1.An analysis of a telephone survey on knowledge of iodine deficiency disorders in Shanghai residents

Objective To find out whether Shanghai residents have mastered the knowledge related to iodine deficiency disorders (IDD),and how they choose different kind of salt.Methods Residents were selected by stratified random sampling from all 18 districts(counties) of Shanghai in 2010.Simple random sampling was used at the first level; random function was used at the second level to produce the last 4 numbers of a phone number.People who own the number were selected to be called.Results Totally 219 people completed the investigation.49.3% (108/219) of the residents only selected iodized salt,and 25.6%(56/219) choose non-iodized salt; 6.8% (15/219) selected both,and 18.3% (40/219) don't care.About the reason of choosing iodized salt,25.9% (28/108) thought it can prevent IDD,6.5% (7/108) thought it's good to children's intelligence.About the reason of choosing non-iodized salt,35.7% (20/56) thought they were not iodine deficiency,17.9% (10/56) thought Shanghai was not an IDD epidemic region.Among the 126 people who had heard of iodine deficiency disorders,7.1% (9/126) believed that iodine deficiency disorders can lead to varying degrees of mental impairment,65.1% (82/126) thought it can lead to endemic goiter; 45.2%(57/126) thought eating iodized salt and 33.3% (42/126) thought eating kelp and laver can prevent IDD.58%(127/219) had no idea of IDD and/or its hazards.Conclusions The resident's knowledge on iodine deficiency disorders is not satisfactory.We should make more effort in health education and help people to choose salt reasonably.
2.Effect of ecoimmunonutrition on systemic inflammatory response in swine with acute necrotizing pancreatitis

Objective To evaluate the effect of ecoimmunonutrition on systemic inflammatory response in swine with acute necrotizing pancreatitis (ANP). Methods Swine model of ANP was induced by retrograde injection of 5% sodium taurocholate and trypsin into the pancreatic duct. 24h after the ANP models were established, eighteen swine were randomly divided into total parenteral nutrition (PN) group (n = 6), enteral nutrition (EN) group (n = 6), and ecoimmunonutrition (E1N) group (n = 6). Each of the three nutrition formulas was given to each group respectively for eight days. The level of plasma endotoxin and serum concentrations of TNF-α, IL-1β, IL-6 and IL-10 were measured before, 24 h after the induction of ANP and 1,2, 4, 8 d after nutrition support. Results 24 hours after the ANP models were established, the serum level of plasma endotoxin, TNF-α, IL-1β, IL-6 and IL-10, NF-κB activities in EIN group were ( 1.85 ± 0.18) EU/L,(461.59 ± 128.25 ) pg/ml, ( 185.49 ± 58.81 ) pg/ml, ( 354.26 ± 34.63 ) pg/ml, ( 110.32 s 25. 18 ) pg/ml,(51.06 ± 2.27 )%, respectively, which all were significantly higher than those before the ANP models were established, but there were no difference among three groups. Eight days after nutrition support, those levels in EIN group were (1.48 ±0.16 )EU/L, (30.11 ±9.12)pg/ml, (20.17 ±7.04)pg/ml, (36.42 ±7.24) pg/ml and ( 89.46 ± 13.54 ) pg/ml, (9.06 ± 0. 17 ) %, respectively, which all were significantly lower than those of TPN and EEN group ( P < 0. 05 ). The ration of IL-10/IL-6 was 2.51 ± 0.42 in EIN group, and 2.28 ± 0. 19 before the ANP models were established. Conclusions EIN could attenuate endotoxemia, decrease NF-κB activities and serum concentrations of cytokines and maintained the balance of pro- and anti-inflammation.
3.Effect of ecoimmunonutrition on maintenance of the intestinal mucosal barrler function in severe acute pancreatitis Pigs

Objective To evaluate the effect of ecoimmunonutrition on maintenance of the mucosal barrier function in pigs with severe acute pancreatitis(SAP).Methods Twenty pigs were injected with5% sodium taurocholate and trypsin into the pancreatic duct to establish SAP models.Twenty four hours later,18 suvival pigs were randomly given parenteral nutrition(PN group,n=6),elemental enteral nutrition(EEN group,n=6),or ecoimmunonutrition(EIN group,n=6)for 8 days.The serum amylase,endotoxin and intestinal permeability were determined on different times.All pigs were sacrificed 8days later,and their venous blood,mesenteric lymph nodes(MLN),liver,lung,spleen and pancreas specimens were cultured quantitatively and qualitatively with standard techniques.The morphology of small bowel was observed and pathological changes of the pancreas was analyzed.Results Serum amylase of all groups were obviously elevated,but there was no significant difference among them(P>0.05).The plasma endotoxin and intestinal permeability were lower in EIN group[(1.50±0.18)EU/L and(42.8±20.0)×10-3 respeetively]compared to EEN group[(1.98 4±0.20)EU/L and(67.4±23.0)×10-3 respectively]and PN group[(3.96±0.40)EU/L and(197.2±47.4)×10-3.respectively](all P value<0.05).The incidence and the magnitude of bacterial translocation in EIN group were lower than those in PN and EEN groups.However,the ileum mucosa thickness,villus height,crypt depth and the rate of the normal intestinal villi in EIN group were(398.27±52.93)μm,(269.72±41.66)μm,(681.98±58.33)μm and 79%,respectively,but in PN group were(218.32±35.81)μm,(145.76±23.34)μm,(376.20±48.23)μm and13%,respectively and in EEN group were(305.70±42.72)μm,(192.52±38.17)μm,(507.31±68.23)μm and 47%,respectively.Conclusions EIN can maintain the integrity of intestinal mucosal barrier and decreasing intestinal bacterial and endotoxin translocation in SAP pigs.
4.The inhibitory mechanism of rabeprazole on acid secretion of parietal cells in rats

