3.Fistulectomy with external anal sphincter bareness in treatment of trans-sphincteric anal fistula: clinical analysis of 46 cases

Clinical data of 46 patients with trans-sphincter anal fistula treated by fistulectomy with external anal sphincter bareness in Department of Anorectal Surgery, Jiaxing TCM Hospital from July 2018 to July 2019 were retrospectively analyzed. All operations were performed successfully. There were no significant differences in Wexner incontinence scores (2.00±0.68 vs.1.99±0.70, P<0.05), mean anal resting pressure [(75.60±8.60) vs.(73.60±8.20)mmHg(1 mmHg=0.133 kPa), P<0.05] and maximum systolic pressure [(109.60±7.80) vs.(107.20±8.30)mmHg, P<0.05] before and 6 months after operation. There were 1 case with postoperative incision bleeding and 2 cases with postoperative infection. All patients were followed up for 6 months and there was no recurrence and changes in anal shape during the follow-up. Results indicate that the fistulectomy external anal sphincter bareness is safe, efficient with well preserved sphincter function for patients with trans-sphincter anal fistula.
4.Qualitative research on risk factors of lymphedema for patients with breast cancer

Objective To deeply understand the risk factors of lymphedema for patients with breast cancer after surgery. Methods The phenomenological method was applied in this study. Semi-structured interview was used to collected data from 9 female breast cancer patients with lymphedema after surgery in our hospital from June to September 2016 for generic analysis. Results The risk factors of lymphedema could be categorized into four themes:(1)choice of treatment is the primary cause:axillary lymph node dissection; radiotherapy; chemotherapy; (2)not paying enough attention to lymphedema:lacking the knowledge of lymphedema; imbalance of physical activities for the affected limb; lacking awareness of exercise and protection of the affected limb. Conclusions Axillary lymph node dissection after radical surgery for patients with breast cancer is the primary cause of lymphedema, and paying not enough attention is an important factor, especially lacking the consciousness of prevention, so the nurses should emphasize education about prevention of lymphedema after surgery for patients, to improve the consciousness of them to reduce the occurrence of lymphedema and its influence on their quality of life.
5.Visual analysis of researches on nursing sensitive quality indicators based on CiteSpace

Objective:To compare research hotspots and frontiers of nursing sensitive quality indicators at home and abroad and to understand the current research status of nursing sensitive quality indicators, so as to provide reference for improving nursing quality.Methods:Using "nursing sensitive quality indicators" and "nursing sensitive quality indicators" as the Chinese and English search terms, CiteSpace Ⅴ software was used to conduct a quantitative analysis of the reviews and research literatures collected by CNKI from the establishment of CNKI database to June 2020 and by Web of Science core collection database from 2014 to 2020. High-frequency keywords, keyword co-occurrence and burst words were carried out visual analysis.Results:A total of 262 valid Chinese reviews and 789 English reviews were retrieved and the overall publication volume was on the rise. Foreign research focused on the outcome of nursing sensitivity quality indicators, the application of specialist nursing practice and the methods of indicator development, while domestic research focused mainly on adverse events, quality control and other aspects.Conclusions:Domestic researches on nursing sensitivity quality indicators has been continuously strengthened. In the future, in-depth researches can be continued in the aspects of processing, development methods and application promotion of indicator information.
6.Impact of group cognitive behavioral therapy counseling on cognitive control among college students with high obsessive compulsive traits

Objective:
To explore the effect of group cognitive behavioral therapy (GCBT) on cognitive control among college students with high obsessive compulsive traits, to provide basic information for the psychological counseling intervention for college students.
Methods:
From March to April 2019, 687 students were conveniently selected from 2 universities in Hefei. According to the inclusion and exclusion criteria, 58 students with high obsessive traits were selected and divided into experimental group ( n =29) and control group ( n =29) by random number table method. The experimental group received cognitive behavioral group counseling for 4 weeks (1.5 h each time, twice a week), while the control group receive no intervention. The Obsessive Compulsive Inventory Revised (OCI-R), Stroop Color Word Test (SCWT), Digital Span Test (DST), and Wisconsin Card Sorting Test (WSCT) were used to assess in two groups at baseline and 4 weeks later.
Results:
After 4 weeks, the scores of OCI-R in the GCBT group (10.28±7.22) was lower than that of in the control group (15.90±10.20) ( t=2.42, P<0.05). Before and after intervention, compared with the control group [(21.89±6.63, 20.52±7.37)s, (8.62±4.43, 8.04±4.84)s] in Stroop C and Stroop interfere effects (SIE), the GCBT group [(22.14±4.92, 16.81±3.43)s, (8.36±3.87, 4.82±1.86)s], the interaction of time group was statistically significant ( F =14.60, 10.54, P <0.05). Compared with the control group (6.21±1.35, 6.55±1.45)times, the scores of DST reverse in the GCBT group (6.31±1.44, 7.24±1.38) times were statistically significant ( F=3.96, P <0.05).
Conclusion
It suggests that cognitive behavioral group counseling can improve the inhibitory control and working memory of college students with high obsessive compulsive traits, but does not change the cognitive flexibility.
7.Progress of Research on Macrophage Polarization in Inflammatory Bowel Disease

