[Objective] To observe the effect of Rebi Kang Capsule (RKC) for damp-heat rheumatoid arthritis. [Methods] Eighty-six cases of damp-heat rheumatoid arthritis (Group A) were treated with RKC (mainly composed of Radix Gentianae Macrophyllae, Radix Ledebouriellae, Ramulus Cinnamomi, Radix Clematidis, etc.) and 40 cases treated with diclofenac sodium (served as control, Group B) . 2.Accuracy of ottawa ankle rules to exclude fractures of the ankle and mid-foot

ObjectiveTo identify the reasons of missed diagnosis with ankle sprains by analyzing OAR in order to guide the rational use of OAR.MethodsOutpatient with sprained ankles were recruited from the surgical clinic in Sichuan Luzhou Medical College from Mar.2005 to Mar.2010.OAR was used for clinical diagnosis of ankle sprains.The accuracy of OAR diagnosis was examined within 24 h and at 15 d by X-ray.ResultsIn the 105 cases of ankle sprain,16 cases was diagnosed of fractures by OAR.X-ray results showed I case of missed diagnosis,with the rate of missed diagnosis of 6.2%.While both OAR and X-ray diagnosed 21 cases in 50 patients with sprained foot fractures,with a rate of missed diagnosis of 0.Conclusion OAR diagnosis for ankle sprains and fractures has high success rate although with the possibility of missed diagnosis which needs attention clinically.
3.Role of vascular endothelial growth factor, interleukin-2 and tumor necrosis factor-alpha in diabetic retinopathy

BackgroundDiabetic retinopathy (DR) is a progressive vision-threatening complication of diabetes mellitus,but its pathogenic mechanism is still unclear. Recent studies showed that it may be associated with the inflammation response of retinal capillary. Cytokines can cause induction of proinflammatory and adhesion molecules and thereby increase monocyte endothelial cell adhesion, which is now accepted as the early key event in the development of DR. ObjectiveThe present study was to determine the relationship between the stages of DR and the levels of serum vascular endothelial growth factor (VEGF) , interleukin-2 ( IL-2 ), tumor necrosis factor-alpha (TNF-α) in diabetic patients. Methods This was a pilot case-controlled study. Ninety patients with type 2 diabetes mellitus were included in this clinical trial and 30 healthy individuals were enrolled as controls. The patients were grouped into the non-diabetic retinopathy(NDR) group,background DR group and proliferative DR(PDR) group according to the results from ophthalmoscopic examination and fundus fluorescein angiography(FFA) ,with 30 patients for each group. The levels of serum VEGF,IL-2,TNF-α were assayed by ELISA and compared among the 4 groups.Written informed consent was obtained from each subject before received any related medical examination to this study. ResultsThe mean serum VEGF levels were(217.35±27. 87)ng/L,(298.31±49.26)ng/L,and(341.23±40. 18)ng/L, respectively, and mean serum IL-2 levels were( 12. 12± 1. 57 )ng/L, (16.43 ±2. 26 )ng/L, and (21.36±0. 86) ng/L,respectively and mean serum TNF-α levels were( 11.63±0. 94) ng/L, ( 17. 52±0. 65) ng/L,and(22. 01±0. 87 ) ng/L respectively in the patients with NDR ,background DR and PDR, showing significant differences from healthy controls with( 193.46±37. 39 ) ng/L for serum VEGF, ( 8. 99 ±0. 57 ) ng/L for serum IL-2 and ( 7.31 ±0. 52 ) ng/L for serum TNF-α ( F =126. 38, P<0. 0 1 ;F =120. 37, P<0. 01 ;F =99. 84, P<0. 01 ). The levels of serum VEGF, IL-2, and TNF-α in the patients with the NDR,background DR and PDR were increased significantly. The level of serum VEGF showed the positively significant correlation with serum IL-2 level and TNF-α level ( r =0. 749, P < 0.01 ; r =0. 631,P<0. 01 ). The serum levels of VEGF, IL-2 and TNF-α showed a significantly positive correlation with the prolongation and severity of DR(r=0. 791 ,P<0. 01 ;r=0. 665 ,P<0. 01 ;r=0. 632,P<0. 01 ). ConclusionsVEGF, IL-2 and TNF-α play active roles in the generation and development of diabetic retinopathy, and the level of serum VEGF is closely associated with the levels of serum IL-2 and TNF-α. during the development of DR.
4.Toxic effects of participate matter components in vivo on the NOS/NO system in the endothelial cells from rat thoracic aorta

