OBJECTIVE:To systematically review the efficacy and safety of nedaplatin or cisplatin combined with paclitaxel in the treatment of advanced non-small cell lung cancer(NSCLC)and esophageal cancer. METHOD:Retrieved from PubMed,CJFD and Wanfang Database, randomized controlled trials(RCT)about nedaplatin combined with paclitaxel(TN)or cisplatin combined with paclitaxel (TP) in the treatment of advanced NSCLC and esophageal cancer were collected. Meta-analysis was performed by using Rev Man 5.3 software after data extract and quality evaluation. RESULTS:Totally 7 RCTs were included, involving 505 pa-tients. Results of Meta-analysis showed, there were no significant differences in the short-term efficacy [NSCLC:OR=1.08,95%CI (0.66,1.75),P=0.76;esophageal cancer:OR=1.44,95%CI(0.68,3.06),P=0.34],1-year survival rate [NSCLC:OR=1.21, 95%CI(0.60,2.44),P=0.59],2-year survival rate [NSCLC:OR=1.01,95%CI(0.38,2.68),P=0.99],the incidence ofⅢ/Ⅳleuko-penia [esophageal cancer:OR=1.36,95%CI(0.62,2.96),P=0.44],incidence ofⅢ/Ⅳthrombocytopenia [NSCLC:OR=1.37,95%CI(0.64,2.92),P=0.42;esophageal cancer:OR=0.97,95%CI(0.30,3.18),P=0.96] and incidence of Ⅲ/Ⅳ neutropenia [NSCLC:OR=1.38,95%CI(0.70,2.74),P=0.35]. Compared with TP,TN can significantly reduce the incidences of nausea and vomiting [NSCLC:OR=0.34,95%CI(0.20,0.60),P<0.001;esophageal cancer:OR=0.16,95%CI(0.07,0.35),P<0.001] and nephrotox-icity [esophageal cancer:OR=0.10,95%CI(0.02,0.55),P=0.009]. CONCLUSIONS:Both TN and TP show good efficacy in the treatment of NSCLC,but TN has lower incidences of astrointestinal reactions and nephrotoxicity.

AIM: To study p42/p44 mitogen - activated protein kinases ( MAPK ) signal transduction pathway effect on vascular endothelial growth factor ( VEGF ) expression induced by elevated glucose concentration in cultured human retinal pigment epithelium ( hRPE) .
METHODS:hRPE cells were cultured and divided into four groups:normal glucose group (NG) (5. 6mmol/L), high glucose group ( HG1:15mmol/L D-glucose, HG2:20mmol/L D - glucose, HG3:30mmol/L D - glucose ), PD98059 group: hRPE cells were treated by an efficient and selective inhibitor PD98059 (20μmol/L) of p42/p44MAPK signal transduction pathway and solvent dimethyl sulfoxide group ( DMSO group) . The expression of VEGF and pigment epithelium derived factor ( PEDF ) mRNA was detected by RT-PCR. VEGF protein expression in cultured hRPE supernatants was detected by enzyme-linked immumosorbent assay ( ELISA) .
RUSULTS: VEGF mRNA and protein expression induced by elevated glucose concentration increased significantly. VEGF mRNA and protein expression were restrained in PD98059 group. Ratio of ( VEGF/β-actine)/( PEDF/β - actine ) in PD98059 group decreased significantly compare with that in high glucose group.
CONCLUSION: p42/p44MAPK signal transduction pathway might play a part in VEGF expression induced by elevated glucose concentration in cultured hRPE cells.
9.NT-pro-BNP in the evaluation of daunorubicin-indued cardiotoxicity in acute childhood leukemia.
Chinese Journal of Hematology 2010;31(9):621-623
OBJECTIVETo evaluate the sensitivity of NT-pro-BNP in daunorubicin (DNR) induced myocardial damage by monitoring the level of NT-pro-BNP and myocardial enzymes (CK, CKMB) before and after DNR treatment in childhood acute leukemia (AL) and performing control study.
METHODSSixty-two cases (total 194 samples) which diagnosed as primary AL were enrolled and had received the conventional chemotherapy. According to the cumulative dose of DNR, they were divided into three groups: cumulative dose ≤ 60 mg/m(2) (group A); cumulative dose 60 - 120 mg/m(2) (group B); cumulative dose > 120 mg/m(2) (group C) and 15 cases with idarubicin (IDA) or mitoxantrone (MXR) as altervative to DNR (group D).
RESULTSThere was a significant difference (P = 0.000) in the level of NT-pro-BNP before and after DNR therapy, but did not in the myocardial enzymes activities (CK, P = 0.085 and CKMB, P = 0.076). The level of NT-pro-BNP appeared obviously elevated (P = 0.001) when DNR cumulative dose > 60 mg/m(2). While the level of CKMB did (P = 0.022) until DNR cumulative dose > 120 mg/m(2). In the 15 cases used IDA or MXR as alternative to DNR, the level of NT-pro-BNP fall from (239.9 ± 230.0) ng/L to (137.0 ± 131.9) ng/L (P = 0.024).
CONCLUSION(1) Compared with myocardial enzymes detection, NT-pro-BNP level can predict earlier DNR-induced cardiotoxicity. (2) Selection of the second or third generation anthracycline to treat AL can significantly reduce the cardiotoxicity in children.

Objective ① To investigate the level of the vitamin D endocrine system in peripheral relationships with bone mineral density (BMD) and the disease activity respectively. Methods The level of the 25-hydroxylate vitamin D3 (25OHD3) and 1,25-dihydroxyvitamin D3 [1,25(OH)D3] in plasma from 43 SLE patients and 44 normal controls were detected by enzyme linked immunosorbent assay. Vitamin D receptor (VDR) gene expression was determinied by real-time PCR in peripheral blood. BMD measurements in the lumbar spine (L1-4) and left proximal femur (femoral neck) were performed using dual X-ray absorptiometry before treatment. The relationship between the vitamin D endocrine system and the bone mass were studied. We also discussed the relationship between the vitamin D endocrine system and the disease activity. Results The levels of 25OHD3 and 1,25 (OH)2D3 were lower in the initial SLE patients than normal controls (P<0.01, P<0.01). The expressions of VDR gene were significantly increased in initial SLE compared with normal controls (P<0.01). The initial SLE patients had significantly lower BMD values, and higher frequency of osteopenia (35%) at both sites of measurement compared with matched healthy controls (P<0.01). The initial SLE patients were divided into two groups by BMD, abnormal group and normal group. There were no differences in 25OHD3, 1,25 (OH)D3 and VDR gene expression (P0.05). There was no correlation between the vitamin D endocrine system and BMD in initial SLE patients. There was no correlation between the vitamin D endocrine system and the disease activity either. Conclusion Vitamin D endocrine system may play an important role in SLE, but the level of VDR gene is not correlated with BMD and disease activity.
