1.Analysis of blood pressure, heart rate, NIHSS score, GCS score of cerebral infarction patients with cerebral hemorrhage after recombinant human tissue-type plasminogen activator (rt-PA) therapy

Objective To analyze the blood pressure,heart rate,the US national institutes of health stroke scale(NIHSS) score,Glasgow coma scale(GCS) score for cerebral infarction patients with cerebral hemorrhage after recombinant human tissue-type plasminogen activator (rt-PA) therapy.Methods From April 2011 to April 2013,48 cases with cerebral infarction after intravenous rt-PA thrombolysis were collected.These cases were divided into two groups:the first group (32 cases):without cerebral hemorrhage after thrombolysis; the second group (16 cases):with cerebral hemorrhage after thrombolysis.The systolic blood pressure,diastolic blood pressure,heart rote,NIHSS score and GCS score were compared between the two groups before and after thrombolysis.Results After thrombolysis,the systolic and diastolic blood pressure were significantly increased in cerebral hemorrhage cases compared with before thrombolysis and patients without cerebral hemorrhage.After thrombolysis,the NIHSS score was significantly reduced in the cases without cerebral hemorrhage compared with that before thrombolysis,while the NIHSS score was significantly higher in the cases with cerebral hemorrhage compared with that before thrombolysis.After thrombolysis,the heart rate and GCS score showed no significant difference between the patients with and without cerebral hemorrhage.Conclusions The rising of systolic blood pressure,diastolic blood pressure and NIHSS score helps us determine the early cerebral hemorrhage after thrombolysis in cerebral infarction patients.
2.Value of serum HE4, CYFRA21-1, CA199 in the diagnosis of ovarian benign and malignant tumor

Objective To investigate the diagnosis significance of human epididymis (HE4),cytokeratin-19-fragment (CYFRA21-1),carbohydrate antigen 199 (CA199) on the malignant ovarian tumor,and to evaluate the value of individual and combined detection in judging high risk ovarian cancer.Methods The serum HE4,CYFRA21-1,CA199 levels of 45 patients with primary ovarian cancer,56 patients with benign ovarian tumor and 50 healthy check-up women in our hospital were detected by electrochemiluminescence immunoassay (ECL).The positive rates and coincidence rates of them were analyzed and the diagnostic value of them were compared.Results HE4 and CYFRA21-1 levels of ovarian cancer group were significantly higher compared with the benign tumor group and healthy control group and the differences were statistically significant (Z=-8.61,P<0.001;Z=-8.39,P<0.001;Z=-8.60,P<0.001;Z=-8.39,P<0.001),however there is no difference between benign tumor group andcontrol group(Z =-1.31,P =0.189;Z =-1.29,P =0.191).The difference of CA199 between benign tumor group and control group was statistically significant (Z =-8.79,P < 0.001).For the malignant tumors,the specificity and positive predictive value of HE4 (99.8%,99.7%),CYFRA21-1 (99.0%,96.7%) were very high and the diagnostic accordance rates were relatively high (93.4%,88.7%),but the sensibilities of them were lower compared with CA199.CA199 was increased both in benign and malignant ovarian tumors with different degrees,but the specificity was not very high (85.8%),and the positive predictive value and the diagnosis accordance rate were low (70.6%,84.1%).The combined detection of HE4,CYFRA 21-1 and CA199 could improve the sensitivity and of the diagnosis of ovarian cancer (100%) and accuracy (89.4%).Conclusions HE4 and CYFRA21-1 only increas in malignant ovarian tumors,and they are mainly used for the diagnosis of ovarian cancer;CA199 level is increased both in benign and malignant ovarian tumors,but the extent is different and it can be used for distinguishing and diagnosing of benign and malignant ovarian tumors.Combining the three indexes can improve the sensitivity and accuracy of the diagnosis of ovarian cancer,and has an important significance for the early diagnosis and treatment of ovarian cancer.
3.Analysis of the electromyographieal character of Parkinson's tremor

