This thesis discourse upon blood stagnant in cerebral venation is the pivotal pathogenesis of cerebral ischemia stroke,and discuss once the cerebral ischemia occur,it can derive more phlegms,toxic heat and other pathological outcomes,resulting in inflammatory injury of cerebral ischemia,forming a vicious circle of "inflammation-thrombus",so that aggravating the injury with ischemia and anoxia of never cells,even causing inreversible putrescence of cerebral ischemia.And this condition should be the springs of the development and aggravation of the illness.Therefore,applying promoting blood circulation for removing blood stasis measures can removing blood stasis,eliminating pathogenic wind-fire and cleanning out phlegm,so the pyogenic infections can be dispelled,and the pathological state of inflammation,injury and microcirculation obstacle in the brain can be meliorated.Then,the brain vascula become smooth and the blood supply resume,the symptoms with never functional impairment alleviate or meliorate,thereby,the rate of death and cripple decrease obviously.
2.Efficacy of rehabilitation in open wards on chronic schizophrenics

ObjectiveTo explore the effect of rehabilitation in open wards on chronic schizophrenic patients.Methods48 chronic schizophrenic patients were shifted to open wards with comprehensive rehabilitational therapy.They were evaluated with Positive And Negative Symptoms Scale(PANSS),Nurse's Observation Scale for Inpatient Evaluation(NOSIE),Social Disability Screening Schedule (SDSS) before and 6 months after the shifting.ResultsThere was significant difference in the total scores, negative symptoms scales score, general psychopathological scales score and response deficient factor score of PANSS, all the factors scores except psychiatric manifestations and depression of NOSIE, and scores of SDSS pre- and post-shifting(P<0.01).ConclusionRehabilitation in open wards can improve the negative symptoms and social function of chronic schizophrenics.

Objective: To investigate the relationship between P27,P53 and PCNA expression in human gastric carcinoma tissues and clinicopathological parameters. Methods: The expression of P27,P53 and PCNA in 62 human gastric carcinoma tissues was examined with immunohistochemistry SP method. Results: Positive rates of P27,P53 and PCNA expression were 37.1%, 40.4%,83.9%. P27 expression was related with Bormann type, infiltrative depth, lymph node and distant metastasis and clinical stage. P53 expression was related with sex of patients, distant metastasis and clinical stage. PCNA expression was related with age of patients and infiltrative depth of tumor. P27 positive expression group was higher than negative group as to 5-year survival. P27 expression was in reverse relation with PCNA expression. Conclusion: The expression of P27, P53 and PCNA may be regarded as an important marker in judging malignant degree of gastric carcinoma,distant metastasis and prognosis.
5.Changes of protein kinase-like endoplasmic reficulum kinase and glucose-regulated protein 78 expression in rats after focal ischemic preconditioning

ObjectiveTo investigate the effect of focal ischemic preconditioning (IPC) on the expression of protein kinase-like endoplasmic reticulum kinase ( PERK ) and glucose-regulated protein 78 (GRP78) mRNA and protein after focal cerebral ischemia/reperfusion (I/R) in rats.MethodsAll 120 male SD rats were randomly divided into three groups: sham-operation group,middle cerebral artery occlusion (MCAO) group and brain ischemia preconditioning (BIP) group.Each group was further divided into 4 subgroups according to 12 h,1,2 and 3 d after I/R.The IPC models were made in order to measure the expression of PERK,GRP78 mRNA and protein by in situ hybridization and Western blot,and the apoptosis rate of neuron by flow cytometry. Results ①The expression of PERK mRNA increased and reached the peak at 12 h,then decreased continuously after 1 d.BIP could decrease its expression.The expression of PERK protein increased at 12 h and reached the peak at 24 h,then decreased continuously after 2 d.BIP could decrease its expression.②The expression both of GRP78 mRNA and its protein all increased and reached the peak at 12 h,then decreased continuously.BIP could increase their expression (mRNA:12 h: 136.70±9.53,F=32.265; 24 h:147.54 ±9.97,F=54.920; 2 d:158.16 ±9.44,F=45.374; 3d: 165.85±10.26,F=16.493,P<0.05; protein:12 h: 1.319±0.116,F=5.619,P<0.05; 24 h: 1.226±0.108,F=33.742,P<0.01; 2 d:1.183 ±0.112,F =46.556,P <0.01; 3 d:1.115± 0.098,F =11.730,P<0.05).③The rate of apoptosis neuron of rats in MCAO increased markedly at 12 h after reperfusion,and reached the peak at 1 d,then decreased continuously.BIP could decrease the rate of apoptosis neuron. Conclusion BIP can protect neurons through inhibiting the expression of PERK and inducing the expression of GRP78 after endoplasmic reticulum stress in rats.
6.Relaxation Training and Sleep related Behavior Modification in Patients with Insomnia

