1.Relationship of lab results and prognosis among children with severe intestinal virus infection

Objective To explore the clinical characteristic of lab results and prognosis of severe intestinal virus infection dis -ease and to provide the objective basis for early intervention .Methods All children who were admitted in pediatric intensive care unit ( PICU) by laboratory diagnosis as cases of severe intestinal virus infection disease in Hunan Children 's Hospital from January 6 to No-vember 25, 2010 were selected as cases of this study .Their medical records were collected .The data was analyzed by χ2 test, Fisher's exact test , and binary logistic regression .Results A percentage ( 33.1%) of patients had abnormal white blood cell ( WBC ) counts, and the difference rates of younger group (under 1 year old) and upper 5 years old group were higher than that in 1~5 years old group with statistically significance (χ2 =12.13, P <0.01).The difference rates of aspartate aminotransferase (AST) and cardiac troponin I(cTnT) were 23.8%and 31.5%, respectively.A percentage (52.9%) of patients had reduced CD4 lymphocytes.Abnor-mal WBC, C reactive protein (CRP), and prognosis have no significant difference ( P >0.05).There were statistically significant differences in procalcitonin (PCT), AST, cTnT anomalies and prognosis ( P <0.01 or P <0.05).Conclusions Abnormal PCT, AST and cTnT are associated with the prognosis of children with severe intestinal virus infection , while WBC and CRP are not .
2.Protective effect of alginate polysaccharide JM on brain ischemia injury in rats

Objective To evaluate the effect of alginate polysaccharide JM on rats impaired by brain ischemia,and to elucidate its mechanisms.Methods By using the middle cerebral artery occlusion(MCAO) induced by nylon surgical thread inserted through the internal carotid artery into the anterior cerebral artery in rats,the effects of JM on neurological dysfunction and infarct volumn in rats brain were investigated.Neuroblastoma SH-SY5Y cells were employed to study the inhibitory effects of JM on cytotoxicity induced by hypoxia-hypoglycemia.Results JM at the doses of 12.5 and 25 mg?kg-1,iv,30 min before ischemia impressed neurological dysfunction,characterized by the decreased behavioral obstacle scores of MCAO rats.The infarct volumn declined 24h after ischemia as well.Further investigation by flow cytometry revealed that JM significantly reduces the overloading of intracellular free calcium on and suppresses apoptosis induced by hypoxia-hypoglycemia in human neuroblastoma SH-SY5Y cells.Conclusion JM showed protective effects on brain ischemia,probably related to its inhibitory effect on the overloading of intracellular free calcium ion(\i) and cell apoptosis.

Objective To analyze the clinical features and prognosis of hyperglycemia and the relationship between the blood glucose level and the severity of disease in critically ill children.Methods A total of 349 critically ill children admitted in Pediatric Intensive Care Unit (PICU) from November 2009 to April 2010 were restrospectively analyzed.According to the levels of venous blood glucose within 24 h after admission,they were divided into very high level group (blood glucose ≥11.1 mmol/L,n =67 ),slightly high level group (blood glucose 6.3-11.1 mmol/L,n =134) and normal level group (blood glucose ≤6.3mmol/L,n =148).Blood glucose levels were measured within 24 hours,3 days and 7 days after admission.Electrolytes,inflammatory markers,cardiac enzymes,liver and kidney function as well as other biomarkers related to the severity and the prognosis of the patients were recorded after admission.The categorical variables were analyzed with Chi -squared test,the continuous variables were analyzed with t-test,F-test,U-test andH-test,and the correlation analysis was calculated by using Pearson Coefficients. Results In the very high level group,slightly high level group and normal level group,the average blood glucose levels were 16.98 ±7.08 mmol/L,8.25 ± 1.40 mmol/L and 4.89 ± 0.98 mmol/L ( P < 0.01 ),respectively;and the Pediatric Critical Ⅲ Scores at admission were 81.22 ± 8.25,86.71 ± 6.40 and 86.15 ± 6.99 ( P <0.01 ),respectively,and the incidences of sepsis or septic shock were 55.23%,30.59% and 14.18%,respectively (P <0.01 ),and the incidences of MODS were 46.26%,22.39% and 16.23%,respectively (P <0.01 ).The blood glucose levels of patients with one organ failure and two organ failure were 8.27 ± 3.75 mmol/L and 8.88 ± 5.42 mmol/L,respectively ( P < 0.05 ).The blood glucose levels of patients with two organ failure and multiple organ failure were 8.88 ± 5.42 mmol/L and 13.09 ± 8.23 mmol/L,respectively (P<0.01).The mortality rates of three groups were 47.76%,14.93% and 10.13% (P <0.01 ),and the blood glucose levels at admission in survival group and death group were 7.57 ±4.11 mmol/L and 12.46 ± 8.17 mmol/L ( P < 0.01 ).Conclusions Patients with hyperglycemia are often found in the PICU.It not only partially reflects the severity of the disease,but also serves as an important indicator for the prognosis.The blood glucose level is positively correlated to the number of compromised organs and the severity of the disease.Dynamic monitoring of blood glucose may be essential for controlling the symptoms and prediction of prognosis.
5.Lower Respiratory Tract Klebsiella Pneumoniae Infection:Clinical Manifestations and Susceptibility of 96 Cases