Objective To investigate the effects of different fine particulate matter (PM2.5) components (water-soluble, organic, water-soluble+organic) in vivo on the NOS/NO system in the endothelial cells from rat thoracic aortas. Methods Three PM2.5 components were used to inject by vein in rat tail. ELISA and immunohistochemical technique were used to examine the change of NOS/NO system in the blood-serum and the endothelial cells from rat thoracic aorta. Results Compared with control group, three PM2.5 components increased NO level of rat blood-serum [control, (38.00±5.40) μmol/L; water-soluble group, (44.66±16.59) μmol/L, t =3.58, P< 0.05; organic groups, (58.28±12.30) μmol/L, t =12.85, P <0.01; mixtural component group, (84.02+19.24) μmol/L, t =31.39, P<0.01]; increased iNOS level of rat blood-serum [control, (17.47±5.33) U/ml; water-soluble group, (21.87±4.63) U/ml, t =4.92, P <0.05; organic groups, (25.27±6.58) U/ml, t = 6.22, P<0.05; Mixtural component group, (32.79±5.86) U/ml, t =32.84, P <0.01]. Mixtural component decreased eNOS level of rat blood-serum [(12.58±4.59) pg/ml vs (17.47±2.97) pg/ml, t =5.91, P <0.05], no significant difference on eNOS level of blood-serum among other component groups (P >0.05). Three components decreased the expression of eNOS protein in the endothelial cells from rat thoracic aorta(control, 164.68±4.80; water-soluble group, 185.43±5.63, t =4.26, P <0.05; organic groups, 194.07±9.77, t =5.26, P <0.05; Mixtural component group, 209.24±15.43, t =43.19, P <0.01), increased the expression of iNOS protein (control, 160.17±5.79; water-soluble group, 134.78±8.22, t =5.21, P <0.05; organic groups, 134.78±8.22, t =5.98, P <0.05; Mixtural component group, 81.62 ±10.59, t =34.98, P <0.01) in the endothelial cells from rat thoracic aortas.Conclusion Three components of PM2.5 induced the dysfunction of endothelial cells and disorder of NOS/NO system in the endothelial cells.
5.Echocardiography in evaluation of unroofed coronary sinus

Objective To conclude the diagnostic methodology of unroofed coronary sinus (UCS) by two-dimensional echocardiography(2DE). Methods By analyzing the echocardiographic results of 16 UCS patients who were diagnosed by 2DE and confirmed by operation, the diagnostic methodology of UCS by 2DE was summarized. Results Sixteen patients with UCS were involved in this study. Among them, 12 cases were diagnosed as complete UCS,others were partial UCS(PUCS). Twelve patients were complicated with persistence of left superior vena cava (LSVC) that was connected to left atrium (LA) through UCS. All of preoperative diagnosis conducted by 2DE were finally confirmed to be consistent with the results of operation. With the analysis of acquired echo images,key points of diagnosis were concluded as follow: 1) normal coronary sinus (CS) could not be detected in the routine 2DE views referring CS. PUCS showed partial absence of CS roof,while complete UCS displayed as total absence of CS. 2)Inter-atrial shunt would definitely be found in UCS and the opening to right atrium must be coronary sinus orifice. The shunt direction was depended on the combined cardiac malformations. 3) With the occurrence of UCS,LSVC would be in junction with LA through UCS. 4) The inter-atrial shunt resulted in cardiac morphologic and hemodynamic changes. Conclusions Better understanding of the anatomic, morphological and hemodynamic characteristics of UCS would greatly contribute to accurate diagnosis on UCS.
6.Expression of Heat Shock Proteins 27,70 and 60in Psoriatic Epidermal Keratinocytes

