1.Diagnosis and treatment of testicular torsion

Objective To investigate the reason why the misdiagnostic rate of testicular torsion is high and improve its diagnosis and treatment.Methods The clinical data of 20 cases of testicular torsion were retrospectively analysed.Results All 20 cases had the symptom of sudden colic of scrotum and went to hospital in 2 h.Only 5 cases were diagnosed as testicular torsion in 6 h by color Dopple ultrasonography.Three cases were cured by surgical detorsion and the testicles of 17 cases were removed for necrosis.The other testicle of all 20 cases was fixed.Conculusion The early diagnosis of testicular torsion is the most important and color Dopple ultrasonography is a reliable easy way.The sufficient knowledge of this disease by the doctors is crucial in improving the diagnosis and treatment.
2.Evaluation of the effectiveness of cough test during tension-free vaginal tape procedure in preventing post-operative voiding dysfunction

Objective To evaluate the value of cough test in the tension-free vaginal tape (TVT)procedure.Methods A cohort of 85 women with stress urinary incontinence underwent the TVT procedure with cough test (n =41) or without cough test (n =44).Patients in cough test group were performed according to the Ulmsten’s method strictly,with the stress of tape adjusted in light of cough test; whereas in other 44 operations,the tape was placed on the urethral tract without stress,and no cough test was performed.The urine catheter was removed after 48 hours postoperatively and follow-up evaluation was carried out at 12 month postoperatively.Results TVT procedure was carried out successfully in all patients by a single experienced surgeon.Four cases of urinary retention and 5 cases of voiding difficulty were observed in the cough test group.However,urinary retention or voiding difficulty was not detected in the nun-cough test group.Based on the twelve-month follow-up results,the cure rate was 92.6% (38/41) in the cough test group and 93.1% (41/44) in the non-cough test group.Flow-pressure study indicated that 11 cases in cough test group were in the obstruction zone,while only 3 cases in the obstruction zone were detected in the non-cough test group.Conclusions TVT is a safe as well as effective minimally invasive surgical procedure to treat female stress urinary incontinence.However,Adjusting stress of tape in accordance with cough test during the TVT may potentially increase the incidence of urinary dysfunction postoperatively.Therefore,no convincing evidence was gained to support the efficacy of cough test during TVT in terms of preventing postoperative voiding dysfunction.

Objective:To observe the effect of combining scalp acupuncture with feedback training on muscle tone, motor ability, plantar pressure distribution and joint range of motion of stroke survivors with lower limb spastic paralysis.Methods:A total of 212 stroke survivors were randomly divided into a combination group ( n=70), an exercise group ( n=70) and a scalp acupuncture group ( n=72) according to a random number table. All of the patients received routine medication and 120 minutes of rehabilitation training six days a week for 8 consecutive weeks. The rehabilitation training included guidance on good posture, posture change training, balance training and gait training. The scalp acupuncture and exercise groups were additionally provided with 40 minutes of scalp acupuncture and lower limb intelligent feedback training, while the combination group was given both. Muscle tension, balance and lower extremity movement were quantified before and after the training, and a gait evaluation system was used to detect the plantar pressure distribution on the affected side and the range of motion range of the lower limb joints during walking. Results:After the treatment, significant differences were observed in the average modified Ashworth scores, modified Berg scale scores, Fugl-Meyer lower extremity ratings and modified Barthel Index scores compared with before the treatment. The average scores of the combination group were then significantly better than those of the other two groups. The average load bearing ratios of the affected toes, metatarsal bones Ⅰ-V, medial arch, external arch, medial heel and external heel had improved significantly in all of the groups, with the improvement of the combination group again significantly greater than those of the other groups. The same pattern of improvement was observed in the groups′ average pelvic rotation angles, hip flexion angles, knee flexion angles and ankle dorsiflexion angles.Conclusion:Combining scalp acupuncture with intelligent feedback can significantly improve lower extremity functioning after a stroke. That should improve performance in the activities of daily living of stroke survivors.
5.Clinicopathologic features and prognosis of membranous nephropathy after renal allografting

Objective:To explore the clinicopathologic features and renal prognosis of patients with post-transplant membranous nephropathy (MN).Methods:Patients with allograft biopsy-proven MN were reviewed retrospectively and divided into unknown etiology group (A, n=12) and recurrent membranous nephropathy (rMN) group (B, n=7). Their clinicopathological data and renal prognosis were assessed and compared.Results:No differences existed in the proportion of living-related donor or post-transplant allograft function. Group B had recurrence at 16.4 months after transplantation and it was significantly shorter than group A. Allograft impairment manifested as proteinuria, nephrotic syndrome and/or renal insufficiency in both groups. The positive rate of serum anti-PLA2R antibody and renal PLA2R staining was significantly higher in group B than that in group A. Similarly, the intensity of IgG4 subtype staining was also stronger in group B than that in group A. The 5-year cumulative renal survival rates from end-stage renal disease (ESRD) were 77.8% and 66.7% in groups A and B respectively. No significant inter-group difference existed in renal prognosis.Conclusions:Anti-PLA2R antibody plays an important role in the recurrence of rMN after renal allografting. PLA2R staining is useful for detecting primary disease and its sensitivity is higher than that of serum anti-PLA2R antibody. Rituximab is an effective treatment for post-transplant MN. Follow-up studies with a larger sample size are required for further verification.

ObjectiveTo analyze the research status, research hotspots and trends at home and abroad in the field of rehabilitation assistive devices in the past ten years. MethodsThe relevant articles of rehabilitation assistive devices from 2012 to 2021 in CNKI and Web of Science (WoS) core collection database were retrieved. The authors, countries, institutions, research hotspots, and trends were extracted with CiteSpace and VOSviewer to draw knowledge mapping. Results and ConclusionThe annual articles published at home and abroad has shown an upward trend year by year. The team with the most articles in China is the Yu Hongliu team, and the institution with the most articles is China Assistive Devices and Technology Center for Persons with Disablities. The team with the most foreign articles is the Maysam G team, and the institution with the most articles is the University of Pittsburgh. The United States has the largest number of literatures and the highest intermediary centrality. Research hotspots related to rehabilitation assistive devices at domestic and abroad mainly focus on the design and development of rehabilitation assistive device products, the application of rehabilitation assistive devices in special/vulnerable populations, the application effect of rehabilitation assistive devices and related professional services; the development trend of intelligent assistive device product design and application research based on intelligent perception, brain-like computing, deep learning and virtual/augmented reality is predicted.
