? AIM: To investigate the efficacy and safety of phacoemulsification combined with intraocular lens implantation in the treatment of shallow anterior chamber with cataract.?METHODS: Retrospective case series. From February 2014 to July 2015 in our hospital,65 eyes in 65 patients with cataract were enrolled and divided into mild and high risk of shallow anterior chamber group. Best-corrected visual acuity ( BCVA ) , intraocular pressure ( IOP ) , central anterior chamber dept ( CACD ) , angle opening distance ( AOD ) , complications pre- and post treatment, were observed and analyzed as outcome measures.?RESULTS: In this study, the mild shallow anterior chamber group included 34 eyes; postoperative BCVA were improved in 29 eyes, with 4 eyes remaining stable and decreased in 1 eye; BCVA was improved in 16 eyes, with 10 eyes remaining stable and decreased in 5 eyes in high risk of shallow anterior chamber group postoperatively. BCVA had a better prognosis in the mild shallow anterior chamber group than another group ( t=-2. 956, P<0. 05). Meanwhile, IOP decreased by 5. 71± 2. 07mmHg and CACD increased by 1. 37 ± 0. 38mm in the mild shallow anterior chamber group, by 9. 77±4. 04mmHg and 1. 67±0. 43mm respectively in high risk group, and the difference has statistical significance ( t=-5. 02,-3. 04; P<0. 05). The mean preoperative nasal AOD500 was 200. 57± 33. 74μm, and they were 346. 62 ± 101. 37μm and 410. 75 ± 137. 48μm and 398. 69 ± 122. 28μm respectively at postoperative 1d, 1 and 3mo, and all nAOD500 comparing with preoperative were increased obviously, and the difference has statistical significance (F=203. 75, P<0. 01). And AOD500 at temporal, superior and inferior presented similar trends. Complications were corneal edema ( 5 eyes ) , transient intraocular hypertension ( 2 eyes ) , posterior capsular opacification ( 4 eyes ) , and posterior capsular rupture (1 eye).?CONCLUSION:Micro incision cataract surgery is useful, effective and safe in patients with cataract and shallow anterior chamber which can stabilize or improve BCVA, reduce IOP, deepen CACD and open the anterior chamber angle.

BACKGROUND:Cybex NORMTM isokinetic dynamometer exhibits precise measurement,but the operation is too complex and the cost is too high to apply on routine examination.OBJECTIVE:To compare orthopedics tool measurement apparatus(OTMA)and Cybex NORMTM dynamometer in evaluating the range of motion after the treatment of shoulder.hip and knee joint diseases,and to verify tlle practical value of OTMA as a scientitic patent achievement.DESIGN,TIME AND SETTING:Clinical randomized control trial was carried out in the Rcsearch Center for Orthopaedics and Artificial Joint.Peking Union Medical College Hospital from October 2004 to March 2005.PARTICIPANTS:A total of 121 patients were randomly divided into two groups.OTMA group comprised 22 patients with shoulder periarthritis(22 shoulders).1 5 patients with avascular necrosis of the femoral head(21 hips),and 24 patients with knee osteoarthritis(33 knees).Cybex group comprised 21 patients with shoulder periarthritis(21 shoulders),16 patients with avascular necrosis of the femoral head(21 hips),and 23 patients with knee osteoarthritis(33 knees).METHODS:Two groups patients were treated with OTMA or Cybex NORMTM dynamometer,respectively,the range of motion was measured before treatment and 3 months,1 year after treatment.The Hospital for Special Surgery(HSS)knee rating scale,Karisson evaluation criterion of shoulder function and Harris hip score were applied to evaluate the patients,Visual analog scale(VAS)pain scale and short-form health survey(SF-36)were also used in this study.MAIN OUTCOME MEASURES:The range of motion in shoulder,hip and knee;Karlsson,Harris and HSS scores;VAS pain score and SF-36 score.RESULTS:OTMA measurement had a coincidence with Cybex NORMTM dynamometer measurement as 98.3%.All patients accepted and completed the measurement.The range of motion Was associated with HSS.Karlsson and Harris scores (P=0.002).CONCLUSION:According to three-dimensional space measurement,the digital OTMA may obtain the outcomes similarly with Cybex NORMTM dynamometer,it is also near to the practical value of joint angles.

BACKGROUND: Measurement is frequently performed in diagnosis of orthopedics, and involved in various correction andtreatment of congenital and acquired deformity. The perioperative measurement of affected knees of patients undergoing totalknee replacement (TKR) is significant.OBJECTIVE: To clinically evaluate and validate 4 domains of a self-designed simple medical orthopedics tool measurementapparatus (OTMA) according to the results of Cybex Norm measurement system, an international standard measurementapparatus, for osteoarthritis patients during TKR.DESIGN, TIME AND SETTING: Stratified, randomized, Controlled experiment. The study was performed at the Department ofOrthopedics, Peking Union Medical College Hospital between October 2004 and March 2006.PARTICIPANTS: A total of 47 patients (66 knees) with osteoarthritis of knee, undergoing TKR, were selected and randomlydivided into 2 groups: OTMA group (n=24, 33 knees) and Cybex group (n=23, 33 knees).METHODS: Range of motion (ROM) of two groups was measured respectively using OTMA and Cybex Norm, before and 3months after treatment as well as the final time of 1-year follow up. The ROM results were evaluated by Hospital for SpecialSurgery Knee Score (HSS) scale for knee joint. The visual analog scale and MOS 36-item Short Form Health Survey (SF-36)scale were filled to verify the applicability and reproducibility of OTMA. The correlation of measurement results and subjectivescale evaluation was analyzed to verify the evaluation efficiency of OTMA.MAIN OUTCOME MEASURES: Coincidence of ROM of two groups at different time points; correlation of scale scores and ROM.RESULTS: All patients were followed up. The coincidence rate of measurements between OTMA and Cybex Norm was 98.3%,HSS scale scores were increased with increasing ROM in both groups (P < 0.01). Correlation analysis showed that ROM washighly correlated with scale results, consistent with the clinical manifestation of patients.CONCLUSION: The measurement process was accepted by all patients, and could be performed repetitively. OTMA displaysprecise measurement, which could reflect clinical symptoms of patients and assist clinical diagnosis.

