1.Determination of hyperoside in Bushen-Yijing wan by Ion-Pair HPLC

Objective To establish a HPLC method for the determination of Hyperoside in Bushen-Yijing wan. Methods The HPLC system consisted of the Agilent HC-C18(4.6 mm×250 mm, 5μm)column was adopted. The mobile phase was consisted of acetonitrile(10)-HK2PO4(90)(0.85 g/L sodium heptanesulfonate,H3PO4 adjusted pH to 4.8), the flow rate was 1.0 ml/min, the column temperature was 30 ℃, and the UV detector was set at 360 nm. Results The linear response range was 3.0~60.0μg/ml(r=0.999 9). The average recovery of hyperoside was 98.2%, and RSD1.53%. Conclusions The method is simple, rapid, accurate and repeatable. It can be applied in determination of Hyperoside in Bushen-Yijing wan.
2.Determination of spinosin in Anshenbao granule by HPLC

Objective To establish a HPLC method for the determination of Spinosin in anshenbao granule. Methods The HPLC was adopted with Phenomenex LUNA-C18(4.6 mm×250 mm, 5μm)column, and the mobile phase consisted of Acetonitrile(19)-0.005 mol/L Kaliumdihydrogenphosphorate(81). The flow rate was 1.0 ml/min, the column temperature was 30℃ and the UV detector was set at 250 nm. Results The linear response range was 1.83~36.60μg/ml(r=0.9998). The average recovery of spinosin was 98.4%, and RSD1.22%. Conclusion The method was simple, rapid, accurate and repeatable. It can be applied in determination of spinosin in Anshenbao granule.
3.Performance evaluation of Candida spp.latex immunochromatography

Objective To preliminarily evaluate the performance and species specificity of Candida spp.latex immunochromatography kit for detecting Candida spp.in vaginal secretion.Methods The vaginal secretions in 354 cases of suspected vaginitis and 9 genus of 22 species of common vaginal infecting organisms were by adopting the smear/Gram staining and Candida spp.kit.The results were compared with those detected by smear/Gram staining semi-quantitative method.The detection performance and species specificity of Candida spp.kit for detecting clinical specimens were evaluated.Results Compared with the smear/Gram staining method,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of Candida sp.kit were 93.81%,99.10%,97.31%,98.14% and 96.90%,respectively;compared the smear/Gram staining semiquantitative method,the Candida spp.kit for detecting Candida spp.in vaginal secretion still had higher specificity and acceptable sensitivity when the Candida spp.concentration was lower in specimens;in the speciese specificity evaluation,6 kinds of Candida spp.were positive and other 16 kinds of common vaginal infecting organisms all were negative.Conclusion The Candida spp.kit for detecting Candida spp.in vaginal secretion has higher clinical and species specificity,sensivity,positive prediction value and negative prediction value.
5.Evidence-based medicine proof and development trends of first-line treatment for advanced hepatocellular carcinoma

Tyrosine kinase inhibitors (TKIs) are used as the primary first-line treatment for advanced hepatocellular carcinoma, and the combination of immune checkpoint inhibitors (ICIs) and angiogenesis inhibitors have also been recommended as first-line treatment recently. For hepatocellular carcinoma patients with portal vein tumor thrombus, hepatic artery infusion chemotherapy (HAIC) is supported by progressively more evidence in improvement in the overall survival benefit. Based on relevant literatures and combined with clinical practices, the authors investigate the clinical application and development trends of TKIs, ICIs and HAIC in the first-line treatment for advanced hepatocellular carcinoma.
6.Research progress concerning intramedullary nail fixation for calcaneal fractures

Calcaneal fractures, common injury to foot, usually require surgery if complicated with an obvious displacement. Since open reduction and internal plate fixation often leads to wound infection, more and more minimally invasive surgery has been adopted for calcaneal fractures. As a minimal invasive treatment, intramedullary fixation can be used to treat calcaneal fractures of Sanders types Ⅰ-Ⅳ. Calcanail and C-Nail are 2 major types of intramedullary nails that have been used to treat calcaneal fractures in Europe and the United States since 2011. This article reviews the basic structural features, biomechanical properties, surgical procedures and clinical efficacy of these intramedullary nails.
7.Differentiation of Pc and P2 variants in class 1 integron by high-resolution melting analysis

