1.Characteristics of electroencephalogram in patients with anti-N-methyl-D-aspartate receptor encephalitis

Objective To investigate the characteristics of electroencephalogram (EEG) in patients with anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis. Methods In this retrospective study, patients with anti-NMDAR encephalitis who admitted to Xuanwu Hospital of Capital Medical University from January 2012 to June 2015 were enrolled. All patients accepted EEG monitoring at least once, and tumor screening, symptomatic therapy, as well as immunotherapy. Outcomes were assessed by modified Rankin Scale (mRS) 6 and 12 months after immunotherapy, and mRS score 0-2 was defined as favorable outcome. Results Forty-one patients aged (27±12) (13-58) years were enrolled, 19 of whom were male. All patients received EEG monitoring 55 times totally. Among 41 patients in disease peak period, 39 patients (95.1%) had abnormal EEG, 2 patients had normal EEG. The abnormality of brain MRI scan accounted for 51.2%. Abnormal EEG included diffuse slowing (12 patients, 29.3%), epileptic discharges (9 patients, 22.0%), rhythmic delta activity (6 patients, 14.6%), extreme delta brush (EDB) (6 patients, 14.6%), focal slowing (4 patients, 9.8%), and rhythmic delta frequency activity without EDB (2 patients, 4.9%). Clinical items did not show statistically significant difference between the patients with EDB and those without EDB. The patients with normal EEG, epileptic discharges, rhythmic delta activity, or rhythmic delta frequency activity without EDB all had favorable outcomes after 12 months′ treatments. Conclusions The great majority of patients with anti-NMDAR encephalitis had abnormal EEG. EEG could sensitively reflect the changes of brain function and could be helpful in early diagnosis and treatment of this disease. The most common electrographic pattern was found to be diffuse slowing in patients with anti-NMDAR encephalitis. EEG of some patients showed EDB.
2.Relationship of hyperuricemia with all-cause mortality in patients with atrial fibrillation

Objective:To explore whether hyperuricemia was an independent risk factor for all-cause mortality in patients with atrial fibrillation.Methods:Patients with atrial fibrillation who were confirmed by 12-lead electrocardiogram in 11 hospitals of Kailuan Group from 2006 to 2007 were selected as the research objects.All patients were followed up by prospective cohort study, and all-cause deaths were observed.The last follow-up time was December 31, 2013.Kaplan-Meier curve and Cox proportional hazards model were used to analyze and compare the risk of all-cause mortality in patients with atrial fibrillation in the hyperuricemia group compared with the normal uric acid group.Results:A total of 388 community-based patients with atrial fibrillation were included in the final statistical analysis, with 136 all-cause deaths occurred during an average follow-up period of 6.93 years.The incidence of all-cause mortality was 9.24% per year(36/390)in the hyperuricemia group, whereas 5.16% per year(100/1 937) in the normal uric acid group.In the univariate Cox proportional risk model analysis, the risk ratio (95% CI) of all-cause death in patients with atrial fibrillation in the hyperuricemia group (95% CI) was 1.84(1.26-2.69) times that in the normal uric acid group ( P<0.01). After adjusting for potential confounding variables, the adjusted risk ratio (95% CI) of all-cause death in patients with atrial fibrillation in hyperuricemia group was still 1.94(1.32-2.85) times of that in normal uric acid group ( P<0.01). After adjustment for potential confounding variables, for each 0.01 g/L increase in uric acid (1 g/L=5 950 μmol/L), the risk of all-cause death in patients with atrial fibrillation increased by 1.15 (1.05-1.26) times ( P<0.01). Conclusion:Hyperuricemia was an independent risk factor for all-cause death in patients with atrial fibrillation in community.
3.MRS in quantitative assessment of lumbar discs degeneration