Objective To investigate the electromyngraphy eharacteristies of Parkinsen's disease (PD) by studying tremor frequency, intensity and drug response. Methods Sixty-six patients with PD hospitalized during March 2005 to May 2006 were re-evaluated and recruited into the study. A ten-channel electromyography recorded the frequency and intensity of the rest tremor and postural tremor. Tremor analysis soft was used to statistically analyze differences between the two types of tremor, correlation between the frequency and intensity, and changes after regular administration of dopaminergic medication. Results The frequencies of rest tremor ((4. 79±0.69 ) Hz) and postural tremor ( (5.54±1.18) Hz ) showed statistical differences (F = 33.5, P<0. 01), but the intensity of rest tremor (0.016-421.700 (rag)2) and postural tremor (0.062-217.900 (mg)2) didn't show statistical differences (F=1.917, P>0.05 ). No regression relation was revealed between the frequency and intensity of tremor (r=0.09, P>0.05). After dopaminergic medication treatment, the intensity declined significantly from 0.016-421.700 (mg)2 to 0.027-82.200 (mg)2(F=5.179,P <0.05), but the difference of frequencies of tremor between before ( (5.09±0.85) Hz) and after treatment ( (5.20 ± 1.08 )Hz) was not significant (F = 0.474, P > 0.05). Conclusions There may be different mechanisms for the rest tremor and postural tremor in PD. There is no correlation identified between the frequency and intensity in PD and this is may be resulted from variability of tremor intensity. Dopaminergic can relieve the symptom of tremor, but it doesn't reduce the frequency of tremor.
4.Application of electromyogram in the evaluation of subjects to N-hexane

Objective To explore the values of electromyography to examine birth injuries.Methods 34 cases of birth injuries were studied,the electromyography and motor nerve conduction of brachial plexcy were recorded.Then,the location and severity of birth injuries were determined.Results Among 34 cases,according to the location of injuries,there were 10 cases of upper trunk injuries,7 cases of upper and middle trunk injuries,5 cases of middle and lower trunk injuries,5 cases of lower trunk injuries,and 7 cases of whole brachialplexus injuries.On the other hand,according to the severity of injuries,there were one case of complete injuries and 33 cases of incomplete injuries which included 2 cases of severe injuries and 31 cases of slight injuries.Conclusion Electromyography is the reliable method to identify the location and severity of brachial plexus injuries,and this examination is necessary to diagnose birth injuries with great clinical values.
5.Expressions of Toll-like receptors 2 and 4 in skin lesions and peripheral blood from patients with chloasma

Objective To investigate the relationship of Toll-like receptors (TLRs) 2 and 4 with the occurrence of chloasma.Methods Peripheral blood samples were collected from 40 patients with chloasma and 40 healthy human controls,and skin samples were also collected from the lesions of 10 of the patients and normal skin of 10 of the healthy controls.Real time (RT)-PCR was performed to measure the mRNA expressions of TLR2 and TLR4 in skin lesions and blood samples.An immunohistochemical test was conducted to observe the expressions of TLR2 and TLR4 in skin lesions.Statistical analysis was carried out by t test.Results The expressions of TLR2 and TLR4 mRNAs were both significantly higher in skin lesions of the patients than in normal skin of the controls (9.72 ± 2.93 vs.5.10 ± 2.69,t =3.67,P< 0.01; 9.52 ± 2.88 vs.4.77 ± 1.90,t =4.36,P< 0.01),while no significant difference was found in the mRNA expressions of TLR2 or TLR4 in peripheral blood between the patients and controls (both P > 0.05).As the immunohistochemical test revealed,TLR2 was absent in both the epidermis and vascular endothelial cells in 6 normal control skin samples,weakly expressed in the basal layer of the epidermis but absent in vascular endothelial cells in 4 normal skin samples,and no TLR4 expression was observed in either the epidermis or vascular endothelial cells in these control skin samples.Among the 10 skin samples from chloasma lesions,3 showed TLR2 expression in the whole epidermis,7 in both basal cell layer and prickle cell layer but not in vascular endothelial cells in the superficial dermal layer,all showed strong TLR4 expression in the basal cell layer and weak TLR4 expression in the prickle cell layer,and 3 exhibited TLR4 expression in vascular endothelial cells in the superficial dermal layer.Conclusion TLR-mediated immune responses in local skin might be related to the occurrence of chloasma.
6.Effect of Tibet-medicine Ratanasampil on serum β-amyloid protein and inflamatory cytokine levels in patients with Alzheimer's disease