OBJECTIVE: To learn the clinical features of patients infected with Klebsiella pneumoniae in our hospital.METHODS: From 2006 and 2007,a total of 96 inpatients in our hospital that were confirmed to be infected with Klebsiella pneumoniae by sputum culture were collected for a study of their clinical features and drug susceptibility test results.RESULTS: Patients with Klebsiella pneumoniae infection represented 12.7% of the total concurrent pulmonary infection cases,of which,57 Klebsiella pneumoniae infection cases had concurrent pulmonary tuberculosis(TB),and 39 Klebsiella pneumoniae infection cases had no concurrent TB.Drug susceptibility tests showed that the resistance rates of Klebsiella pneumoniae to third generation cephalosporins and quinolones were higher than those reported in the literature,and the resistant rate of Klebsiella pneumoniae to quinolones in Klebsiella pneumoniae infection patients complicated with TB was higher than those without TB.CONCLUSION: Klebsiella pneumoniae infection was more common in patients with low body resistance,which has typical clinical manifestations.Recently,the resistance of Klebsiella pneumoniae to the third generation cephalosporin has been increasing,and its resistance to quinolones in patients with pneumonia Klebsiella infection complicated with TB was significantly higher than in those without TB.
6.Construction and fluorescence intensity of lipid ultrasound microbubbles with monoclonal antibody targeted to leukaemia inhibitory factor receptor

Objective To investigate fluorescence intensity of lipid ultrasound microbubbles constructed in vitro and targeted to leukaemia inhibitory factor receptor (LIFR) with a monoclonal antibody.MethodsThe LIFR-targeted ultrasound mierobubbles (MB-BSB-LIFR-AB) were constructed using a technology of biotin-avidin bridge.FITC labeled Avidin was incubated with lipid ultrasound microbubbles (MB) and biotinylated lipid microbubbles (MB-B).Two dilutions (1:4 and 1:16) of DTAF second antibody were incubated with four types of ultrasound microbubbles,including MB,MB-B,biotinavidin-MB (MB-BS),MB-BSB-LIFR-AB.The fluorescence intensity of microhubbles were graded as 0,1,2to 3.ResultsAfter incubating with FITC-avidin,MB-B displayed bright green fluorescence ( grade 3),but MB had no fluorescence ( grade 0).After incubating with two dilutions of DTAF second antibody (1:4 and 1:16),MB-BSB-LIFR-AB displayed brightest green fluorescence (grade 3) in both concentration,while MB-BS and MB-B only displayed dim green fluorescence (grade 1 ) at the dilution of 1:4,with MB displaying no fluorescence at either dilution (grade 0).Conclusions LIFR monoclonal antibody can be effectively conjugated to MB-B with biotin-avidin bridge.Fluorescence detection is a simple method for investigating the conjugation reliability of targeted lipid ultrasound microbubbles.
7.Adhesive behavior of dual-targeted microbubbles carrying both Sialyl Lewisx and anti-ICAM-1 monoclonal antibody in vitro

