2.Expression of neutrophil extracellular trap and B lymphocyte in renal tissue of patients with lupus nephritis and its significance

Objective To investigate the association of expressions of neutrophil extracellular trap (NET) and B lymphocyte in renal tissue with lupus activity in patients with lupus nephritis (LN). Methods Immunohistochemistry was.used to detect the expression of NET (citrullinated histone H3 as the marker) and the infiltration of B lymphocyte (CD19 as the marker)in renal specimens of three groups [LN group,n=20; minimal change disease (MCD) group,n=8;healthy control group,n=3].The chronic index and SLE-disease activity index (SLE-DAI) in renal tissues of LN and their correlations with NET and B lymphocytes were examined. Results The expression of NET was not found in the renal tissues of healthy control group and MCD group,while it increased significantly in LN group,especially in glomeruli with moderate and severe mesangial cells proliferation, cellular crescents, and tubulointerstitium with inflammatory cell infiltration.Compared with other types of glomeruli,the expression of NET was significantly upregulated in glomeruli with moderate and severe mesangial cell proliferation (P<0.01).In the glomeruli with moderate and severe glomerular mesangial cell proliferation,the mean number of =NET positive cells was positively correlated with renal pathological active index (r=0.620,P=0.004),the score of SLE-DAI (r=0.492,P=0.027) and the mean number of NET positive tubular cells (r=0.558,P=0.011).In renal interstitium,the NET positive cells were positively correlated with CD19 positive B lymphocytes (r=0.573,P=0.008) and renal pathological chronic index (r=0.645,P=0.002). Conclusion NET is widely expressed in the renal tissues of lupus nephritis,which may play a role in the active damage of glomeruli.
3.Effect of glutamine on heat shock protein-70 and tumor necrosis factor-? expession in endotoxemic rats

AIM: To study the protective effect of glutamine (Gln) against endotoxemia by observing the effect of glutamine on heat shock proteins (HSPs) and tumor necrosis factor-? (TNF-?) in endotoxemic rats. METHODS: The rats were randomly divided into 3 groups, lipopolysaccharide group (LPS), glutamine-treated group (Gln) and control group (C). The blood was drawn from lateral tail vein for analysis of cytokine levels at 0, 2, 4 and 6 h post-lipopolysaccharide (LPS) challenge. TNF-? was measured by radioimmunity assay. Multiple tissues were harvested from the rats, and HSP70 was detected by immunohistochemistry. At the same time, lung, liver, and ileum tissue section were stained with hematoxylin and eosin. 4.Clinical study of femtosecond laser in vitrectomy combined with phacoemulsification and intraocular lens implantation

AIM: To analyze the clinical value of femtosecond laser in vitrectomy combined with phacoemulsification and intraocular lens (IOL) implantation.
METHODS: We selected 100 patients ( 102 eyes ) undergoing vitrectomy combined with phacoemulsification and IOL implantation in our hospital from October 31 2013 to October 31 2015. They were divided into routine operation group, 50 cases (52 eyes), and femtosecond laser group, 50 cases (50 eyes). The parameters were compared between the two groups, including total operation time, postoperative complications ( elevated intraocular pressure, uveitis, vitreous hemorrhage, and retinal detachment ), postoperative intraocular pressure and postoperative half of the visual acuity ( near visual acuity, visual acuity, uncorrected visual acuity, corrected visual acuity).
RESULTS: The difference on total operation time of the two groups was statistically significant ( P < 0. 05). The differences between the two groups on postoperative intraocular pressure, uveitis, vitreous hemorrhage, retinal detachment were not statistically significant ( P >0. 05). There were no significant differences on internal pressure, near visual acuity, far visual acuity, uncorrected visual acuity, corrected visual acuity at 6mo after surgery (P>0. 05).
CONCLUSION: Femtosecond laser in vitrectomy combined with phacoemulsification and IOL implantation has a high safety and efficacy with the advantages as accurate, efficient.
5.Short-term and long-term outcomes of moxifloxacin treatment in acute exacerbations of COPD

