1.Performance of T cell infected with Mycobacterium tuberculosis assay in diagnosis of smear and culture negative pulmonary tuberculosis

Objective To evaluate the application value of T cell infected with Mybacterium tuberculosis assay in diagnosis of smear and culture negative pulmonary tuberculosis .Methods In total ,145 patients with smear and culture negative pulmonary tu‐berculosis and 45 patients with non‐tuberculosis lung disease were enrolled in the study .All patients received T cell infected with Mybacterium tuberculosis test and sensitivity ,specificity ,positive predicted value ,negative predicted value of testing for the diagno‐sis of smear and culture‐negative TB patients were calculated .Results The sensitivity of specificity T cell infected with Mybacteri‐um tuberculosis assay in diagnosis of smear and culture negative pulmonary tuberculosis was 85 .5% ,the specificity was 84 .4% , positive predicted value was 94 .7% ,negative predicted value was 64 .4% .No statistical significance in age‐dependent groups(P>0 .05) .Conclusion T cell infected with Mybacterium tuberculosis assay has high positive predicted value in diagnosis of smear and culture negative pulmonary tuberculosis and is suitable for clinical auxiliary diagnosis .
2.PCR Detection of N.coenophialum and N.lolii

18 fungal strains including N.coenophialum,N.lolii, N.huerfanum、N.chisosum、N.aotearoae、N.sp.and 8 varieties of grass seeds belonging to Festuca arundinacea and Lolium perenne have been studied.With amplification of IS1~IS3 and F1~R1 of genomic DNA, the primers Tub-2-F~Tub-2-R from Tubulin-2 gene and F3~R3 from NC25 gene have been designed.A PCR method to detect N.coenophialum and N.lolii was established, and also a nested-PCR method to detect N.coenophialum and N.lolii in single seed was established.These PCR detection methods are strongly special and much credible and rapid-speeded.
4.Spiral CT reconstruction for typing of tibial plateau fracture to guide surgical therapy.

OBJECTIVETo typing of tibial plateau fracture based on spiral CT reconstruction and to explore effect of the typing method for treatment.
METHODSA hundred and twenty-six cases with tibial plateau fracture (male 95, female 31, age from 23 to 58 years old), the fractures were classified based on reconstruction image of spiral CT. Including central compression type in 13 cases, split type in 8, split compression type in 79, comminution type in 26. According to the different typing the suitable incision of operation and fixed method for fracture were select.
RESULTSA hundred and twenty-six cases were followed up for 0.5-4 years with an average of 1.2 years. According to Hohl system score to knee joint function, there were statistical significance in the pain,active movement,active range of motion between before and after operation (P < 0.01) and there were no statistical significance in stability and self-evaluation (P < 0.01).
CONCLUSIONTyping of tibial plateau fracture based on spiral CT reconstruction helpful to choose operative approach, reduction and fixed method and obviously improve clinical effect.

5.Ceramic-on-ceramic versus ceramic-on-polyethylene bearing surfaces in total hip arthroplasty:a meta analysis of efficacy and safety

BACKGROUND: Greatly importance has been attached to ceramic-on-ceramic bearing surface due to its excel ent wear resistance. But the risks of squeaking and ceramic fracture also go with it. Up til now, the choice between ceramic-on-ceramic and ceramic-on-polyethylene bearing surfaces in primary total hip arthroplasty remains controversial. OBJECTIVE: To compare the clinical outcomes and safety between ceramic-on-ceramic versus ceramic-on-polyethylene bearing surfaces in total hip arthroplasty based on meta analysis. METHODS: We electronical y searched databases including PubMed/Medline, Embase, Web of Science, Cochrane Col aboration database, Chinese Biomedical Literature Database (CBMdisc) and China National Knowledge Internet for randomized control ed trials on the comparison between ceramic-on-ceramic versus ceramic-on-polyethylene bearing surfaces in total hip arthroplasty from inception to January 2015. References of the included studies were also retrieved. Investigators severely selected the studies, extracted data and assessed the quality according to the inclusion and exclusion criteria. Then, meta-analysis was performed using RevMan 5.2 software. RESULTS AND CONCLUSION: Nine randomized control ed trials were included, involving 1 231 hips with ceramic-on-ceramic prosthesis and 932 hips with ceramic-on-polyethylene prosthesis. Meta analysis showed that both bearing surfaces achieved satisfied function recovery. But ceramic-on-ceramic had significantly increased risks of squeaking and ceramic fracture, meanwhile ceramic-on-polyethylene showed significantly higher wear rate. There were no significant differences in intra- or post-operative dislocation, osteolysis and other complications and prosthesis failure with any reason between two bearing surfaces. These results suggest that during the short- to mid-term fol ow-up period, no sufficient evidence can tel that ceramic-on-ceramic was obviously super than ceramic-on-polyethylene. Long-term fol ow-up is required for further evaluation.

