1.Diagnostic significance of c-kit and Ki-67 expression in the baseal cell adenoma and adenoid cystic carcinom of salivary gland

Objective To study the expression of c-kit and Ki-67 in the basal cell adenoma(BCA)and adenoid cystic carcinoma (ACC) of salivary gland. Methods The expression of c-kit and Ki-67 was determined by immunohistochemistry method in BCA and ACC of salivary gland. Results The positive immuostaining of c-kit in BCA and ACC were 72.2 %(13/18) and 83.3 %(35/42),respectively, There was no significant difference in the expression of c-kit between the two tissues (x2=0.11,P>0.05).The expression of Ki-67 was significantly different between BCA[(3.72±1.41) %]and ACC[(23.81±10.19)%](t=14.145,P<0.01).Conclusion Detection of Ki-67 might be helpful to distinguish between BCA and ACC,but c-kit has no effect in differentiating BCA from ACC.
2.Comparison of postoperative analgesic effect of flurbiprofen axetil and parecoxib sodium in patients with humeral shaft fracture in painless ward

Objective:To compare postoperative analgesic effect of flurbiprofen axetil and parecoxib sodium in patients with humeral shaft fracture in painless ward.Methods:All of 200 hospitalized humeral fractures patients were retrospectively studied in the painless ward of the Forth Medical Center of PLA General Hospital from January 2017 to September 2019 , the clinical effects of flurbiprofen axetil and parecoxib were compared.Results:Postoperative visual analogue scale (VAS) scores after 3 d of two groups of patients were significantly lower, compared with preoperative results: (4.26±0.96) scores vs. (6.09±1.38) scores, (4.04±1.19) scores vs. (6.04±1.11) scores, and the differences were statistical significantly ( P<0.01). Postoperative VAS score after 3 d of two groups had no statistical significance ( P>0.05). Two groups had different degree of adverse reactions after operation, and flurbiprofen axetil group had singnificant gastrointestinal adverse reaction: 22 patients vs. 3 patients( P<0.05). The number of patients in the parecoxib group were more than that in the flubiprofen axetil group without troubled sleep: 20 patients vs. 8 patients. Two groups of patients were satisfied with the pain care during hospitalization. Conclusions:Two analgestic drugs can obtain obvious analgesic effect in the treatment of bones surgery. The side effects and sleep disturbance in the flurbiprofen axetil group are higher than those in the parecoxib group. Good pain control can improve patients satisfaction with pain care.
3.The effects of BMP-2 expression in the related periodontal tissue on bone remolding of rapid tooth movement by resistance reduction and distration method

Objective:To investigate the effects of bone morphogenetic protein 2(BMP-2)expression in the related periodontal tissue on bone remodeling under different distracting force during rapid tooth movement by resistance reduction and distraction. Methods:1 2 Beagle dogs were randomly divided into 4 groups as follows:5 d distraction,1 5 d distraction,1 5 d distraction and 1 0 d retaining and 1 5 d distraction and 90 d retaining.4 4 were distalized.6 teeth in each group were randomly assigned to re-sistance and distracting method,resistance and conventional method and conventional method,and there were 2 teeth in each group.Moving teeth models were prepared regularly.BMP-2 expression was examined by immunohistochemistry.Results:The BMP-2 positive expression of the 3 groups of different distraction schedule showed similar distribution area,and it reached peak at the end of 1 5-day distration,but the group of resistance and distracting method showed the maximum peak(P <0.05)and maximum tooth movement(P <0.01 ).In different stages of distraction,the positive expression in the group of resistance reduc-tion and conventional method was higher than in the group of conventional method but lower than the group of resistance reduction and extraction method(P <0.05).There was no significant difference in BMP-2 positive expression among 3 groups when retai-ning was 90 d(P >0.05).Conclusion:Resistance reduction with sustained strong distracting force can significantly increased the positive expression of BMP-2 and effectively accelerate new bone formation in periodontal tissue.
4.Influence of socio-economic status on dietary quality of children and adolescents in Fangshan District, Beijing

Objective:
To assess the effects of Socioeconomic status on the dietary quality of children and adolescents, and to provide the reference for improving primary students’ dietary quality.
Methods:
A total of 2 496 students from 16 schools (4 urban primary schools, 4 rural primary schools, 4 urban middle schools, and 4 rural middle schools) in Fangshan District, Beijing, were included using a stratified cluster random sampling method. Information on 7-day intake of cereals, vegetables, fruits, meat and dairy was collected using questionnaires and dietary quality was graded according to the Dietary Guidelines for Chinese Residents (2016).
Results:
The dietary quality for urban children and adolescents was better than that for rural ones. Compared with rural children, their urban peers had higher scores for intake of vegetables, fruits, and dairy (all were P<0.01), but lower scores for intake of meat (P=0.01). For children with parental education level of senior high school, the dietary scores were higher in urban areas (P<0.05), whereas no urban-rural disparity was found for children with higher educated parents (junior college or above, P=0.17). In children with monthly household income less than 5 000 yuan, the urban dietary scores were higher than that of rural areas (P<0.01), whereas in the group above 5 000 yuan, the difference between urban and rural areas was no longer statistically significant(P>0.05). The multi-factor analysis showed that living in urban areas, being girls, being younger, with higher educated parents and with higher monthly household income were protective factors for dietary quality. However, the effect of household income on dietary quality differed between urban and rural areas (P<0.05).
Conclusion
In the process of dietary guidance and intervention, children with relatively low socio-economic level should be given a priority to provide appropriate nutrition education and welfare policies.
5.Parental influences on screen time among children and adolescents and associated gender disparity

