1.Technical refinements and clinical application of Studer neobladder

Objective To evaluate the clinical value of modified Studer ileal neobladder. Methods Technical refinements on Studer ileal neobladder have been divised and applied to 35 patients of which 31 with multiple or invasive transitional cell carcinoma of bladder,2 advanced stage of neurogenic bladder and 2 extensive polypoid cystitis glandularis.All patients were male,with an average age of 61 years. Results Mean operative time was 6.5 hours (range 5.5~8.5 hours),mean intraoperative blood loss was 650 ml(range 300~1 200 ml),and transfusion was needed in 21 case.Perioperative complications included stress ulcer in 6,ARDS in 1.Follow-up ranged from 3 to 38 month.All the patients voided without difficulty,only 5 patients had small amount of residual urine (40~60 ml).The day time urinary continent rates at 3 months and 6 months postoperatively day time were 91% and 100% and the night time 87% and 96%,respectively.The upper urinary tract function was excellent.Mean capacity of the ileal neobladder was 410 ml,maximal filling pressure 21.4 cmH 2O,and Qmax 16.8 ml/s in 6 months postoperatively. Conclusions Modified Studer ileal neobladder has advantages of technically simple,lower complication rate,shorter recovery time and similar to physiological voiding.It is recommended to be widely used in selective candidates.
2.Clinical efficacy and safety of intracoronary verapamil during percutaneous coronary intervention in patients with acute coronary syndromes

Objective To examine the clinical efficacy and safety of intracoronary verapamil during percutaneous coronary intervention(PCI) in patients with acute coronary syndromes (ACS).Methods Totally 98 patients with ACS undergoing PCI were randomly assigned to two groups:verapamil group (immediately intracoronary verapamil 200 μg/2 ml heparinised saline,n=48) and intracoronary saline control group (immediately intracoronary 2 ml heparinised saline,n=50) after deploying stent.The follow up time was 3 months.Thrombolysis in myocardial infarction (TIMI)flow grade (TFG), corrected TIMI frame count (CTFC),TIMI myocardial perfusion grade (TMPG), myocardial blush grade (MBG) were assessed pre- and post-PCI and after drug administration.Echocardiography were performed one week after PCI. Incidence of major adverse cardiac events in hospital and 3 months follow-up were compared between the two groups. Results The differences in values of CTFC,TFG,TMPG,MBG after PCI were not found between two groups (P>0.05). However,after intracoronary drug administration,verapamil group was superior to control group in terms of CTFC (t=6.47,P<0.01),TFG (x2=5.17,P<0.01),TMPG(x28.25,P<0.01)and MBG(x2=2.12,P<0.05).After correcting the influencing factors,only CTFC was still improved in verapamil group than in control group (x2 =2.36,P<0.05).There were no significant differences between the two groups in TFG(x2 =0.58,P>0.05)and MBG(x2 =0.91,P>0.05) and TMPG (x2 =0.68,P>0.05).Echocardiographic results after PCI were similar between two groups (x2 =0.65,P>0.05).There was no difference in major adverse cardiac events between two groups (x2 =0.71,P > 0.05 ). Conclusions Application of intracoronary verapamil after deploying stent is effective,safety and worthy of popularization in view of improving post procedural coronary flow in patients with ACS.
3.Building and operation of the pharmacovigilance system in the hospital

Objective Effective intervention on the setbacks and safety loopholes of clinical medication, in an effort to explore, establish and improve the ADR watchfulness system in the hospital step by step. MethodsCollection and compilation of ADRs and drug compatibility cases found in 2010 in the hospital. Intervention on drugs of safety risk exposure or medication factors by means of the ADR prewarning mechanism, medical rounds by hospital's directors regarding drug issues, communication between pharmacists and clinicians, ADR monitoring leadership team, and pharmaceutical affairs council. Results 143 cases of ADR were interfered with among all the 393 ADR cases reported by clinical departments in 2010, accounting for 36.4% of all the reported cases. ConclusionHospitals need to pay more attention to the monitoring, analyzing and evaluation of ADRs, and take efficient intervention means to make drug use more rational and ensure safe medication for the people.
4.Microglia and Parkinson's disease

BACKGROUND: Because dopaminergic neurons are particularly sensitive to oxidative stress, microglia is characteristics of being prone to activation, and activated microglia is the main source of oxygen free radical production, so microglia activation is more important in the pathogenesis of Parkinson's disease and illness progress.OBJECTIVE: To summarize the correlation between microglia and Parkinson's disease.METHODS: An online computer-based retrieval was performed by the first author among the Chinese Journal Full-Text Database (CNKI: 2000/2010) and Medline (2000/2010) Database, with key words of Parkinson's disease, microglia in English and Chinese. The correlation between microglia and Parkinson's disease was discussed through two aspects, one is the influence of cell factors and toxic substance produced after microglia activation on Parkinson's disease, the other is the inhibition of microglia and prevention of nerve toxic factors on Parkinson's disease progress.RESULTS AND CONCLUSION: A total of 112 articles were screened out according to inclusion and exclusion criteria, and 27 of them were involved in the analysis. Results showed that microglia activation will damage dopaminergic neurons, and cause Parkinson's disease. And the occurrence and development of Parkinson's disease may further reduce the neurotransmitter dopamine, continue to damage dopaminergic neurons and release the inflammatory factor, thus promoting microglia activation. Inhibition of microglia activation is likely to stop the progress of Parkinson's disease.
5.Considerations of curriculum arrangement for professional English education for pharmacog-nosy-major graduate students

