1.Dynamic assay of serum inhibin B and estradiol concentrations obtained after gonadotrophin therapy as predictors of ovarian response in vitro fertilization cycle

Objective To study prospectively the values of basal and dynamic measurement of inhibin B(INHB), estrodiol (E_2)following gonadotrophin therapy in vitro fertilization(IVF)cycles in predicting ovarian response to stimulation in young women. Methods A total of 57 patients younger than 40 years of age undergoing their first cycle of long protocol IVF treatment were studied and serum INHB, E_2 levels were detected on day 3 of the menstrual cycle, before the first administration of gonadotrophin (day 0), on days 1 and 5 after gonadotrophin therapy(day 1, day 5 ). Ovarian response was presented by: number of oocytes/total rFSH dose and square root of (number of follicles/total rFSH dose). According to number of follicles, number of oocytes and presence or absence of ovarian hyperstimulation syndrome(OHSS), we categorized the results into three different groups: poor response, normal response and OHSS. Results (1) Hormone levels on days 1,5 of rFSH stimulation: There were positive correlation between INHB (r_S:0.69-0.73), E_2 (r_S:0.60-0.73) and ovarian response (P=0.000). Objective To control study the treatment effect of functional dyspepsia of stagnation of liver-qi type treated by liver-coursing depression-resolving decoction added psychological counseling.Methods130 patients with functional dyspepsia of liver-qi stagnation type who were treated in the inpatients and outpatients in our hospital from January 2015 to January 2016 were selected, to randomLy grouping.The 65 cases of the control group was given the conventional therapy, and the another 65 cases of the treatment group was given liver-coursing depression-resolving decoction added psychological counseling at the the same term based on the control.After the continue treatment for 4 weeks, to compared the clinical curative effect of them.ResultsThe treatment effective rate of the treatment group was 94%, he treatment effective rate of the control group was 78%, there was statistical significance (P<0.05).To compared with the grade of traditional Chinese medicine symptom (distending pain in the stomach, satiation, belching, inhibited defecation) and discovered that the grades of them were lower after treatment, and there has the significant improvements that compared with that before the treatment, there was statistical significance (P<0.05), and the treatment group was significantly lower than the control group, there was statistical significance (P<0.05).The Hp Infection and the bile regurgitation of them after treatment was significantly improved, the treatment group was significantly better than the control group, there was statistical significance (P<0.05), the treatment group was significantly better than the control, there was statistical significance (P<0.05).To compared the 71% of the negative conversion ratio of Hp urease detection of the treatment group that was significantly greater than the 40% of the control group after detection, there was statistical significance (P<0.05).The gastric emptying time of them was significant improved after treatment, and compared with that before the treatment, there was statistical significance (P<0.05).For the gastric emptying time, to compared with the control group, the improvement effect of the treatment group was outstanding, there was statistical significance (P<0.05).After treatment, the grade of anxiety and depression was significantly decreased of them, and to compared with the control group after treatment, the grade of anxiety and depression of the treatment group was significantly improved, there was statistical significance (P<0.05).There was no obvious untoward effects of them, there was no statistical significance.ConclusionThat has an exact effect of functional dyspepsia of stagnation of liver-qi type treated by liver-coursing depression-resolving decoction added psychological counseling, to provide an effective way for the clinic treatment of the functional dyspepsia (stagnation of liver-qi type).Meanwhile, to provide the new theoretical foundation for the traditional chinese medicine therapy of functional dyspepsia.
3.Analysis of impact factors for 273 medical dispute cases mediated with the people's mediation mechanism in Guangdong province

Objective To analyze impact factors for resolving cases of medical disputes with people's mediation mechanism,for improvement of this mechanism.Methods This paper picked 273 cases mediated by Guangdong Medical Dispute People Meditation Committee from June 2010 to December 2011,for analysis from doctor-factors,patient-factors,mediator-factors and composite factors.Results Under such mechanism,doctor-factor has not significant difference on the mediation success ratio and mediation timeliness of the cases; the success rates of cases of death,disability,and no obvious impairment suffered by patients are 68.57%,55.81% and 76.80% respectively.Such results are found by chi-square test as P<0.05,significant statistically.With cases reported via hospitals,the mediation timeliness is much better than other cases.Conclusion The mechanism is advantageous in effectively avoiding influences of case factors,yet emphasis should be placed on mediation of disability cases,and doctors should be encouraged to report the cases,contributing to higher success rate of mediation.
4.Upper airway forms of the adults with classⅡsagittal facial type

Objective:To analyze the characteristics and differences of upper airway forms in the adults with class Ⅱ sagittal facial type of different vertical facial types and investigate the correlation between craniofacial structures and upper airway forms.Methods:Cephalograms of 135 adults with classⅡsagittal facial type of different vertical facial types were collected.60 cases were males and 75 cases were females.All the cephalograms were measured in sagittal upper airway depths.The differences were compared between genders,and also among different vertical facial type groups.Results:Among different vertical facial type groups,only the sagittal depths of nasopharynx(PNS-Ba、PNS-R)of the males and PNS-Ba of the females in high angle groups were smaller than that in average and low angle groups(P0.05),but the values of V-LPW of the males were larger than that of the famales in all vertical facial type groups(P
5.Effects of microscrew implant anchorage in extraction space closing by one step and retraction of anterior teeth

