. Results:The technical success rate of LAMS placement in 51 patients reached 100%, and all patients underwent ETD and DEN. The median number of necrosectomy sessions in the early removal group was significantly lower than in the traditional removal group, 2.0 sessions vs 3.0 sessions and the difference was statistically significant ( P<0.05). Postoperatively, 15.7% of patients required percutaneous catheter drainage (PCD) and 5.9% required surgery, with no significant difference between the two groups. The clinical success rate and mortality rate in the early removal group were 79.2% and 8.3%, respectively, compared to 81.5% and 3.7% in the traditional removal group, with no statistically significant difference. In terms of safety, the early removal group exhibited a significantly lower rate of adverse events during stent retention with statistically significant difference (12.5% vs 37.0%, P<0.05) compared to the traditional removal group. A total of 46 patients were followed up for six months. In the early removal group, the rates of disease recurrence, need for endoscopic reintervention and occurrence of long-term complications were 20.0%, 10.0% and 20.0%, respectively. These rates did not show a significant increase compared to the traditional removal group, which were 7.7%, 3.8% and 38.5%, respectively, without significant differences between the groups. Conclusions:In the treatment of WON, early removal of LAMS is safe and effective to a certain extent. In comparison to the traditional practice of removing LAMS after two weeks, early removal does not reduce clinical success rates, nor does it increase the rates of disease-related mortality, recurrence, or long-term complications. On the contrary, it may reduce the occurrence of adverse events during stent retention and decrease the number of necrosectomy procedures subsequently.