Objective To evaluate the value of MRS in quantitative assessment of degeneration of lumbar discs.Methods Totally 82 patients with lumbago underwent lumbar MR scanning.All the discs were classified with Pfirrmann grades in the sequences of sagittal T2WI.The area under N-acetyl peak,under water peak and the ratio of N-acetyl/Water were measured by MRS.Correlation between MRS values and Pfirrmann grade,age were analyzed.Results In 82 patients,204 lumbar discs were measured by MRS.There were 89,73,39,3 discs in Pfirrmann Ⅱ,Ⅲ,Ⅳ,V respectively.The areas of N-acetyl,water peak and N-acetyl/Water ratio of nucleus region were positively correlated with Pfirrmann grading,respectively (rs =-0.460,-0.204,-0.526,all P<0.05).There were 62,25,37,51,29 discs in patients aged <30,30-39,40-49,50-59,>59 years respectively.The ares of N-acetyl peak,N-acetyl/Water ratio of nucleus region was negatively correlated with the age (rs=-0.247,-0.385,both P<0.05).Conclusion MRS can be used for quantitative assessment of lumbar discs degeneration.
4.The influence of humanistic management atmosphere on nurses′ intention to stay

Objective:To explore the influence of humanistic management atmosphere on nurses′ intention to stay in hospital, so as to provide reference for stabilizing nursing team.Methods:The convenience sampling method was used to investigate 1 044 clinical nurses in 5 tertiary hospitals in Shandong province by using the general information questionnaire, nurses′ perception of humanistic management questionnaire and nurses′ intention to stay questionnaire in May 2019.Four dimensions of demographic factors and nurses′ perception of hospital humanistic management were used as independent variables, and the total score of nurses′ intention to stay was used as dependent variable for stepwise regression analysis.Results:The total scores of humanistic management perception and intention to stay were 166.70±29.42 and 23.30±4.29 respectively. There was a positive correlation between humanistic management perception and intention to stay( P<0.05). The three dimensions of human management perception, emotional management perception and cultural management perception, as well as the professional title, working years and employment form could explain 27.5% of variation. Conclusions:The nurses′ intention to stay is in the middle and upper level, which is related to the hospital′s humanistic management. The perception of humanistic management of nurses has a positive predictive effect on the intention to stay, especially in talent management, emotional management and cultural management. Hospital managers should improve the humanistic management mode, enhance nurses′ perception of humanistic management in order to stabilize the nursing team and improve nurses′ intention to stay.
6.The 482nd case: vomiting, paroxysmal dizziness and anhidrosis

A 31-year-old woman was admitted to Peking Union Medical College Hospital presented with intermittent vomiting and abdominal pain for 2 years, and recurrence with paroxysmal dizziness for 1 month. This patient was diagnosed with systemic lupus erythematosus (SLE) 2 years ago with involvement of gastrointestinal and urinary tracts. One month ago, repeated vomiting and nausea recurred. No laboratory and imaging abnormalities were found in central nervous system and gastrointestinal evaluation. Orthostatic hypotension and fluctuation of blood pressure were recorded during hospitalization. Combined with sexual dysfunction, left adie pupil, anhidrosis and abnormal sympathetic skin response, autonomic nerve dysfunction related to SLE was diagnosed. After treated with pulse glucocorticoids and intravenous immunoglobulin, the patient′s symptoms improved remarkably. Orthostatic hypotension in SLE patients may link to autonomic nerve dysfunction.
7.Areca catechu L.ameliorates chronic unpredictable mil stress-induced depression behavior in rats by the promotion of the BDNF signaling pathway