Objective To assess the adhesive behavior of dual-targeted microbubbles carrying both Sialyl Lewisx and anti-ICAM-1 monoclonal antibodies in vitro. Methods Selectin-targeted (with Sialyl Lewisx) microbubbles (MB-S),ICAM-1-targeted (with anti-ICAM-1 monoclonal antibodies) microbubbles (MB-Ⅰ),and dual-targeted (with both ligands) microbubbles(MB-D) were prepared by attaching the ligands to the biotinylated lipid-microbubbles via multi-step avidin biotin bridging chemistry. A parallel plate flow chamber combined with a novel automated tracking algorithm,were used to analyze the transient velocities,rolling and firmly adherent numbers of microbubbles at various shear stress (0. 6,2.0 and 4.0 dyn/cm2)over 6 min. Microbubbles detachments were tested by ramping up the shear stress at 30 s intervals. Results At 0.6 dyn/cm2 shear stress, the rolling numbers of MB-S and MB-D were remarkably more than that of MB-I( P<0.05), while at 2.0 and 4.0 dyn/cm2 MB-S performed higher rolling efficiency as compared with either MB-I and MB-D ( P< 0.05). In all flow conditions, the adhesive numbers of MB-D to the targets were obviously greater than those of MB-S and MB-I ( P< 0.05). Half-maximal detachment decreased gradually in MB-I, MB-D and MB-S by turns ( P< 0.05). Conclusions MB-I, MB-S and MB-D have different adhesive behaviors. MB-I exhibites primarily firm adhesion with low rolling efficiency, while MB-S reveales unstable or transient adhesion with high rolling efficiency,and MB-D exhibites firm adhesion with high rolling efficiency. MB-D may be suitable for molecular imaging in high-flow vessels.

Objective:To detect the expression of microRNA-218 (miR-218) in human acute lymphocyte leukemia (ALL) T lymphocytes ( CCRF-CEM) ,explore its effects on the biological features of CCRF-CEM cells and the expression of its target gene c-kit, so as to provide new insights for leukemia treatment.Methods: Using the quantitative real-time polymerase chain reaction ( qRT-PCR) ,we detected the expression of miR-218 in the normal peripheral blood T lymphocytes and CCRF-CEM cells.Forty-eight hours after the miR-218 mimic was transfected into the CCRF-CEM cells,the expression of miR-218 in the CCRF-CEM cells was detected by qRT-PCR.The effect of miR-218 on the CCRF-CEM cell viability was detected using MTT.The effect of miR-218 on the proliferation and apoptosis of CCRF-CEM cell was analyzed using flow cytometry.c-kit gene was identified to be a target gene of miR-218 by luciferase reporter enzyme system,and the effect of miR-218 on the expression of KIT protein in cells were determined using Western blot.Results:As shown by qRT-PCR,compared with that in the normal peripheral blood T lymphocytes,the expressions of miR-218 in ALL T lymphocytes cell lines were significantly decreased ( P<0.01 ) .Compared with the control group, the expression of miR-218 increase significantly in CCRF-CEM cells transfected with miR-218 mimic for 48 hours ( P<0.01).MTT showed that the cell viability decreased significantly after the over-expression of miR-218 in the CCRF-CEM cells ( P<0.05 ) .Flow cytometry showed that the S-phase fraction significantly declined after the over-expression of miR-218 ( P<0.01 ) , and meanwhile the apoptosis of cells also significantly increased (P<0.01).Luciferase reporter gene assay showed that,compared with the control group,the relative luciferase activity significantly declined in the miR-218 mimic transfection group (P<0.01).Compared with the control group,the expression of KIT protein in the CCRF-CEM cells transfected with miR-218 mimic for 48 hours significantly decreased ( P<0.01).Conclusion:The expression of miR-218 decreases in ALL T lymphocytes cell lines.MiR-218 can negatively regulate the expression of KIT protein,inhibit the proliferation and increase the apoptosis of CCRF-CEM cells.Treatment based on the enhanced expression of miR-218 may be a promising strategy for leukemia.
10.Percutaneous argon-helium cryoablation for hepatic malignancies

Objective To observe the efficacy of percutaneous hepatic cryoablation for the treatment of primary or meta-static hepatic malignancies (<5 cm in diameter).Methods A total of 31 patients (39 tumors <5 cm in diameter) were treated with argon-helium cryoablation system under the guidance of CT or ultrasound.Results Tumor ablation range was 90%-100% in 39 lesions,including 69.23% (27/39) complete ablation.The 1- and 2-year survival rate was 90.32% (28/31) and 61.29% (19/31),respectively.No bleeding and injury of blood vessel or bile duct was noted.Complications of cryoablation included intraoperative shivering in 4 (12.90%) patients,postoperative fever (37.12-38.25℃ in 7 (22.58%) patients and hepatic pain in 6 (19.36%) patients.One patient had severe pain relief until 2 h after cryosurgery with ice-cold skin temperature and stable life index,analgesic had little effect,and no bleeding was found on CT image.Other patients had slight or moderate pain and remained untreated.Conclusion Percutaneous targeted argon-helium cryoablation is a feasible and safe technique in the treatment of small primary or metastatic hepatic malignancies not suitable for resection.
