1.Application of process engineering to remove lignocellulose fermentation inhibitors.

Fermentation inhibitors are toxic to cells, which is one of the bottlenecks for lignocellulose bio-refinery process. How to remove those inhibitors serves a key role in the bioconversion of lignocellulose. This article reviews the sources and the types of the inhibitors, especially the updated removal strategies including physical methods, chemical methods, biological methods and inhibitor-tolerant strain construction strategies. Based on these, we introduce a new bio-refinery model named "fractional conversion", which reduces the production of inhibitors at pretreatment stage, and a novel in situ detoxification method named "fermentation promoter exploitation technology". This review could provide new research ideas on the removal of fermentation inhibitors.

2.Utilization reseach of medical insurance drugs in the hospitals of Suqian district

Objective To analyze the current situation and developing trend of utilizaion of medical insurance drugs.Methods Data were collected from Suqian Hospital.Drug Utilization Analysis System and DDD were taken as the basic unit of measurement.Results The consumption of cost,DDDs and variety of type A medical insurance drugs were rising year by year,but the proportion in total was in declining trend.When analyzed according to DDDc,the type A,non medical insurance drugs and the type B ranked the first,the second and the third place during the 2009-2010,the DDDc of type A was lower than the non medical insurancedrugs and the type B.For example,the DDDc of type A was 3.32 yuan/day,the DDDc of the non medical insurance drugs and the type B was 15.04 and 15.97 yuan/day respectively.Conclusion Analyzed in respect to DDDs,the protortion of type A was in high level and the type B was in low.But analyzed in respect to the comsumption cost,the protortion of type B was in high level and the type A was in low.
3.Rotary self-locking intramedullary nail for treating long tubular bone fracture of extremities

Objective To evaluate the results of rotary self-locking intramedullary nail (RSIN) for treating long tubular bone fracture of extremities, and discuss the current problems. Methods One hundred and twenty-two patients with long tubular bone fracture of extremities,including 59 femoral fractures,57 tibial fractures and 6 humeral fractures,who had been treated by RSIN were retrospectively investigated. Results All of the patients achieved clin-ical healing,with an average time of 24 weeks. Nobody was found to appear maluniun,infection and the break of inter-nal fixture. Internal fixations were removed after the fracture healing,with an average time of 14 months. It was diffi-cult to remove the internal fixations in 5 cases, and one case refractured after removing the internal fixations. Conclu-sion RSIN has the advantages such as easy operation,less trauma,no pendulum effect,early motion after operation and so on, except the characters of general interlocking intramedullary nail such as anti-rotation, anti-crispition and anti-displacement.
4.Risk analysis on duo paternity tests

Objective To discuss the risk in duo paternity testing. Methods 22 fictitious duo families formed by 22 pairs of unrelated individuals having one or zero inconsistent locus were selected after detected by GoldeneyeTM 20A Kit. The 22 fictitious duo families were further tested with STRtyper-10G kit and/or AGCU 21+1 STR kit until there were more than 3 inconsistent loci and the cumulative paternity index(CPI) value was less than 0.0001. According to the three excluding rules, ① number of inconsistent loci>3; ② CPI ≤ 0.0001; ③ accord with both ① and ② , using multiple STR systems, such as 19 STR loci, 26 STR loci, 39 STR loci and 46 STR loci to test and discuss whether there is difference among the excluding result of unrelated individual. Results Among those 22 fictitious duo families, using three excluding rules, None was excluded by 19 STR loci, and all was excluded by 39 STR loci. Conclusion Duo paternity tests may get a wrong result using only 19 loci system. To reduce the error risk 39 STR-loci systems would be suggested.
5.Comparison and Enlightenment of the Contents of Pharmaceutical Administration in Hospital Accreditation Standards between China and JCI