OBJECTIVE To investigated the anti-de-pressant effects of the fruit Areca catechu L.(ACL)and elucidated its potential underlying mechanism using a rat model of chronic unpredictable mild stress(CUMS).METHODS CUMS was induced in rats to establish a depression animal model for 28 d.According to the baseline sucrose preference,the male rats were divided into six different groups.They were treated with parox-etine hydrochloride,ACL,and water once a day until the behavioral tests were performed.The levels of corticoste-rone(CORT),malondialdehyde(MDA),catalase(CAT),and total superoxide dismutase(T-SOD)in serum were de-tected using a commercial kit,and the concentrations of 5-hydroxytryptamine(5-HT)and dopamine(DA)mono-amine neurotransmitters in the brain tissues were detect-ed by liquid chromatography-tandem mass spectrometry.Doublecortin(DCX)expression in the hippocampal den-tate gyrus(DG)was determined by immunofluorescence,and the relative abundance of brain-derived neurotrophic factor(BDNF),TrkB,PI3K,p-Akt/Akt,PSD-95,and p-GSK-3β/GSK-3β of brain tissues were assayed by West-ern blotting.RESULTS ACL markedly increased sucrose preference,decreased the immobility time,and short-ened the feeding latency of CUMS-induced rats.CUMS induction resulted in marked changes in the contents of the monoamine neurotransmitters(5-HT and DA)in the hippocampus and cortex of brain tissues and the levels of CORT,MDA,CAT,and T-SOD in serum,whereas ACL administration alleviated these considerable changes.ACL promoted DCX expression in DG and increased the protein levels of BDNF,TrkB,PI3K,p-Akt/Akt,PSD-95,and p-GSK-3β/GSK-3β in the brains of CUMS-induced rats.CONCLUSION Our results indicated that ACL may improve depression-like behaviors in CUMS-induced rats by decreasing the hyperfunction and oxidative stress of the hypothalamic-pituitary-adrenal axis,stimulating hippo-campal neurogenesis,and activating the BDNF signaling pathway.
8.Association of Galectin-3 with atrial fibrillation

Objective:To investigate the effect of galactose lectin 3 (Gal-3) on the pathogenesis of atrial fibrillation.Methods:This study adopts a case-control study method. 55 patients with non valvular atrial fibrillation (atrial fibrillation group) admitted to the First People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from February to July 2019 were selected, and another 55 healthy individuals who underwent physical examination at our hospital during the same period were selected as the control group. Compare the general data and levels of various laboratory indicators between two groups, including blood routine, fasting blood glucose, blood lipids, liver and kidney function, and plasma Gal-3. Analyze the influencing factors of atrial fibrillation and the predictive value of plasma Gal-3 levels for the onset of atrial fibrillation. The measurement data with normal distribution and the measurement data converted to normal distribution after taking natural logarithm are expressed in xˉ± s. The comparison between the two groups is performed by independent sample t test; The measurement data of non normal distribution is represented by [ M ( Q1, Q3)], and Wilcoxon signed rank sum test is used for inter group comparison; The counting data is represented by examples (%), and the comparison between groups is conducted using χ 2 test. The influencing factors of atrial fibrillation were analyzed using logistic regression analysis. Results:The age, NLR, and blood creatinine levels in the atrial fibrillation group were higher than those in the control group [(71.16±9.17) years vs (60.71±10.11) years, (2.32±0.85) vs (1.74±0.81), (74.18±21.61) μmol/L vs (64.69±18.30) μmol/L, t-values are 5.68, 3.66, 2.48, P-values are <0.001, <0.001, 0.015], total cholesterol, HDL-C, LDL-C Albumin and eGFR water were on average lower than those in the control group [(4.31±1.67) mmol/L vs (5.13±0.78) mmol/L, (0.96±0.21) mmol/L vs (1.21±0.32) mmol/L, (2.35±0.65) mmol/L vs (3.04±0.62) mmol/L, (39.58±3.83) g/L vs (44.66±5.61) g/L, (94.84±29.22) mL/(min·1.73 m 2) vs (111.77±21.51) mL/(min·1.73 m 2)] ,The t-values are 3.30, 4.87, 5.69, 5.54, 3.46, and the P-values are 0.001,<0.001,<0.001,<0.001, 0.001, respectively. The plasma Gal-3 levels in the atrial fibrillation group were higher than those in the control group [(12.79±4.24)] μg/L vs (7.31±2.28) μg/L], the difference was statistically significant ( t=8.43, P<0.001), and the plasma Gal-3 level in the persistent atrial fibrillation group was higher than that in the paroxysmal atrial fibrillation group [(14.03±3.95) μg/L vs (11.51±4.21) μg/L], the difference was statistically significant ( t=2.29, P=0.026). The results of multivariate logistic regression analysis showed that after excluding other factors, Gal-3 remained an independent influencing factor for atrial fibrillation (odds ratio=1.66, 95% confidence interval: 1.29-2.12, P<0.001). Conclusions:Plasma Gal-3 is an influencing factor for the onset of atrial fibrillation. After excluding other factors, Gal-3 remains an independent influencing factor for atrial fibrillation, with an increase of 1 μg/L in Gal-3 increases the risk of atrial fibrillation by 1.66 times.
9.Measurements of glomerular filtration rate with iohexol plasma clearance: method selection and evaluation