Objective: To investigate the effects and mechanism of microscrew implant anchorage (MIA) in speeding up the closure of extraction space by one step and retraction of anterior teeth. Methods: Twenty-three patients with teeth extraction and required strong posterior anchorage were treated with MIA to close the extraction space. The extraction spaces were closed by two methods respectively. The extraction spaces were closed by the method of one step and retraction of anterior teeth(Ni-Ti spring from MIA to the hook on the wire combined with the elastic pull from MIA to cuspid) on the experiment sides and by the traditional one step method(Ni-Ti spring from MIA to the hook on the wire alone) on the control sides. Which side treated as trial side or control side was selected at random. The period of the space closing, the way and position of anterior teeth movement were observed on each side and the period of the space closing of two sides were tested by paired t test. Results: ①The average period of the spaces closing which was (6.69±1.07) months on the trial side and (9.56±1.19) months on the control side was significantly different (P<0.05).② After the closure of extraction spaces, the cuspid achived better position on the trial side than the contral side on which cuspid presented distal-lingual rotation. Conclusion: The extraction spaces can be closed in all cases by two methods respectively. Using MIA to close the extraction space by one step and retraction of anterior teeth can not only shorten the period of space closing but also achieve nice outcome by moving the cuspid in spongy bone.

9 adults aged 23 to 43 years old were treated with bidimensional self-ligating lingual bracket technique for upper teeth and labial ceramic or aesthetic bracket for lower teeth.After treatment the labioclination of anterior teeth in Angle's II patients decreased from 28.12°± 10.05°to 19.25°±3.12°(P <0.05),in Angle's III patients from 32.14°±4.71°to 28.15°±3.26(P <0.05).

Objective To investigate the expression pattern and significance of two importantoocyte-secreted factors:growth differentiation factor 9(GDF9)and bone morphogenetic protein 15(BMP15)during oocyte maturation in women with polycystic ovary syndrome(PCOS)and infertile women due to husband factors.Methods Total of 25 oocytes[9 at germinal vesicle GV stage,9 at M Ⅰ stage and 7 at M Ⅱ stage]were obtained from 12 patients with PCOS and 82 oocytes(29 at GV stage,26 at M Ⅰ stage and 27 at M Ⅱ stage)were from 56 controls.The nested quantitative real time(RT)PCR was uscd to detect the abundance of GDF9 and BMP15 mRNA in each oocyte.Results(1)The expression level of GDF9 mRNA at the GV stage,M Ⅰ stage and M Ⅱ stage in PCOS group were 44.8(4.2-529.0),27.6(9.8-172.7)and 49.0(0.2-65.9)respectively,the expression in were 149.9(55.4-387.9),29.9(2.5-205.8)and 657.8(149.4-1376.2)in control group,respectively.The expression of GDF9 mRNA at M Ⅱ stage was significantly lower in PCOS group than in controls(P < 0.01),however,the differences didn't reach statistical significant at GV or M Ⅰ stage between the two groups(P > 0.05).The expression of GDF9 mRNA displayed some changes at different maturation stage in controls(P < 0.05,P < 0.01),however,the expression didn't demonstrate any dynamic changes in PCOS group(P > 0.05).(2)The expression level BMP15 mRNA at the GV stage,M Ⅰ stage and M Ⅱ stage in PCOS group were 0.1(0.1-22.0),3.2(0.6-55.0)and 6.4(3.2-8.5),respectively,the expression were 41.6(6.5-96.1),4.0(2.0-10.4)and 49.7(2.3-139.5)in control group,respectively.The expression of BMP15 mRNA at GV stage was significantly lower in PCOS group than in controls(P < 0.01),however,the differences were not significant at M Ⅰ or M Ⅱ stage between the two groups(P >0.05).The expression of BMP15 mRNA also displayed some changes at different maturation stage in controls(P < 0.05),however,the level didn't demonstrate any dynamic changes in PCOS group(P > 0.05).Conclusion It was suggested that the low expression of oocyte secreted factors in mature oocytes from PCOS patients might be associated with impaired oocyte quality and developmental competence in PCOS.
8.A study of Bolton tooth-size discrepancies of malocclusion patients.