OBJECTIVE:To provide reference for further refinement and improvement of pharmaceutical administration in hospital accreditation standards in China.METHODS:According to Joint Commission International Accreditation Standards for Hospitals (6th edition) [called "JCI standards (6th edition)" for short] and Implementation Rules of Level 3 General Hospital Accreditation Standards in China (2011 edition) [called "Implementation Rules of Standards (2011 edition)" for short],the similarities and differences of pharmaceutical administration were studied and compared;the advantages and disadvantages of Implementation Rules of Standards (2011 edition) were analyzed to put forward some suggestions on this basis.RESULTS:The number of the items,standards and key points of pharmaceutical administration in JCI standards (6th edition) were 8,22,89,respectively.The number of above indexes in Implementation Rules of Standards(2011 edition) were 11,39,280,respectively.The similarities mainly reflected in relevant laws,regulations,rules and regulations to be followed in pharmaceutical administration,personnel requirements,drug procurement,reserve,storage and recall,drug dispensing and preparation,special drugs management,prescription management,antibiotics management,drug use monitoring,ADR reporting,etc.The specific regulation of JCI standards (6th edition) involved drug management system documentation review,drag acquisition at night or after pharmacy closed,drug list examination,comparison of drug use list before admission and the first medical order,etc.The specific regulation of Implementation Rules of Standards (2011 edition) involved the promotion of National Essential Medicine System and the construction of clinical pharmacist system.CONCLUSIONS:In China,Implementation Rules of Standards (2011 edition) have clearly defined the National Essential Medicine System and the construction of clinical pharmacist system,and are more in line with the development requirements of medical and health services in China at the present stage;but there are also some deficiencies compared to JCI standards (6th edition),including the revision update,the management of the patient's own medicine,the safety management of the patient's identity,measurability of the accreditation standards.Implementation Rules of Standards should be further refined and perfected by updating version regularly,strengthening the management of details,increasing measurability of the accreditation standards,strengthening drug management outside the department of pharmacy,enhancing drug supply management,etc.
6.Diuretic efficacy of torasemide in nephrotic syndrome: bolus injection versus continuous infusion

Objective To compare the diuretic efficacy of torasemide as a 2-hour continuous infusion and as a bolus injection of equal dose in patients with nephrotic syndrome,and to investigate a preferable administration mode of torasemide for these patients.Methods Twenty-three hospitalized patients were randomized to receive torasemide 20 mg or 40 mg per day by either 2-hour intravenous infusion or bolus injection,and interchanged after 48 hours of washout.Results Patients received torasemide by 2-hour intravenous infusion exhibited significantly higher daily urinary volume,chloride excretion,sodium excretion and fractional excretion of sodium (FENa) within 24 hours than those by bolus injection (P < 0.05).Significantly lower bound-state torasemide excretion,higher ratio of urinary volume to torasemide excretion and a markedly larger area under the curve in the plasma concentrationtime profiles were also observed in the infusion group (P < 0.05).Conclusion 2-hour continuous infusion delivers a better diuretic effect compared with a bolus injection of equal dose of torasemide in patients with nephrotic syndrome.
7.Study on the modeling method and pathological parameters of chronic atrophic gastritis

Objective:To explore an ideal method for establishing a mouse model of chronic atrophic gastritis (CAG).Methods:CAG mouse models were established with five different modeling methods ( N-methyl- N′-nitro- N-nitrosoguanide (MNNG), sodium salicylate, sodium deoxycholate, Helicobacter pylori infection, and combinations of them) in BALB/c and C57 mice. The effect of each modeling method was evaluated by histological observation of gastric mucosa, plasma biochemical parameters, inflammatory response score, and the expression of anti-inflammatory factors. Results:The results of histological observation of gastric mucosa showed that all of the 5 methods could successfully establish CAG mouse models. In BALB/c mice, compared with the healthy control group, significant features of CAG accompanied with intestinal metaplasia was found in the model group established by combination of MNNG-free drinking, 2% sodium salicylate and 20 mmol sodium deoxycholate. From the results of serological detection, compared with the normal control group, the mRNA expression levels of related anti-inflammatory factors interleukin-2, interleukin-10, interleukin-13 and growth differentiation factor-15 of each model group decreased, which indicated that the mice of each CAG model group had different degrees of inflammation. The results of plasma biochemical parameters indicated that plasma gastrin of each group decreased and the ratio of pepsinogen Ⅰ and pepsinogen Ⅱ significantly dropped. The above results demonstrated that in BLAB/c mice, MNNG-free drinking, 2% sodium salicylate and 20 mmol sodium deoxycholate was better than other four modeling methods. For C57 mice, it was also found that simple chemical drug mutagenesis and Helicobacter pylori replication method both could successfully establish CAG models. No matter from pathological observation, relative expression of anti-inflammatory factors and analysis of plasma biochemical parameters, the effects of combination of the two methods was better. Conclusion:The CAG mouse model established by MNNG-free drinking, 2% sodium salicylate and 20 mmol sodium deoxycholate can provide a certain reference for the establishment and application of mouse model in CAG experiments in the future for pharmacological research.
8.Effect of Mini-Clinical Evaluation Exercise combined with direct observation of procedural skills in ophthalmology practice training in integrated teaching of sensory system diseases

