1.Clinical Observation of Combined Epidural Analgesia with Morp hine

To investigate amethod which could prolong the efficient time of morphine and decrease side effect. Met hods:100 patients were randomly divided into four groups, receiving the different dispensation of analgesia. Analgesic solutions were given into epi dural space 510 minutes before operation was finished. Analgesic efficiency, efficient time, and side effects were recorded. esult: Sol utions diluted with hypertonia glucose injection might prolong the efficient time obviously (P<0.01). When combined with bupivacaine or scopolamine the sid e effects were decreased. Conclusion: Morphine combined wi th bupivacaine or scopolamine diluted with hypertonis glucose injection, used in epidural analgesia, can bring longer efficient time and less side effects.
2.Serum level of 25-hydroxyvitamin D and its impact on glucose metabolism and bone mineral density in type 2 diabetic patients

Objective To investigate the change of 25-hydroxyl vitamin D level and its impact on glucose metabolism and bone mineral density in type 2 diabetic patients.MethodsTwo hundred and fifty-eight cases of type 2 diabetic patients in our hospital were collected.In accordance with 25 (OH) D =50 nmol/L for the critical values,they were grouped into vitamin D deficiency group ( n =192) and the relative lack of vitamin D group ( n =66).Dual-energy X-ray absorptiometry (DXA) measurement of the lumbar vertebrae L2 - L4 and femoral neck bone mineral density were undertaking.Taking the total value of the lumbar spine,femoral neck and total value as judgment indicators,homeostasis model assessment of insulin resistance (HOMA-IR),we collected and statistically analyzed glucose metabolism and bone metabolism indicators in patients with diabetes.Results Among 258 cases of type 2 diabetes,there were 57 cases of osteoporosis accounting for 22.1%.There was significant difference on the duration of diabetes [ (7.98 ± 1.09 ) years old vs (3.77 ± 1.21 ) years old,t =4.849,P <0.05],FINS [ (6.42 ± 1.30) mU/L vs (5.79 ± 1.08) mU/L,t =3.871,P <0.05] and HOMA-IR [ (2.35 ±0.54) vs ( 1.85 ±0.41 ),t =2.705,P <0.05],but no significant difference on the FPG and HbA1c ( P > 0.05 ) between two groups.PTH [ ( 36.51 ± 7.59) ng/L vs ( 32.02 ± 6.89 ) ng/L,t =2.008,P < 0.05 ],bone mineral density in the lumbar total value [ (0.87 ±0.14) g/cm2 vs (0.99 ±0.12) g/cm2,t =2.799,P <0.05 ],femoral neck Neck [ ( 0.70 ± 0.10 ) g/cm2 vs ( 0.79 ± 0.11 ) g/cm2,t =2.564,P < 0.05 ] and Total [ (0.84 ± 0.14) g/cm2 vs (0.97 ± 0.15 ) g/cm2,t =3.340 P < 0.05 ] were statistically different,but no significant difference on the rest of the indicators.Conclusion The prevalence of vitamin D deficiency was found in patients with diabetes.Vitamin D level could affect insulin resistance and glycemic control and bone mineral density level.Therefore,routine vitamin D testing to the diabetic patients and giving vitamin D replacement therapy to vitamin D deficiency patients in a timely manner were necessary.
3.Risk factors of antidepressant treatment emergent suicidal ideas or attempted suicide among patients with bipolar depression

Objective To explore the risk factors of antidepressant treatment emergent suicidal ideas or attempted suicide among patients with bipolar depression. Methods Out of 623 patients with bipolar disorder visiting between June 2006 and June 2009,68 who was initially diagnosed as unipolar disorder and treated with antidepressants were retrospectively surveyed. Suicide and activation symptoms during the process of antidepressant treatment were estimated based on the cases record and semi-structural interviews. Unconditional bivariate logistic regression was conducted to analyze the potential risk factors. Results Of 68 patients,7 ( 10. 3% ) experienced antidepressant treatment emergent suicidal ideas and 4 ( 5.9% ) attempted suicide after 2 to 63 ( 17.45 ± 5.90 )days following the treatment. The antidepressant treatment emergent suicidal ideas or attempted suicide were related to the suicidal level before treatment (P= 0. 012, OR = 2. 072 ) and the combined use of benzodiazepine (P =0. 043, OR = 0.236). Clinical subtypes, gender, age, symptoms feature, types of antidepressant and antidepressanttreatment emergent activation symptoms had little impact on the antidepressant treatment emergent suicidal ideas orattempted suicide (P > 0.05 ). Conclusion The antidepressant treatment emergent suicidal ideas or attempted suicide among patients with bipolar depression are positively related to the suicidal level before treatment while negatively associated with the combined use of benzodiazepine.
4.Study Advances in Traditional Chinese Medicines Used in Diabetic Refractory Wounds

