1.Determination of aloin of compound aloe ointment by HPLC

Objective To establish a method for the determination of aloin of compound aloe ointment. Methods HPLC method was used with Agilent Zorbax-C18(250 mm×4.6 mm,5μm)column and the mobile phase consisted of acetonitrile-0.08%Sodium 1-heptanesulfonate (22∶78). The flow rate was 1.0 ml/min,the column temperature was 30 ℃, and the UV detector was set at 355 nm. Results The linear ranges of aloin was 0.037~0.74 mg/ml(r=0.999 8). The average recovery of aloin was 98.7%. Conclusion The method is simple, repeatable and accurate, it can be applied in quantitative determination of aloin in compound aloe ointment.
2.Determination of berberine hydrochloride in modified Ermiao pill by HPLC

Objective To establish a HPLC method for the determination of berberine hydrochloride in Jiawei Ermiao Wan.Methods The RP-HPLC system consisted of the Luna-C18 (4.6 mm × 250 mm,5 μm) column,the mobile phase consisted of acetonitrile-0.1%H3PO4(45:55),the flow rate was 1.0mL · min-1,the column temperature was 30℃,The UV detector was set at 245nm.Results The linear response range was 0.071~1.42 mg· ml-1 (r=0.9998).The average recovery of mangiferin was 99.2%,and RSD<2%.Conclusion The method was simple,rapid,accurate and repeatable.It could be applied in the determination of berberine hydrochloride in Jiawei Ermiao Wan.
3.Construction of tissue-engineered blood vessel stent using different materials: Characteristics and effectiveness

OBJECTIVE: To evaluate the characteristics and application prospect of tissue-engineered blood vessel stent materials. METHODS: Using "tissue engineering, tissue engineering blood vessel, intravascular stent" both in Chinese and English as key words to search documents published between January 1993 and October 2009. Literatures concerning biomaterials and tissue engineered blood vessel were included. Repetitive research or Meta analysis was excluded. After selection, 30 documents were included to discuss the kinds and characteristics of tissue-engineered blood vessel materials. RESULTS: Cellular tissue matrix is a kind of ideal material for intravascular engineering, which had widely applied in tissue-engineered stent. The fibrin-based stents are characterized by good biocompatibility, biodegradation, compatibility, as well as promote vascularization and tissue repair. Gelatin can implement its own "vascularization" due to less antigenicity and can be fully biodegraded. Accordingly, both the natural biomaterials and synthetic polymeric material have certain limitations, thus, the combination of these two materials would construct a tissue-engineered blood vessel stent that has good characteristics.Nano-modifled technology is hopeful to be applied in the future transplantation of tissue-engineered blood vessel stent.CONCLUSION: The tissue-engineered blood vessel developed rapidly in these years. However, as yet, there is not an ideal intravascular stent material. The natural biomaterials arose more attention, but the mechanical function of which is far from the stent requirement. Thus, to reach the aims of repair and reconstruction, new materials are the urgent need in tissue-engineered intravascular stent.
4.Effect of scale method on vancomycin through concentration and nephrotoxicity in ICU patients

Objective To evaluate the vancomycin trough concentration and nephrotoxicity in ICU patients with scale method.Methods The patients in ICU were evaluated from September 2011 to December 2013 in the hospital by prospective randomized controlled study.Experience using drug was applied in control group (n=116).Scale method was applied in test group (n=117), vancomycin concentration and renal toxicity were compared between two groups.Results The percentage of patients with an initial vancomycin trough concentration 15.0μg/mL or higher increased in the test group as compared with control group (73%vs 38%, P=0.004).The test group also demonstrated that an increase in the percentage of patients with initial trough concentration from 15.0 to 20.0μg/mL (41% vs.19%, P=0.008), and no statistical difference in the percentage of patients with an initial vancomycin trough concentration above 20μg/mL (31% vs.17%, P=0.340).There was no difference in nephrotoxicity in test group compared with control group (17% vs.16%, P=0.953).Conclusion Use of scale method increases the percentage of initial vancomycin trough concentrations 15.0μg/mL or higher in ICU patients and is not associated with an increased occurrence of nephrotoxicity.
5.Non-ablative treatment for hyperpigmented skin diseases with combined intense pulse light and Q switched Nd :YAG laser

Objective To observe the effect of non-ablative treatment combined intense pulse light (IPL) and Q switched Nd :YAG laser on hyperpigmented skin diseases. Methods Total 250 cases of Fitzpatrick skin types Ⅲ and Ⅳ with hyperpigmented skin disorders received 4 treatments with combined 560nm IPI. and 532 nm Q switched Nd :YAG laser. The interval between two treat-ments was I month. After 3 months the effect of IPL was observed. If it was not good, we used the Q switched Nd :YAG laser. Efficacy and adverse effects were evaluated 3-6 months after the final treat-ment. Results 3-6 months after the last treatment, 50%-70% of patients had their telangiectasia, enlarged pores, hyperpigmentation and wrinking of facial photoaging improved. The degree of im-provement was more than 60 %. Common side effects were minor blistering and erythema. Conclusion Combined IPI. and Q switched Nd :YAG laser treatment is an idea non-ablative therapy for telangi-ectasia, enlarged pores, hyperpigmentation and wrinking of facial photoaging in Fitzpatrick skin types Ⅲ and Ⅳ in Asian.
6.Surgical treatment of carotid stenosis:a report of 28 cases