Objective To investigate the inhibitory mechanism of rabeprazole on acid secretion of parietal cells in rats. Methods Seventy two Spraque-Dawley rats were divided into control group(normal saline), low dose of rabeprazole group (rabeprazole 10 mg/kg) and high dose of rabeprazole group (rabeprazole 20 mg/kg) with 24 each. Four rats in each group were treated at 1 h, 2 h, 4 h, 6 h, 12 h and 24 h, respectively, pH value and activity of H+-K+-ATPase were tested by NaOH titrimetric method and chromatometry, respectively. The changes of ultrastructure of parietal cells were observed by electron microscope. Results Compared with control group(1.97±0.30), pH value in low (3.37± 0.97)and high (5.96±0.26)dose groups were significantly increased(P<0.01), and the activity of H+-K+-ATPase was inhibited obviously at 1 h after administration(3.28±0. 41 vs 1.47±0.27 and 0.92±0.07, P<0.05). Those differences could be found even at 12 h(P<0.01). The ultrastructural changes of parietal cells were consistent with the increase of pH value and decreased activity of H+-K+-ATPase. The significant difference was found in acid depression and excellent time between low and high dose of rabeprazole groups. Conclnsions The uhrastructural changes of parietal cells and the activity of H+-K+-ATPase can accurately reflect the acid secretion in rats. Rabeprazole has powerful and rapid effects in inhibiting acid secretion which correlates with the dosage.
5.The effects of EPO on immunofunctions in patients with chronic renal failure

Objective:To investigate the effect of EPO on humoral and cellular immunity and the monocyte antigen presenting function in patients with chronic renal failure(CRF).Methods:60 patients with CRF were divided into three groups.Group A patients accepted hemodialysis and treated with EPO.Group B patients treated with EPO and didn't accept hemodialysis.Group C patients didn't accept EPO treatment and hemodialysis.After 8 weeks treatment,the subpopulations of T lymphocyte were analyzed with detected by flow cytometry analysis method;Immunoglobulins were measured with radioactive immuntzation method.In addition,the infection rate of patients with CRF in every group was compared.Results:The parameters of immune functions of patients with CRF were comparable between groups before treatment.After treated with EPO for 8 weeks,RBC numbers and Hb were significantly increased in patients of group A and group B,but not in patients of group C.IgG and IgA were increased in patients of groups A and groups B and have statistic differences as compared with that in patients of group C(P
6.Quantitative Analysis on Staging of Chronic Viral Hepatitis B and Hepatic Fibrosis by Real-time Shear Wave Elastography

Objective To explore the application value of real-time shear wave elastography (SWE) technique in diagnosing and staging of chronic viral hepatitis B and hepatic fibrosis and to establish Young's modulus reference range for diagnosing and staging of hepatic fibrosis.Methods Forty-eight patients with chronic hepatitis B and fifty-eight healthy adults were enrolled and their Young's modulus values of S5 and S6 segments of liver were measured.Histopathologic examination was performed on 48 patients with chronic hepatitis B.Comparative analysis was conducted between the pathological findings and Young's modulus values,by means of which Young's modulus reference range for diagnosis and staging of hepatic fibrosis was obtained.Results There was significant difference in Young's modulus values of S5 and S6 segments of liver between chronic hepatitis B group and the normal control group (P<0.05).Young's modulus values of S5 and S6 segments of liver in chronic hepatitis B group were (11.7 ± 2.9) kPa and (12.1 ± 3.2) kPa respectively,which were significantly higher than those in the normal control group,(5.7 ± 1.1) kPa and (5.8 ± 1.3) kPa respectively.Significant differences of Young's modulus values were detected in every staging of hepatic fibrosis (P<0.05).S5 segment of liver Young's modulus values in S0-S4 stages were (5.8 ± 2.2) kPa,(7.3 ± 1.9) kPa,(10.3 ± 2.8) kPa,(10.3 ± 2.8) kPa,and (25.3 ± 3.6) kPa,respectively.S6 segment of liver Young's modulus values in S0-S4 stages were (5.7 ± 2.3) kPa,(9.2±2.1) kPa,(10.5±2.1) kPa,(14.7±4.5) kPa,and (26.1 ±2.1) kPa,respectively.Young's modulus value of the liver rose with the increase of S stage.Conclusion SWE technique can establish the Young's modulus reference range for hepatic fibrosis stage.Besides,it features high sensitivity,specificity and accuracy.
7.Research on the Improvement of Quality Standard for ShutonganⅠGranule

