1.Dynamic measurement of CRP, IL-6 and Fib levels of unstable angina and its clinical significance

Objective To study the clinical significance of dynamic determination of serum CRP,IL-6 and fibrinogen(Fib) levels in unstable angina pectoris (UAP) patients.Methods 162 UAP patients were selected as UAP group(including 52 cases of low risk group,53 cases in medium risk group and 57 cases in high risk group),58 healthy people were selected as control group.The CRP,IL-6 and Fib contents were observed in UAP patients after cardiac events.Results At discharge,CRP,IL-6,Fib in UAP low risk group were (3.47 ±0.55)mg/L,(10.38 ± 1.57) ng/L,(1.64 ± 0.65) g/L,which were significantly lower than the admission contents [(6.50 ±0.87) mg/L,(14.35 ± 1.22) ng/L,(3.01 ± 0.28) g/L] (t =2.761,2.945,2.118,P =0.028,0.003,0.031).CRP,IL-6,Fib levels in the medium risk group were (6.87±0.75) mg/L,(14.35 ± 1.02) ng/L,(2.26 ± 0.33) g/L,which were significantly lower than the admission contents [(9.21±1.03) mg/L,(19.24±1.58) ng/L,(4.15 ±0.47) g/L] (t =2.985,3.157,3.548,P =0.003,0.001,0.000).CRP,IL-6,Fib levels in the high risk group were (8.71 ±0.84) mg/L,(17.37 ± 1.26) ng/L,(4.67 ±0.85) g/L,which were significantly lower than the admission contents [(12.48 ± 1.24) mg/L,(25.13 ± 1.84) ng/L,(7.28 ± 0.64) g/L] (t =3.472,4.018,5.097,P =0.000,0.000,0.000).Six months after hospital discharge,in CRP,IL-6,Fib elevated groups,the incidence rates of cardiac event were 40.20%,40.35% and 47.50%,which were significantly higher than those in normal group (5.0%,4.17%,14.29%,x2 =23.655,2.211,14.442,P=0.000,0.000,0.000).Conclusion CRP,IL-6 and Fib levels in UAP patients were significantly higher than those in healthy people,and with remission,contents would be lower,which was the index to reflect the severity of UAP,but its specificity is bad,need to use caution.
2.The correlation between renal dysfunction and prognosis of patients with decompensated heart failure

