1.The study of 3DCRT plan for hepatic tumors

Objective To study the 3D CRT plan for hepatic tumors. Methods The 3DCRT plan of 50 cases of primary or metastasis hepatic cancer were designed according to the request of the doctors for CTV and the restricted receiving dose of the peripheral normal sensitive organs. The best plan was chosen referring the dose-volume histogram. Results By using ICRU dose reference point (the central point of tumor) as the dose 100 % and 80 % ~ 90 % dose line can be surrounded above 90 % of the PTV(planning tumor volume). The degree of 90 % dose line surrounding PTV was related to the scope of target area and its location. Conclusion The focus in different localization can be treated with different design when devise the treatment plan of the hepatic tumors. The reaction to radiotherapy was directly influenced by the volume of dosage of radiation to the normal hepatic tissues, and it also had great influence to the choice of prescription dose and the dose division style.
2.Relationship of hs-CRP with the state of anxiety and depression in patients with acute coronary syndrome

Objective To explore the relationship of the presence and severity of anxiety and depression with the increased inflammatory activity,as marked by the serum levels of high sensitivity C reactive protein(hs-CRP)after acute coronary syndrome(ACS).Methods Serum hs-CRP levels were measured in 647 ACS patients within 36 hours after onset of event.Depression and anxiety were evaluated by self-reporting standardized questionnaire,using a validated Chinese version of Hospital Anxiety and Depression Scale(HADS)(14 items)within 7 days.Results In ACS patients,serum levels of hs-CRP(mg/L)were lower in those with anxiety than those in control group[(10.43?3.55)mg/L vs(13.19?4.90)mg/L,P0.05].Conclusion Presence and severity of depression is associated with increased activity of inflammation in patients with ACS;however,anxiety does not have such an association with inflammation in patients with ACS.

4.The exploration of improving perfraction dose by 3DCRT

Objective To evaluate the feasibility of increasing the perfraction dose in treatment of neoplasms by 3DCRT (three- dimention confornial radiation therapy). Methods From May 1998 to June 2002, the radiation therapy plans of 300 out- cranial neoplasms patients were analysed retrospectively, including 143 patients with chest neoplasms and 157 patients with abdomen neoplasms. The PTV was 7.0 ~ 1 478 cm3, major PTV was encircled by 90 % isodose curve, minor 95 % PTV were encircled by 80 % isodose curve. Prescription dose was 90 % reference point dose, perfraction dose was 5 ~ 10 Gy, a majority of dose was 6 ~ 8 Gy, period of treatment was 5 ~ 15 days with an interval of 0 ~ 1 day. The general dose was given to radical cure dose or appeasement dose. The biological effect increased 10 % ~ 30 %. Results All treatment plans were accomplished and there were not complication which reduced patients' QOL. Conclusions 1.Owing to the f factor, increasing dose of perfraction, shortening general period of treatment and improving radiative biological effect were possible during the 3DCRT. 2. It was suggested that the larger out- cranial neoplasms should be treated by 3DCRT firstly, but precise plan, precise design and precise treatment can not intensely be pursued because of the limit of knowledge. 3. During the 3DCRT for out- cranial neoplasms, 2 ~ 3 times routine radiation therapy dose was secure, credible and effective according to different purpose.
5.Fingerprints of Different Ethanol Extracts of Tripterygium Wilfordii Hook.f.and Spectrum-activity Relationship in Rat Mesangial Cells

Objective:To investigate the proliferation of rat glomerular mesangial cells (GMC) influenced by different ethanol extracts of Tripterygium wilfordii HooK F.(TWHF).Methods:An HPLC method was used to establish the fingerprints of 5 different ethanol extracts of TWHF,and GMC was chosen to study the effects of different ethanol extracts of TWHF on cell proliferation.After statistical analysis,the spectrum-activity relationship was analyzed by using partial least squares regression(PLSR).Results:The HPLC fingerprints of the 5 different ethanol extracts of TWHF were established,and 32 characteristic peaks were characterized by the HPLC fingerprints.60%,70% and 95% ethanol extracts and glycosides tablets showed dose-effect relationship,and with the increase of dose,the more significant inhibition of cell proliferation was exhibited.The absorbance values of the 60% ethanol extracts at medium and high doses were lower than those of the other extracts at the same dose.The proliferation inhibition rate of GMC was used as the potency index and analyzed by PLSR,and 20 peaks were potency peaks at high dose(40 μg·L-1),17 ones were potency peaks at medium dose(20 μg·L-1) and 15 ones were potency peaks at low dose(10 μg·L-1).Conclusion:Part of the potency peaks has regular dose-effect relationship with the changes of dose.
6.Overexpression and clinical implication of MDM2 oncogene in acute leukemia

Objective To study the over-expression and clinical implications of the oncogene MDM2 in acute leukemia (AL). Methods The expression of MDM2 gene in 100 patients with newly diagnosed and relapse or refractory AL and 20 healthy as control was measured by relative quantitative reverse transcriptase polymerase chain reaction (RT-PCR),then the results was measured by χ2-test,t-test and one-way ANOVA to compare expession positive rate and intensity of MDM2. Results Among 100 patients,fifty-eight had the high expression of MDM2 gene (58 %). The expression level of MDM2 gene in patients was higher than that of health controls(P <0.05). The expression positive rate of MDM2 is higher in poor outcome group (67.9 %,19/28)than that in general outcome group (33.9%,19/56) (P<0.05). Conclusion Our results suggest that the expression of MDM2 gene plays an important role in the pathogenesis and poor outcome of AL.
7.Quantitative analysis of lineage-specific chimerism using real-time PCR and single nucleotide polymorphisms

