1.Delayed diagnosis of narcolepsy and differential diagnosis and treatment of epilepsy

Narcolepsy is a rare sleep-wake rhythm disorder in clinic practice, mainly characterized by recu-rrent unstoppable sleep during the day and often accompanied by cataplexy, sleep paralysis and hypnagogic hallucinations.Clinicians′ insufficient knowledge about narcolepsy is one of the main causes of misdiagnosis and delayed diagnosis.Moreover, narcolepsy may get easily confused by epilepsy because of complex and diverse types of epileptic seizures.Therefore, it is necessary to distinguish the two forms each other.When they are comorbidity, the diagnosis and treatment will be much more difficult.In this article, the clinical characteristics of narcolepsy and the causes of delayed diagnosis were analyzed, differential diagnosis between narcolepsy and epilepsy was investigated, and practical expe-rience in diagnosis and treatment of comorbidities were summarized, so as to raise clinicians′ awareness of narcolepsy and its comorbidity with epilepsy and improve patients′ prognosis and their quality of life.
2.HBV Contaminated Blood Worktable Surface: Investigation and Analysis

OBJECTIVE To discuss the possibility of HBV contaminating environment and transmission through the polluted surroundings. METHODS On the basis of that ascertain the contaminated environment by HBV marker(HBVM) and the relationship between HBVM and HBV infectivity in the blood,to discuss the probability of HBV contaminating and be spreading through the circumstance. RESULTS The positive rate of HBsAg and HBeAg was 26.39% and 11.11%,respectively in these specimens.While all of samples in control group were negative.The HBV DNA positive rate was 73.72% and 100.00%,respectively in the positive blood specimens of HBsAg and HBeAg.The infectivity was 0-10~9 and 10~2-10~9 ID/ml,respectively. CONCLUSIONS The blood worktable surface is contaminated by HBVM.The pollution is from patients′ blood.So the probability exists that HBV is transmitted through environment pollution.
3.Curative effect of Jianwei Yuyang Granule on chronic gastritis

AIM:To observe the effect of Jianwei Yuyang Granule(Radix Bupleuri,Radix Codonopsis,Rhizoma Atractylodis macrocephalae,Rhizoma Corydalis,etc.) on the expressions of PS_2 and platelet activating factor(PAF) of helicobacterpylori-associated gastritis and study the potential mechanisim. METHODS:80 cases were divided into two groups.Jianwei Yuyang Granule group(42 cases) was given Jianwei Yuyang Granule and the control group(38 cases) was given three kinds of western medicines combined.H.pylori was detected by urease test on biopsy specimen of gastric mucosa and Warthin-Starry method and the changes of expressions of PS_2 and platelet activating factor(PAF) were observed.The contents of aninohexose and phosphatide in gastric mucosa were determined at the same time.All patients were rechecked gastroscope in the 6th week. Objective To investigate the effect of hydroxycam ptothecine on PS2 and COX-2 in nude mice by human bile duct carcinoma QBC939 cells in order to analyze the correlation between PS2 and COX-2 expressions and the development of the tumor.Methods After the model of the transplanted tumor of subcutaneous tissues located the nude mice was constructed by human bile duct carcinoma QBC939 cells,30 nude mice were randomly divided into 3 groups,namely,control group,hydroxycamptothecine low-dose group and high-dose group.The growth of the transplanted tumor was observed and immunohistochemical staining combined with image analysis was used to determine the expression of PS2 and COX-2 in the tumor tissues.Results Compared with that of the control group,the content of PS2 in the tumor tissues was increased in hydroxycamptothecine low-dose group and high-dose group.The content of COX-2 was decreased and the growth of the transplanted tumor was inhibited in both hydroxycamptothecine low-dose group and high-dose group(P

Nitrogen source is one of the important factors that affect the microalgae growth and lipid accumulation. We studied the effects of various nitrogen sources (i.e. NaNO3, CO(NH2)2, NH4Cl and CH3COONH4) and amount on the growth density, lipid yield, and eicosapentaenoic acid (EPA) content of Nannochloropsis oculata by single factor experimental method. The results show that N. oculata preferred NH4+ as nitrogen source rather than NO3- and CO(NH2)2. NH4+ could promote the growth and lipid accumulation of N. oculata. With the increase of nitrogen concentration, the biomass and the content of polyunsaturated fatty acid (PUFA) increased, but the content of lipid decreased. CH3COONH4 was the most suitable for growth, accumulation of lipid and EPA of N. oculata among the four investigated nitrogen sources. The optimal concentration was 5.29 mmol/L.

6.Study of the Sea Buckthorn Procyanidins's (SBPC) potencional mechanisim against peptic ulcer

AIM: To investigate the potencional mechanism of Sea Buckthorn Procyanidins(SBPC) against peptic ulcer. METHODS: 72 gastric ulcer patients(final diagnosis by gasroscope),were randomly divided into two groups,including Sea Buckthorn Procyanidins(SBPC) treatment group(36 cases),and Ranitidine control group(36 cases),among them,50 Hp-positive PU patients were confirmed by endoscopy,histological instection,urease test.Normal group was 12 cases.Detection on Biospy specimens of gastric mucosa was used to observe the change of ITF for Immunohischemical,PS_2 for western blotting and the contents of aninohexose and phosphatide in ulcerated gastric mucosa. 7.Relaxation Training and Sleep related Behavior Modification in Patients with Insomnia