Objective To demonstrate the diffusion tensor imaging(DTI)characteristics of multiple sclerosis(MS)plaques,periplaque white matter regions and normal appearing white matter (NAWM)regions in patients with MS,and to evaluate the clinical values of DTI and three-dimensional brain fiber tracking for the diagnosis of MS.Methods Conventional MRI and DTI were performed in 32 patients with MS and 32 age-matched control subjects.Fractional anisotropy(FA)and apparent diffusion coefficient(ADC)maps were generated and coregistered with T_2-weighted MR images,FA and ADC values were calculated in regions of interest in plaques,periplaque white matter regions,NAWM regions and white matter regions in control subjects.And three-dimensional brain fiber tracking maps were generated by using the DTI.Results TheADCwas(1.233?0.119)?10~(-3)mm~2/s in MS plaques,(0.973?0.098)?10~(-3) mm~2/s in periplaque white matter regions,(0.748?0.089)?10~(-3)mm~2/s in NAWM,and(0.620? 0.094)?10~(-3)mm~2/s in control subjects.The FA was 0.225?0.052 in MS plaques,0.311?0.050 in perip]aque white matter regions,0.421?0.070 in NAWM,and 0.476?0.069 in control subjects. Objective To evaluate the value of measuring ankle brachial index (ABI) for diagnosing peripheral arterial disease(PAD)compared with conventional digital subtraction angiography (DSA) as the reference standard.Methotis A total of 383 consecutive inpatients (245 male.mean age 64.1 ±11.7 years) underwent both conventional DSA and ABI measurements.Results The rate of statin intervention was 90.9%,ACEI 69.2%,antiplatelet 96.6% and β-blockers 67.9%.The intravascular stenosis was classified into six degrees:normal,<30%,30%-49%,50%-69%,70%-89% and≥90%.Compared to the traditional gold standard (DSA) in diagnosis PDA.the ABI value decreased in proportion to the sevefity of PAD (the ABI value was 1.08±0.11,1.05±0.16,0.99±0.17,0.66±0.24,0.55±0.28 and 0.54±0.00 respectively in the six ranks).There was a significant correlation between DSA and ABI in diagnosis PAD.Conclusion ABI measurement is an accurate and reliable non-invasive altemative to conventional DSA in the assessment of lower extremity arteries in patients with peripheral arterial disease.
8.Clinical features of children with incomplete Kawasaki disease.

OBJECTIVETo investigate the clinical features of children with incomplete Kawasaki disease (IKD), and to provide reference for the early diagnosis of IKD.
METHODSThe clinical data of 22 hospitalized children with IKD were analyzed retrospectively and compared with the data of 63 children with Kawasaki disease (KD) who were hospitalized during the same period of time. Another 20 children with pyrexia were enrolled as the control group.
RESULTSPyrexia was observed in all children. Compared with the KD group, the IKD group had significantly lower proportions of children with changes in the limbs, conjunctival hyperaemia, and cervical lymphadenectasis (P<0.05), a significantly higher serum level of glutamic-pyruvic transaminase (P<0.05), and significantly lower levels of plasma albumin, serum sodium, and interleukin-6 (P<0.05). There was no significant difference in the rate of γ-globulin application between the IKD and KD groups; however, the IKD group had a significantly higher incidence rate of coronary artery lesion than the KD group (P<0.05).
CONCLUSIONSThe symptoms and signs in children with IKD are untypical. The liver function test and serum hyponatremia and IL-6 measurements may be useful for the diagnosis of IKD.

9.Study of event-related potential on the visual attention of aging under region cues

Objective To investigate the brain mechanisms of aging of the visual attention. Methods Through the precue-target visual search paradigm using the event-related potentials technique,16 young and 16 elderly subjects participated the electrophysiological experiment.The background was comprised of three homocentric black circles and eight English capital letters consisted of each circle.The letter “T” was designed as the target stimulus.T was a target only when it located the circle clued to the same size of the precue.For example,when the cue was the “large”,“T” may appear within 3 circles.When the cue was the “median”,“T” may appear within either the median or small circles.When the cue was small,the target “T” may appear only within the small circle. Results The reaction time of the two groups of subjects became quick with the reduction of the cue scale,while the amplitudes of P1 and N1 components of event-related potentials increased with the decrease of the cue scale.Old subjects showed longer response time than did young subjects,and the posterior P1 component was enhanced significantly and N1 component was inhibited obviously.The P2 component was manifested as significantly inhibitory effect not only in the amplitude but also in the abnormal and unstable waveform.Conclusions The cognitive function of elderly subjects declines in the research of target stimuli,which suggests that the age-related changes could lead to deficit in the posterior area of the brain to visual spatial attention (involuntary attention).
10.Association between gestational weight gain and birth weight (2001-2009) :a cohort study carried outin Kunshan city, Jiangsu province, China

Objective To identify the association between gestational weight gain and birth weight over the past 9 years in Kunshan city,Jiangsu province,China.Methods This population-based study was conducted between 2001 to 2009.Data were retrieved from Perinatal Monitoring System of Maternal and Child Health Care Hospital of Kunshan.The study population consisted of 33 631 women and singleton live fetus.Gestational weight gain was defined as the total weight gain during the last and first prenatal care program and divided by the interval weeks.Results From 2001 to 2009,the average incidence of low birth weight was 1.86%,while the average incidence of macrosomia was a bit higher,fluctuating around 8.47%.On those underweight mothers,after adjustment for potential confounders,and stratified by the BMI levels,which were evaluated at the first prenatal care program,we found that weight gain in the 3rd and 4th intervals,could reduce the risk of low birth weight (less than 2500 g).With those mothers with normal-weight,weight gain in the 2 rid,3 rd and 4th intervals,would reduce the risk of low birth weight.Risks in the 4th quantile among underweight and normal-weight group were prevalence odds radio (POR) 95%CI:0.51 (0.32-0.80) and 0.58 (0.42-0.79),respectively.The risks showed a significant downward trend in underweight and normal- weight groups with increased gestational weight gain.As for macrosomia (≥4000 g),the risks increased (POR 95%CI) 4.69(2.82-7.81 ) in underweight,4.15 (3.43-5.03) in normal-weight,in overweight,3.64 (2.62-5.06) and 1.96 (1.48-2.60) in obese mothers with increased levels of gestational weight gain.Trend tests indicated that the risks of marcosomia increased in all levels of BMI,with the increase of gestational weight gain.Conclusion Findings from this population-based study suggested that gestational weight gain could reduce the risks of low birth weight among underweight and normal-weight groups,while increase the risks of macrosomia in all parturients,as compared with lowest levels of gestational weight gain.