Objective To study the effect of ratanasampil (RNSP) which is Traditional Tibetan Medicine on the levels of serum β-amyloid protein, interleukin and tumor necrosis factor alpha (TNF-α) in patients with mild to moderate Alzheimer's disease (AD). Methods One hundred AD patients were divided into two groups in randomized controlled study, including treatment group (RNSP 1 g/d) and control group (piracetam 2.4 g/d). The treatment lasted 12 weeks. The Mini Mental State Examination (MMSE), Alzheimer' s disease Assessment Scale-cognitive subscale (ADAS-cog) and Activity of Daily Living Scale (ADLs) were taken to evaluate the efficacy. Serum levels of amyloid peptides (Aβ40 and Aβ42 ) were measured by ELISA assay. The radioimmunologic assay was used to determine the serum levels of IL-1β, IL-2, IL-6, IL-8 and TNF-α. Results The scores of MMSE, ADAS-cog and ADL significantly improved at 12 weeks after RNSP treatment (P<0.01, 0.01, 0.05, respectively), while had no significant changes in piracetam group (P<0.05).The levels of TNF-α, IL-1β, IL-6 and Aβ42 were significantly lower in RNSP group than in Piracetam group (P<0.01). There was a decrease trend of the Aβ42/Aβ40 ratio at 12 weeks after RNSP treatment (P<0. 05, P<0.01 ). The serum Aβ42 level had strong correlations with TNF-α, IL-1 β and IL-6. There were no significant differences in Aβ40 and IL-8 between RNSP group and piracetam group. No obvious drug side effect happened on the groups. Conclusions The reductions of serum TNF-α, IL-1β and IL-6 levels after RNSP treatment may lead to decrease of Aβ42 production in AD patients. RNSP may decrease the Aβ42/Aβ40 ratio and slow down the progress of AD. It may improve the learning and memory ability in treating patients with mild to moderate AD and is well tolerated and safe.
7.Perioperative nursing for a patient with mayer rokitansky kuster hauser combined with rectovestibullar fistula

Objective To investigate the perioperative nursing experience for patients with mayer rokitansky kuster hauser (MRKH).Methods We conducted multidisciplinary care to a MRKH patient,including personalize psychological support,bowel preparation,two surgical nursing convergence and vaginal mold care during hospitalization.Results The patient discharged after clinical symptoms remission,the patient sexual life was satisfaction,no complications were found in the six months follow-up.Conclusion The key of operation success is good coordination and cooperation of multiple departments,close observation and effective nursing in the perioperation period.
8.A Randomized,Double-Blind,Multi-Center Clinical Trial with Butenafine Hydrochloride 1% Aerosol Versus Bifonazole 1% Aerosol for the Treatment of Tinea Pedis,Tinea Corporis or Tinea Cruris

Objective To investigate the efficacy and safety of butenafine hydrochloride 1% aerosol in the treatment of tinea pedis,tinea cruris or tinea corporis.Methods A randomized,double-blind,multi-center clinical trial was conducted.Efficacy was assessed in terms of mycological cure,total clinical sign and symptom scores,and clinical response,at baseline,mid-term,end of study,and 2 weeks after treatment.Results One hundred and seventeen patients with tinea cruris or tinea corporis were randomly allocated to individual groups treated with either butenafine 1% aerosol (n = 58,male 53,female 5,age 29.45 ? 11.80,course of disease 3.0 ? 5.0 months) or bifonazole 1% aerosol (n = 59,male 49,female 10,age 34.12 ? 12.98,course of disease 3.0 ? 11.0 months).One hundred and nineteen patients with tinea pedis were also allocated to two groups treated with either butenafine (n = 59,male 59,age 22.97 ? 3.97,course of disease 24.0 ? 36.0 months) or bifonazole (n = 60,male 60,age 23.77 ? 4.12,course of disease 36.0 ? 48.0 months).The cure rates and total response rates were 25.86% vs.40.68%,and 86.21% vs.91.53%,in the study group and the control group,respectively,at the end of study,and 58.62% vs.74.58%,and 96.55% vs.96.61% in 2 weeks following-up,for the patients with tinea cruris or tinea corporis.Also,the cure rates and total response rates were 23.73% vs.25.00%,81.36% vs.78.33%,in the study group and the control group,respectively,at the end of study,and 37.29% vs.41.57% and 81.36% vs.90.00% in 2 weeks following-up,for the patients with tinea pedis.Local adverse reactions were recorded in 13 of butenafine group,and 20 of bifonazole group.The differences of above data between two groups were not statistically significant.Conclusion Butenafine hydrochloride 1% aerosol is effective and well tolerated for the treatment of tinea pedis,tinea cruris or tinea corporis.
9.Competencies in health education among pediatric nurses: analysis from GDP percapital level