Objective To investigate the effect of renal dysfunction on the prognosis of hospitalized patients with decompensated heart failure (DHF).Methods 191 patients with DHF hospitalized between June 2011 and June 2013 in Baoshan Branch of Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine were enrolled. These patients were divided into three groups according to the glomerular filtration rate (eGFR): normal renal function group (eGFR ≥ 90 mL·min-1·1.73 m-2, 63 cases), mild renal function descend group (eGFR 60 - 89 mL·min-1·1.73 m-2, 80 cases) and moderate or severe renal function descend group (eGFR < 60 mL·min-1·1.73 m-2, 48 cases). The general clinical data were recorded; the serum tumor necrosis factor-α (TNF-α) and interleukins (IL-1, IL-6, IL-8, IL-10, IL-13) were determined by enzyme-linked immunosorbent assay (ELISA). After discharge, the patients were followed-up for 1 year, and their outcomes were compared among the three groups.Results In 191 hospitalized patients with DHF, there were 67.0% with renal function impairment. Compared with normal renal function group and mild renal function descend group, the patients in moderate or severe renal function descend group were older (years: 83.4±5.1 vs. 66.2±5.4, 76.8±6.3), their cardiac functions were poorer, and their incidences of complications were higher than those in the normal renal function group [hypertension: 66.7% (32/48) vs. 42.9% (27/63), diabetes: 65.6% (31/48) vs. 41.3% (26/63), anemia: 37.5% (18/48) vs. 15.9% (10/63), acute myocardial infarction (AMI): 25.0% (12/48) vs. 9.5% (6/63), old myocardial infarction: 31.3% (15/48) vs. 11.1% (7/63), pulmonary infection: 29.2% (14/48) vs. 11.1% (7/63), allP < 0.05]. The complication incidences of hypertension [66.7% (32/48) vs. 51.3% (41/80)], diabetes [65.6% (31/48) vs. 48.8% (39/80)], anemia [37.5% (18/48) vs. 25.0% (20/80)] and pulmonary infection [29.2% (14/48) vs. 16.3% (13/80)] had no statistically significant differences between the moderate or severe renal function descend group and mild renal function descend group (allP > 0.05). The complication incidence of AMI [25.0% (12/48) vs. 10.0% (8/80)] and old myocardial infarction [31.3% (15/48) vs. 11.3% (9/80)] in moderate or severe renal function descend group was obviously higher than that in mild renal function descend group (bothP < 0.05). There were no statistically significant differences in the complication incidences of chronic obstructive pulmonary disease [COPD, 12.7% (8/63), 17.5% (14/80), 20.8% (10/48)], atrial fibrillation [30.2% (19/63), 27.5% (22/80), 29.2% (14/48)], ventricular premature beat [9.5% (6/63), 11.3% (9/80), 10.4% (5/48)] and cerebrovascular disease [20.6% (13/63), 22.5% (18/80), 22.9% (11/48)] among the three groups (allP > 0.05). Compared with normal renal function group, the levels of inflammatory cytokines in serum TNF-α, IL-1, IL-6, IL-8, IL-10, IL-13, and the mortality, the re-admission rates due to heart failure, rates of malignant arrhythmia in the two renal function descend groups were increased significantly, the increment being more remarkable in moderate or severe renal function descend group [TNF-α (ng/L): 235.8±20.9 vs. 121.6±10.7, IL-1 (ng/L): 345.9±40.8 vs. 203.5±34.7, IL-6 (ng/L): 502.8±64.2 vs. 321.9±53.8, IL-8 (ng/L): 723.9±210.3 vs. 431.5±110.5, IL-10 (ng/L): 155.4±23.5 vs. 103.1±13.2, IL-13 (ng/L): 184.5±27.3 vs. 136.8±20.2, the rate of mortality in the first time of hospitalization: 14.6% (7/48) vs. 5.0% (4/80), mortality within one year after discharge: 25.0% (12/48) vs. 18.0% (9/80), readmission rate due to heart failure: 47.9% (23/48) vs. 30.0% (24/80), rate of relapse of coronary events: 72.9% (35/48) vs. 37.5% (30/80), malignant arrhythmia rate: 39.6% (19/48) vs. 20.0% (16/80), allP < 0.05]. There were no significant differences in the rates of stroke among moderate or severe, mild and normal renal function descend groups [4.2% (2/48), 3.8% (3/80), 3.2% (2/63),P > 0.05].Conclusions The incidence of renal dysfunction in patients with DHF is relatively high, and their mortality, re-admission rate and their levels of inflammatory cytokines are high obviously. Thus, the intervention of renal dysfunction may have important significance in the improvement of their prognoses.
3.Effect of different external nasogastric tube fixations on unplanned nasogastric tube removal after radical esophagectomy for esophageal cancer

Objective To investigate the effects of three external nasogastric tube fixations on unplanned nasogastric tube removal after radical esophagectomy for esophageal cancer.Methods Two hundred and thirty four esophageal cancer patients who underwent radical esophagectomy with nasogastric intubation were assigned into conventional group I(n=74),conventional group II (n=79)and experiment group(n=81)using random number table.For external fixation of the nasogastric tube,the traditional group I used“3M”silk tape,the traditional group II used a white flat strap with two knots and the experiment group used anI-shaped nasal fixation tape combined with a flapping cheek fixation tape.The three groups were compared in terms of displacement of the indwelling nasogastric tube and slippage of the tube.Results The displacement rate of nasogastric tube and the rate of slippage in the experiment group were both significantly lower than those in both conventional groups I and II(all P<0.05).Conclusion TheI-shaped nasal fixation tape combined with a flapping cheek fixation tape in fixing the nasogastric tube can effectively reduce the incidence of unplanned removal of nasogastric tube and meanwhile the patients may feel comfort and satisfactory.