Refractory wounds are the common complication of diabetes,and the therapeutic scheme and methods are paid more and more attention nowadays. The influence of traditional Chinese medicines on diabetic wound healing, such as promoting fibroblasts proliferation, enhancing granulation tissue formation, increasing the secretion of growth factors and reducing the wound scar, was re-viewed in the paper to provide scientific basis and reference for traditional Chinese medicines used in diabetic refractory wounds.
5.Advances in influence of UV-B radiation on medicinal plant secondary metabolism.

Stratospheric ozone depletion results in an increased level of solar UV-B radiation (UV-B, 280-320 nm) reaching the earth surface. By the effect of UV-B radiation, various medicinal active ingredients changed because of the change of gene expression, enzyme activity and secondary metabolism, clinical effect is also changed. The research status of UV-B radiation and the accumulation of plant secondary metabolites in the past 10 years were summarized in this paper to supply reference for cultivation and exploitation of the medicinal plants.

6.Correlation between environmental factors and liposoluble and hydrophilic constituents of Polygalae Radix.

OBJECTIVETo analyze the correlation between environmental factors and the lipophilic and hydrophilic constituents of Polygalae Radix.
METHODThe contents of lipophilic constituent were determined by GC-MS and hydrophilic constituents by HPLC. Geographical factors were collected by on-site inspection and climate factors by the local meteorological data. The relationship between the content of the lipophilic and hydrophilic constituents and the factors were analyzed by SPSS 18.0.
RESULTThere was linear relationship between the content of lipophilic constituent and climate factors such as average temperature of July, average temperature of January. There was also linear relationship between hydrophilic constituents and climate factors such as annual average temperature, latitude, annual average rainfall.
CONCLUSIONThe main climate factors that affect liposoluble constituent content were average temperature of July, average temperature of January, and the main climate factors that affect hydrophilic constituent content were annual average temperature, latitude, annual average rainfall. The study would contribute to the quality evaluation and suitability of origin of Polygalae Radix.

7.Comparison of efficacies of surgeries via frontal keyhole approach assisted by endoscope and via temporal keyhole approach assisted by microscope in treatment of basal ganglia cerebral hemorrhage

Objective:To compare the clinical efficacy and safety of surgeries via frontal keyhole approach assisted by neuro-endoscope and via temporal keyhole approach assisted by microscope in cerebral basal ganglia hemorrhage. Methods:One hundred and five patients with basal ganglia cerebral hemorrhage admitted to our hospital from January 2017 to January 2020 were chosen in our study; 51 patients underwent surgeries via frontal keyhole approach assisted by neuro-endoscope (neuro-endoscopy group) and 54 patients underwent surgeries via temporal keyhole approach assisted by microscope (microscopy group). The clinical data of these patients were retrospectively analyzed; and the differences of hematoma clearance rate, intraoperative blood loss, duration of surgery, length of hospital stays, Glasgow Coma Scale (GCS) scores one week after surgery, incidence of postoperative complications, and activity of daily living (ADL) scores 6 months after surgery were compared between the 2 groups. Results:There were no significant differences in hematoma clearance rate and length of hospital stays between the 2 groups ( P>0.05). As compared with the microscopy group, the neuro-endoscopy group had significantly lower intraoperative blood loss, significantly shorter duration of surgery, and statistically higher GCS scores one week after surgery ( P<0.05). There were no significant differences in incidence of postoperative complications and ADL scores 6 months after surgery between 2 groups ( P>0.05). Conclusion:Both surgeries via frontal keyhole approach assisted by neuro-endoscope and via temporal keyhole approach assisted by microscope can effectively clear the intracranial hematoma in patients with cerebral hemorrhage in the basal ganglia and protect neurological function; however, surgeries via frontal keyhole approach assisted by neuro-endoscope has advantages of shorter duration of surgery and lower intraoperative blood loss, and earlier neurological function recovery.
