1.Expression and clinical significance of Survivin and cyclooxygenase-2 in non-small cell lung cancer tissues

Objective To study relationship of expression of Survivin and cyclooxygenase(COX)-2 in the tissue of non-small cell lung cancer (NSCLC) with differentiation, lymph node metastasis and clinical stage. Method The expression of Survivin and COX-2 were detected by immunohistochemical SP staining in the specimens of 43 cases of NSCLC and 20 cases of para-cancerous lung tissue. Results The positive rate of Survivin and COX-2 was 67.4% (29/43) and 55.8% (24/43) respectively in NSCLC,was 0 respectively in the para-cancerous lung tissue. Compared with para-cancerous lung tissue,there was statistically significant difference(P< 0.01). Both Survivin and COX-2 had significantly higher expression in NSCLC with poor differentiation,lymph node metastasis and clinical stage of Ⅱ-Ⅲ than those with well differentiation,without metastasis and clinical stage of Ⅰ (P <0.05). Conclusion The expression of Survivin and COX-2 are high in NSCLC,they both have relationship with differentiation,lymph node metastasis and clinical stage of NSCLC.
2.Study on Effective Polysaccharide of Anti-Human Lung Cancer A549 Cells in Zhou’s Prescription for Resolving Mass

Objective To study content and ascription of the composition of the polysaccharides from Zhou’s Prescription for Resolving Mass. Method The polysaccharide was extracted by ethanol precipitation, and purified by savage method. DEAE-cellulose anion exchange resin was used to prepare the composition of polysaccharides. UV-spectra was used to detect protein and IR-spectra was used to analyses the ascription of the composition of polysaccharides. The content of the composition of polysaccharides was determined by anthranone-sulfuric acid method. Result There was no absorption at 240 nm and 260 nm. The IR spectrum graph of the polysaccharides was similar to Polygonatum odoratum, Radix Ranunculi Ternati and Agrimonia pilosa Ledeb. The content of the composition of polysaccharides was 1.41%. Conclusion The composition of polysaccharides does not contain proteins and nucleic acids, and come from Polygonatum odoratum, Radix Ranunculi Ternati and grimonia pilosa Ledeb.
3.Advances in the diagnosis and treatment of dipeptidyl-peptidase-like protein 6 antibody- associated encephalitis

Dipeptidyl-peptidase-like protein 6 (DPPX) antibody-associated encephalitis, mediated by DPPX antibodies in serum and/or cerebrospinal fluid, is a novel autoimmune encephalitis. It presents with multifocal and diverse neurological disorders characterized by the triad of prodromal weight loss and/or gastrointestinal symptoms (diarrhea predominately), cognitive-mental dysfunction, and central nervous system hyperexcitability. Brain magnetic resonance imaging shows non-specific abnormalities. Immunotherapy can result in clinical improvements, but the disease is prone to relapse.
4.Prenatal screening for major fetal heart defects by training sonographers in township hospitals and county health centers:a feasibility study

Objective:To explore the feasibility of screening for major fetal heart disease by training sonographers in township or county level hospitals .Methods:Training of B ultrasound scan for congeni-tal heart defects was given to the sonographers from one county hospital , and thirteen township hospitals ( or the district hospitals ) , and training of fetal echocardiography was given to sonographers from four city/county hospitals.The trained sonographers who had passed the examinations and had obtained quali-fications after six months of independent practice began to screen fetal congenital heart defects .To evalu-ate the effectiveness , sensitivity and specificity of screening was calculated by using the diagnosis of ex-pert neonatal/fetal echocardiographers as the gold standard .Results: A total of 3 425 fetuses received one fetal B ultrasound screening , one fetal echocardiography and one neonatal echocardiography from April 1, 2004 to December 31, 2005.One hundred and sixty-five B ultrasound screening images (4.9%) from township hospitals and fifty-six fetal echocardiography images (1.7%) from county or city centers couldn ’ t be reviewed because of poor quality .The sensitivity of fetal B ultrasound screening in the township and county hospitals was 30%and 0, and the specificity 93.3%and 99.9%, respectively. Nine fetuses with a major congenital heart disease were eventually found by the trained sonographers , and two cases were misdiagnosed and two unnoticed .The total sensitivity and specificity of fetal echocardio-graphy were 81.8% and 99.9%, respectively.The sensitivity in the county and city hospitals was 66 .7% and 100%, respectively .The specificity in the county and city hospitals was 99 .9%and 100%, respectively .Conclusion: Under the current circumstances , township hospitals are unable to perform effective fetal cardiac screening .Screening on fetal congenital heart disease is suggested to be taken by trained sonographers in county and city level medical centers .
5.Relationship between plasma homocysteine level and stroke