Objective To assess the effectiveness of carotid endarterectomy(CEA) in the surgical treatment of stenosis of carotid artery.Methods From December 2003 to June 2006,28 patients of carotid arteriosclerotic stenosis received CEA and vascular repair or plasty(VP).Before and after operation,the hemodynamic parameters including inner diameter of internal carotid canal,blood flow rate,blood flow and blood flow rate of middle cerebral artery of 28 patients were measured by color Doppler flow imaging(CDFI) and transcranial Doppler(TCD).Results The inner diameter of internal carotid canal in all patients recovered to normal range.Blood flow rate and blood flow after operation were improved.No postoperative complications occurred,and no aneurysms formed.Conclusion CEA is one of the effective ways to manage carotid stenosis and angioplasty can improve the long-term efficiency.
8.Defective Fas function of T lymphocytes in patients with abdominal aortic anenrysm

Objective To assess apoptosis mediated by the cell death receptor Fas in peripheral T lymphocytes of patients with abdominal aortic aneurysm. Methods The apoptotic pathway was triggered by anti-Fas monoclonal antibody in cultured and activated peripheral T-cells from 20 AAA patients. Control groups consisted of 15 patients with aortic atherosclerotic occlusive disease(AOD)and 25 healthy individuals. Cell survival and death rate were assessed. Results Cross-linkage of Fas receptor exerted a strong apoptotic response on T cells from AOD patients and healthy controls, while the effect on T cells was very limited from that of AAA patients. The evaluation of cell Survival rate showed a significantly higher percentage in AAA group(98.9%±10.3%)than in the AOD subjects(58.9%±15.2%)or the healthy group(59.4%±12.9%;P<0.001=.Apoptosis assessment by annexin V and propidium iodide staining and flow cytometry showed similar results. The defect in AAA group was not due to decreased fas expressed at normal levels. Moreover,it specifically involved the Fas system because cell death was induced in the normal way by methylprednisolone. Conclusions Fas-induced apoptosis in activated T cell from AAA patients is impaired. This may disturb the normal down-regulation of the immune response and thus provide a new insight into possible mechanisms and routes in the pathogenesis of AAA.
9.Percutaneous argon-helium cryoablation for hepatic malignancies

Objective To observe the efficacy of percutaneous hepatic cryoablation for the treatment of primary or meta-static hepatic malignancies (<5 cm in diameter).Methods A total of 31 patients (39 tumors <5 cm in diameter) were treated with argon-helium cryoablation system under the guidance of CT or ultrasound.Results Tumor ablation range was 90%-100% in 39 lesions,including 69.23% (27/39) complete ablation.The 1- and 2-year survival rate was 90.32% (28/31) and 61.29% (19/31),respectively.No bleeding and injury of blood vessel or bile duct was noted.Complications of cryoablation included intraoperative shivering in 4 (12.90%) patients,postoperative fever (37.12-38.25℃ in 7 (22.58%) patients and hepatic pain in 6 (19.36%) patients.One patient had severe pain relief until 2 h after cryosurgery with ice-cold skin temperature and stable life index,analgesic had little effect,and no bleeding was found on CT image.Other patients had slight or moderate pain and remained untreated.Conclusion Percutaneous targeted argon-helium cryoablation is a feasible and safe technique in the treatment of small primary or metastatic hepatic malignancies not suitable for resection.
10.Summary of surgical experience of eight cases with pentalogy of cantrell

Objective To summarize surgical experience of eight patients with pentalogy of Cantrell. Methods Six male and two female patients with pentalogy of Cantrell,aged from 4 months to 26 years old, average 7.35 years old, underwent surgical therapy for intracardiac anomalies and extracardiac anomalies from July 2007 to June 2009. Eight case with intracardiac anomalies include one case with only VSD, one case with only ASD, two cases with DORV, four cases with VSD and ASD or PTO. Experts majoring in cardiovascular surgery cooperated with doctors majoring in thoracic surgery and general surgery for satisfactory correction of intracardiac anomalies and extracardiac anomalies and repositioning heart to thoracic cavity. Results Ectopic heart of the first patient was simply repositioned into thoracic cavity following surgery of double outlet of right ventricle in another hospital two years before. Correction of introcardiac anomaly and reposition of ectopic heart finished at one time in 7 cases. Eight patients got full recovery except that residual shunt occurred in the second case which also got full recovery after transcatheter therapy. Ventricular diverticulum was removed in the fourth case because of difficult reposition of ectopic heart.With the help of general surgeon and thoracic surgeon, partial coronary ligament of liver and falciform ligament of liver in the left was cut in the first case and the left half lobe of liver was pushed downward. Bilateral pleural and marginal costal costochondral was cut and make thoracic wall upward so that ectopic heart can reset into thoracic cavity. And then, defect of diaphragm and abdominal wall were repaired with Proceed patch. In the other seven cases, bilateral pericardium and mediastinal pleura was cut and the 7th and 8th cartilage was transected and bilateral costal arch was closed so for complete thoracic angioplasty.Left ventricular dysfunction occurred in the fifth case with DORV and also got full recovery after symptomatic treatment. Full recovery was got in all cases after followingup from 1 to 23 months. No adverse complications occurred and every case live a wonderful life. Conclusion Pentalogy of Cantrell can be cured at one time by accurate correction of cardiac anomalies, cutting of bilateral pleural and marginal costal costochondral to make thoracic wall upward and enlarge thoracic space for repositinning of ectopic heart and using artificial patch to repair defect of diaphragm when necessary.