OBJECTIVE:To establish the quality standard for ShutonganⅠgranule.METHODS:TLC was conducted for the qual-itative identification of Citrus aurantium and Forsythia suspensa. HPLC was conducted for the content determination of naringin in the preparation;the column was Agilent C18 with mobile phase of acetonitrile-water(20:80,V/V)at a flow rate of 1.0 ml/min,the detection wavelength was 283 nm,the column temperature was 30 ℃,and the injection volume was 10 μl. HPLC was also used for the content determination of phillyrin;the column was Agilent TC-C18 with mobile phase of acetonitrile- water(23:77,V/V)at a flow rate of 1.0 ml/min,the detection wavelength was 277 nm,the column temperature was 30 ℃,and the injection volume was 10 μl. RESULTS:The C. aurantium and F. suspensa of TLC showed clear spots and good separation. The linear range was 57.843-289.214 μg/ml for naringin(r=0.999 9)and 37.434-187.169 μg/ml for phillyrin(r=0.999 6);RSDs of precision,stability and reproducibility tests were less than 2%;recoveries were 97.20%-102.04%(RSD=1.7%,n=9)and 97.14%-102.27%(RSD=2.0%,n=9),respectively. CONCLUSIONS:The standard can be used for the quality control of ShutonganⅠgranule.

Objective To observe the effect of atorvastatin on intraabdominal fat and microalbuminuria (MAU) in patients with metabolic syndrome (MS). Methods Forty-four MS patients were divided into the atorvastatin group and the control group. Blood pressure and blood glucose were controlled in both groups, in addition, atorvastatin was administered to the patients in the atorvastatin group. ObjectiveTo evaluate the effect of enteral ecoimmunonutrition (EIN) on hepatic injury in swine with acute necrotizing pancreatitis (ANP).MethodWith 20 pigs,swine model of ANP was induced by injecring 5% sodium taurocholate into the pancreatic duct.24 hours after the ANP models were established,2 pig were dead,and the remaining 18 pigs were randomly divided into total parenteral nutrition (TPN) group ( n =6),elemental enteral nutrition (EEN) group ( n =6 ),and EIN group ( n =6 ),and received nutrition support for 8 days.The level of plasma endotoxin and serum concentrations of AST,ALT,TNF-α,and IL-6 were measured at different time points ( before model establishment,24 hours after model establishment,and the 1,2,4,8 days of nutrition support).All the pigs were sacrificed after the 8-day nutrition support,and ascites quantity,hepatic malondialdehyde (MDA),superoxide dismutase (SOD) were measured.The mRNA expression of TNF-α and IL-6 in hepatic tissue were detected by RT-PCR.Pathologic changes of the livers were analyzed.Results24 hours after the ANP models were established,the plasma endotoxin,AST,ALT,TNF-α,IL-6 in EIN group were significantly higher than the levels before model establishment ( all P < 0.01 ),but not significantly different from the 9ther 2 groups.After 8 days of nutrition support,the above 5 indexes in EIN group were significantly lower than those in TPN group and EEN group ( all P < 0.05 ).In addition,the ascites quantity,hepatic concentration of MDA,the expression of TNF-α and IL-6,and hepatic pathological score in EIN group were lower than those of TPN group and EEN group,while hepatic SOD activity was higher in EIN group than in the other 2 groups (all P < 0.05 ).ConclusionEarly EIN could attenuate hepatic injury in ANP from various aspects.
10.The role of BCR/ABL isoforms in the presentations and outcome of Philadelphia-positive acute lymphoblastic leukemia in adult patients

Objective To investigate the difference of clinical characteristics and outcomes between different isoforms of BCR/ABL in adults with Philadelphia-positive acute lymphoblastic leukemia (ALL).Methods The data of 106 adults with Ph+ALL diagnosed in our hospital from January 1, 1996 to December 31, 2007 were reviewed. The difference of clinical characteristics between different subgroups of BCR/ABL was compared and their relation with outcomes was studied. Results The median age of the 106 patients was 34 years and the median white blood cell count at baseline was 28. 5 × 109/L. Comparative analysis demonstrated that patients in p210 group had an older age, higher blood platelet count (BPC) and more frequent occurrence of splenomegaly. Referring to the outcomes, the complete remission (CR) rate of the two groups were 92. 2% and 93.9%, respectively. The median overall survival (OS) and relapse free survival (RFS) in p190 group were 13 months and 10 months, the 1,3-year estimated OS were (54. 7±6. 7)% and (5.5±5.2)%, and the 1,3-year estimated RFS were (40. 2±6. 8)% and (7. 8±6. 7)%,while in p210 group, the median OS and RFS were 15 months and 10 months, respectively, the 1,3-year estimated OS were (65.8±8. 9)% and (14. 5±7.4)%, and the 1,3-year estimated RFS were (48. 3±9. 4)% and (12. 9±7. 7)%. All of the above data had no statistic significance between the two groups.Conclusion Majority of the adults with Ph+ALL is p190 positive and patients with p210 have older age, higher BPC and more frequent occurrence of splenomegaly, while there is no significant difference between p190 group and p210 group in CR rate, RFS and OS.