BACKGROUND: Hyperhomocysteinemia has been suggested to be a possible independent risk factor for stroke.OBJECTIVE: To explore the relationship between hyperhomocysteinemia and cerebral infarction and hemorrhage, and analyze the factors that affect plasma homocysteine level.DESIGN: Case-controlled clinical trial.SETTING: Department of Neurology, Second Hospital Affiliated to Medical College of Zhejiang University.PARTICIPANTS: Totally 57 patients including 21 with cerebral hemorthage and 36 with brain infarction were treated in the Department of Neurology, Second Hospital Affiliated to Medical College of Zhejiang University Between January and November 2003. Twenty-eight healthy volunteers were also recruited from the subjects coming for routine physical examination.METHODS: Two milliliters of fasting venous blood was collected from all subjects in the morning for detecting the contents of plasma homocysteine,vitamin B12, folic acid, creatinine and so on. All patients were scored for clinical neurological impairment, with the hematoma volume calculated in patients with brain hemorrhage determined on the basis of CT scanning.acid, vitamin B12, clinical neurological impairment score and hematoma volume.RESULTS: Valid results were obtained from all the 57 stroke patients and in male and female patients of both cerebral infarction group and cerebral hemorrhage group than that of the subjects of the same gender in the control group [(25.2±21.4), (18.3±10.9), (11.5±2.9) μmol/L for male subjects;(22.8±18.9), (14.7±7.4), (10.8±2.6) μmol/L for female subjects, P< 0.05-0.01].The level of homocysteine was similar between cerebral infarction group and cerebral hemorrhage group, homocysteic acid level showed obvious inverse correlation with folic acid level (r=-0.442, -0.531, P < 0.05), but without relation to vitamin B12 level (r=-0.086, -0.111, P > 0.05). Homocysteine level was not obviously correlated to the neurological impairment scores in cerebral infarction group (r=-0.139, P > 0.05), nor was it related to the scores or hematoma volume in cerebral hemorrhage group (r=0.225,0.425, P > 0.05).CONCLUSION: Hyperhomocysteinemia is risk factor for cerebral infarction and hemorrhage. Plasma homocysteine level is inversely correlated with folic acid level, but not obviously related to vitamin B12, clinical neurologicla impairment score or hematoma volume.
6.Features of clinical and therapy in juvenile myoclonic epilepsy: an analysis of 87 cases

Objective To analyze clinical characteristics of patients with juvenile myoclonic epilepsy in China. Methods Eighty-seven patients were retrospectively studied in the aspects of family history, febrile seizures, clinical features, EEG, treatment effect. Results There was a female preponderance of incidence. In contrast to the earlier studies we found a high incidence of febrile seizures and a low incidence of family history. myoclonic seizures began at age of ( 13.1?3.4) years. That combined with generalized tonoclonic seizures began at age (14.3?3.8) years. Absence seizures began at age (10.0?3.3) years. The correct diagnosis was delayed at a mean of 2.2 years from onset of the disease. The incidence of abnormal EEG discharge could be enhanced by hyperventilation, photic stimulation and sleep. Sixteen patients who had received carbamazepine or phenytoin were experienced aggravation of seizures. Forty-five patients who received monotherapy with sodium valproate remained seizure-free in a follow-up longer than 0.5 years. Conclusions Failure to recognize JME may result in uncontrolled seizures, and even aggravated of seizures by using antiepilepsy drugs. Effective treatment was achieved with small doses of sodium valproate.

Objective To investigate the effect of the extracts of Centella asiatica(L.) Urban (ECA) on the depressive-like behavior alteration in rats and mice. Method 60 Wistar rats were randomly divided into 6 groups (10 rats each), the normal group, model group, Fluoxetine group, ECA high dose group (component 10.08 mg+component 24.48 mg/kg), ECA middle dose group (component 1 0.04 mg+component 2 2.24 mg/kg) and ECA low dose group component 1 0.02 mg+component 2 1.12 mg/kg. Chronic unpredictable mild stress (CUMS) induced depression in rats and sugar consumption to detect the antidepressant effect of ECU. 60 Kunming mice were randomly divided into six groups as above. Locomotor activity, forced swim, tail suspension test were used to detect the effect of ECA on the behavioral alteration in mice. Results Compared with model group, sugar consumption in ECA groups was significant increased (P<0.01). ECA treatments also decreased duration of immobility in the tail suspension test and forced swim. Conclusion ECA could improve the depressive-like behavior in rats and mice.

9.Effect of health education pathways on hospitalized patients with type 2 diabetes: A Meta-analysis

Objective To evaluate the therapeutic effect of health education pathway on type 2 diabetic patients by using the Meta-analysis.Methods The domestic main databases were searched,the quality of included studies which screened by certain standards was evaluated.The Review Manager 5.0software was taken for analysis.Results Totally 12 studies were included,containing 2115 cases of type 2 diabetic patients.The Meta-analysis showed there were significant differences between the health education pathway group and the control group in hospitalization days and hospitalization expenses,rate of patients' satisfaction and rate of mastering knowledge of diabetes mellitus,fasting blood glucose and 2h postprandial blood glucose.Conclusions Health education pathway for type 2 diabetic patients can shorten hospitalization days,decrease hospitalization expenses,increase rate of patients' satisfaction and rate of mastering knowledge as well as lower fasting blood glucose and 2h postprandial blood glucose.
10.Clinical study on the stomach tube insertion in the lateral recumbent position in rescuing the unconscious patients

Objective To study the application of stomach tube insertion in the lateral recumbent position in rescuing the unconscious patients,to reduce the discomfort of patients and increase the one-time success rate in the insertion of stomach tube. Methods 100 cases of unconscious patients were divided into the control group and the treatment group with 50 cases in each, they adopted routine tube insertion method and tube insertion in the lateral recumbent position.The success rate of tube insertion and incidence of complications were collected and analyzed in the two groups.Results The experimental group had a higher success rate and less complications compared with the control group.Conclusions The lateral recumbent position for stomach tube insertion is the ideal position for the unconscious patients.
