1.Expression of androgen receptor and embryonic stem cell associated transcript 4 in breast cancer patients with over-expression of human epidermal growth factor receptor 2 and their clinical significances

Objective To investigate the expression and clinical significance of androgen receptor (AR) and embryonic stem cell associated transcripts 4 (NANOG) in breast cancer patients with human epidermal growth factor receptor 2 (HER-2) over-expression, and to analyze its relationship with clinicopathologic features of breast cancer. Methods 143 breast cancer patients with HER-2 over-expression were selected from the screening of 1052 cases of invasive breast cancer according to estrogen receptor (ER), progesterone receptor (PR) and HER-2 status. The protein expression of AR and NANOG was assayed by using immunohistochemistry.The relationship between AR expression and clinicopathological features was analyzed by χ2 test. The correlation between AR expression and NANOG expression was analyzed by Spearman correlation analysis. Results The positive expression of AR was 35.7 % (51/143). The AR expression was not associated with age and menstruation status (both P>0.05), and was associated with tumor size, clinical TNM staging and lymphatic metastasis (all P< 0.05). The positive rate of NANOG were 53.1 %(76/143), and NANOG proteins were negative in adjacent normal breast tissue and benign breast lesions. The positive rate of AR was 27.6%(21/76) in NANOG-positive cases, whereas the positive rate of AR was 44.8%(30/67) in NANOG-negative cases, and the difference was statistically significant (χ2=4.526, P=0.033). There was an inverse correlation between NANOG and AR expressions (r= -0.255, P= 0.002). Conclusion AR and NANOG may be new targets for endocrine therapy and molecular biological therapy.
2.Clinical study of thinning and replanting combined with VAC negative pressure drainage in the treatment of large area of retrograde skin avulsion injury

Objective:To study the clinical effect of thin replantation combined with vacuum assisted closure (VAC) in the treatment of large area retrograde skin avulsion injury.Methods:A total of 42 patients with large scale retrograde skin avulsion injury admitted to the trauma center of Zhuzhou Central Hospital from April 2017 to April 2019 were enrolled in this study, and were treated with VAC continuous negative pressure drainage after operation. The wound survival rate, wound survival area, wound infection, replantation skin performance and joint mobility were observed.Results:All of the 42 patients were followed up for 8-50(23.56±3.56)months. 35 patients survived the stage 1 skin grafting, the skin flap survived, and the wound had no obvious skin defect. After active dressing change, the wound healed well, and no second operation was needed. Small area necrosis occurred in the first stage wound of 7 patients due to large skin defect, and the wound healed well after the second stage surgical transplantation and enhanced dressing change. The Hospital for Special Surgery (HSS) score of 26 patients was 80-95(87.96±3.21), and the American Orthopaedic Foot and Ankle Society (AOFAS) score of 22 patients was 80-96(88.79±3.41). All patients had good skin sensation, elasticity, pressure resistance, wear resistance and color, and joint mobility was good without obvious limitation of movement.Conclusions:Thinning replantation combined with VAC negative pressure drainage in the treatment of large area retrograde skin avulsion injury can significantly promote the application of wound surface and skin graft, which is conducive to drain the drainage fluid out of the body, reduce the wound infection rate, improve the survival rate of skin grafting and improve joint function.
3.Differences in expression of Mohawk transcription factors and collagen in anterior cruciate ligament grafts between two remodeling outcomes under arthroscopy