Objective To investigate the aetiology of acute exacerbations of COPD (AECOPD),and the effects of moxifloxacin in the treatment of AECOPD.Methods Patients with stable COPD based on GOLD criteria were included in the study.Sputum collected at first exacerbation was analyzed for bacteria count and culture.IL-6,IL-8 and TNF-?were measured by enzyme- linked immunosorbent assay (ELISA).Eligible patients were randomized to receive moxifloxacin (400 mg qd for 5 days) or ce- faclor (250 mg q8h for 7 days).Efficacy parameters were evaluated at 7 and 14 days after treatment initiation and 1 year later. Results Of the 46 patients with moderate or severe COPD (male 38,moderate 24),21 (45.65%) were microbiologically evalu- able at baseline.The main pathogen was Haemophilus influenzae (10/21).Clinical efficacy rate was 87.0% in moxifloxacin group and 82.6% in cefaclor group.Bacterial eradication rate was 80.0% and 72.7% respectively.The difference between groups was not statistically significant in terms of clinical or microbiological efficacy.In moxifloxacin arm,the frequency of ex- acerbation was 2.6?1.0,significantly lower than control arm (3.5?1.4,P
6.Clinical Feature of Children with Acquired Immunodeficiency Syndrome

Objective To explore the causes,clinical feature,diagnoses and prognosis of children with acquired immunodeficiency syndrome (AIDS) and improve our awareness of this disease.Methods The clinical and laboratory data of 12 children with AIDS were analyszed retrospectively and the clinical feature was summed up.Results Among 12 children with AIDS,9 cases were boys and 3 cases were girls,aged from 45 days to 9 years old.The parents of 11 cases were positive in serological human immunodeficiency virus (HIV) antibody or died of AIDS.The main manifestations were developmental delay and various degree cacotrophia(12 case,100%),the others included abnormity fever,discontinuity cough and hepatosplenomegaly(10 cases,83.3%),recurrent diarrhea and lymphadenopathy (8 cases,66.7%),skin lesion(6 cases,50%),cerebral hypoplasia (4 cases,33.6%),oral candidiasis(3 cases,26.7%),and tympanitis(2 cases,16.7%).Laboratory tests showed:all of 12 cases were positive in serological HIV antibody by test for 2 times.The percent of CD4+ and the ratio of CD4+ to CD8+ were decreasing.One case was dead and the others had bad effect with allopathy.Conclusions Children with AIDS have short clinical latency,diversified clinical manifestation,low cell-mediated immunity function and bad effect with allopathy.To improve our awareness of the di-sease should be helpful for diagnosing and treating it early and prolonging the patients' life.
8.Progress on the cause and mechanism of a separation of clinical symptoms and signs and imaging features in lumbar disk herniation.

A few of patients with lumbar disk herniation having a separation of clinical symptoms and signs and imaging features, can be found in clinic, but the traditional theory of direct mechanical compression of nerve roots by herniated nucleus pulposus can't be used to explain this abnormal protrusion of lumbar intervertebral disc. The clinical symptoms and signs of the atypical lumbar disk herniation are affected by multiple factors. The indirect mechanical compression and distraction effect of spinal nerve roots may play an important role in the occurrence of the separation, and the appearance of abnormal clinical symptoms and signs is closely related to the migration of herniated nucleus pulposus tissue, transmission of injury information in the nervous system, and the complex interactions among the nucleus pulposus, dural sac and nerve roots. Moreover,the changes of microcirculation and inflammation secondary to the herniated nucleus pulposus tissue, the hyperosteogeny in the corresponding segment of the lumbar vertebrae and the posture changes all results in a diversity of symptoms and signs in patients with lumbar intervertebral disc herniation. Besides, there exist congenital variation of lumbosacral nerve roots and vertebral bodies in some patients, and the misdiagnosis or missed diagnosis of imaging finding may occur in some cases. However, the appearance of a separation of clinical symptoms and signs and imaging examination in patients may be caused by a variety of reasons in clinic. The exact mechanism involved in the interaction among nucleus pulposus tissue, dural sac and nerve root, secondary changes of pathophysiology and biomechanics around the nucleus pulposus, the determination of lesioned responsible segments, and how to overcome the limitations of imaging all need the further researches.

Stereotactic body radiation therapy (SBRT) has been increasingly used in the treatment of early-stage NSCLC,especially for patients who cannot undergo surgery.Compared with other imaging modalities,PET/CT has higher sensitivity and specificity in diagnosis and staging of NSCLC as well as higher prediction value for prognosis.The progress of 18F-FDG PET/CT in evaluation of the therapeutic response and prognosis after SBRT for early-stage NSCLC is summarized in this review.
