1.Effect of BiPAP noninvasive ventilation on patients with acute left ventricular failure

Objective To observe the effect of Bi-level positive airway pressure(BiPAP) noninvasive ventilation on acute left ventricular failure(LVF). Methods BiPAP group :32 patients with severe LVF from various causes were treated by BiPAP with traditional drugs. Control group:32 patients were treated with traditional drugs alone,and the results were compared with those of BiPAP group. Results Dyspnea was relieved in all patients in BiPAP group,both the respiratory rate and heart rate were slowed,the vital signs were stable,and PaO2 ,SaO2 went up. The total effective rate was 100%, which was only 68. 75% in control group. Conclusion The use of BiPAP noninvasive ventilation therapy is feasible and effective on LVF.

This article reviews the editorial work in publishing Journal of Guangzhou University of Traditional Chinese Medicine (TCM) in the past 20 years, which includes the background when the Journal was first published, its characteristics and achievements. It is believed that TCM characteristics, advanced academic content and standardization of edition are the three important principles in publishing the Journal. It is sticking to these principles that the Journal has increased its communicating efficacy and its academic impact.
3.Features of Computed Tomography Perfusion of Mediastinal Lymphadenopathies: a Pathology-based Retrospective Study.

OBJECTIVETo explore the features of various mediastinal lymphadenopathies using computed tomography perfusion (CTP).
METHODSCTP parameters (CTPs) of the selected mediastinal nodes from 59 patients with pathology-proven malignant lymph nodes and of those from 29 patients with clinically diagnosed or pathology-proven inflammatory lymphadenopathies were collected. Patients were divided into subgroups by etiology and phase of primary disease, including different pathological malignant nodes and diverse inflammatory nodes. CTPs were defined as blood flow (BF), blood volume (BV), mean transit time (MTT), permeability (PMB), and time to peak (TTP). Differences of CTPs were compared between malignant and benign nodes, and among subgroups, respectively.
RESULTSIn the mediastinum, no significant differences of CTPs were found between malignant and benign groups (all P>0.05), the same for subgroups of malignant nodes (all P>0.05). Acute lymphadenitis had higher BF and BV than chronic inflammatory, lymphoid tuberculosis, sarcoidosis and malignant nodes. The BF of malignant nodes was markedly slower than that of acute lymphadenitis (P=0.01), but faster than chronic inflammatory nodes (P=0.04) and sarcoidosis (P=0.03), with no significant difference compared with lymphoid tuberculosis. Pneumonia-complicated lymphoid tuberculosis showed the longest MTT while sarcoidosis displayed the shortest MTT, and inflammatory nodes, lymphoid tuberculosis without complicated pneumonia and malignant nodes had moderate MTT.
CONCLUSIONCTPs show promising potential in distinguishing various lymphadenopathies in the mediastinum, but more studies are needed to improve their specificity.

4.Achillis Tendon-Sliding Procedure and Subtalar Joint Capsula Release by the Same Cut and Individualization in Treatment of Congenital Clubfoot

Objective To explore achillis tendon-sliding procedure and subtalar joint capsula release by the same cut and individualization treatment used in the correction of congenital clubfoot.Methods Forty eight cases (76 feet) of congenital clubfoot who were first visit were treated by achillis tendon-sliding procedure and subtalar joint capsula release by the same cut,meantime,footplate fascia release and transfer of the anterior tibial tenden were made by individualization.Results All cases were followed-up,the average time follow-up was 1 year and 6 months,excellent were 71% and good were 15.7%.Five cases were recurrence and its rate was 6.94%.Conclusions Achillis tendon-sliding procedure and subtalar joint capsula release by the same cut can solve rotation of displacement between calcaneus and talus and individualization treatment can also solve high arch deformity and adduction of foot.This technique is a simple,safe and effective treatment for congenital clubfoot.
5.Expression of Platelet-derived growth factor and Collagen type Ⅰ in hepatic tissue in oral drinking arsenic exposed mice

Objective To study the expression and its significance of Platelet-derived growth factor(PDGF) and Collagen type Ⅰ (Col- Ⅰ) mRNA in hepatic tissue in mice exposed to arsenic in drinking water. Methods Fifty mice were divided into control group, sodium arsenite group(iAs3+ group, 300 mg/L) and sodium arsenate group (iAs5+ group, 300mg/L) at random. The mice were sacrificed after 10 months for liver function test (ALT, AST and GLB examined by automatic biochemical analyzer) and pathologic examinations. Masson staining and immunohistochemical SABC methods were used. Through the result of Masson staining fibrosis semi-quantitative analysis of the fibrosis number was carried out in the per unit area. Using immuonhistochemical SABC methods, the expression of PDGF and Col- Ⅰ was detected. Results ①Serum ALT was statistically significant among control group [(36.67±3.51) U/L], iAs3+ group[(61.46±13.85) U/L] and iAs5+ group[(43.31±4.21) U/L, F= 6.56, P < 0.05], especially between the control and iAs3+ group (P < 0.05) ;Serum AST had a statistical significance among control group [(135.00 ± 20.42)U/L], iA3+ group [(510.86 ± 59.01)U/L] and iAs5+ group[(258.93 ± 22.40) U/L, F= 83.36, P < 0.05] ;Serum GLB had no statistical significance among control group [(20.86 ± 0.61)g/L], iAs3+ group [(26.94± 3.73)g/L] and iAs5+ group[(24.59 ± 5.27)g/L, F = 2.80, P < 0.05]. ②Masson staining revealed notable liver cell necrosis, regeneration, infiltration of inflammatory cells in portal area. Masson staining Semi-quantitative results showed significantly increasing size of fibrosis in model group. Compared with control group(0.069 ± 0.013) and iAs5+ group(0.143 ± 0.122), fiber hyperplasia of iAs3+ group(0.192 ± 0.108) had statistical significance (F = 1.91, P < 0.05). ③Immunohistochemistry results showed higher expression of PDGF in the portal area, sepetal and preisinus cells in model group. The heavy expression of Col- Ⅰ distributed in the surrounding of blood vessels and bile duet and along portal wall extensting to the liver in model group. Semi-quantitative results showed that PDGF among control group(202.79 ± 7.46), iAs3+ group(174.38 ± 7.71) and iAs5+ group(177.64 ± 7.81) had a statistical significance(F= 102.91, P < 0.05);Col-Ⅰ had a statistical significance among control group(200.11 ± 7.46), iAs3+ group (176.47±10.20) and iAs5+ group (177.378 ±7.948, F = 78.51, P < 0.05). Conclusions Exposed in oral drinking arsenic, mice can develop hepatic injury and hepatic fibrosis. The PDGF and Col-Ⅰ play significant effects on the process of hepatic fibrosis caused by oral drinking arsenic exposed mice.
6.Experimental observation of Hep-2 cell cycle progression by elemene combined with heperthermia