Objective To investigate the differences in expression of Mohawk (MKX) transcription factors and collagen of types Ⅰ and Ⅲ in anterior cruciate ligament (ACL) gratis between 2 remodeling outcomes under arthroscopy.Methods Enrolled for this study were 17 patients who had undergone arthroscopic single-bundle ACL reconstruction with autogenous hamstring tendons and secondary arthroscopic exploration 48 to 131 months (average,83.1 months) after removal of tibial internal fixator at Department of Sports Medicine,The First Affiliated Hospital to Shenzhen University from March 2017 to December 2017.They were divided into a good remodeling group (11 cases) and a fair remodeling group (6 cases) according to the graft quality under arthroscopy (synovial and vascular coverage,and apparent tension,thickness and retear of the grafts).During the secondary arthroscopic procedures,biopsy of the central ACL grafts was performed.Moreover,normal ACL tissues were harvested from 8 contemporary controls of < 60 years old who underwent total knee replacement.Immunohistochemical assay and quantificational real-time polymerase chain reaction were conducted to detect the expression of transcription factors and collagen of types Ⅰ and Ⅲ in all the samples.Results In the samples from good remodeling and control groups,there were abundant well-arranged collagen fibers of types Ⅰ and Ⅲ and MKX-positive cells;in the fair remolding group,the collagen fibers of types Ⅰ and Ⅲ and MKX-positive cells were much decreased in number and the fibers were not well arranged.The former 2 groups scored in immunohistochemical assay significantly higher than the latter one (P < 0.05).qRT-PCR showed that the expression levels of MKX gene (0.44 ± 0.30),COL1A1 gene (0.52 ± 0.27) and COL3A1 gene (0.60 ± 0.22) in the fair remolding group were significantly lower than in the control group (1.00 ± 0.00,1.00 ± 0.00 and 1.00 ± 0.00) and than in the good remolding group (0.97 ± 0.67,0.99 ±0.38 and 1.00 ± 0.35) (P < 0.05).Conclusion Good remodeling ACL grafts with histological maturation under arthroscopy are more similar to normal ACL than fair remodeling ACL grafts in expression of MKX transcription factors and collagen of types Ⅰ and Ⅲ.
4.Clinical significance of vasculogenic mimicry and its correlation with extracellular matrix metalloproteinase inducer expression in gliomas

Objective To investigate the expression of vasculogenic mimicry (VM) and its relations with extracellular matrix metalloproteinase inducer (EMMPRIN) expression and clinical data in gliomas.Methods Sixty-eight glioma specimens were collected in our hospital from January 2004 and April 2014, including 4 of grade I, 17 of grade Ⅱ, 23 of grade Ⅲ and 24 of grade Ⅳ.All specimens were preserved in paraffin embedded by standard procedures after surgery.Immunohistochemical staining of CD34 and PAS (dual staining) was employed to verify the existence of VM and EMMPRIN expression.The relations of VM with clinical data and EMMPRIN expression were analyzed.Results VM channels were found in 14 specimens of human gliomas (20.58%);higher incidence of VM in high-grade gliomas (WHO Ⅲ: 5.88%;WHO Ⅳ: 13.24%) was noted as compared with that in low-grade gliomas (WHO Ⅰ: 0.00%;WHO Ⅱ: 1.47%, Z=2.843, P=0.008).There were no significant differences of VM appearance between patients of different age, gender and tumor sizes (P<0.05).EMMPRIN expression in glioma slices of VM appearance was significantly higher than that in glioma slices without VM appearance (Z=2.748, P=0.004).Conclusion VM exists in gliomas and VM is not only positively correlated with tumor grade, but also with EMMPRIN expression level;and it is not correlated with general clinicopathological features such as gender, age and tumor sizes.
5.Herbal Textual Research on Lablab Semen Album in Famous Classical Formulas

In this paper, the name, origin, quality evaluation, producing area and processing methods of Lablab Semen Album in the famous classical formulas were researched by reviewing the ancient materia medica, medical books, prescription books and modern literature. The results showed that the name of Lablab Semen Album in the past dynasties was mostly derived from its shape and color, called Biandou and Baibiandou. The mainstream origin used in the past dynasties was Lablab purpureus, the medicinal parts were mainly white mature seeds, with the addition of the leaves in the Song dynasty and the flowers in the Ming dynasty. Since modern times, the authentic producing areas of Lablab Semen Album are Suzhou, Zhejiang and other places, and now mainly produced in Chuxiong and Xinping, Yunnan and Panzhihua, Sichuan. The traditional quality evaluation of Lablab Semen Album is evaluated as large, solid, full and white. The harvesting time of this herb is recorded from the eighth to the ninth lunar month in related literature, the pods are picked when the seeds are ripe, and the seeds are dried in the sun. In ancient times, the processing of Lablab Semen Album mainly consisted of frying the seeds with skin and then pulverizing for use, or soaking and peeling seeds for raw use. Based on the conclusion of the textual research, it is recommended that the seeds or flowers of the white flowering plants of L. purpureus, a member of the leguminosae, should be used in the famous classical formulas, and the dried seeds or dried flowers of Lablab Semen Album can be used as medicine if the formula did not clearly indicate processing requirements.
