1.Evaluation of regional myocardial mechanics of right ventricle in patients with liver cirrhosis using velocity vector imaging combined with isometric stress test

Objective To study regional myocardial function of right ventricle at different stages of cirrhosis,using velocity vector imaging combined with standard isometric grip strength test to reveal the mechanical function in patients with liver cirrhosis at early stage further,and to benefit reliable quantitative evaluation and efficacy indicators for the establishment of the early diagnostic criteria of cirrhotic cardiomyopathy by echocardiography,and found myocardial mechanics basis for explicit pathogenesis of cirrhotic cardiomyopathy.Methods Accorded to Child-Pugh score,the cirrhosis experimental groups were divided into three groups:group A (42 patients),group B (28 patients) and group C (12 patients).There were 31 patients in group A to be conducted standard isometric grip strength test.While 34 healthy people were selected as the control group.The dynamic gray scale long-axis view images of the right ventricle were collected in 3 complete cardiac cycles in apical four-chamber view.Peak systolic longitudinal strain (Smax) and global ejection fraction of the right ventricle were derived from 6 myocardial segments with the Syngo VVI workstation off-line.Results 1)Compared with the control group,in group A,global ejection fraction of the right ventricle was normal,there were not significant change of Smax of each segment (P >0.05).In group B,there was not significant change of global ejection fraction of the right ventricle,the Smax of each segment were increased mainly (P <0.05-0.01).In group C,global ejection fraction of the right ventricle was significantly lower (P < 0.01),the Smax of each segment reduced mainly (P < 0.05-0.01).2)Compared with group A,in group B,there was no significant difference of blood pressure and global ejection fraction of the right ventricle,while including the Smax of each segment in systole and diastole.Compared with group A and group B,in group C,systolic and diastolic blood pressure and global ejection fraction of the right ventricle decreased significantly; the Smax of each segment were reduced mainly (P < 0.05-0.01).3)Compared with the resting state,after the standard isometric grip strength test,the global ejection fraction of the right ventricle decreased significantly,there was a significant reduction mainly in Smax of each segment (P < 0.01).Conclusions The regional myocardial function of right ventricle in patients with liver cirrhosis is abnormality even at early stage and could be revealed by standard isometric stress test.The right ventricular systolic dyfunction is related to the stages of liver cirrhosis disease.
2.Evaluation of surgery-related quality of life in patients with meningioma

BACKGROUND: With development of modern medical sciences, the out comes after meningioma surgery cannot be fully assessed using morbidity,livability and mortality merely. Quality of life (QOL) is a health-related multivariable index, which provides integrated inf ormation associated with physiological,sychological and social adaptation status of the patients to doctors. Assessment of the QOL in meningioma patients may be one of a relatively good method for guiding surgery and reducing reoccurrence of tuOBJECTIVE: To evaluate the QOL in meningioma patients and then to provide relevant data for assessing the outcomes of meningioma surgery comprehensively.DESIGN: It was a single-sample investigation. SETTING: Neurosurgery Department, Second People's Hospital of Wuxi City of Nanjing Medical University.PARTICIPANTS: Totally 147 patients underwent meningioma surgery in the Neurosurgery Department of the Second Affiliated Hospital of Wuxi City affiliated to Nanjing Medical University between January 1995 to January 2001 were selected, 61 males and 86 females, with a male/female ratio of 2:3 and an age ranging from 5-77 years old, in average of 43 years old, and the median was 43 years old.METHODS: Based on World Health Organization Quality of Life 100item questionnaire (WHOQOL-100) and Karnofsky Performance Scale (KPS), we designed a questionnaire on QOL in patients with meningioma.Outcomes of the transverse investigation on 147 meningioma patients were compared with healthy controls. Surgery-related QOL curve was used for identifying the threshold satisfactory point of QOL. In addition, with selfcontrol, influence of surgery on QOL was assessed.MAIN OUTCOME MEASURES: Relationship between QOL and clinical histories, radiological analysis, operation grading, histological properties and reoccurrence were investigated.RESULTS: All of the 147 patients entered the statistical analysis. ①In patient with meningioma, Cronbach's coefficient was 0.9521 and the correlated coefficient was 0.8685, which suggested that QOL questionnaire for patients with meningioma had a good reliability and validity. ②QOL ≥70 represented that the patient was satisfied with the QOL. ③In patients underwent surgical treatment, their physiological functions, self-help and self-care except for psychological functions were significantly improved (P < 0.001 ).CONCLUSION: Surgical treatment enables patients to get a satisfied QOL. QOL questionnaire for patients with meningioma is helpful to provide relevant data for meningioma operation.
3.Analysis for the clinical pathological characters of extranodal Nasal-type natural killer cell/T cell lymphoma

Objective To summarize the clinical histopatholngical characteristics of extranodal na-sal-type natural killer cell / T cell lymphoma. Methods 21 cases of nasal-type extranodal natural killer cell / T cell lymphoma were studied by retrospective analysis on its connection with their clinical manifesta-tions and histopathological features. Immunohietochemical SP method was used to detect the expression of immunophenotype, virus( EBV), and etc, in 21 cases. Results Capillaries had varied degrees hyperplasi-a in all cases under the microscope. Tumor cells showed definite devastating infiltration around the blood vessels, its infiltrating representation was found below intima and among vessel wall, and multiple mixed in-flammatory cells were found in it as well. Squamous epithelial pseudotumor-like proliferating can be seen in 1 case. All tumor cells'immnnophenotype were T-cell differentiation antigen CD45RO( + ), T-cell related antigen CD56 (+), T-cell particle-associated antigen TIA-1 (+), EBV (+). Conclusion Nasal-type extranodal natural killer cell / T cell lymphoma has characteristic clinical expression and histopathological changes. Accurate diagnosis can be obtained on the basis of its typical clinical expression, pathomorphology changes, immunophenotype and EBV( + ) in situ hybridization.
4.Tissue engineered cartilage constructed by growth factor-induced bone marrow mesenchymal stem cells and allogenic bone matrix gelatin