OBJECTIVETo analysis the sum and frequencies of Bolton tooth-size discrepancies Angle Class I, II, III malocclusion patients.
METHODSMeasured each tooth crown mesial-distal size between the first molar of 439 dental plaster casts of malocclusion patients and obtained the sums of six anterior tooth-size and twelve tooth-size between the first molar of upper or lower and calculated the anterior and total tooth-size discrepancies by Bolton rate standards, then statistic analyses were done.
RESULTSPatients whose sums of anterior tooth-size discrepancy were pass 1.5 mm or less than -1.5 mm was 14.02% in Angle Class I malocclusion patients and 9.49% in Class II and 19.32% in Class III. Patients whose sums of total tooth-size discrepancy were pass 1.5 mm or less than -1.5 mm was 19.63% in Angle Class I malocclusion patients and 15.33% in Class II and 20.45% in Class III. The upper sum of anterior tooth-size of Class I malocclusion patients whose sums of anterior tooth-size discrepancies were pass 1.5 mm or less than -1.5 mm was always smaller than normal and the lower sum was always larger. The sums of anterior tooth-size discrepancy of Class I, II, III and total tooth-size discrepancy of Class I were always between 1.5 mm-2.5 mm or -2.5 mm(-)-1.5 mm. Patients whose sums of anterior tooth-size discrepancy were pass 3.5 mm or less than -3.5 mm was 2.34% in Class I malocclusion patients and 0 in class II and 0 in class III. Patients whose sums of total tooth-size discrepancy were pass 3.5 mm or less than -3.5 mm was 4.21% in Class I and 1.46% in Class II and 4.54% in Class III.
CONCLUSIONTooth-size discrepancy of malocclusion patients was not the general cause of malocclusion.

9.Factors associated with the efficacy of cardiac resynchronization therapy in patients with chronic heart failure.

OBJECTIVETo observe the clinical effects of cardiac resynchronization therapy (CRT) in patients with chronic heart failure, and compare the clinical characteristics and outcome between responders and non-responders to define factors related to the efficacy of CRT.
METHODSWe retrospectively analyzed the data of patients underwent CRT-P/D implantation from January 2006 to December 2012 in our Hospital. All patients received long-term follow-up including NYHA classification, left ventricular ejection fraction (LVEF) and left ventricular internal dimension at end diastole (LVIDd).
RESULTSA total of 204 patients were included (130 males, mean age (64.8 ± 11.9) years). The total response rate of CRT was 61.3%. Women, QRS duration ≥ 150 ms, and left bundle branch block (LBBB) were related with better response after CRT (all P < 0.05). Multivariate regression analysis showed that QRS duration was an independent determinant for CRT response. All-cause mortality rate was significantly lower in responder group than in non-responder group (P < 0.001).
CONCLUSIONSIn patients with chronic heart failure, women, QRS duration ≥ 150 ms, and LBBB are related with better CRT response rate post CRT. QRS duration ≥ 150 ms is an independent predictor of CRT response, and positive response is associated with lower all-cause mortality in this patient cohort.

10.Complications and prognosis of urgent-start peritoneal dialysis and urgent-start hemodialysis in end-stage renal disease patients

Objective To compare the complications and outcomes of urgent?start peritoneal dialysis (PD) and hemodialysis (HD) in end?stage renal disease (ESRD) patients, and explore the safety and effectiveness of PD which was as an urgent?start dialysis modality in ESRD patients. Methods All patients for urgent?start dialysis, who initiated dialysis without a long?term dialysis access or had the long?term dialysis access under 30 days in Renji Hospital from January 1st 2013 to December 31st 2014, were enrolled. According to the dialysis modalities, patients were divided into PD group and HD group. Participants were followed up until death, transferred to other centers, lost of follow up or January 1st 2016. Dialysis?related complications within 30 days of implantation, complications of reimplantation and the occurrence of bacteremia between two groups were compared, and their survival rates were tested by Kaplan?Meier curves. Results Among 178 patients in this study, there were 96 (53.9%) patients in PD group and 82 (46.1%) patients in HD group. Compared with those of HD group, patients of PD group presented more cardiovascular disease [21(21.9%) vs 8(9.8%), P=0.029], higher serum potassium [(4.5±0.8) mmol/L vs (4.3±0.8) mmol/L, P=0.038], but less heart failure (NYHA Ⅲ?Ⅳ) [26(30.2%) vs 40 (48.8%), P=0.014], lower brain natriuretic peptide (BNP) [328.5 (129.5, 776.8) ng/L vs 503.5(206.0, 1430.0) ng/L, P=0.008], higher hemoglobin [(81.5 ± 17.7) g/L vs (75.3 ± 22.5) g/L, P=0.039], higher serum albumin (33.5±5.7) g/L vs (31.3±6.7) g/L, P=0.022] and higher serum pre?albumin (304.5±78.0) mg/L vs (257.0 ± 86.1) mg/L, P<0.001]. PD group presented less dialysis?related complications [5 (5.2%) vs 20(24.4%), P<0.001], less dialysis?related complications requiring reimplantation [1(1.0%) vs 20(24.4%), P<0.001] and less bacteraemia [3(3.1%) vs 11(13.4%), P=0.011]. The 3?, 6?and 12?month patient survival rates of PD and HD group were 97.9% vs 98.4%, 97.9% vs 98.4%, and 92.1%vs 93.0% respectively, and no significant difference was found (Log ? rank=0.004, P=0.947). Conclusions Patients with urgent?start PD have less complications within 30 days of implantation and occurrence of bacteremia than patients with urgent?start HD, and the same survival rates. PD may be a feasible and safe urgent?start dialysis modality for ESRD patients.