ATM: To explore the effect and mechanism on blood pressure and heart protection of renal hypertension in rats treated with ?_1 receptor antisense oligodeoxynucleotides (?_1-AS-ODN) by delivery with cationic liposomes DOTAP/DOPE. METHODS: 24 SD rats were divided randomly into 4 groups of 6 each group: two groups of 2K1C rats were treated via tail vein injection with ?_1-AS-ODN or inverted oligonucleotides against rat ?_1-AR mRNA (?_1-IN-ODN) of 0.5 mg?kg~(-1) once. 6 untreated 2K1C rats and shamed rats served as positive and normal controls. Blood pressure (BP), cardiac haemodynamics, and left ventricular index (LVW/BW) were measured. 5.Determination of IL-2 Content in IL-2 Targeting 5-FURP Liposomes by Ultraviolet Spectrophotometry

OBJECTIVE:To determine the content of IL-2 in IL-2 targeting liposomes of 5-FURP. METHODS: The sample solution was stained by Coomassie brillant blue G-250 solution. Using blank liposome as blank,the absorbability of the IL-2 was determined by ultraviolet spectrophotometry at a wavelength of 595 nm;meanwhile its content was determined. RESULTS: The linear range of IL-2 was 1~100 ?g?ml-1(r=0.999 7) and the average recovery rate was 99.26% (RSD=0.59%,n=5). CONCLUSION: The method was proved to be simple,rapid,sensitive and reproducible,and it is applicable for the determination of IL-2 in liposome of 5-FURP.
6.Effect of empowerment educational program on AIDS prevention and treatment in one university

Objective:
To apply and evaluate the effect of empowerment educational program on AIDS prevention and treatment among freshmen in one university.
Methods:
The method of two stage stratified sampling was used to select the experimental and control group. The traditional health education was implemented among the control group, and the empowerment education was implemented for the experimental group. The effect of the two groups was compared before and after intervention.
Results:
For experimental group, the awareness rate of AIDS(65.02%) ( χ 2=61.02, P <0.01) and the overall score of attitude and behavior(16.71± 2.53 )( t =-2.66, P <0.05) were significantly improved after intervention(82.96%,18.58±1.95). For the control group, there was significant difference in awareness rate of AIDS after intervention(67.70% vs 96.02%, χ 2=18.64, P <0.05), while there was no statistical difference in overall score of attitude and behavior after intervention(16.52±1.50 vs 17.16±1.57, t =-1.51, P =0.14). There was no significant difference in awareness rate between the two groups before intervention ( χ 2=0.36, P =0.55), but there was a statistical difference after intervention ( χ 2=20.42, P <0.01). There was statistical difference in attitude and behavior scores between the two groups after intervention ( P <0.05).
Conclusion
Empowerment educational program can improve the awareness rate of AIDS among college students, help to establish an objective attitude towards AIDS and infected patients, and to reduce high risk sexual behavior, also it is more effective compared to traditional education method.
7.Surveillance and drug resistance analysis of thermotolerant Escherichia coli in sewage from hospitals in Shaoxing