Pharmacognosy (Chinese Medicine) discipline has a long history and International cooperation is more and more frequent, but the communication of the Chinese traditional medicine cul-ture is very difficult. In order to solve this problem, it is urgent to improve the professional English level of the students. Thus, combined with the education survey and interview results, as well as the reports in previous studies and our own experience, we think about how to set pharmaceutical profes-sional English course and put forward some suggestions such as the innovation of curriculum organiza-tion strategy, convergence of basic English course, teaching content and form being closely linked to academic activities, and paying attention to highlighting the subject characteristics of pharmacognosy (Pharmacy).
6.Hospitalized eye injury in a Chinese urban population: a retrospective analysis

AIM: To present the epidemiology, cause of injury, ocular status and final visual acuity after management of severe ocular trauma required hospitalization during 7 years in a representative urban Chinese population.METHODS: A retrospective analysis of the hospital admission files of ocular trauma patients admitted to the Daping hospital from January 2000 to December 2006 was carried out.RESULTS: A total of 268 patients were open-globe injury and the remaining 294 patients were closed-globe types. The most common causes of ocular injuries were metal (29.4%), explosive (14.6%) and stone (13.9%). And the visual outcomes of most of eye injury patients in this study were poor; half of injured eyes ended with visual acuity worse than 0.1.CONCLUSION: Therapeutic methods to ocular trauma make a great progress in recent years, but the visual outcomes are poor.
7.CK20 expression in exfoliated urothelial cells of bladder cancer

Objective To study whether CK20 expression could be served as a noninvasive test for detecting and monitoring bladder cancer. Methods Reverse transcriptase polymerase chain reaction (RT PCR) was used to determine the expression of CK20 in cells collected from the urine of 10 healthy volunteers,41 patients with bladder cancer and in 20 hematuria patients with bladder cancer excluded. Results CK20 amplification band (370bp) was obtained with mRNA extracted from TCC cells of bladder cancer. Of 41 patients with bladder cancer, 37 specimen were positive.No false positive cases were found in the 10 healthy controls and only in 3 of the 20 hematuric cases.Of the 3 false positive, 2 had pelvic cancer and 1 chronic inflammation. CK20 was positive in 23 of the 25 G 1 patients,in 8 of the 9 G 2 patients,and in 6 of the 7 G 3 patients. Conclusions The preliminary results showed that CK20 might be a potential biomarker for the noninvasive detection of bladder cancer by means of assaying urothelial cells from the voided urine specimen with RT PCR.
8.Preconditioning with desflurane, sevoflurane and isoflurane increase the preservation of adenosine triphosphate in anoxia-reoxygenation myocardial cells

Aim To study the effects of preconditioning with desflurane, sevoflurane and isoflurane on adenosine triphosphate (ATP) in anoxia-reoxygenation myocardial cells. Methods Rat ventricular myocytes, cultured for 4~5 days, were randomly allocated to five groups: Control group, anoxia-reoxygenation group and groups preconditioned with 1.5 MAC desflurane, sevoflurane or isoflurane following anoxia-reoxygenation. The content of intracellular ATP ,the activities of lactic dehydrogenase(LDH) and creatine kinase(CK), and the cell viability were measured at the end of experiment.Results Preconditioning with 1.5 MAC desfllurane, sevoflurane or isoflurane significantly attenuated the great reduction in ATP and cell viability and the increase of LDH and CK caused by anoxia-reoxygenation. 10.Effects ofXiaotan Sanjie Decoction on Ultramicrostructure in MKN-45 Human Gastric Cancer Orthotopic Xenograft

Objective To observe the effects ofXiaotan Sanjie Decoction on ultramicrostructure in MKN-45 human gastric cancer orthotopic xenograft;To discuss its mechanism for gastric cancer. Methods MKN-45 human gastric cancer cell line was used to establish subcutaneous tumor model in nude mice as an experimental model which passed three generations of tumor source. Then OB glue was used to establish human gastric cancer orthotopic xenograft model. 40 model nude mice were divided into control group, model group (received gavage with saline),Xiaotan Sanjie Decoction group (received garage withXiaotan Sanjie Decoction) and Tegafur group (received gavage with Tegafur), 10 mice in each group. Mice in the control group were under normal diet, without medicine intervention. After 6 weeks of treatment intervention, tumor weight was detected, inhibitory rate was calculated, the ultrastructural changes of gastric cancer cells were detected through transmission electron microscopy.Results The tumor weight inXiaotan Sanjie Decoction group was significantly lower than that in control group and model group (P<0.05,P<0.01). The tumor inhibition rate was 46.2%. Under the electron microscope, MKN-45 gastric cancer cells strain changed to typical apoptosis.ConclusionXiaotan Sanjie Decoction can lead to apoptosis effects on MKN-45 human gastric cancer orthotopic xenografts. All evidences indicate that inducing apoptosis may be one of the most important mechanisms ofXiaotan Sanjie Decoction in treating gastric cancer.
