1.Observation of the effect of Bupiyichang pills combined with levocetirizine in the treatment of 56 patients with chronic urticaria

Objective To observe the effect of Bupiyichang pills combined with levocetirizine in the treatment of 56 patients with chronic urticaria,so as to provide clinical guidance and basis.Methods 118 patients with chronic urticaria were selected for our study,and after informed consent they were randomly divided into observation group and control group.The observation group were was treated by Bupiyichang pills combined with levocetirizine,and the control group were was treated by levocetirizine.When they were curingAfter treatment for 10,20,and 30 days,the effective rates in the two groups were calculated and compared with each other.And at the day before treating and the day after treating for 20 days,the serum IgE levels in each group was detected and compared.The falling ranges between the two groups were calculated and compared.Results When they were curing treated for 10,20,and 30 days,the effective rates in the observation group were 76.79%,94.64 %,85.71%,respectively,which in those of the control group were 59.68%,74.19%,64.52%,respectively,the differences between the two groups were statistically significance significant (x2 =3.94,9.11,6.97,all P < 0.05).At the day before treating and the day after treating for 20 days,the serum IgE levels in the observation group were (384.71 ± 65.62) U/mL and (214.59 ±40.71) U/mL respectively,the difference was statistically significance significant (t =10.93,P < 0.05);and the serum IgE levels in the control group were (380.25 ± 67.81)U/mL and (256.99 ± 53.21)U/mL respectively,the difference was statistically significancesignificant (t =9.37,P < 0.05).The falling ranges in the observation group and the control group were (170.12 ± 35.59) U/mL and (123.26 ± 53.31) U/mL respectively,the difference was statistically significance significant between the two groups (t =7.82,P < 0.05).Conclusion Bupiyichang pills combined with levocetirizine is more effective than single medication of levocetirizine.
2.The clinical effects of the packet therapy combined with Chinese prescription soaking in the treatment of ;49 patients with verruca plantaris

Objective To explore the clinical effects of the packet therapy combined with Chinese prescrip-tion soaking in the treatment of patients with verruca plantaris,so as to provide clinical guidance and basis.Methods 101 patients with verruca plantaris were chosen as study objects,and they were divided into two groups,namely obser-vation group and control group.Observation group was treated by the packet therapy combined with Chinese prescrip-tion soaking,and control group was treated by Chinese prescription soaking.When they were cured for 4 weeks,the main symptoms and effective rate in the two groups were calculated and compared.For the recovery patients,when their drugs were withdrew for 8 weeks,the recurrence rate between the observation group and control group was compared.Results After being treated for 4 weeks,both of the skin lesion and pain degree between the observation group and control group had significant differences (Z=4.48,P<0.05;Z=4.18,P<0.05).The effective rates in the observation group and control group were 85.71% and 48.08%,respectively,and the difference between the two groups was statistically significant (χ2 =16.00,P<0.001).The recurrence rates in the observation group and control group were 13.33% and 27.27%,respectively,and the difference between the two groups was no statistically signifi-cant (P>0.05).Conclusion The packet therapy combined with Chinese prescription soaking is more effective than Chinese prescription soaking.
3.Effective dosage of sirolimus for seizure treatment of immature C57BL/6 mice induced by kainic acid

