1.Oservation of Lacidophilin vaginal capsules in the treatment of pregnant woman with bacterial vaginosis

Objective To discuss the curative effect of Lacidophilin vaginal capsules in the treatment of pregnant woman with bacterial vaginosis .Methods According to the digital table ,80 pregnant women with bacterial vaginosis were selected and divided into the observation group and control group .The patients in observation group were given Lacidophilin vaginal capsules in posterior fornix of vagina by one in the evening for 10 days,and the pa-tients in control group were given Metronidazole in posterior fornix of vagina by one in the evening for 10 days.The curative effect after 1 week was observed ,and the reoccurrence rate after 6 months was followed up .Results The to-tal clinical efficiency of the observed group was much higher than that of the control group (χ2 =5.16,P<0.05).The reoccurrence rates in control group and combined group were 12 and 3 cases after 6 months′treatment,and the reoc-currence rate in the observation group was much lower than that in the control group (χ2 =9.53,P <0.01). Conclusion Lacidophilin vaginal capsules has reliable curative effect on bacterial vaginosis in pregnant woman , which can obviously reduce the reoccurrence rate and has treatment and prevent function of the reoccurrence hereof .
2.Clinical observation of macular epiretinal membrane

Objective To investigate the clinical features, etiological classification and staging of epiretinal macular membrane(MEM). Methods Clinical materials of 194 cases of MEM diagnosed by fundus fluorescein angiography in outpatient department of eye clinic in this hospital from 1983 to 2000 were retrospectively analyzed. Results There were typical clinical symptoms and signs of MEM in all of this 222 eyes of 194 patients. Etiological classification revealed that 4 cases were congenital(2.12%), 22 cases were secondary(11.34%), and 168 cases were idiopathic(86.60%). Staging of course of disease indicated that 119 eyes were in early stage(53.60%), 72 eyes were in middle stage(32.43%), and 31 eyes were in late stage(13.96%). Conclusion MEM may be classified as congenital, secondary and idiopathic type according to its pathogenesis, as early, middle and late stage according to the clinical course of disease. This can be helpful in treating the disease.
3.Analysis on the Changes of Snail Status of Schistosomiasis Before and After Embankment Collapsed in Dongting Lake Region

Objectives To know the affecting degree of snail status by the flood in schistosomiasis epdemic areas.Methods The eight embankments suffered from disasterous floods and collapsed were selected in Dongting Lake; the changes of snail states and snail-killed etc.Before and after disasterous floods were surveyed by retrospectively. Results In the eight embankments, the snail areas inside embankment increased at different degree except two embankments where snail was not found after disaster, it was 10.8 times of that before disaster; Furthermore, after disaster the snail was found in the embankment enclosing village areas inside where it was not found before disaster; in most of embankments collapsed, the mean density of living snail inside embankment and mean density of infected snail and positive rates of snail outside embankment descended at different degree after disaster. Conclusions Inside embankment, the disastrous floods could lead to snails speading in most of embankments collapsed. Otherwise, outside embankment the flood led to reduce the mean density of infected snail and positive rates of snail in most of embankments collapsed, and the degree of snails spreading was affected by the factors such as the snail states at the collapsed place, mean density of living snail,immersed area by water, mud and sand ect.
4.Clinical analysis of the results of macular edema in diabetic retinopathy

ObjetiveTo explore the relationship between the classification of diabetic macular edema(DME) and the stages of the diabetic retinopathy (DR), the diabetic duration and the visual loss.MethodsRetrospectively analyzed the clinical data of fundus fluorescein angiography (FFA) and other related information of 1 521 patients who were diagnosed as DR. Classified DR according to national standard of the diagnosis and classification of DR, and classified DME according to the standard made by the early treatment diabetic retinopathy study research group of United States. The occurrence of DME in DR in each stage and the relationships between DME and the disease course and the vision were analyzed.ResultsIn 1 521 patients, 791 eyes in 468 patients had DME (30.77%), including 361 eyes (45.64%) with focal DME and 430 eyes (54.36%) with diffuse DME. The occurrence of DME was 1.13% in I-stage DR, 7.84%in Ⅱ-stage DR, 41.98% in Ⅲ-stage DR, and 48.93% in Ⅳ-stage DR. Focal and diffuse DME usually occurred at the Ⅲ and Ⅳ stage of DR respectively, with 178 eyes (22.51%) with focal macular edema at the Ⅲ stage of DR, and 249 eyes (31.48%) with diffuse DME at the Ⅳ stage of DR. Patients with DME were hardly found at the Ⅴand Ⅵ stage of DR because of retinal proliferation and vitreous hemorrhage or other complications which made the condition of macula region blurred. The visual acuity of diffuse DME was worse than focal DME. DME often occurred within 10 years in the diabetic duration, and its severity and incidence increased year by year.ConclusionsDME is the main cause of visual impairment of DR. The incidence of DME increased as the course of the DR prolonged. Along with the development of retinopathy, the incidence of DME increased, and the severity of DME aggravated, but the development of DME and its classification can not be brought into definite correspondence or unification with the classification of DR, hence the typing of DME in another individual classification in DR is of course necessary.
5.Synthesis and bioactivity of the folate receptor targeted gamma-cyclodextrin-folate inclusion-coated CdSe/ZnS quantum dots.