Objective To investigate the role of heat shock poteins(HSPs)in the pathogenesis of psoriasis.Methods Expression of HSP27,HSP70and HSP60was detected by immunohistochemical and image analysis in epidermal keratinocytes of pre-and post-treatment psoriatic lesional,and non-lesional skin in25psoriatic patients and6healthy controls.Results There was constitutive expression of HSP27and HSP70in epidermal keratinocytes of psoriatic non-lesional skin and normal controls,and the weak expression in psoriatic lesions.There was expression of HSP60in keratinocytes of psoriatic skin,but no expression of HSP60in non-lesional and normal skin.Expression of HSP27and HSP70recovered gradually in post-treatment psoriatic lesions,and no expression of HSP60was observed in lesional skin after treatment.Conclusion HSPs may play a certain role in psoriatic stress protective mechanism.
7.Discussion on Current Situation and Developing Policy for Medical Digest Periodicals in China

Objective Through the investigation and analysis to "Chinese Medical Digest" and its relative periodicals to get a general idea of the survival and situation of these periodicals and to establish the pattern for their survival and development. Methods Such methods as telephone consultation, questionnaire survey, interview, and conference discussion were adopted to collect the relative information of medical digest periodicals and thus an analysis was made upon it. Results There were 18 kinds of "Chinese Medical Digest" periodicals originally. Of all these 18 kinds of the periodicals, 6 kinds are still publishing, 10 kinds have transferred its nature to first-hand literature periodical, and 2 kinds have ceased their publishment. Conclusion As a result of the computer, the network and the full text database development, the survival of medical digest periodicals are endangered. However, regarding to the different domain of subject, different reforming measures should be taken.
8.Evaluation on the ventricular dyssynchrony in patients with pulmonary hypertension using velocity vector imaging

Objective To determine whether strain and strain rate derived from velocity vector imaging ( VVI) allowed assessment of regional and global right ventricular ( RV) dysfunction and intra‐and inter‐ventricular dyssynchrony in patients with pulmonary hypertension ( PHTN) . Methods Ninety‐nine patients were enrolled , including 35 controls and 64 PHTN patients . All participants underwent echocardiography examinations and off line strain/strain rate analysis . Bi‐ventricular regional peak systolic strain ,strain rate ,and the time‐to‐peak systolic variables were measured at the RV free wall ,inter‐ventricular septum ,and LV lateral wall in apical four chamber views . Standard deviations of the time‐to‐peak systolic strain (SD‐Ts) and strain rate (SD‐Tsr) at six RV and three left ventricular (LV) segments were calculated to define intra‐and inter‐ventricular dyssynchrony . Results Compared with the controls , segmental and global peak systolic strain and strain rate decreased in the PHTN patients . And intra‐right ventricular dyssynchrony was present in the PHTN patients [SD‐Ts:(28 ± 20)ms in controls vs (61 ± 62) ms in patients , P = 0 .003 ] . The difference between the time‐to‐peak systolic strain rate at the basal segment of RV free wall and LV lateral wall indicated earlier RV free wall contraction in PHTN patients , relativetoLVlateralwall,particularlyinthoseseverePHTNpatients[(13±13)msincontrolsvs( -49± 14) ms in patients , P < 0 .01] . Conclusions PHTN impairs RV myocardial contraction and induces to ventricular mechanical dyssynchrony .VVI could monitor RV function during the treatment of PHTN .
9.Effect of Notoginsenoside on Cerebral Infarcted Neurotic Dysfunction

Objective: To study the effect of notoginsenoside in various doses on local ischemic neurotic dysfunction and the ultrastructure. Methods: 35 Wistar rats were divided at random. The reversible middle cerebral artery occlusion (MCAO) model was made according to the assessment of the literature. The changes of SEP contents of cerebral cortex NO and SOD, and the changes of the ultrastrcture were recorded by evoked potential electrograph before and after MCAO. Results: notoginsenoside 200mg?kg -1 and 400mg?kg -1 could remarkably alter MCAO, shorten the latent period, improve the dysfuntion induced by MCAO, lower the concentration of NO and enhance the activity of SOD. Objective To observe the effects of polysaccharide extracted from Yangwei Mixture on immunologic functions in mice.Method Immuno-suppressant mice model was established by intraperitoneal injection of CTX.The effects of the polysaccharide were observed through phagocytosis test of mice celiac macrophagocyte and serum hemolysin level test as well as lymphocyte transformation test.Result The polysaccharide can obviously increase the percentage of phagocytosis of mice celiac macrophagocyte,accelerate the formation of hemolysin and promote the lymphocyte transformation.Conclusion Polysaccharide extracted from Yangwei Mixture is a good immuno-stimulant,and can counteract the immuno-suppression induced by CTX.