OBJECTIVE To explore the safe and effective dose of sirolimus (Rapamycin,Sir) and its effect on seizure comorbidities. METHODS Immature C57BL/6 mice at postnatal 10 d of age were administered with kainic acid(KA) 12.0 mg · kg-1 intraperitoneally by a single injection to induce acute seizure. Sir 0.3, 1.0 and 3.0 mg · kg-1 was injected 24 h after seizure every other day until 3 d, 1 week, 3 weeks, 5 weeks and 6 weeks. Western blotting analysis was used to detect the expression and phos?phorylation level of S6 protein and to determine the minimum effective dose of Sir. Effect of the mini?mum effective dose of Sir on cognitive function and body growth was observed by several evaluations. Immunofluorescent intensity of Doublecortin (DCX) immunofluorescent staining was conducted to evaluate the development of neurons in the hippocampus. Morris water maze was used to assess the cognitive function. Tail suspension test, O maze and new object recognition test were used to study the anxiety-like behaviors of mice. RESULTS The result of Western blotting showed that Sir 0.3 mg · kg-1 had no significant effect on the phosphorylation of S6 protein in normal mice or KA mice, whereas 1.0 and 3.0 mg · kg- 1 could significantly inhibit the phosphorylation of S6 protein in KA mice (P<0.05). Sir 1.0 mg·kg-1 had no obvious effect on DCX-positive cells or body wass. Morris water maze showed that KA-induced seizure resulted in prolonged escape latency and swimming length (P<0.05), and a decreased crossing number of target quadrant (P<0.05). Sir 1.0 mg·kg-1 significantly reversed the deficit of cognitive function of KA-induced seizure mice (P<0.05), whereas no significant difference was found between Sir group and normal control group. Compared with normal control group, model group showed increased freezing time in tail suspension test (P<0.05), decreased migration length and reten?tion time in open arms in O maze (P<0.05), decreased retention time and touch frequency with new objects, migration length and average speed in new object recognition test (P<0.05). Sir 1.0 mg · kg-1 significantly reversed the above anxiety and depression status, whereas no significant difference was found between sirolimus group and normal control group. CONCLUSION Sir 1.0 mg · kg-1 inhibits the abnormal activation of mTOR pathway and the formation of epilepsy comorbidity in immature mice. Along with its mild side effect in development, Sir 1.0 mg · kg-1 will be an ideal dose to be used in the treatment of seizure in immature mice.
4.Detection and Genotyping of Treponema pallidum by a Nested PCR

Objective To develop a nested PCR for the detection of early syphilis and genotyping of Treponema pallidum (TP), and to investigate the distribution of genotypes of TP in Guangzhou. Methods Specimens were consecutively collected from genital ulcers of patients with suspected chancre during 2002-2004, and were detected by dark-field microscopy and nested PCR. The acidic repeat protein (arp) gene and the T. pallidum repeat (tpr) gene family were amplified with the positive specimens above. The number of repeats presented in the arp gene and the restriction fragment length polymorphism by Mse I in the tpr gene were analyzed by electrophoresis. The strains were genotyped according to Pillay's criteria. Results Out of 62 patients with suspected chancre, 33 cases (53.2%) were positive by dark-field microscopy and 54 cases (87.1%) by nested PCR. Of 47 TP-positive specimens genotyped by arp gene, 36 (76.6%) were type 14, while of 49 cases genotyped by tpr gene 39 (79.6%) were type d. By combining genotypes of arp and tpr genes, 7 genotypes were found, including 14d (31, 66.0%), 13d (5, 10.6%), 14b (4, 8.5%), 12b (3, 6.4%), 12d (2, 4.3%), 15d(l, 2.2%) and 14i (1, 2.2%). Conclusions Nested PCR shows a high sensitivity in early detection of TP. Genotype 14d seems the predominant type of TP in Guangzhou.
5.Mechanism of Neiyi Kangfu Tablets in treatment of adenomyosis through regulating GPER-Ras-STAT3 signal pathway

AIM To study the therapeutic effects of Neiyi Kangfu Tablets (Trionycis Carapax,Ostreae concha,Rhei Radix et Rhizoma,etc.) on adenomyosis and its possible mechanism of action.METHODS The mouse model for adenomyosis was set up by the pituitary allograft,and received the gavage of Neiyi Kangfu Tablets,Dan'e Fukang Decoction (Salviae miltiorrhizae Radix et Rhizoma,Curcumae Rhizoma,Paeoniae Radix rubra,etc.) and gestrinone,respectively,for three months.Western blot,real-time PCR,IHC were used to measure the expression of GPER-Ras-STAT3 in ectopic endometrium and entopic endometrium.RESULTS High doses of Neiyi Kangfu Tablets could significantly lower GPER protein and gene expression;down-regulate the Ras and STAT3 protein expression;reduce the lesion severity score of ectopic tissue morphology.Its curative effect was better than Dan'e Fukang Decoction and gestrinone.CONCLUSION Neiyi Kangfu Tablets may control the development of ectopic lesions by inhibiting the GPER-Ras-STAT3 pathway activity and weakening the production of estrogen.
6.Effect of the evidence-based nursing to prevent the deep vein thrombosis after orthopaedic surgery:a Meta-analysis

