2. Analysis of long-term outcome of patients with acute promyelocytic leukemia treated with different regimens after complete remission

Objective: To explore a more effective treatment regimen for patients with acute promyelocytic leukemia (APL) after complete remission (CR). Methods: Fifty patients with newly diagnosed APL were induced to CR by all-trans retinoic acid (ATRA). They were divided into two groups. Twenty-three patients in group A were given with alternative therapy with compound realgar and natural indigo tablet (CRNIT) and combined chemotherapy. Twenty-seven patients in group B were treated with alternative therapy with ATRA and combined chemotherapy. Long-term clinical outcomes were compared between the two groups. Results: Eight of the total 50 patients relapsed. The relapse rates in group A and group B were 4.3% and 25.9%, respectively. The difference was significant (P = 0.038). The three-year and five-year disease-free survival (DFS) rate of group A was markedly superior than group B (100% and 91% vs 79% and 58%; P = 0.023). Conclusion: Alternative therapy with CRNIT and combined chemotherapy for APL patients after CR was more effective than that of ATRA and combined chemotherapy.
3. Interaction of magnolol with HSA/BSA and analysis of the feasibility of modeling equation

OBJECTIVE: To investigate the reaction mechanisms of magnolol (MG) binding with bovine serum albumin (BSA) and human serum albumin(HSA) by using different equations. METHODS: The characteristic spectra of HSA/BSA-MG interaction system were determined by using spectral experiment. The binding constants, energy transfer parameters and thermodynamic parameters were calculated by logarithmic equation and nonlinear equation, and the influence of MG on molecular conformation of BSA/HSA was also investigated. A comparative analysis of the interaction of MG with HSA/BSA was performed. The different equations were applied to deal with the experimental data and the feasible modeling equation was suggested. RESULTS: Static quenching exited between MG and SA with significant bond strength. The value of binding distances (r) was low, which indicated the occurrence of energy transfer. Fluorescent phase diagram technical analysis showed that the reaction conformational pattern of MG-BSA was "two-state" model; MG-HSA was a sequence change process. According to the obtained thermodynamic parameters, the main interactional forces between MG and SA were judged to be hydrogen bonds and Van der Waals forces. The results of molecular modeling revealed that hydrogen bonds and Van der Waals forces were the main binding forces in the MG-BSA/HSA system, and hydrophobic interaction was also present. These results were in accordance with spectral experiments. CONCLUSION: There are differences between the interaction mechanisms of MG-BSA and MG-HSA. Logarithmic equation is the most suitable equation for this system.
4. Preparation of inclusion complex of piperine with β-cyclodextrin and exploration of the properties of inclusion complex system

OBJECTIVE: To prepare a novel inclusion complex of the piperine with β-cyclodextrin (β-CD) by the saturated solution method and characterize the inclusion mechanism by fluorescence spectroscopy and ultraviolet spectrometry. METHODS: The dissolution properties and the balance properties of the inclusion complex were analyzed by ultraviolet spectrum and the Gaussian method was used to calculate the inclusion complex's configuration and thermodynamic parameters. The interaction performance of β-cyclodextrin-piperine supermolecular system was explored by spectroscopic experiment combined with theoretical calculation. RESULTS: The results showed that the spectral method better characterize the piperine-β-cyclodextrin. The stability and solubility of piperine was significantly improved after it was included with β-cyclodextrin. Gaussian calculation successfully obtains the preferred molecular configuration and the total energy change effect of inclusion system, the thermodynamic parameters of the piperine-β-cyclodextrin were also be obtained and showed that the inclusion process of the inclusion complex is a spontaneous binding reaction, which was further confirmed by spectral experiments. CONCLUSION: The stability and solubility of piperine can be significantly improved after β-cyclodextrin including, the results can provide certain reference for the study of inclusion complex supermolecular system.
5.Analysis on related factors of depression in chronic hemodialysis patients

Objective To explore the social economic factors related to depressive in patients with long-term hemodialysis.Methods A total of 122 patients with hemodialysis for more than 3 months from December 1st 2008 to February 1st 2009 in Dongfang Hospital and Wangjin Hospital,were divided into a depression group (41 cases) and a non-depressed group (81 cases).Informations such as economic status,social interaction,education,hobbies,etc were collected by questionnaire and the relationship between these factors and depression was analyzed.Results There was significant difference between the two groups in the factors of economic status,social interaction,education,and hobbies (x2 values were 14.671 、6.256、6.744、6.390,P<0.05).There was no significant difference in the factors of gender,drinking,relationship of family,profession,the condition of housing,social status,seasons of born,and careers,etc (x2 values were 3.651、0.765、2.034、3.093、0.772、5.956、2.687、5.138,P>0.05).Conclusion Thepatientswithpooreconomic,lower education,less social communication,fewer hobbies,and no smoking habits are easy to be depressive.
6.Effect of adoptive immunotherapy with CIK cells combined with interleukin-2 on immunologic function and life qualities of patients with non-small cell lung cancer