The gamma-cyclodextrin-folate (gamma-CD/FA) inclusion-coated CdSe/ZnS quantum dots (QDs) with folate-receptor (FR) targeted were synthesized by simple and convenient sonochemical method. The products were studied using Fourier transform infrared (FTIR), proton nuclear magnetic resonance (1H NMR), utraviolet-visible spectrometry (UV-vis), fluorescence spectrum and transmission electron micrographs (TEM). The results showed that the gamma-CD/FA-coated CdSe/ZnS QDs not only have good monodispersity and smaller size, but also have good optical performance, such as higher quantum yield (QY) and a long fluorescence lifetime. The cytotoxicity experiments showed that the gamma-CD/FA-coated CdSe/ZnS QDs have lower cytotoxicity and could more effectively enter cancer cells with FR over-expression. The QDs with 4-5 nm in diameter were relatively easy to enter the cell and to be removed through kidneys, so it is more suitable for biomedical applications for bioprobes and bioimaging.
6.Clinical studies of early enteral immunonutrition in patients with severe acute pancreatitis

Objective To compare the efficacy and clinical value of early enteral immunonutrition (EIN) combined with glutamine with early standard formula enteral nutrition (EEN) in patients with severe acute pancreatitis(SAP).Methods Twenty-eight patients with SAP were divided into two groups by random number table.Both groups were fasted,given treatments of anti-infection,gastrointestinal decompression,acidoresistance,spasmolysis,pancreatic enzyme inhibition,reduce the burden on the pancreas insulin,water electrolyte balance and parenteral nutrition upon admission into hospital.Spiral nasointestinal tube was implemented within 72 hours of patients' admission.The 14 cases in the EEN group were given treatment of early standard enteral nutrition by nasojejunal tube.The 14 cases in the EIN group received early enteral immunonutrition (glutamine granules,0.2 g/(kg · d),at least three times oral administration by dissolving in enteral nutritious agent peptison with a small amount of warm water).During the treatment,the dose of glutamine granules was reduced gradually until withdrawal when the patients in the EIN group started normal diet.The changes were observed in patients in terms of APACHE Ⅱ,Ranson score,inflammatory markers,nutritional and immune indicators,while monitoring the first defecation time,ICU care unit stay,total hospital stay,total hospitalization costs,complication rate and mortality data.Results Patients of both groups tolerated early enteral nutrition treatment.Fourteen days after treatment,the value of the immune indicator IgG was significantly higher in the EIN group than in the EEN group((19.14 ± 2.03) g vs.(13.79 ± 3.29) g,P <0.05).The first defecation time,ICU care days,total hospital costs were significantly shorter or less in patients in the EIN group than in those in the EEN group (first defecation time:((4.29 ± 1.64) d vs.(5.36 ± 0.84)d,t =2.179 ; ICU care:(2.57 ± 0.85) d vs.(3.64 ± 1.60) d,t =2.213 ; total hospital costs:(343.6 ±35.9)thousand yuan vs.(438.6±94.3) thousand yuan,t =3.518; P <0.05).Conclusion It is feasible and safety to implement EIN treatment in patients with severe acute pancreatitis.EIN and EEN could significantly reduce the inflammatory response and improve the nutritional status of patients with severe acute pancreatitis.But EIN is better in improving immune function and the prognosis of patients with severe acute pancreatitis than the EEN.
7.Expression of interleukin-8 protein in villi and decidua of patients with recurrent spontaneous abortion

ObjectiveTo study the expression of interleukin-8 (IL-8) protein in villi and decidua of patients with recurrent spontaneous abortion(RSA). MethodsThe local and quantitative expressions of IL-8 in villi and decidua of 30 RSA patients were shown and measured by immunohistochemistry and Western blotting. Results IL-8 protein immunohistochemical signals were located in the chorioepithelium cytoplasm and decidual cells plasma in RSA group. While in control group the immunohistochemical staining was negative in decidul cells, but positive in glandular epithelium. The quantitative analysis of IL-8 protein by Western blotting, intensity of the immunostaining was higher in RSA group than that in the control group. Conclusion The higher expression of IL-8 in villi and decidua is releated to RSA, IL-8 might take part in the pathologic process of RSA.
8.Determination of polymethoxylated flavones in Citri Reticulatae Pericarpium from eleven cultivar origins

