1.Relationship between Pancreatic and Pulmonary Injury in Rat Acute Pancreatitis

Rat acute pancreatitis was induced by the injection of dog bile into a closed duodenal loop. It was found that lung index and extravascular lung water were significantly increased 6 h after the induction of pancreatitis. Histological examination revealed pulmonary congestion, edema, and atelecta-sis. There was a strong correlation between pancreatic and pulmonary injury. Increased serum and lung lipid peroxide (LPO) was parallel to the degree of pancreatic and lung injury. The results indicate that increase in LPO may be a result of pancreatitis-induced lung injury.
2.THE CHANGES OF CYCLIC NUCLEOTIDES IN PLASMA, PANCREAS AND LUNGS DURING EARLY PHASE OF ACUTE HEMORRHAGIC PANCREATITIS IN DOGS

Cyclic nucleotides in plasma, lung and pancreas were determined by radio-competitive protein binding assay before and after acute experimental pancreatitis in dogs. Plasma cAMP was increased three-fold 30 minutes after pancreatitis and remained at high level for 5 hours. cAMP was increased in the lungs and decreased in the pancreas,as compared with the controls.There were no significant changes for cGMP. The mechanisms of the above changes were discussed.

4.Current status of diagnosis and management of rectal cancer in China

Colorectal cancer is one of the most common malignancies in the world, and rectal cancer accounts for a large percentage of this disease in Chinese patients. Surgical extirpation is the mainstay of therapy, Intraluminal ultrasound is highly accurate in detecting depth of invasion and lymph node metastasis, and is also helpful in preoperative staging of rectal cancer. Preoperative adjuvant therapy could down-stage tumors, improve the radical resection rates of advanced lower rectal cancer, increase the rate of sphincter-saving surgery, and decrease the local recurrence rate. Total mesorectal excision could signifi-candy decrease the local metastasis of rectal cancer, and rectal cancer resection with preservation of pelvic autonomic nerve is effective in preventing urinary or reproductive dysfunction. Recently, minimally invasive surgery has been introduced for the management of rectal cancer, and transanal endoscopic microsurgery is a new method with advantages in rectal cancer treatment.

Some studies corresponding modernization of Chinese materia m edica(CMM)were performed in the institute for the new technologies,materials,and facilities based on the achievements and experiences gotten in the past two dec ades.The technologies of large-scale propagation of plant cell,tissue,and organ were applied to the products of cell metabolism,biotransformation,and artificial breeding and rooting.Many new types of large-scale bioreactors were designed an d used not only for laboratory but also for manufactory.Reaction/separation inte gration technology,microwave-assisted extraction,and the other new extraction te chnologies were studied to realize high efficiency and low energy.Besides tradit ional separation and purification methods,new technologies,including extraction in reversed micelles,one step three-phase extraction on Penicillin,foam fraction ation,membrane separation,and high-speed counter-current chromatography(HSCC),we re developed.HSCCC is a kind of liquid-liquid partition chromatography without a ny solid matrix,which eliminates irreversible adsorption of samples on solid sup port in the conventional chromatographic column.It has been successfully applied to the analysis and separation of various natural products.At the same time,man y types of microsphere and microencapsules for controlled release and separation media were prepared and lots of attention was paid on drug modifying and embedd ing techniques.HSCCC was also applied as a new method to the study of CMM finger print and showed good precision and repeatability.High performance liquid chroma tography(HPLC),gas chromatography(GC),high performance capillary electrophoresis (HPCE),and mass spectrometer(MS)played a very important role in fingerprinting.C ontinuous propagation-continuous collection technological process was set for la rge-scale propagation of micro-algae in the study of marine drugs.Transgenic alg ae were cultured to prduce genetically engineered products.
6.Clinical analysis of 76 cases of gastric stump cancer after subtotal gastrectomy

Objective To observe the treatment and outcomes of gastric stump cancer(GSC) after subtotal gastrectomy,and to analyse the possible related factors for the development of GSC. Methods The clinical data of 693 patients with subtotal gastrectomy were retrospectively analysed.The development,treatment and outcomes of GSC were observed,and the possible factors associated with the development of GSC were analysed. Results Seventy-six cases of GSC emerged in the 693 patients with subtotal gastrectomy.Fifty-one of the 76 cases were perfomed radical operations(n=36) or palliative resection(n=15).No death was found during the perioperative period,and the median survival time was 32.7 and 14.2 months,respectively.Those 25 without surgical management died 3 to 11 months after the development of GSC.Among the 76 cases with GSC,stomach was found as the original site of ulcer in 59(22.3%,59/259),and duodenal bulb in the other 17(3.9%,17/434)(P
7. Transapical aortic valve implantation with a new self-expanding valved stent in canines: An observation of short-term outcomes