Objective:to do clinical study on effect of sleep-related behavior modification and relaxation training on insomnia.Method:44 outclinic patients with insomnia received sleep-related behavior modification and relaxation training for 8 weeks. Using self-designed inventory to assess the outcome.Result:the time of sleep increased since the third week, and increased continuously. After the intervention, the rate of satisfaction about sleep increased from 23% at the baseline to 86.1%. 36 of the 44 patients completed the 8-week clinical study. Conclusion:sleep-related behavior modification and relaxation training can improve sleep of patients with insomnia.

AIM: To evaluate the effect and safety of Rhodamine 123 (Rh123)-mediated photodynamic treatment (PDT) on acute graft versus host disease. METHODS: An acute graft versus host disease (aGVHD) mice model was established using C57B/6 mice as donors and BALB/c mice as recipients. Mixed lymphocytic cells were cultured with Rh123 (50 nmol/L) and irradiated by argon laser 30 mW/cm2 for 3 min, then transplanted to BALB/c recipient mice mixed with donor bone marrow. Hepatopoietic recovery, aGVHD occurrence, survival time after transplantation and pathological changes were observed. In addition, CD3+CD69+ positive rates of MLC were examined by flowcytometry. RESULTS: Occurrence of aGVHD decreased, degree of pathological manifestation became milder, survival rates were higher than non PDT groups. CD3+CD69+ rates of MLC cells treated with photodynamic therapy (PDT) and cultured for 24 h significantly decreased. CONCLUSION: Rh123-mediated PDT can effectively prevent aGVHD of allogeneic bone marrow transplantation in mice.

9.Prevalence and relevant factors on echocardiographic left ventricular hypertrophy among community-based hypertensive patients in Shanghai.

OBJECTIVETo investigate the prevalence and relevant factors on the echocardiographic left ventricular hypertrophy (LVH).
METHODA cross-sectional study was conducted among the hypertensive patients in an urban community.
RESULTSThe prevalence of LVH was 29.2% in 1 686 hypertensive patients, with 25.4% in males and 34.5% in females, respectively. The prevalence was significantly higher in females than in males (chi(2) = 16.17, P < 0.01). The rate was increasing with age and significantly higher prevalence was observed in 45-, 55-, 65- age groups of females (P < 0.05). Moreover, elevated systolic blood pressure and higher BMI were related to the LVH in hypertensive patients, while higher education level seemed a protective factor.
CONCLUSIONThese results implied that a comprehensive intervention should be taken in the prevention of LVH.

We produced (S)-4-cyano-3-(4-chlorophenyl)-butyrate by highly stereoselective biocatalyst in this study. A nitrilase-producing strain, named Gibberella intermedia WX12, was isolated by 3-(4-chlorophenyl)-glutaronitrile as substrate in the screening with phenol-sodium hypochlorite method. The fermentation conditions and catalytic properties of this strain were investigated. The preferred carbon and nitrogen sources for nitrilase production were lactose (30 g/L) and peptone (20 g/L). After being cultivated for 96 h, the cells were collected for use in biotransformation. The hydrolysis of 3-(4-chlorophenyl)-glutaronitrile was performed at 30 degrees C in phosphate buffer (pH 8.0, 50 mmol/L) for 24 h to give (S)-4-cyano-3-(4-chlorophenyl)-butyric acid with 90% yield and > 99% of ee, which can be used for the synthesis of (R)- and (S)-baclofen. The configuration of product was determined by chemically converting it to baclofen and comparison with the authentic sample by chiral HPLC analysis.