Objective:To explore whether the inequality of economic development in different provinces in China leads to differences in pediatric nurses ′ health education literacy and to analyze related factors affecting pediatric nurses ′ health education literacy. Methods:Self-designed and tested online questionnaire of competencies in health education (scoring scale 10-50) were distributed to pediatric nurses in China in October 2018. We examined the influencing factors of competencies in health education and its relationship with the province-level data on gross domestic product (GDP) per capita.Results:A total of 15 443 pediatric nurses from 31 provinces were eligible for the analysis. At the regional of GDP per capital over than 20 000 US dollars, 15 000 to 20 000 US dollars, 10 000 to 14 900 US dollars and less than 10 000 US dollars, the health education literacy scores were 40.76±4.52, 40.66±4.08, 40.50± 4.02 and 39.69±4.32 respectively. Significant difference was found between the competencies in health education of pediatric nurses and provinces with different GDP per capita ( F value was 9.21, P<0.001). Regression and hierarchical analysis models based on GDP per capita showed that: nurses with senior professional titles, bachelor degrees or above, aged over 40, and those working in emergency rooms have higher competencies in health education ( OR value was 0.296-4.766, P<0.05) . Lower competencies in health education were demonstrated on nurses who have been working less than 5 years ( OR value was 0.319, P<0.05). Conclusions:Economic development is one of the main factors that affect the competencies in health education of pediatric nurses in China. Pediatric nurses who were young, had limited working experience, with low office titles, with low education background, and who working at non-emergency rooms require more training.
10.Study on Gastric Cancer Susceptibility Gene Profiling and Risk Assessment in An Area in Shanghai

Aims:To explore gastric cancer susceptibility gene profiling in an area in Shanghai,and to assess the risk of gastric cancer susceptibility. Methods:A total of 152 patients with primary gastric cancer at Shanghai Pudong Hospital were enrolled,and 152 demographic characteristics matched patients with non-gastrointestinal diseases,non-tumor were served as controls. Gene polymorphism was determined by allele specific polymerase chain reaction. Susceptibility gene of gastric cancer was screened. Multiple genes interactions were analyzed and multiple genes risk was evaluated by DEMCHUK model. Results:Univariate analysis showed that CYP2E1,NAT2M1,NAT2M2,NAT2,XRCC1194,MTHFRA1298C,VDR TaqⅠ were susceptibility genes of gastric cancer. Multivariate analysis showed that CYP2E1(C1/ C1),NAT2M1(T/ T),NAT2M2(A/ A),XRCC1194( T/ T)and MTHFRA1298C(A/ C)were susceptibility genotypes. Synergistic effect was found between genes except MTHFRA1298C (A/ C)with NAT2M1(T/ T)and NAT2M2(A/ A)(P ﹤0. 05). Multiple genes risk analysis showed that combination OR of multiple genes was highly correlated with gene frequency,and the risk of gastric cancer was increased with the increasing number of susceptibility genes. Conclusions:CYP2E1(C1/ C1),NAT2M1(T/ T),NAT2M2( A/ A),XRCC1194( T/ T) and MTHFRA1298C( A/ C) are susceptibility genotypes of gastric cancer. Carrying multiple susceptibility genes can significantly increase the risk of gastric cancer.