Objective To investigate the changes of immunologic functions and life qualities after the auto-cytokine induced killer (CIK) cells combined with interleukin-2 (IL-2) being transfused back to the patients with non-small cell lung cancer (NSCLC).Methods Thirty-eight NSCLC patients were enrolled,and 19 patients received not only chemotherapy and radiotherapy but also adoptive immunotherapy with CIK cells combined with IL-2 compared with the other 19 patients who only accepted radiotherapy and chemotherapy.The changes of immunologic functions and life qualities after the CIK cells combined with IL-2 being transfused back to the patients with NSCLC were observed.Immune targets of the treatment group and control group,such as peripheral blood CD3 +,CD4 +,CD8 + T cell percentage,ratio of CD4 +/CD8 + and Th1/Th2,were observed at the end and after 3 month of the radiation and chemotherapy.Results Both of the two groups were neither having obvious adverse reactions.After CIK cells combined with IL-2 treatment,the life qualities of patients improved,such as 18 cases of mental improvement,15 cases of appetite improvement,12 cases of sleep improvement,and 3 cases of fatigue symptoms improvement.The percentage of CD3 +,CD4 + cells in the treatment group increased [(66.39 ± 9.22) % vs (42.98 ± 7.23) %,t =5.45,P =0.00 ; (32.27 ± 3.75) % vs (26.38 ±2.51)%,t =5.73,P =0.00],the percentage of CD8 + cells declined [(17.51 ± 1.85)% vs (20.90 ± 2.31) %,t =5.21,P =0.00],the rate of CD4 +/CD8 + increased and the rate of Th1/Th2 obversed [(1.86±0.32) vs (1.27±0.19),t=7.13,P=0.00;(1.15±0.48) vs (0.91 ±0.30),t=2.42,P=0.02].There were significant differences between the two groups.Conclusion Adoptive immunotherapy with CIK cells combined with IL-2 is safe in clinical,which can improve the immunologic functions and life qualities of the patients with NSCLC.
7.Clinical Application of CT Simulation in Radiotherapy for Esophageal Carcinoma

Objective To evaluate method of CT simulation (CT sim) in the treatment planning setup of esophageal carcinoma. Methods From Dec.1999 to Jun.2002, 49 patients with pathologically proved esophageal carcinoma were analyzed. All patients underwent CT sim and treatment planning was configured with three symmetrical fields in the isocenter mode. A few comparisons have been made between CT sim and conventional simulation (Con sim) in the treatment planning setup. Results The difference in isocenter position were observed between CT sim and Con sim with 3~18 mm.When the esophageal lumen was taken as portal center, the 90% isodose curve in 42.8%(21/49) of patients were able to cover the whole lesion. When taking the tumor as the portal center, the whole lesion of all patients was totally covered by the 90% isodose curve. Conclusion The CT sim has an advantage in the delineation of lateral extension of esophageal tumor than the Con sim .Selection of irradiation portal should be done according to the size the silhouette of the tumor.

9. Research of binding reaction mechanism for adenosine in traditional Chinese medicine with serum albumin

OBJECTIVE: To study the binding mechanism between adenosine (Ade), an active component of some traditional Chinese medicines, and human serum albumin (USA). METHODS: The binding constant K and binding sites n were obtained by using fluorescence and UV absorption spectroscopy. In addition, the reaction mechanism was discussed. The energy transfer parameters of the reaction system were determined as was the effect of Ade on HSA's conformation on the basis of Fōrster theory. The binding reaction model of HSA with Ade was constructed through molecular simulation and compared with the experiment results. RESULTS: Ade could bind HSA to form static complex. The binding constant (K) was 1.39 × 10 L3 · mol-1 and binding sites (n) was 0.94. The binding distance r between Ade and HSA was very short, which indicated the phenomenon of energy transfer. Ade changed the hydrophobic environment of the binding domain in HSA and caused certain changes for micro area conformation. Computer molecular docking technology showed that the interactions of Ade with HSA were mainly hydrophobic interaction and hydrogen bonding between purine ring of Ade and amino acid residues of HSA. CONCLUSION: The theoretical calculation and experiment results were in consistency, which provided certain reference for the study of pharmacological mechanism for the active ingredient of some traditional Chinese medicines, adenosine. 10.The correlation between serum levels of IL-33,TNF-α and disease severity,prognosis in patients with acute pancreatitis

Objective To explore the reference value of interleukin(IL)-33 and tumor necrosis factor-alpha(TNF)-α in early diagnosis,illness evaluation and prognosis of acute pancreatitis(AP) by dynamically monitoring the serum levels of IL-33 and TNF-α.Methods A total of 86 patients with AP were selected as objectives which were divided into two groups according the severity of illness which include 59 cases of mild AP group(MAP) and 27 cases of severe AP group(SAP),52 cases of systemic inflammatory response syndrome(SIRS) group and 34 cases of non SIRS group distinguished by SIRS,75 cases of survival group and 11 cases of death group identified by prognosis of AP.All of patients were assessed by Bedside Index of Severity in Acute Pancreatitis(BISAP) score within 48 h after admission and detected the serum levels of IL-33 and TNF-α at the first day,third day,seventh day and fourteenth day during hospital.A total of 63 healthy persons were recruited into control group.Results The serum levels of IL-33,TNF-α and BISAP score of SAP group and MAP group were higher than those of the control group in different monitoring time,but the SAP group increased more obviously than MAP group.The serum levels of IL-33 and TNF-α of SAP group were higher than those of MAP group at the first day,third day,seventh day and fourteenth day during hospital,and the differences between them had statistical significance(P<0.05).The serum levels of IL-33 and TNF-α of SAP group increased to peak at the third day and reduced gradually after conventional treatment(the seventh day in the hospital).The serum levels of IL-33 and TNF-α of SIRS group were significant higher than those of non SIRS group(P<0.05).The serum levels of IL-33,TNF-α and BISAP score of death group were significant higher than those of survival group(P<0.05).Spearman correlation analysis suggested that there were positive correlations between BISAP and the serum levels of IL-33 and TNF-α of AP patients.Conclusion There is an important clinical value to the early diagnosis,severity classification,guiding clinical treatment and the assessment of prognosis of AP by dynamically monitoring the serum levels of IL-33 and TNF-αof AP patients.