Objective:
To analyze the effect of parental correlates on screen time among children and adolescents and to explore possible gender disparity,and to provide the reference for taking measures to control and prevent the phenomenon.
Methods:
By stratified cluster sampling, 4 schools were selected from each of the urban middle school, urban primary school, rural middle school and rural primary school in Fangshan district, Beijing city. Students and parents in the selected school were recruited and investigated on general demographic characteristics and screen time. The effect of parental correlates on screen time and associated gender disparity were analyzed.
Results:
The prevalence of high screen time among children and adolescents was 10.4%, with boys’(11.9%) higher than girls’(8.9%). Students whose father spent more than 180 min in moderate and vigorous physical activity weekly were at lower risk for high screen time(OR=0.64). Students whose father spent more than 120 min screen time during weekday or on average were at higher risk for high screen time(OR=1.88,1.91). Students whose mother spent more than 120 min screen during weekday, during weekend or on average were at higher risk for high screen time compared with control group(OR=1.87,1.52,1.76). Father’s screen time during weekday or on average showed impact on screen time of their daughters and sons. Mother’s screen time only showed impacts on their daughters.
Conclusion
A strong effect of parental daily screen time on children’s screen time has been observed. Parental practice and roles should be considered to tailor interventions for healthy screen time among children and adolescents.
6.Cannulated screws with sutures for treatment of transverse patellar fractures

Objective:To evaluate the clinical efficacy of cannulated screws with sutures in the treatment of patellar transverse fractures.Methods:A retrospective analysis was performed of the data of 70 patients with patellar transverse fracture who had been admitted to Senior Department of Orthopedics, The Fourth Medical Center of Chinese PLA General Hospital from January 2017 to March 2021. According to the construction methods for the tension band, the patients were divided into 3 groups. In group A of 21 cases subjected to fixation with cannulated screws with sutures (Fiber-Tape), there were 8 males and 13 females with a median age of 55.0 (48.0, 65.0) years; in group B of 32 cases subjected to fixation with Kirschner wire tension band, there were 15 males and 17 females with a median age of 52.5 (41.5, 63.0) years; in group C of 17 cases subjected to fixation with Cable-Pin system, there were 5 males and 12 females with a median age of 55.0 (37.0, 65.0) years. The 3 groups were compared in terms of complications, secondary surgery (removal of internal fixation), operation time, intraoperative blood loss and knee function rated by the Lysholm and B?stman scores at the last follow-up.Results:There were no significant differences in the preoperative general data between the 3 groups, showing they were comparable ( P > 0.05). There was no significant difference in the operation time, intraoperative blood transfusion or follow-up time among the 3 groups ( P > 0.05). The incidence of soft tissue irritation [4.8% (1/21)] and the secondary operation rate [4.8% (1/21)] in group A were significantly lower than those in group B [43.8% (14/32) and 37.5% (12/32)] and group C [41.2% (7/17) and 35.3% (6/17)] ( P < 0.05), but there was no statistically significant difference between group B and group C ( P > 0.05). In groups A, B and C, respectively, the Lysholm knee score was 84.0 (69.0, 88.0), 89.0 (71.5, 95.0) and 82.0 (63.0, 90.0), and the B?stman knee score 26.0 (23.0, 28.0), 26.5 (24.0, 27.5) and 26.0 (22.0, 28.0), showing no significant difference ( P > 0.05). There was no significant difference either in the incidence of other complications among the 3 groups ( P > 0.05). Conclusion:In the treatment of patellar transverse fractures, compared with the Kirschner wire tension band and Cable-Pin system, cannulated screws with sutures (Fiber-Tape) may lead to a lower incidence of soft tissue irritation and a lower rate of secondary surgery, but no significant differences in operation time, intraoperative blood loss, other complications or postoperative functional scores.
7.Correlation between mitral regurgitation grading and left ventricular ejection fraction in elderly patients: a follow-up study.