Objective To use the method of Meta-analysis to evaluate the effect of the evidencebased nursing in preventing the deep vein thrombosis after orthopaedic surgery,to provide the basis for clinical prevention of DVT.Methods The randomized controlled trials (RCT) on the evidence-based nursing in preventing the DVT were collected using the databases of China National Knowledge Infrastructure (CNKI),Wanfang Periodical Databases,Chinese Science Technology Periodical Databases (VIP) and PubMed and analyzed by RevMan 5.2 software.Results Nine RCTs were included in the study.The results of Meta-analysis revealed that the incidence of the DVT could be effectively prevented by evi-dencebased nursing.Conclusions The evidence-based nursing can reduce the incidence of the DVT after orthopaedic surgery.
7.Study on the clinicopathologic features of 88 cases with mucinous cervical adenocarcinoma

Objective To analyze the clinicopathologic features of mucinous cervical adenocarcinoma (ADC) and supply some reference for its diagnosis,treatment and prognosis.Methods Totally 88 cases with primary mucinous ADC diagnosed between January 2003 and December 2007 in Jiangxi Maternal and Child Health Hospital were retrieved.Their clinical and pathological data were reviewed and analyzed.Results Among 88 patients,70 cases of them were endocervical ADC,11 cases were minimal deviation adenocarcinoma (MDA),2 cases were intestinal subtype and 5 cases were villoglandular ADC.The positive rate of cervical Pap smear cytology was only 28% (10/36).There were 10 cases were performed several times of cervical Pap smear cytology test and biopsies,while still no abnormality was found.Finally,7 cases of them were diagnosed by cervical excisional biopsy and endocervical curettage,including 5 cases with endocervical ADC and 2 cases with MDA; 2 cases (1 case with stage Ⅰ b1 endocervical ADC and 1 case with Ⅰ b1 MDA) were diagnosed by cold knife conization,and 1 case with stage Ⅰ b1 endocervical were diagnosed by uteroscopically directed biopsy.The overall 5-year survival rate was 60.0%.Using univariate analysis results showed that age (P =0.046),stage (P =0.007),tumor size (P =0.007) and therapeutic methods (P =0.009) were significant prognostic factors.Multivariate analysis showed that therapeutic methods was an independent predictor for survival (P =0.013).Stage Ⅱ b-Ⅲ b MDA patients occupied 7/11 of all MDA patient,while there was only one such patient of 5 cases with villoglandular ADC.The overall HPV infection rate was 65% (57/88),and there was no obvious differences about the HPV infection rates among different types of mucinous ADC.Conclusions Cervical smear cytology is of little significance to the diagnosis of mucinous cervical adenocarcinoma.Early stage mucinous ADC is difficult to be detected,especially for MDA.Cervical excisional biopsy,endocervical curettage,cold knife conization,uteroscope are beneficial to its diagnosis.Therapeutic methods is an important prognostic factor for mucinous ADC and comprehensive treatment should be given to those patients with risky postsurgical factors.

9.The impact of nursing intervention based on MEWS for potential accidents in critical patients:a systematic review

Objective To evaluate the impact of programmed nursing intervention based on MEWS for potential accidents in critical patients.Methods The randomized controlled trials (RCT) on programmed nursing intervention based on MEWS for potential accidents in critical patients were collected using the databases of PubMed,Medline,FMJS,CNKI,Wanfang database,VIP.Data were analyzed with RevMan5.0 software.Results Four RCTs were included in the study.All were Chinese articles.The incidence of accidents in the intervention group was significandy lower than that of the control group,the difference was statistically significant.Conclusions Programmed nursing intervention based on MEWS can effectively reduce the potential accidents happened in critical patients.

Objective To evaluate the effect of the bundle of care interventions in preventing ventilator-associated pneumonia (VAP) in the mechanic ventilated patients.Methods The randomized controlled trials (RCT) on the bundle of care interventions in preventing ventilator-associated pneumonia (VAP) were collected using the databases of PubMed,Medline,Embase,Cochrane library,China National Knowledge Infrastructure (CNKI),Wanfang Periodical Databases and Chinese Science and Technology Periodical Databases (VIP),data were analyzed by RevMan5.0 software.Results Ten RCTs were included in the study.The results of Meta-analysis revealed that the incidence of ventilator associated pneumonia,mechanical ventilation time,ICU length of hospital stay and the mortality rate in the intervention group were different from the control group,the difference had statistical significance.Conclusions The bundle of care interventions can reduce the incidence of ventilator-associated pneumonia,mechanical ventilation time,ICU length of hospital stay and the mortality rate.
