Chinese Journal of Preventive Medicine 2016;50(2):163-167
OBJECTIVETo assess the quantitative risk of the polycyclic aromatic hydrocarbons (PAHs) dietary exposure from edible fats and oils in China.
METHODSOne hundred samples of edible fats and oils were collected from the supermarkets and the farmers markets in 11 provinces of China from December in 2013 to May in 2014. Then they were tested for EU15+1 PAHs (16 PAHs were controlled in priority by European Food Safety Authority) by two test methods which were QuECHERS-GC-MS-MS and GPC-HPLC-FLD. Data of PAHs concentration and edible fats and oils consumption which were from Chinese National Nutrition and Health Survey in 2002 were combined to evaluate carcinogenic risk of PAHs in edible fats and oils by the method of margin of exposure (MOE). In this process, we divided the population into 6 groups, namely male adults (older than 18 years old), female adults (older than 18), male youths (13-17), female youths (13-17), school-agers (6-12) and preschoolers (2-5), and thought carcinogenicity as the critical toxicity end point of PAHs. Two quantitative risk assessment methods, i.e. point assessment and probability assessment, were used to evaluate the dietary exposure and MOEs.
RESULTSEU15+1 PAHs in one of 100 samples were not detected, other samples were polluted in different degrees; the detection rates were 3%-98% and the average contents were 0.26-3.26 μg/kg. The results of PAHs dietary exposure from both of point assessment and probability assessment were the same. The average exposures of PAH8 were as the following: male adults were 10.03 and (9.34 ± 12.61) ng·kg(-1)·d(-1)(The former was from point assessment and the latter from probability assessment, the same below), female adults were 9.95 and (9.60 ± 15.04) ng · kg(-1)·d (-1), male youths were 11.09 and (10.84 ± 16.54) ng·kg(-1)·d(-1), female youths were 10.06 and (9.58 ± 12.87) ng·kg(-1)·d(-1),school-agers were 15.29 and (15.62 ± 25.54) ng·kg(-1)·d(-1), preschoolers were 19.27 and (19.22 ± 28.91) ng·kg(-1)·d(-1). MOEs of mean and 50% exposure levels in different group of people were more than 10,000, while MOEs of 95% exposure levels in school-agers and preschoolers were less than 10,000.
CONCLUSIONFor general consumers, the health risk of PAHs exposure is very low. However, for high-end consumers (95% exposure level) from the sensitive groups (school-ager and preschooler) has a potential health risk.

Objective:to investigate the efficacy of drugs in the treatment of laryngeal granuloma after hypothermic plasma radiofrequency surgery in early glottic carcinoma.Methods:Thirty-two cases of laryngeal granulomacoming from 289 patients with early glottic carcinoma treated by plasma radiofrequency surgery under endoscope-supported laryngoscopefrom January 2011 to January 2021 in Dalian Municipal Central Hospital were enrolled. All patients were given oral treatment of zinc gluconate tablets (70 grams per tablet, containing 10 grams of zinc). The usage was 3 tablets each time, twice a day. If the granulation was located in the vocal cord, 20 mg of esomeprazolec was added twice a day, and taken orally on an empty stomach. The total course of treatment was 6-12 weeks.Results:Follow-up and reexamination of electronic laryngoscope showed that the granulation of all patients began to become smaller after 3 weeks of drug treatment and gradually disappeared after 6-12 weeks. After the granulation disappeared, the drug was stopped, and there was no recurrence. Two patients developed nausea and epigastric discomfort after oral administration of zinc gluconate tablets for 3 weeks, and the discomfort disappeared after reducing the dose. Conclusions:Oral administration of zinc gluconate tablets alone or in combination with esomeprazoleis an effective, safe and low recurrence method for laryngeal granuloma after hypothermic plasma radiofrequency ablation in early glottic carcinoma.