Objective To compare the consistency between single-(I-GFR-SS) and dual-(I-GFR-DS) sample methods with three-sample method (I-GFR-TS) of iohexol plasma clearance in chronic kidney disease (CKD) patients for choosing the optimizing project of glomerular filtration rate (GFR) measurement.Methods The multiple-sample methods were performed in 174 patients with CKD admitted to the Department of Nephrology,Shanghai Ruijin Hospital from August 2017 to July 2018.Plasma concentrations of iohexol were measured three times at different time points after receiving 5 ml iohexol (300 g/L) intravenous injection,according to estimated GFR (eGFR) grouping.The first blood sample was collected at 2 hours,and the time for the last sample was delayed from 4 hours to 6 hours with reduction of eGFR.The synchronized Gates (99mTc-Gates-GFR) method was detected as control.With I-GFR-TS as the golden standard,the accuracies of I-GFR-DS,I-GFR-SS and 99mTc-Gates-GFR were compared.Results The median differences of I-GFR-DS,I-GFR-SS and 99mTc-Gates-GFR in overall patients were-0.15,-1.00,6.76 ml· min-1· (1.73 m2)-1 comparing with I-GFR-TS;P10(percentage of the GFRmeasurements that was within 10% of the standard method) were 95.4%,74.1%,28.7%,and P30 were 100%,93.7%,72.4% separately.In the patients with eGFR < 30 ml· min-1· (1.73 m2)-1,I-GFR-SS was more accurate when last point collecting extended to 6 h from 4 h [P10:43.5% vs 17.4%,P=0.055;P30:73.9% vs 43.5%,P<0.05].Conclusions The dual-sample plasma clearance of iohexol is recommended in clinical practice,and the single-sample method can be a secondary option because of its slightly poor accuracy but more convenient.Sample-collection protocol should be adjusted according to eGFR especially in moderate-to-severe CKD patients.The Gates method is not recommended.
10.The clinical effect of modified arrow flap for nipple reconstruction

Objective:To investigate the clinical effect of modified arrow flap for nipple reconstruction.Methods:From Jan. 2018 to Oct. 2019, 10 patients in Nanjing Hospital of Traditional Chinese Medicine who received the modified arrow flap for nipple reconstruction were collected. The rate of nipple retraction, patient satisfaction and the incidence of local flap complications were evaluated.Results:The operation time of 10 patients was 13-18 minutes, and the average operation time was (15.10±1.52) minutes. All patients were followed up for 3-24 months, with an average follow-up time of (14.60±1.07) months. The retraction rate of nipple height was 23.5%-33.2%, the average retraction rate of nipple height was (28.53±3.02) %, the diameter retraction rate of nipple was 7.1%-10.5%, and the average diameter retraction rate of nipple was (8.92±1.05) %. The patients’ satisfaction was 96%-100%, with an average of (97.9±1.60) %. None of the 10 patients had complications such as bad wound healing, incision dehiscence or flap necrosis.Conclusion:The design of the improved arrow flap is simple, the operation is simple, the curative effect is satisfactory, and the incidence of postoperative complications is low.