AIM To determine the contents of polymethoxylated flavones in Citri Reticulatae Pericarpium from eleven cultivar origins.METHODS HPLC was applied to screening extraction solvents (methanol,anhydrous ethanol,95% ethanol,ethyl acetate and petroleum ether).Ultraviolet-visible spectrophotometry was adopted in the content determination of polymethoxylated flavones in eighteen batches of samples,nobiletin was taken as a reference substance,and the detection wavelength was set at 330 nm.RESULTS Ethyl acetate exhibited the best extraction effect,so it was selected as extraction solvent.Nobiletin showed a good linear relationship within the range of 3.024-13.104 μg/mL (R2 =0.999 8),whose average recovery was 101.37% with the RSD of 1.34%.The contents of polymethoxylated flavones had obvious differences among samples from different cultivar origins,which were relatively high from Citrus reticulata ‘ Chachi’,Citrus reticulata ‘ Dahongpao’,Citrus reticulata ‘ Tangerina’,Citrus reticulata ‘ Ponkan’,Citrus reticulata ‘ Kinokuni’and Citrus reticulata ‘ Shiyueju’(more than 0.4%).CONCLUSION This simple and accurate method can effectively eliminate the interference of another flavonoids,which is suitable for the content determination of polymethoxylated flavones in Citri Reticulatae Pericarpium.
9.Random control trial study on effect of narrative medicine mode on rehabilitation of patients underwent radical nephrectomy

Objective To investigate the effect of narrative medicine mode in perioperative medical and nursing care for patients underwent radical nephrectomy. Methods A total of 124 patients received radical nephrectomy from February 2014 to October 2016 were selected and divided into experimental group and control group by block randomization grouping method with 62 cases each. Narrative medical treatment and care was applied in experimental group while control group received conventional treatment and care. The psychological status and social support of the patients were evaluated by Zung Self-rating Anxiety Scale(SAS), Self-rating Depression Scale (SDS) and social support rating scale(SSRS), as well as rehabilitation indexes including complications, first feeding time, hospitalization time, costs, etc. Results Anxiety and depression status were existed in both experimental and control groups before operations, and there were no significant difference in all pre-operative scales between two groups (P>0.05). The score of SAS, SDS, SSRS was (44.71 ± 4.67), (40.84 ± 4.05), (43.21 ± 3.60) points after operation and (54.98 ± 5.93), (46.52 ± 5.07), (31.87 ± 3.22) points before operation in experimental group. The score of SAS, SDS, SSRS was (53.24±5.97), (44.63±4.37), (32.61± 2.72) points after operation and (54.92 ± 5.83), (46.53 ± 4.89), (32.16 ± 2.48) points before operation in control group. Significant differences were found between before and after operation in experimental group (t=18.73, 8.85, 18.18, all P<0.01). However, no significant difference could be found between before and after operation in control group (t=1.590, 1.400, 1.910, P>0.05). After operation, the complications, first feeding time, hospitalization time and costs was 22.58%(14/62), (2.50±0.92) d, (11.16±3.72) d, (24.6±4.0) thousand yuan in experimental group, which was 48.39%(30/62), (3.98 ± 1.44) d, (15.48 ± 5.44) d, (40.2 ± 3.1) thousand yuan in control group. Significant differences were found between experimental group and control group (χ2=9.02, t=2.07, 8.06, 3.93, P<0.05 or 0.01). Conclusions Narrative medicine mode can provide medical care filled with respect, empathy, and humanistic concern, andimprove patients′psychological status, thus promoting rehabilitation.
10.Comprehensive rehabilitation intervention for stroke patients with unilateral spatial neglect

Objective To observe the iffectiveness of comprehensive rehabilitationinterventions on stroke patients with unilateral spatial neglect (USN). Methods A total of 245 cases if strijd were examined to diagnose USN. Of the 245 patients,86 cases were diagnosed as being with USN, and divided into a control group(n=43 cases) and a treatment group (n=43 cases) randomly. The control group was treated with Bobath and Rood techniques in addition to routine clinical medical interventions, while the treatment group was treated with a comprehensive rehabilitation protocol for USN in addition to the same interventions for the control group. Both groups were assessed with regard to motor, balance function, walking performance, and USN severity as well as the activities of daily living (ADL) performance. Results After 8 weeks of treatment,both groups improved, but there showed a significantly statistical difference between the 2 groups in terms of Fugl-Meyer motor function scores (P<0.01), balance function scores (P<0.01) Holden walding function classifications (P<0.01), Barthel index(P<0.001) and USN severity scores(P<0.01). Conclusion Comprehensive USN rehabilitation intervention could improve motor, balance, walking functions and ADL performance and alleviate the USN severity in stroke patients with USN.