Objective To construct tissue engineered cartilage with bone marrow mesenchymal stem cells (MSCs) and allogeneic bone matrix gelatin (BMG). Methods Bone marrow was aspirated and MSCs were separated by consecutive 0.70(70%)(V/V) Percoll gradient centrifugation and fibronectin adhesion. In experimental group, rhTGF-?1 and rhIGF-Ⅰ were applied into DMEM to induce proliferation and chondrogenic transformation, while in control group only DMEM was used. Colony forming efficiency (CFE) of MSCs was calculated, procollagen ?1(Ⅱ)mRNA expression in cells was detected by RT-PCR, hexuronic acid in culture medium was measured by carbazole-sulfuric acid method. Allogeneic bone matrix gelatin (BMG) was prepared by successive defatting and decalcification, observed by scanning electronic microscope. Chondrocyte progenitor cells induced from MSCs were seeded into BMG to construct tissue engineered cartilage. Culture masses were sectioned and stained by Masson's trichrome. 5.BLOOD SUPPLY OF THE ANTERIOR PART OF THE ILIAC CREST

The blood vessels supplying the anterior part of the iliac crest of 126 (84 males, 42 females) Chinese adult cadavers were observed. The findings are as following:1. The superficial circumflex iliac artery has an average external diameter of 1.45?0.41 mm and supplies mainly the skin and the superficial structures of the groin region and the region adjacent to the anterior superior iliac spine.2. The ascending branch of the lateral circumflex femoral artery has an average external diameter of 2.62?0.51 mm and supplies the tensor fasciae latae and the outer border of the anterior superior iliac spine.3. The deep upper branch of the superior gluteal artery has an average external diameter of 2.87?0.56 mm and supplies the glutei medius et minimus and the outer border of the anterior part of the iliac crest.4. The deep circumflex iliac artery has an average external diameter of 2.77?0.47 mm and may originate from above (18.0%), behind (41.5%), or below (40.5%) the inguinal ligament. It supplies the abdominal muscles adjacent to iliac crest and the inner border of the anterior part of the iliac crest.In this study particular attention was paid to investigating the course, branches, anastomosis of the deep circumflex iliac artery and its relationship to the anterior part of the iliac crest.5. The findings suggest that the deep circumflex iliac vessel may be used as a nutritive pedicle in free microvascular transplantation of the iliac bone.

Objective To analyze the distributional characteristics of NPC1L1 1 735 C>G polymorphism and the relation-ship between serum lipid levels and obesity in Guangxi Han population.Methods Genotyping of the NPC1L1 1 735 C>G poly-morphism of 409 cases of Han was performed by polymerase chain reaction and restriction fragment length polymorphism combined with gel electrophoresis in all subjects.The relationship of different genotypes in blood lipid levels and obesity were evaluated.Re-sults The frequency of CC,CG and GG genotypes was 39.40%,46.00% and 14.60% in Han.The frequency of C and G alleles was 62.35% and 37.65% in Han.The levels of BMI,LDL-C,ApoB was different among the three genotypes (P <0.05).The G al-lele carriers had higher serum LDL-C and ApoB levels than the G allele noncarriers (P <0.05 ).Serum lipid parameters and BMI were also correlated with some environmental factors.Conclusion The 1 735 C>G polymorphism in NPC1L1 gene may be correla-ted with serum lipid profiles and obesity,the G allele might increase the risk of hyperlipemia and obesity.

E-selectin is an inducible adhesion molecule,and it is correlated with various molecules during cerebral ischemia/reperfusion injury.This article mainly reviews the relationship between E-selectin and intracellular adhesion molecule-1,tumor necrosis factor-?,NF-kB,leucocyte function-associated antigen-1 in cerebral ischemia/reperfusion injury,so as to provide assistance for E-selectin in the prevention,diagnosis and treatment of ischemic cerebrovascular disease.

Objective To investigate the pathogentic factor of severe acute pancreatitis(SAP)-associated lung injury and the protective function of octreotide combined with alinastatin on SAP-associated lung injury in rats.Methods Eighty SD rats were randomly divided into 5 groups.Sham operative(S)group,SAP group,(octreotide)(O)group,ulinastatin(U)group and octreotide + ulinastatin (O+U)group,and each group was divided into 6h,12h sub-groups.After AP models were induced,the serum concentration of amylase(AMY),tumor necrosis factor?(TNF-?),malondiadehyde(MDA),and the concentration of(myeloperoxidase)(MPO) in lung tissue were determined;and the pancreas and lung pathology were graded,the changes of the above-mentioned indexes after using octreotide and ulinastatin were compared.Results(1)Compared to SAP group,AMY,TNF-?,MDA,MPO,and pancreas pathology score were decreased significantly in each of the 3 therapy groups during the same period;and at 12h,in O+U sub-group,lung pathology score also decreased compared to SAP group(P

Morphology and relationships of piriformis and its tendon,and their relations withsciatic nerve in 100 cases have been observed in order to analyse the cause of sciatica.Piriformis was divided into four types and piriformis tendon into five types.In79% of piriformis there were additional origins.In 5% of cases synovial bursa waspresent below the end of piriformis tendon.The relation between piriformis andsciatic nerve was anomalous in 38%.Sciatic nerves and their branches in 16%passed between the two tendons of piriformis,and along the anterior or posteriorsurface of a tendon.It is concluded that various anomalous relationship between piriformis tendonand sciatic nerve is an important factor giving rise to sciatic pain.