OBJECTIVE To observe the inhibition of elemene combined with heperthermia on the cultured Hep-2 cell line(human laryngeal cancer cell line).METHODS MTT was applied to detect Hep-2 cell proliferation inhibition rate.Flow cytometry was applied to detect cell cycle and apoptosis rate of Hep-2 cells and electronic microscope was applied to detect subcellular changing of Hep-2 cells.RESULTS The inhibition rates were 28.8%,25.7% and 47.1% in elemene group,heperthermia group and elemene combined with heperthermia group respectively.The result of elemene combined with byperthermia group was the most effective.Elemene combined with hyperthermia can significantly increase the proliferation inhibition rate and the apoptosis rate of the Hep-2 cell.The cell cycle progression can be changed obviously.The number of G1 phase cells increased,the number of S phase cells decreased while the cells in G2/M phase increased relatively.At the subcellular level,the swelled mitochondrial,the chromatin agglutination edge trended and apoptotic bodies were observed under electronic microscope.CONCLUSION Elemene combined with hyperthermia can keep Hep-2 from proliferation and inhibit the changing from G1 phase to S phase,so the apoptosis rate can be induced.

Objective To investigate the effects of Rhizoma Zingiberis Extract (RZE) on the cardiac functions of rabbits with heart failure. Methods Forty pure-bred New-Zealand rabbits were randomly allocated to 4 groups: control group, low-dosage RZE group, moderate-dosage RZE group and high-dosage RZE group. The rabbit model of heart failure was established by intravenous dripping of 20 g/L pentobarbital sodium. Oral tube perfusion of RZE was given after model establishment.The hemodynamic changes were observed before and after the modeling and 5,10,15,20,30,45,60,90,120,150 minutes after treatment by a RM-6000 4-graph physiological monitor. 10.Clinical efficacy of modified transforaminal lumbar interbody fusion for the treatment of lumbar spinal stenosis with lumbar instability in the elderly

Objective To explore the feasibility and clinical efficacy of modified transforaminal lumbar interbody fusion for the treatment of lumbar spinal stenosis with lumbar instability in the elderly.Methods Retrospective study was done on 45 elderly patients diagnosed as lumbar spinal stenosis with lumbar instability treated by modified transforaminal lumbar interbody fusion from June 2011 to December 2012.There were 25 males and 20 females aged from 65 to 78 years [mean (70.64.0) years].The operation time,blood loss,and complications were recorded and analyzed.The visual analog scale (VAS) score,and Japanese Orthopaedic Association (JOA) score were used to assess clinical outcomes before and 3 months after treatment and at the last follow-up.According to the criteria of Brantigan-Steffe,intervertebral fusion was evaluated.Results The operation of 45 patients was successful,and there were no severe complication.The average operative time was (126 23) min,(range,100~ 185 min),and the average amount of blood loss was (272±89) ml (range,180-550 ml).There was no injury of nerve root,dural tear,or deep infection.All patients were followed up for 12 months to 30 months with an average of (20.6±5.8) months.The VAS score of low back pain was decreased from (4.8 ± 1.7) before operation to (1.6 ± 0.5) at 3 months after operation and (1.3±0.3) at last follow-up.The VAS score of leg pain was decreased from (6.7±1.6) before operation to (1.2±0.5) at 3 months after operation and (0.6±0.3) at last follow-up.The JOA score was increased from (13.2±4.9) before operation to (23.8±4.0) at 3 months after operation and (24.1 4.2) at last follow-up.There were significant differences in the VAS score and JOA score between pre-and post-operation (F=68.35,98.58,89.73,all P<0.05),but no significant difference between 3 months after operation and final follow-up (all P > 0.05).We observed no pedicle screw loosening,breaking,orany pullingout of intervertebral fusion cage.All patients showed evidence of fusion in each operated segment according to the criteria of BrantiganSteffe.Conclusions Modified transforaminal lumbar interbody fusion has the advantages including less invasion,sufficient decompression,and less interference to neural structures,and it may provide an ideal surgical method for lumbar spinal stenosis with lumbar instability in the elderly.