Objective To develop a real-time quantitative PCR method with TaqMan probe to analysis the lineage-specific chimerism based on single nucleotide polymorphisms (SNP).Methods CD3 positive and CD15 positive cells were separated by magnetic cell sorting system from cord blood,and a quantitative method were established using real-time quantitative PCR and SNP.Detect the artificial chimerism and mark the standard curves.The reaction system was optimized,and the sensitivity and specificity were evaluated.Results Separation purity of blood cells by magnetic cell sorting system was up to 94 %-97 %.Discrimination between donor and recipient was possible.Dilution experiments of the mock chimerism sample revealed that Ct values correlated linearly with the logarithm of recipient/donor DNA fraction (r > 0.98),and the limit of detection for a minor DNA percentage was under 0.1%,and the specificity was also good.Quantitative analysis of 4 clinical cases in same period were made by real-time fluorescence quantitative SNP-PCR,the fitted rate was 87.50 % based on the chimera standard curve calculated for 1 case.Conclusion The method shows high sensitivity and specificity,and will be used to quantify the lineagespecific chimerism after allogeneic hematopoietic stem cell transplantation.
8.Detection of ATM deletion in chronic lymphocytic leukemia by fluorescence in situ hybridization

Objective To investigate ATM deletion [del (ATM)] in chronic lymphocytic leukemia (CLL) and study its correlation with the clinical stage. Methods Spectrum Orange~(TM) labeled sequence specific DNA probe for ATM locus on 11q22.3 and fluorescence in Situ hybridization (FISH) was used to examine del (ATM) in 28 newly diagnose patients with CLL. FISH analysis were performed on bone marrow smears. Clinical staging was done according to Binet Method.Fisher exact propability was used to study the relations between del (ATM) and clinical feature. Results 4 out of the 28 cases were found with deletion of ATM. The incidence of del (ATM) in BinetA, BinetB and BinetC was 1/9 (11.1 %), 1/8 (12.5 %), 2/11 (18.2 %), respectively. Fisher exact propability showed that deletion of ATM was not associated with its clinical feature. Conclusion Application of FISH on archival bone marrow smears is a simple, liable method, and can be readly used to retrospective study of clonal blood system diseases. Deletion of ATM was common cytogenetic changes in CLL patients.And the significance of del (ATM) in the prognosis of CLL in China needs to be further investigated.
9.The application of SimMan 3G situational simulation method in anesthesiology teaching

Objective To evaluate the application value of situational simulation method based on SimMan 3G in anesthesiology teaching. Methods 5-years anesthesiology under-graduates of 2012 grade and 2013 grade in Wannan Medical Colloge anticipating in clinical practice of anesthesia were selected as observation group. All students were divided into two groups: control group (teachers demonstrated cases, 2012 grade, n=35) and observation group (students role-played simulation cases, 2013 grade, n=36). In control group, teachers firstly applied a multimedia courseware, and then demonstrated the cases to adopt the SimMan 3G simulated system. In observation group, SimMan 3G situational simulation method was used to teach the course , and then the students played roles and videotaped the whole course in the simulated operating room. After training, the students discussed their performance in the simulated training (by the playback of the simulated video), and the teachers commented and summarized the case. The effect of teaching was evaluated by questionnaire and examination. The date were analyzed through Chi-square and t-test test. Results There was no significant difference in essential characteristics, including age, gender, the scores of specialized theory at routine examination between two groups (P>0.05). Statistical difference was found in the theoretical assessment between observation group and control group [(79.44±5.41) vs. (73.46± 4.62), P=0.001]. Meanwhile, questionnaire survey of teaching effect showed that in observation group 31 students (86.1%) were satisfied with the teaching contents, 33 students (91.7%) had consolidated clinical thinking, and 34 students (94.4%) believed it improved their teamwork and communication ability (P=0.031, P=0.018, P=0.013). Conclusion Using SimMan3G for role-playing situational simulation teaching can significantly improve student's comprehensive theoretical scores, facilitate the exercise of clinical thinking, and improve their teamwork and communication skills.
10.Comparison of two health education intervention on tuberculosis prevention in schools

Objective:
To compare the effectiveness of Internet plus mode with tradition education mode on tuberculosis prevention in schools,so as to provide the reference for reducing the risk of catching tuberculosis in schools.
Methods:
Two junior and two senior high schools as well as two universities were selected from Baoding. The two same level schools were randomly divided into the traditional education group and the Internet plus group. All the students received 2-month TB health education intervention. Questionnaire survey was conducted before and after intervention. A total of 2 804 and 2 821 students were investigated before and after intervention respectively.
Results:
The awareness rate of TB prevention and treatment core knowledge among the traditional education group and the Internet plus group before intervention was 47.5% and 47.8%, respectively (χ2=0.19, P>0.05). After intervention, the percentage of students with positive attitude and behavior regarding TB was 93.0% and 85.1% in the Internet plus group and the traditional model education group, respectively, including willingness to accept TB test (88.6% vs 81.5%), active reporting to school on TB diagnosis (96.4% vs 90.5%), no spitting in public, cover up when sneeze or cough (94.3% vs 90.6%), opening windows for ventilation (98.1% vs 95.7%), and willingness to share knowledge (98.7% vs 96.4%), active prevention of TB (86.3% vs 78.2%)(P<0.05).
Conclusion
Internet plus mode health education intervention shows more effectiveness on tuberculosis health education, compared with traditional health education.