Objective:To investigate the clinical outcome of continuous expander implantation in the repair of large cicatricial alopecia in children. Methods:From February 2015 to October 2019, 5 cases of large cicatricial alopecia were treated, aged from 3 to 14, including two boys and two girls. The area of alopecia exceed 1/2 of scalp. One case is intradermal nevus excision. Four cases were scar after burn. All cases were used continuous expander implantation to expand scalp. In the first stage, the expander was implanted and the normal skin was expanded. In the second stage, the expander was removed. After the cicatrix was cut partly, the expanded skin flap was transferred to repair the wound and the expander was placed under the expanded skin flap in a relay way. Then the expansion was continued. In the third stage, the expander was removed, the residual cicatricial alopecia was cut and reconstructed by the expanded flap. Advanced flap and rotational flap were applied and the efficacy was observed. Results:Among the 5 children, 2 cases were left with 10 cm×4 cm alopecia and 6 cm×2 cm alopecia. The other 3 cases were completely restored. The incision scar was wider in one case. The follow-up period was from 3 months to 3 years. Hair grows well and the shape of the hair was close to normal. The results were satisfactory. Conclusions:Continuous expander implantation in the repairation of large cicatricial alopecia in children have many advantages, including good postoperative results, short course, low cost and highly feasible.

Chinese Journal of Burns 2023;39(11):1014-1020
Objective:To explore the safety and effectiveness of the application of sevoflurane inhalation sedation and analgesia during dressing changes in children with extensive burns. Methods:A prospective randomized controlled research was conducted. From March 2020 to January 2023, 216 children with extensive burns who met the inclusion criteria were admitted to the Department of Burns and Plastic Surgery of Kunming Children's Hospital. According to the random number table, the children were divided into sevoflurane group and ibuprofen group, with 103 cases left in sevoflurane group (67 males and 36 females, aged 1 (1, 2) years), and 98 cases left in ibuprofen group (67 males and 31 females, aged 1 (1, 2) years) after the exclusion of several dropped-out children. Children in sevoflurane group received sevoflurane inhalation for sedation and analgesia during dressing changes, while those in ibuprofen group took oral ibuprofen for analgesia before dressing changes. The heart rate, mean arterial pressure (MAP), and percutaneous arterial oxygen saturation (SpO 2) of the children were monitored and recorded at 30 minutes before the start of dressing changes, immediately after debridement, and at 30 minutes after the completion of dressing changes. The face, legs, activity, cry, and consolability scale and Ramsay sedation scale were used to evaluate the pain intensity and degree of sedation, respectively, at 30 minutes before the start of dressing changes, immediately after debridement, and at 30 minutes after the completion of dressing changes. The duration of dressing changes and the total number of dressing changes during hospitalization were recorded. The Houston Pain Outcome Instrument questionnaire was used to assess the satisfaction of the dressing-changing surgeons and a family member of the child with the analgesic effects during the process of dressing change when the children were discharged from the hospital. The occurrence of adverse reactions such as respiratory depression and hypoxemia that occurred during the process of dressing change were monitored and recorded. Data were statistically analyzed with independent sample t test, analysis of variance for repeated measurement, Wilcoxon rank-sum test, chi-square test, and Fisher's exact probability test. Results:At 30 minutes before the start of dressing changes and 30 minutes after the completion of dressing changes, there were no statistically significant differences in heart rate, MAP, and SpO 2 between children in the two groups ( P>0.05). Immediately after debridement, compared with those in ibuprofen group, the heart rate and MAP of children in sevoflurane group were significantly decreased (with t values of 8.10 and 4.37, respectively, P<0.05), while the SpO 2 was significantly increased ( t=21.77, P<0.05). At 30 minutes before the start of dressing changes and 30 minutes after the completion of dressing changes, there were no statistically significant differences in the score of pain intensity and score of sedation degree between children in the two groups ( P>0.05). Immediately after debridement, compared with that in ibuprofen group, the score of pain intensity of children in sevoflurane group was significantly decreased, while the score of sedation degree was significantly increased (with t values of 42.87 and 72.45, respectively, P<0.05). The duration of dressing changes and the total number of dressing changes during hospitalization of patients in sevoflurane group were (18±3) min and (4.1±1.0) times, respectively, which were both significantly shorter than (26±7) min and less than (6.6±1.4) times in ibuprofen group, respectively (with t values of -4.44 and 14.17, respectively, P<0.05). Upon discharge, the satisfaction scores of dressing-changing surgeons and the family members of children with the analgesic effects during the process of dressing change in sevoflurane group were significantly higher than those in ibuprofen group (with t values of 44.23 and 36.55, respectively, P<0.05). There were no statistically significant differences in the incidence of respiratory depression, hypoxemia, hypotension, coughing, nausea, and vomiting during the process of dressing change between children in the two groups ( P>0.05). Conclusions:Application of sevoflurane inhalation during dressing changes in children with extensive burns can safely and effectively control pain and sedation, shorten the time for dressing change, with fewer adverse reactions. This method can be used for routine dressing change in pediatric burn wards.