Objective:to do clinical study on effect of sleep-related behavior modification and relaxation training on insomnia.Method:44 outclinic patients with insomnia received sleep-related behavior modification and relaxation training for 8 weeks. Using self-designed inventory to assess the outcome.Result:the time of sleep increased since the third week, and increased continuously. After the intervention, the rate of satisfaction about sleep increased from 23% at the baseline to 86.1%. 36 of the 44 patients completed the 8-week clinical study. Conclusion:sleep-related behavior modification and relaxation training can improve sleep of patients with insomnia.
8.Expression and significance of miR-21 and PDCD4mRNA in human colorectal cancer

Objective To investigate the expression of miR-21 and PDCD4mRNA in colorectal cancer and their correlation with clinicopathological characteristics,to elucidate relationship between PDCD4and miR-21 in vivo.Methods Expression of RNA (including miR-21and PDCD4mRNA) and PDCD4 protein were detected respectively by quantitative real-time reverse transcriptase-PCR and SP immunohistochemical staining in 43 cases of colorectal carcinoma and corresponding normal mucous membrane tissue.Results In colorectal carcinoma,expression of miR-21 was higher than that of the control ( P < 0.05 ),and expression of PDCD4mRNA lower than the control ( P < 0.05 ).Expression of miR21 was associated with lymph node metastasis and clinical stages ( Ⅰ + Ⅱ,Ⅲ + Ⅳ ) ( P < 0.05 ).On the other hand,no significant differences were observed regarding sex,tumor site,size,local invasion,distant metastases,clinical stages( Ⅰ,Ⅱ ) (P > 0.05 ).Expression of PDCD4mRNA was associated with local invasion,clinical stages ( Ⅰ,Ⅱ ) ( P < 0.05 ).While no significant differences were found concerning sex,site,size,lymph node metastasis,distant metastases,clinical stages( Ⅰ + Ⅱ,Ⅲ + Ⅳ ) ( P > 0.05 ).miR21 levels was negatively correlated with PDCD4 expression including nuclear and nuclear and cytoplasmic put together (P < 0.05 ),in contrast to PDCD4mRNA and PDCD4 expression in cytoplasmic ( P > 0.05 ).Conclusions ( 1 ) Abnormal expression of PDCD4 mRNA and miR-21 correlate with prognosis in colorectal cancer.(2) miR-21 suppress translation of PDCD4mRNA by binding to the 3'-untranslated region (3'-UTR)of target PDCD4mRNA.
9.Expression of RUNX3 and miR-130b in gastric carcinoma and clinical significance

ObjectiveTo explore the expression of miR-130b and RUNX3mRNA in human gastric carcinoma and the clinical significance. MethodsThe expression of miR-130b and RUNX3mRNA were detected by RT-PCR in 40 cases of gastric carcinoma and corresponding normal mucosa tissue. The expression of RUNX3protein was determined by immunohistochemistry SP method. ResultsThe expression of miR-130b was significantly up-regulated in gastric carcinoma than that in the adjacent normal gastric mucosa tissues (2.18 ± 3.75 ) vs.( 2.59 ± 3.45 ),P < 0.05 ; The expression of RUNX3mRNA in gastric carcinoma tissues was significantly lower than that in adjacent normal gastric mucosa tissues( 8.76 ±2.82) vs.( 7.58 ± 2.87 ),P < 0.05.The expression of miR-130b and RUNX3mRNA were positively correlated with lymph node metastasis and clinical stage ( P < 0.05 ) ; No significant association was found between the expression and age,gender,tumor size,distant metastasis and depth of tumor invasion ( P >0.05 ).The expression of miR-130b was negatively correlated with RUNX3 protein expression in nuclei and cytoplasm (P < 0.05 ).ConclusionsAbnormal expression of miR-130b and RUNX3mRNA correlates with prognosis of gastric carcinoma; Decreased RUNX3 protein expression may contribute to tumourigenesis.
10.p27 gene methylation and clinicopathologic features of colorectal carcinoma

Objective To investigate the relationship between p27 gene methylation and pathology of colorectal carcinoma. Methods p27 gene methylation promotor region and p27 protein expression were detected respectively by methylation specificity polymerase chain reaction and immunohistochemical staining SP in 106 cases of colorectal carcinoma and each adjacent normal mucous membrane tissue and 22 cases of colorectal adenoma tissue. Results The positive expression rate of p27 gene methylation was statistically different in colorectal carcinoma tissue compared with normal mucous membrane and colorectal adenoma tissue (P<0.05). Their positive expression rate were 59.4% (63/106), 18.2% (4/22) and 3.8%(4/106) respectively in colorectal carcinoma tissue,colorectal adenoma and normal mucous membrane tissue (P < 0. 05). p27 gene methylation in poorly differentiated group was significantly higher than that in welldifferentiated group (48.0% vs. 24. 7%, P <0. 05), in Dukes-A + B stage group was significantly lower than that in Dukes C + D stage group(20. 0% vs. 41.2%, P < 0. 05 ), and it was higher in lymph nodes metastases group than that in lymph nodes negative group(41.5% vs. 23. 1%, P <0. 05), that in positive serosa infiltration group was higher than negative serosa infiltration group(32. 5% vs. 24. 1%, P > 0. 05 ).Conclusions Methylated p27 gene protein expression in colorectal carcinoma was significantly higher than normal mucous membrane and colorectal adenoma tissue. The methylation rate of p27 gene in colorectal carcinoma was significantly associated with tumor differentiation, invasive depth, Dukes stage, lymph node metastasis.
