1.Change and clinical significance of serum myocardial zymogram in patients with intracerebral hemorrhage

Objective To explore the changes and the clinical signficance in serum myocardial zymogram of patients with intracerebral be hemorrhage(ICH).Methods To estimate the serum myocardial zymogram.Serum aspartate aminotrasferase(AST),lactate dehychogenase (LDH),creatine kinas (CK) in patients with ICH were measured in 3 days,then to make comparison with control group.Results The serum myocardial zymogram in ICH group were higher than that of control group(P <0.05).There were different changes in serum myocardial zymogram of various parts.The serum myocardial zymogram in group with temporal lobe damage was higher than that of group without temporal lobe damage (P < 0.05).The serum myocardial zymogram of group with colliculus brain damage was higher than that of group without colliculus brain damage(P <0.05) ,the serum myocardial zymogram of group with disorder of consciousness was higher than that of group wakefulness(P < 0.05).Conclusion There are significant changes in the serum myocardial zymogram of patients with ICH.The incidence of the serum myocardial zymogram in ICH was close]y correlated to the severity area and part of ICH,consciousness disorder.To estimate serum zymogram is helpful to judge patients' state and prognosis.
2.Effect Evaluation of Comprehensive Intervention with Low-salt Diet in Elderly Patients with High Blood Pressure

Objective To investigate the role of the extended-care in improving the compliance with low-salt diet in elderly patients with hypertension.Methods We selected 100 cases of elderly patients with hypertension in hospital.The 100 cases were divided into control group (50 cases) and observation group (50 cases) randomly.Observation group was given comprehensive intervention in low-salt diet education and control group was given traditional low-salt diet education.Tow groups were divided into three stages (on the day of admission,seven days after admission,one month after discharge).We analyzed low-salt cognitive scores,low-salt behavioral scores and salt-intake/d comparably.Results On the day of admission,tow groups had no statistically significant difference (P>0.05) in low-salt cognitive scores,low-salt behavioral scores and salt-intake/d;seven days after admission,tow groups had statistically significant difforence (P<0.05) in low-salt cognitive scores,low-salt behavioral scores and salt-intake/d;one month after discharge,tow groups had statistically significant difference (P<0.05) in low-salt cognitive scores,low-salt behavioral scores and salt-intake/d.Conclusion We use comprehensive intervention method to elderly patients with hypertension in low-salt diet education,through personalized low-salt diet education following evidence-based nursing,we can increase the compliance with low-salt diet in elderly patients with hypertension after discharge and improve blood pressure control effectively and to promote the image of the hospital and achieve the higher satisfaction from patients and families.
3.The role of complement activation in the pathogenesis of diabetic complications

Complement system is an important constituent in immune system.Membrane attack complex (C5b-9)is the final ingredient in the complement activation.Recently,a few studies showed that C5b-9 deposition was found in the diabetics with chronic complications,indicating that complement activation might be involved in the pathogenesis of diabetic complications.Our aim is to summarize some newly published works and show a prelimimary work we did in this area.We hope that more attention should be paid to the role of autoimmune mechanism in diabetic complications.

5.Percutaneous reduction combined with bone graft in the treatment of displaced intra-articular calcaneal fractures

[Objective]To introduce the experience and key points of percutaneous reduction combined with bone graft to treat calcaneal fractures.[Method]Percutaneous reduction and internal fixtion combined with bone graft was performed from April 2004 to April 2006 on 15 cases(16 sides) with intra-articular calcaneal fractures including 13 males (14 feet)and 2 females(2 feet),with average age of 36.6 years(24~61 years).All patients underwent radiography including lateral and axial views for calcaneus,oblique view for foot and three-dimensional CT imaging reconstruction. According to Sanders classification,there were 12 feet of type Ⅱ(three type Ⅱa,three type Ⅱb and eight type Ⅱc)and 2 feet of type Ⅲac. The length of caicaneus was recovered through traction by Steinmann pin which passed through calcaneal tubercle perpendicularly and the posterior facet was elevated until reduction by a curve scissors through an 0.5 cm incision along the primary fracture line of lateral calcaneus. The calcaneus was fixed with different cannulated cancellous screws according to the type of fractures.Then bone graft was injected to fill the defect of calcaneus through lateral incision.[Result]All patients were followed up for an average of 18.4 months (ranged,12 to 34 months).No complication such as wound infection,screw breakage and calcaneum varus was found postoperatively. The average time for bone healing was 10 weeks. The results were excellent in 12 cases, good in 4 cases according to the American Orthopaedic Foot and Ankle Society(AOFAS) hindfoot score. The rate of excellent and good clinical results was 100%. Radiography showed basic restoration of Bhler's angle,Gissane's angle and calcaneal shape.[Conclusion]The combination of percutaneous reduction and injectable bone graft is suitable for surgical treatment of Sanders II and III type calcaneal fractures,with advantages of simple operation,fewer complications and good clinical results.
6.Application of trypan blue and indocyanine green in mature and hypermature cataracts

Abstract?AlM: To assess the efficacy and safety of trypan blue and indocyanine green ( lCG ) for continuous curvilineal capsulorrhexis ( CCC ) in mature or hypermature phacoemulsification.?METHODS: A total of 122 eyes of 122 cases with cataracts in mature and hypermature were randomly divided into three groups, trypan blue staining group was 46 eyes of 46 cases as group A, lCG staining group was 40 eyes of 40 cases as group B, control group was 36 eyes of 36 cases as group C. Staining groups were used to 0. 2mL trypan blue or lCG injected into the anterior chamber during operation, respectively. The success rate of CCC, lens posterior capsule rupture and implanted intraocular lens pouch were observed and compared during operation. Anterior chamber inflammation was observed after operation, and compared with the control group to observe and analysis.? RESULTS: The success rate of CCC, implanted intraocular lens pouch were statistically significant difference in trypan blue staining group ( group A ) than that in control group (group C) (P<0. 05). lmplanted intraocular lens pouch was statistically significant difference in lCG group ( group B ) than that in control group ( group C ) ( P<0. 05 ) . The rate of lens posterior capsule rupture was no statistically significant difference in all three groups ( P > 0. 05 ) . Anterior chamber inflammation was no significant difference in the postoperative reaction among the three groups.?CONCLUSlON:The application of trypan blue or lCG for lens capsule staining before CCC in lack of red reflective mature and hypermature cataracts is safe and effective, both results are comparable. lt guarantees a complete CCC and improves the success rate of phacoemulsification.
7.A case of proptosis by traumatic delayed meningo-encephalocele.

A case of traumatic delayed meningo-encephalocele suffered orbital fracture, but bony defects in frontal sinus had not been found on CT scanning. We treated the patient with surgery of intranasal endoscopy and repaired the skull base defect successfully during the first attempt. There was no recurrence in 10 months followed up. The leak site may not correlate with imaging in traumatic delayed meningo-encephalocele by comparing operative findings with the imaging estimate and endoscopy. Therefore, endoscopical approaching is effective in seeking and treatment.
