1.The diagnosis and surgical management of intra and extra middle skull base tumors

Objectives: Inquirying the ability of the earlier diagnosis and studying the surgical management of intra and extra middle skull base tumors. Methods: A retrospective study has been made on 43 cases of malignant skull base tumor extended into cavernouse sinus. CT and MRI evaluation were made for all patients. Different approaches have been utilized to remove tumors. Results: In early period there was no obviously clinical sign. 5 year survival rate was 35.1%. Seven cases of no removing totally tumors in cavernous sinus had been alive for 7 months. Conclusions: The symptoms of skull base carcinoma is less, which often leads to false diagnosis and management. Total resection via different approaches may improve clinical results.
2.Experimental study on the pathogenesis of hemifacial spasm

Objective To investigate the pathogenesis in hemifacial spasm by establishing animal models of hemifacial spasm and combining with electrophysiological technique. Methods 28 New Zealand white rabbits were randomized into four groups: 13 rabbits underwent both artificial demylination and arterial compression (model group). 5 rabbits underwent only arterial compression, 5 rabbits underwent only artificial demyelination and 5 rabbits underwent sham operation control. Abnormal muscle response (AMR) and F wave changes in all groups were tested and compared at the third or sixth week after operation respectively.Results No AMR was recorded in all four groups at the third week after operation, while a clear AMR was elicited in ten animals in model group rather than in the other groups at the sixth week of postoperation. F/M amplitude ratio, the F wave duration and frequency of F wave appearance in model animals significantly increased as compared with those of the other groups ( P 0 05). Conclusion The hyperactivity of the facial motonucleus may be the pathophysiological basis of hemifacial spasm.
3.Development of a big medical database system for clinical research and practice of medical data governance

Objective:To strengthen the integration and sharing of medical data resources, provide high-quality and usable data for clinical researchers, and promote the medical data use in clinical research.Methods:According to the development and application goals of the medical big data platform, data from different major clinical information systems in our hospital are integrated and then cleaned, processed and analyzed, and finally aggregated into a unified platform and turned to valuable and usable data resources.Results:A medical big data platform for clinical research in our hospital has been developed. It has stored over 13.42 million patients′ clinical data of more than 50 million visits since 2004 in our hospital; an analysis-oriented common data model (CDM) for clinical research has been designed; clinical researchers can query and extract clinical data according to CDM; the standard clinical research data service mechanisms have been established.Conclusions:The medical big data platform in our hospital helps to provide usable data of good quality for high-level scientific research based on medical big data, and improve the efficiency and quality of clinical research; at the same time, it also provides a efficient way to manage and control clinical research data use while ensuring data security and regulatory compliance.