Objective To develop a simple high-resolution melting ( HRM) analysis method for differentiation of Pc and P2 variants in class 1 integron.Methods DNA fragments containing Pc and P2 variants were amplified from plasmids pACW ( PcW ) and pACWP2 ( PcW-P2 ) respectively , then these purified PCR products and P 2 promoters were analyed full-length amplicon by HRM .Eight DNA fragments containing different Pc promoters were amplified and site-specific mutated from plasmids pACS ( PcS ) , pACH2 ( PcH2 ) , pACH1 ( PcH1 ) , pACW ( PcW ) , genomic DNA of Klebsiellar pneumonia HS07-68 (PcWTGN-10)and HS05-1792(PcH2TGN-10)respectively.The purified PCR products and eight Pc variants were characterized by HRM analyses of an unlabeled probe and full-length amplicon.This assay was applied to the differentiate Pc and P2 variants in 109 class 1 integrons from 95 urine clinical Escherichia coli isolates in Huashan Hospital during 2004 -2007.The differentiation results were compared with that determined by direct sequencing .Results P2 promoter with a significant higher melting temperature ( Tm ) can be identified by HRM analysis clearly .P2 promoters were identified in 2 class 1 integrons and consistent with direct sequencing results .Eight Pc variants were classified into three groups: PcS, PcSTGN-10 , PcW, PcWTGN-10, PcH1, PcH1TGN-10.Using direct HRM analysis.PcH2, PcH2TGN-10 were classified into four groups:PcS, PcH1, PcH2, PcW, PcSTGN-10 , PcH1TGN-10 , PcH2TGN-10 , PcWTGN-10 according to the melting curves of the unlabeled probe .Combined the HRM analyses of the whole amplicon and unlabeled probe , the eight Pc variants can be differentiated from each other .Five different Pc variants, PcS, PcW, PcH1, PcH2TGN-10 and PcWTGN-10 , were identified and consistent with direct sequencing results .Conclusions This developed a simple Pc and P 2 variants differentiation method via simultaneous HRM analyses of an unlabeled probe and full-length amplicon .This method is cost-effective and accurate , could be used in differentiation of Pc and P2 variants of class 1 integrons in clinical isolates .
8.Percutaneous kyphoplasty in treatment of elderly osteoporotic vertebral compression fractures

Objective To investigate the clinical efficacy of percutaneous kyphoplasty(PKP) in treatment of elderly osteoporotic vertebral compression fractures(OVCF).Methods 122 elderly OVCF patients from Oct.2009 to Oct.2013 in Lihuili Hospital of Ningbo Medical Center were selected,and were randomly divided into group A,and group B.There were 61 cases in each group.Group A received percutaneous kyphoplasty(PKP),and group B were treated with percutaneous vertebroplasty(PVP).The two groups were observed in terms of pain,activity,vertebral compression rate and Cobb angles after treatment.The two groups were followed up at least for 3 months.The postoperative side effects were recorded and analyzed.Results After the treatment,the two groups significantly improved in VAS score and activity(P < 0.05),but no significant difference was found between the two groups (P > 0.05).After the treatment,vertebral compression rate and Cobb angle got better than before in the two groups,but group A recovered more obviously than group B,and the difference had statistical significance (P < 0.05).The postoperative recurrence rate was 18.03 % (11/61) in the observation group and 9.84 % (6/61) in the control group,and the difference had statistical significance (P < 0.05).Conclusion PKP and PVP can significantly relieve pain,but PKP had better effect than PVP in vertebral body and spinal kyphosis improvement,which is worth of clinical application.
9.Optimization of flash type extraction methods forDanzhuye granules by orthogonal test

Objective To optimize the flash type extraction ofDanzhuye Granules.Methods With the total flavonoids and extract yield as indices, the orthogonal test was adopted to optimize the extraction times, the ethanol concentration, solid-liquid ratio, and the duration of extraction forDanzhuye Granules.Results The factors that infuenced the extract efficiency from high to low were the extraction times, volume fraction of ethanol, the ratio of liquid to solid, and the extraction time. The optimum extracting condition included that the origin was extrated by the ethanol volume fraction 80% with the material to liquid ratio 30:1, total of 3 times, with each time of 3 minutes. Validation experiments of the potimal condition showed that the extraction of total flavonoids was 29.1 mg/g, and the extract yield was 23.6%.Conclusions The flash type extraction method is suitable for extraction, which provides a new method for the development and utilization ofDanzhuye Granules.
10.Short and long term results of percutaneous transhepatic cholangioscopic lithotomy in the treatment of intrahepatic duct stones

Objective To explore the short and long term curative effects of percutaneous transhepatic cholangioscopic lithotomy (PTCSL) in the treatment of intrahepatic stone (IHS).Methods 38 IHS patients were enrolled,who were treated with PTCSL between January 2008 and July 2013.Results PTCSL was successfully completed in all the 38 IHS cases.Stone clearance rate was 84.2% and the average episode of stone removal was (2.6 ± 0.9) times.Average diameter of percutaneous transhepatic fistula was (18.4 ± 0.6) F and the average time from percutaneous transhepatic puncture and fistulization to cholangioscopic lithotomy was (7.2 ± 0.7)d.The average operation time was (68 ± 20) min,intraoperative blood loss was (20 ± 13) ml,and hospitalization was (4 ± 2) days.The hepatolith recurrence rate in patients with stones completely removed was 37.5% (12/32),and 1 case developed into biliary cirrhosis.Patients with calculi residual suffered from higher hepatolith recurrence rate of 83.8% (5/6),with biliary cirrhosis found in 1 case.Conclusions PTCSL is safe and effective in treating primary IHS,which is indicated in multiple recurrent IHS especially in after biliary surgery patients.It has the advantages of minimally invasion,less bleeding,less postoperative pain,less complications,and fast postoperative recovery.