Inflammatory bowel disease(IBD)is a chronic inflammatory disease of the gastrointestinal tract that includes Crohn's disease and ulcerative colitis.IBD may be caused by complex interactions between genetic susceptibility,environmental factors,and alterations in the gut microbiota,resulting in dysregulated innate and adaptive immune responses.Recent studies have identified macrophages in the intestinal inflammatory response as having the plasticity to not only regulate inflammation,but also to promote tissue repair and healing.As aberrant macrophage polarization occurs during the development of IBD,the balance between the phenotype and function of pro-inflammatory M1 and anti-inflammatory M2 macrophages is regulated by extracellular and intracellular stimuli,and this process is therefore expected to be a potential target for new therapeutic approaches.This article reviewed the progress of research on macrophage polarization in IBD.
8.Effects of polysaccharide content and anti-inflammatory,hemostatic,and antioxidant activities before and after preparation of Mosla chinensis-Jiangxiangru

Objective Explore changes in polysaccharides in Jiangxiangru before and after ginger juice preparation,and evaluate polysaccharide anti-inflammatory and antioxidant activities before and after processing.Methods The contents of Jiangxiangru polysaccharide(JXRPs)and Ginger juice processed of Jiangxiangru polysaccharide(JZJXRPs)before and after processing were determined by phenol-sulfuric acid method.We used the swelling model in rats and endotoxin(LPS)to establish the RAW264.7 mouse macrophage inflammation model.The optimal administration concentration was determined using a cell proliferation(MTT)assay.Enzyme-linked immunosorbent assay(ELISA)were used to measure Interleukin-6(IL-6),Interleukin-12(IL-12),Nitric oxide(NO),Interleukin-4(IL-4),and Interleukin-10(IL-10).Bleeding time of mice by tail cutting was observed to evaluate the hemostatic effect.The ability to remove 1,1-diphenyl-2-picryl-hydrazyl radical(DPPH)and 2,2'-Azinobis-(3-ethylbenzthiazoline-6-sulphonate)(ABTS)was used to evaluate in vitro antioxidant activity.Results The contents of JXRPs and JZJXRPs were 13%and 22%,respectively.In swollen rats at 4 h after injection,compared with the model group,200 mg/kg JXRPs and 100 mg/kg JZJXRPs significantly reduced rat swelling(P<0.05).In vitro anti-inflammation assessment showed that the polysaccharides before and after processing significantly inhibited secretion of IL-6,IL-12,and NO(P<0.01)and promoted secretion of IL-4 and IL-10(P<0.01),and also that processing post-effects were stronger.The hemostatic experiment show that,compared with the control group,JXRPs increased hemostasis,but without a significant difference,and no significant difference was found using JZJXRPs,although high doses showed a trend to increase hemostasis.In vitro antioxidant activity showed that JXRPs and JZJXRPs had different scavenging abilities for DPPH and ABTS with IC50 values of JXRPs of 0.2215 and 0.2110 mg/ml,respectively,and IC50 values of JZJXRPs of 0.1651 and 0.1884 mg/mL,respectively.Conclusions After Jiangxiangru is produced in ginger juice,it promotes dissolution of polysaccharides and increases polysaccharide content.Anti-inflammatory,hemostasis,and antioxidant capacities are stronger in JZJXRPS than JXRPS,which lays the foundation for follow-up research and clinical applications of JXRPS.
9.Eating Raw Snails Infected with Angiostrongylus Cantonensis Causes Eosinophilic Meningitis: A Case Report

We report a case of a male patient who developed persistent fever and central nervous system symptoms after eating raw snails for 10 days. The patient was diagnosed with Angiostrongyliasis depended on the clinical presentation, epidemiological history, and etiological results. The patient recovered after receiving albendazole anthelmintic and dexamethasone anti-inflammatory therapy. This article incorporates literature review to sort out the diagnosis and treatment of this patient, in order to provide feasible reference for clinicians.
10.Correlation between perceived professional benefits and innovation behavior of oncology nurses

Objective To explore and understand the influence of perceived professional benefits on oncology nurses′innovative behaviors. Methods Totally 943oncology nurses were selected to join thestudy, Using the general information questionnaire, nurses′ perceived professional benefits scale and innovative behaviors scale for investigation. Results The average score of oncology nurses′ perceived Professional Benefits Scale was (114.08 ± 16.45) points, the average score of oncology nurses′ innovative behaviors was (31.00 ± 7.00) points. The score of perceived professional benefits of oncology nurses was positively correlated with score of innovative behavior of nurses (r=0.472, P<0.01). Nurses position, level and relatives and friends identity,their own growth and team belonging in perceived professional benefits were the influencing factors of innovative behavior of oncology nurses (t=2.022-4.486, P<0.05 or 0.01). Conclusion The perceived professional benefits of oncology nurses can play a positive role in promoting their innovative behaviors. Nursing managers should pay attention to improving perceived professional benefits of clinical nurses, promote their innovative behaviors and improve the quality of nursing services.