Objective To understand the iodine deficiency disorders in Xunhua county Qinghai province, and to provide reference values for setting up prevention and control strategy. Methods According to historical data of Xunhua county, 1 township center elementary school and 1 village elementary school were sampled in Qingshui, Baizhuang, Hongqi, Jiezi and Galeng townships. A total of 9 elementary schools were investigated (Caleng has only 1 elementary school). Fourty children aged 8-10 years were checked thyroid size by palpation in every school. Twenty children were chosen to assay their urinary iodine and edible salt iodine in every school. Thirty 5-grade students were surveyed by questionnaire on health education in every school. Ten housewives were inquired prevention knowledge on iodine deficiency disorders and salt iodine level was tested in every village. Results Goiter rate was 7.9%(30/378) in children aged 8-10 years, with Baizhuang and Galeng the highest (11.9%,5/42), and Qingshui the lowest (2.4%, 1/42). The median was 196.5 μg/L in children aged 8-10 years, and urinary iodine level lower than 50 μg/L was accounted for 5.3% (11/208), but that value was 25.0% (5/20)in Galeng. The average qualified rate of using edible salt was 91.4%(159/174), and that rates were 79.0%(15/19), 83.3%(15/18) and 89.5%(17/19) in Chajia, Tuanjie and Minzhu villages, respectively. The average score were 2.8, 2.1 in health education survey of the 5-grade students and housewives. Conclusions After years effort in prevention and control of iodine deficiency disorders, remarkable progress has been made in Xunhua County. The monitoring of iodized salt and the knowledge spreading are still need to be strengthened.

Objective To explore the current research status and existing problems in severe burn nursing by reviewing relevant literature of severe burn nursing published in 6 kinds of nursing core journals. Methods Search 6 kinds of nursing core journals using the following key words: severe burn critical burn serious burn large area burn intensive burn and nursing. Applying bibliometrics to conduct descriptive analysis of the external and internal characteristics of these papers. Results Two hundred and nine papers were included;the main literature research type of thess articles was clinical experience summary, the content of these literature was extensive. Conclusions Severe burn nursing research focus on clinical symptomatic nursing; clinical nurse scientific research ability needs to be strengthened, and subject aspects should be paid more attention to children, the elderly, and clinical nurse physical and psychological conditions.

The rise of advanced nursing practice in the developed countries promotes the development of nursing science, nursing postgraduate education arises at the historic moment. Professional nursing postgraduate education started late in our country and its development is not mature, there exists some problems in the training plan and professional positioning, such as training process not realizing the training objectives, unreasonable curriculum setting and evaluation system, unclear professional direction, disconnection of vocational orientation and the professional qualification and specialist nurse certification. Therefore, nursing professional degree graduate education mode needs further exploration and unity.

Background Aheration of eyeball wall caused by ocular axial extension is associated with multiple complications of high myopia.However,the study on quantitative analysis of choroidal thickness and axial length in adult high myopic patients is less.Objective This study was to investigate the choroidal thickness in high myopic eyes of adult patients and estimate the correlation of choroidal thickness with axial length,age,and spherical equivalent(SE).Methods A prospective cohort study was designed.Seventy-five eyes of 75 adult patients with high myopia were entrolled from December 2012 to May 2013,and 70 eyes of 70 age-and gendermatched healthy volunteers were included in Renmin Hospital of Wuhan University.Enhanced depth imaging (EDI)on Cirrus spectral-domain optical coherence tomography (SD-OCT) was used to measure the choroidal thickness from the outer border of the retinal pigment epithelium through the inner scleral boarder among the 11 meridians in a 500 μm intervals and range of 2 500 μm for each from fovea toward temporal and nasal lateral.The differences of choroidal thickness and axial length were compared between the high myopia group and normal control group,and the correlation of choroidal thickness with axial length,age,SE were analyzed.Results The subfoveal and mean choroidal thickness values were (146±52) μm and (142±63) μm in the high myopia group,and those in the normal control group were (306±60) μm and (271 ±71) μm,with significant differences between the two groups (t =-17.130,P=0.000; t=-15.890,P=0.000).Choroid was thickest in the temporal and then was subfovea and nasal in the high myopia group,but in the normal control group,it was subfovea,temporal and nasal in turn,and the choroidal thicknesses in various areas were thinner in the high myopia group than those in the normal control group.A negative correlation was found between the choroidal thickness and axial length in high myopia group(r =-0.580,P =0.000),and the regression equation determined a decrease of 17.943 μm per millimeter of axial length.Conclusions SD-OCT determines that choroidal thickness is decreased in highly myopic eyes compared with normal eyes.Choroidal thickness varies with the change of axial length in adult high myopia patient.These findings indicate that abnormalities of the choroids may play a role in the pathogenesis of complication of high myopia.