Objective: To discuss the feasibility of using the off-pump antegrade transventricular route for aortic valved stent implantation in canines, and to observe the short-term outcomes. Methods: Fresh porcine pericardium was treated with 0.6% glutaraldehyde solution for 36 h; then it was trimmed and sutured into a valvular ring and fixed on a new self-expanding dumbbell-shaped nickel-titanium shape-memory alloy stent. The valved stents were then implanted off-pump in 8 canines. A limited or full sternotomy approach was used to access the apex of the heart. The crimped valve was introduced through a sheath in the left ventricular apex under ultrasound guidance. The function of valved stents was evaluated with electrocardiogram, echocardiography, computed tomography and DSA angiography early and 3 months after the procedure. Results: We successfully prepared the valved aortic stent. Five canines survived after implantation of the aortic valved stents. Angiographic and echocardiographic observation confirmed that the location and function of the stent were satisfactory, without influencing coronary blood flow and mitral valve function. CT examination showed no migration of the stent 3 months after the procedure, and there were no other prominent complications. Conclusion: Our new self-expanding valved stent can be deployed over the native aortic valves via transapical route, and the short-term outcome is satisfactory.
8. Preparation of valved aortic stent and in vitro implantation to aortic valve position through catheter

Objective: To evaluate the feasibility of implanting a new self-expanding valved stent for aortic valve implantation and its influence on coronary blood flow. Methods: We designed a self-expanding valved stent made from super-elastic memory nitinol alloy, with a tubular shape and a wide mesh in the cavate middle part. A valvular ring made of nitinol wire was sutured on the lower part. Fresh porcine pericardium was decellularized, treated with 0.6% glutaraldehyde solution for 36 h, trimmed into leaflets, and sutured by hand into the valvular ring. The valved stent was pulled into a 14-French sheath by a silk and positioned in the left ventricle of isolated pig heart via the ascending aorta, and then deployed over the native aortic valves by pulling back the outer sheath. Water was injected into the ascending aorta by a silicon tube to evaluate the competence of the prosthetic heart valves and its effect on coronary flow. Results: The prepared valved aortic stent could be stably positioned over the native valves and could be removed after deployment. The prosthetic heart valves showed a satisfactory function and had no influence on coronary flow and mitral valve motion. Conclusion: This self-expanding valved stent is well-designed and allows for aortic valve implantation over the native valves without interfering the coronary flow; it can be evaluated further in vivo.
9.Influence of thyroxine treatment on serum lipid levels in patients with subclinical hypothyroidism:a Meta-analysis

Objective:To examine whether the thyroxine treatment can affect the serum lipid levels in patients with subclinical hypothyroidism by Meta analysis.Methods:Pubmed and China National Knowledge Infrastructure(CNKI)were searched for randomized controlled trials reporting the effectiveness of thyroxine treatment on serum lipid levels in subclinical hypothyroidism patients from January 1980 to September 2006;hand-search was also performed.The included trails were analyzed by RevMan4.2 software.Results:Six studies were finally included and five of them found thyroxine to be significantly more effective than placebo in improving the serum lipid disorders;one of them found thyroxine had no obvious effect.The pooled effect sizes(95%CI)of total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),apoprotein A(ApoA),apoprotein B(ApoB)and lipoprotein a(Lp(a))were -0.39(-0.71, -0.07),-0.02(-0.07,0.04),-0.02(-0.06,0.03),-0.22(-0.31,-0.12),-0.08(-0.11,-0.06),-0.05(-0.08, -0.02),and -0.38(-2.41,1.64),respectively.The analysis results showed that thyroxine treatment was significantly effective on serum levels of TC,LDL-C,ApoA,and ApoB,but not on TG,HDL-C,or Lp(a).Conclusion:This Meta-analysis demonstrates that thyroxine treatment can improve thyroid function and lipid metabolism in patients with subclinical hypothyroidism,reducing the prevalence of artherosclerosis and coronary heart disease.
