Objective To investigate the mechanisms of tigecycline nonsusceptibility in carbapen-ems-resistant Acinetobacter baumannii ( CRAB) strains in order to provide a theoretical basis for a reasonable use of antibiotics and the control of nosocomial infection .Methods Susceptibility testing of 120 non-dupli-cate CRAB strains to tigecycline was performed by using the broth microdilution method .Minimal inhibitory concentrations ( MIC) of tigecycline against the A.baumannii strains were determined by using the broth mi-crodilution method before and after exposing the strains to Carbonylcyanide-m-chlorophenylhydrazone (CCCP), which was the efflux pump inhibitor .Polymerase chain reaction (PCR) was used to amply the ef-flux pumps genes including adeB, adeJ, adeG, abeM, adeE, adeRS, tetX and tetX1.The real-time PCR was performed to measure the expression of efflux pumps genes including adeB, adeJ, adeG, abeM and adeE.Results A total of 120 CRAB strains were collected including 13 (10.8%) tigecycline non-suscep-tible A.baumannii (TNAB) strains and 107 (89.2%) tigecycline susceptible A.baumannii (TSAB) strains. The MIC values of tigecline to the 120 CRAB strains were in a range of 0.25 μg/ml to 8 μg/ml. The adeR and adeJ genes were detected in 90.0%and 92.5%of the 120 CRAB strains, respectively. The positive rates of adeB, adeS, adeG and abeM genes among the 120 CRAB strains were all 94.2%.None of the three genes including adeE, tetX and tetX1 were detected .The mean expression levels of adeB and adeJ in TNAB strains were respectively increased by 18.69 folds and 5.46 folds as compared with those in sensi-tive strains. No significant increase in the expression of adeG and abeM genes was observed in TNAB strains . A 4-fold decrease in the MIC was observed in 8 out of 13 TNAB isolates treated with 10 μg/ml of CCCP .The CCCP could partially reverse the resistance pattern of tigecycline .Conclusion The efflux pump sys-tems of adeABC and adeIJK rather than the abeFGH and abeM systems might play an important role in reduc-ing the tigecycline susceptibility in carbapenems-resistant A.baumannii strains.

evaluate the efficacy and adverse events of suvorexant for Chinese patients with primary insomnia over 6
months. Methods: A total of 120 patients with primary insomnia were assigned randomly to two groups
that received placebo or suvorexant (40 mg) for 6 months. The primary outcomes were the total sleep time
(sTST), time to sleep onset (sTSO), and sleep quality (sQUAL). The secondary outcomes were the Insomnia
Severity Index (ISI) score and adverse events. Results: A total of 111 patients completed the study and
all of them were included in the final analysis. Suvorexant showed greater efficacy than the placebo in
enhancing sTST, sTSO, sQUAL and ISI score at months 1 and 6. Serious adverse events were documented
in 2 patients (3.3%) in the suvorexant group and 1 patients (1.7%) in the placebo group. The most common
adverse event was somnolence, which occurred in 7 patients (11.7%) in the suvorexant group and 2 patients
(3.3%) in the placebo group. No death related to suvorexant treatment was recorded.
Conclusions: Suvorexant was efficacious and well-tolerated in a group of Chinese patients with primary
insomnia over 6 months.

Objective To investigate the effects of different therapeutic formulas on the differentiation of bone marrow mesenchymal stem cells ( BMSC) to cardiomyocyte-like cells in rat myocardium of in vitro micro-environment. Methods 40 SD rats randomly divided into blood-activating stasis-resolving group, qi supplementing blood-activating group, qi supplementing group, and blank group were administered o-rally Chinese medicinals to obtain madicated serum;another 60 SD rats were used to extract, incubate, i-solate and purify BMSC;SD newborn rats were used to extract and incubate myocardial cell, then to es-tablish an external myocardial micro-environment. MTT assay was used to select the optimal compounding
concentration of each treatment group. Flow cytometry was used to measure the proliferation of CD105 +-BMSC of each treatment group. Immunocytochemical method was used to test the CD105 +-BMSC’ s ex-pression of myocardium protein CTNT, GATA-4 while immunofluorescence technique was used to meas-ure the expression of MHC. Results The doubling time of CD105 +-BMSC of each treatment group was 24 h. The cells numbers in blood-activating and stasis-resolving group were significantly higher than those in control group and other treatment groups from the 2nd day, and remained steady. All the proliferation index of blood-activating and stasis-resolving group at every time point were the highest except the 1 st day. The number of cell division was also significantly more than other groups: qi-supplementing and blood-activating group, qi-supplementing group, blood activating and stasis resolving group. From the 2nd day to 5th day, the expressions in the blood-activating stasis-resolving group, qi-supplementing blood-activating group, qi-supplementing group increased markedly with statistical significance compared with the blank group ( P<0 . 05 ) , blood-activating stasis-resolving group was superior to the other two groups(P<0. 05). There were no statistical significance between the qi-supplementing blood-activating group and qi-supplementing group ( P>0 . 05 ) . Conclusion Under the external mimic myocardial mi-cro-environment condition, all therapeutic formulas could promote CD105 +-BMSC ’ s differentiation. They also could promote CD105 +-BMSC ’ s metaplasia to the cardiomyocyte-like cells, however, the blood-activating stasis-resolving medicinal appears to be most effective.

Objective To evaluate the feasibility of quantitative evaluation of fallopian tubes patency by the rising slope of pressure curve produced by bolus injection using a liquid diagnostic apparatus in the four-dimen-sional hysterosalpingo-contrast sonography. Methods A total of 205 patients with infertility were collected for four-dimensional hysterosalpingo-contrast sonography. In the examination, the contrast medium concentration, the liquid volume of the liquid diagnostic apparatus and the speed of the liquid were stationary calculated. The rising slope of the curve was calculated according to the real-time pressure curve produced by the contrast agent injection process. According to the hysterosalpingo-contrast sonography examination's results ,patients were divided into one side patency group, one side partial obstruction group and one side obstruction group. The correlation between the slope and the patency of the fallopian tube was analyzed by binary logistic regression. Results Binary logistic regression showed that the slope and fallopian tube patency had good correlation(all P < 0.05)between one side patency group and one side obstruction group, and also between one side partial obstruction group and one side obstruction group;but had no correlation between one side patency group and one side partial obstruction group(regression coefficient P>0.05,regression constant P=0.000). Conclusion The slope of pressure curve produced by four-dimensional hysterosalpingo-contrast sonography can be used to assess the patency of fallopian tubes quantitatively. It is a new auxiliary index for evaluating the patency of fallopian tubes.