Objective:To investigate the effect of Mini-Clinical Evaluation Exercise (Mini-CEX) combined with direct observation of procedural skills (DOPS) in ophthalmology practice training in the integrated teaching of sensory system diseases.Methods:The medical students in the five-year program who entered the stage of ophthalmology practice in the integrated teaching of sensory system diseases in The Second Clinical College of Chongqing Medical University, were selected as research group, and the medical students in the five-year program who received training from October 2020 to July 2021 were selected as control group. The two groups were compared in terms of department examination score, progressive evaluation of the research group, and the degree of satisfaction with practice. SPSS 23.0 was used for the t-test. Results:Compared with the control group, the research group had significantly better scores of medical history inquiry, medical record writing, and operational skills ( t=2.47, P=0.016; t=5.26, P<0.001; t=2.20, P=0.031). In progressive training, the Mini-CEX and DOPS scores in the second progressive formative assessment were better than those in the first assessment ( t=-21.06, P<0.001; t=-15.09, P<0.001). The degree of satisfaction with performance among the interns and the degree of satisfaction with interns among the examiners in the second assessment were better than those in the first assessment ( t=-5.53, P<0.001; t=-8.19, P<0.001). Conclusion:Mini-CEX combined with DOPS has a good effect in ophthalmology practice training in the integrated teaching of sensory system diseases and thus holds promise for application.
9.Expression of BATF, a member of the activator protein-1 family, in renal tissues of MRL/lpr mice.

OBJECTIVETo investigate the expression of BATF, a member of the activator protein-1 family, in the renal tissues of mice with lupus nephritis.
METHODSThe renal tissues from 24-week-old female MRL/lpr mice and age- and sex-matched C57BL/6 mice were examined for BATF protein expressions using Western blotting and for expressions of BATF, RORγt and IL-17A mRNA using quantitative real-time PCR. The results of laboratory examinations were analyzed in relation with the histopathology of the mice.
RESULTSThe urinary protein and ds-DNA levels were significantly higher in MRL/lpr mice than in the control mice (P<0.05). Compared to normal control mice, MRL/lpr mice showed obvious glomerular fibrosis and inflammatory cell infiltrating with significantly increased BATF protein and mRNA expressions (P<0.05) and RORγt and IL-17 mRNA expressions in the renal tissues (P<0.05). In MRL/lpr mice, the expression of BATF mRNA was positively correlated with the RORγt and IL-17A mRNA expressions in the renal tissues.
CONCLUSIONThe renal expressions of BATF protein and mRNA is increased in MRL/lpr mice. BATF may participate in the immunopathogenesis of lupus nephritis by enhancing Thl7 cell response.

10. The clinical application value of seven tumor-associated autoantibodies in the diagnosis of non-small cell lung cancer

Objective:
This study was designed to evaluate the clinical value of seven combinedtumor-associated autoantibodies (7-TAAB) in the diagnosis of non-small cell lung cancer (NSCLC).
Methods:
This is a cross-sectional study. The 81 newly diagnosed patients with NSCLC were enrolled. 46 patients with benign pulmonary diseases (BLD) and 55 healthy subjects were selected as the BLD group and the healthy control (HC) group, respectively. ELISA was used to detect the concentration of seven TAABs of p53, PGP9.5, SOX2, GAGE7, GBU4-5, MAGE A1 and CAGE in the serum of the NSCLC and the other two groups. The levels of lung cancer tumor markers CEA, NSE, SCC and CYFRA21-1 in serum were also detected in all enrolled subjects. Kruskal-wallis test was used for comparison among the three groups, Mann-Whitney test was used to evaluate the differences between the two groups, and positivity rates were analyzed by using standard χ2 tests and Fisher exact tests. The receiver operating characteristic (ROC) analyses were performed to evaluate the diagnostic efficacy of 7-TAAB or combination of 7-TAAB and traditional tumor markers.