OBJECTIVETo analyze the correlation between mitral regurgitation grading and left ventricular ejection fraction in elderly patients (>60 years of age) in a 2-year follow-up.
METHODSA total of 455 patients with the diagnosis of at least mild mitral regurgitation by echocardiography were divided into ischemic mitral regurgitation (IMR) group and non-ischemic regurgitation (NIMR) group. The patients were followed up with echocardiography every 6 months and the data were analyzed at the end of 24 months.
RESULTSMitral regurgitation grade was inversely correlated with left ventricular ejection fraction (LVEF). Patients with moderate and severe IMR had a lower LVEF than those with NIMR (P<0.05). After adjustment for age, sex, body mass index, high blood pressure, diabetes, atrial fibrillation and cardiomyopathy, the mean LVEF at 2 years was lowered by 2.7% (1.4%-4.1%), 2.7% (1.3%-4.0%), and 5.2% (3.5%-6.9%) in mild, moderate and severe IMR patients, respectively (P<0.04), and by 3.2% (1.6%-4.8%), and 3.0% (1.4%-4.5%), and 1.7%(-0.5%-3.9%) in mild, moderate and severe NIMR patients (P=0.30).
CONCLUSIONThe mean LVEF in IMR patients is significantly lowered compared to that in NIMR patients. The grade of mitral regurgitation is inversely correlated with the regurgitation area in IMR patients. Stratified management might help improve LVEF in severe IMR patients.

8.Signal Conversion and Isolation Processing Technology Used in the Cerebrovascular Stroke Detector.

By using Doppler sensor and pressure sensor, the cerebrovascular stroke detector can be used to measure the blood flow velocity and blood pressure of the carotid artery. In this study, a variety of signal conversion and isolation processing techniques are proposed for processing and feature extraction of the output signals from the sensors. Finally, effective signal output waveforms that can be used to evaluate the cerebrovascular hemodynamics index (CVHI) are obtained, and the sound signal outputs that can reflect the change characteristics of blood flow velocity and blood pressure signals are generated, which realizes the application functional requirements of the detector.

9.Gradual traction-unlocking closed reduction technique for Tile C old pelvic fractures

Objective:To explore the technical points and efficacy of gradual traction-unlocking closed reduction technique (GT-UCRT) for Tile C old pelvic fractures.Methods:From August 2012 to June 2021, 6 patients with Tile C old pelvic fracture were treated and followed up at Department of Orthopedics, The Fourth Medical Center, General Hospital of Chinese PLA. They were 4 males and 2 females with an age of (35.8±10.5) years. By Tile classification: 4 cases of type C1.2, 1 case of type C1.3, and 1 case of type C2; time from injury to surgery: 153.8 (64.3, 204.8) days. The 6 patients were treated with GT-UCRT. The time for gradual traction reduction, operation time, hospital stay, intraoperative blood loss and complications were recorded. The modified Matta score was used to evaluate the reduction quality of pelvic fractures, and the Majeed score was used to evaluate the pelvic function at the last follow-up.Results:The 6 patients were followed up for (40.3±22.9) months (from 12 to 72 months) after surgery. The time for gradual traction reduction was (26.7±4.6) days, operation time (119.2±4.6) minutes, hospital stay (11.5±2.9) days, and intraoperative blood loss (533.3±189.6) mL. By the modified Matta score, the pelvic reduction after surgery was rated as satisfactory in 5 cases and as unsatisfactory in 1 case. The length disparity between both lower limbs in the 6 patients was (6.9±1.6) cm before surgery and (1.0±0.4) cm immediately after surgery, showing a statistically significant difference ( t=11.135, P<0.001). One fracture healed 3 months after surgery and 5 fractures 6 months after surgery. The Majeed pelvic score at the last follow-up was (80.8±9.0) points for the 6 patients, yielding 2 excellent cases, 3 good cases and 1 fair case. Delayed wound healing was reported in 1 patient, calf intermuscular venous thrombosis in 2 cases, and emotional anxiety and sleep disorder in 1 patient. No new lumbosacral plexus injury was found in any patient. Conclusions:In the treatment of Tile C old pelvic fractures, since our self-designed GT-UCRT combines the advantages of Ilizarov technique and unlocking closed reduction technique, it can not only protect the lumbosacral plexus but also obtain satisfactory reduction of the fracture.
10.Tyrosine phosphatase SHP2 inhibitors in tumor-targeted therapies.

Src homology containing protein tyrosine phosphatase 2 (SHP2) represents a noteworthy target for various diseases, serving as a well-known oncogenic phosphatase in cancers. As a result of the low cell permeability and poor bioavailability, the traditional inhibitors targeting the protein tyrosine phosphate catalytic sites are generally suffered from unsatisfactory applied efficacy. Recently, a particularly large number of allosteric inhibitors with striking inhibitory potency on SHP2 have been identified. In particular, few clinical trials conducted have made significant progress on solid tumors by using SHP2 allosteric inhibitors. This review summarizes the development and structure-activity relationship studies of the small-molecule SHP2 inhibitors for tumor therapies, with the purpose of assisting the future development of SHP2 inhibitors with improved selectivity, higher oral bioavailability and better physicochemical properties.