Objective To analyze the drug resistance and detect the drug resistance genes of pathogenic bacteria isolated from hospital sewage , and to investigate the sources of antibiotic resistant microorganisms in the environment .Methods The enteric pathogenic bacteria , such as thermotolerant Escherichia coli , Salmonella and Shigella , were isolated from the sewage of Shaoxing hospitals from January 2016 to December 2016 , and the detection of drug resistance of thermotolerant Escherichia coli , the drug resistance of Escherichia coli isolated from clinical specimens of three hospitals was compared and analyzed . Meanwhile , extended spectrum beta lactamases ( ESBLs ) resistance genes were detected in thermotolerant Escherichiacoliinsewage.Results 48strainsofpathogenswereisolatedinsewage,including32strainsof Escherichia coli , 2 strains of Shigella sonnei and 1 strain of Salmonella arizonae .Escherichia coli has a low rate of resistance to most antibiotics .Compared to Escherichia coli isolated from clinical specimens , there were significant differences among the drug resistance rates of Ampicillin , Aztreonam, Amoxicillin/Clavulanic acid and the first to four generation Cephalosporins (P<0.05).11 strains of ESBLs positive bacteria were detected in sewage .Among them , 7 carried CTX-M9 resistance genes , and 4 carried TEM resistance genes , while 1 strain of CTX-M1 and SHV was carried .There were 2 strains carrying CTX-M9 and TEM resistance genes simultaneously , 1 strain simultaneously carrying TEM and SHV resistance genes . Conclusions There are different kinds of intestinal pathogens in hospital sewage , which may lead to contamination of drug-resistant microorganisms in the environment .Therefore the hospitals should strengthen the quality of sewage treatment and improve the qualified rate of sewage disinfection .
8.Effects of Arg20 mutation on sodium channels activity of JZTX-V.

Jingzhaotoxin-V(JZTX-V) isolated from the venom of the spider Chilobrachys jingzhao is a novel potent inhibitor that acts on tetrodotoxin-resistant and tetrodotoxin-sensitive sodium channels in adult rat dorsal root ganglion(DRG) neurons. It is a 29-residue polypeptide toxin including three disulfide bridges. To investigate the structure-function relationship of the toxin, a mutant of JZTX-V in which Arg20 was substituted by Ala, was synthesized by solid-phase chemistry method with Fmoc-protected amino acids on the PS3 automated peptide synthesizer. The synthetic linear peptide was then purified by reversed-phase high performance liquid chromatography and oxidatively refolded under the optimal conditions. The refolded product was analyzed by matrix-assisted laser desorption/ ionization time-of-flight mass spectrometry(MALDI-TOF MS) and electrophysiological experiments for its relative molecular weight and prohibitive activity of sodium channels respectively. The present findings show that the prohibitive effect of R20A-JZTX-V on TTX-S sodium channels in DRG neurons is almost the same as that of native JZTX-V, suggesting that Arg20 does not play any important role in inhibiting TTX-S sodium currents in DRG neurons. In contrast, the prohibitive level of R20A-JZTX-V on TTX-R sodium channels is reduced by at last 18.3 times, indicating that Arg20 is a key amino acid residue relative to the bioactivity of JZTX-V. It is presumed that the decrease in activity of R20A-JZTX-V is due to the changes of the property in the binding site in TTX-R sodium channels.

9.Isomangiferin, a Novel Potent Vascular Endothelial Growth Factor Receptor 2 Kinase Inhibitor, Suppresses Breast Cancer Growth, Metastasis and Angiogenesis.

PURPOSE: Vascular endothelial growth factor (VEGF) signal transduction mainly depends on its binding to VEGF receptor 2 (VEGFR-2). VEGF downstream signaling proteins mediate several of its effects in cancer progression, including those on tumor growth, metastasis, and blood vessel formation. The activation of VEGFR-2 signaling is a hallmark of and is considered a therapeutic target for breast cancer. Here, we report a study of the regulation of the VEGFR-2 signaling pathway by a small molecule, isomangiferin. METHODS: A human breast cancer xenograft mouse model was used to investigate the efficacy of isomangiferin in vivo. The inhibitory effect of isomangiferin on breast cancer cells and the underlying mechanism were examined in vitro. RESULTS: Isomangiferin suppressed tumor growth in xenografts. In vitro, isomangiferin treatment inhibited cancer cell proliferation, migration, invasion, and adhesion. The effect of isomangiferin on breast cancer growth was well coordinated with its suppression of angiogenesis. A rat aortic ring assay revealed that isomangiferin significantly inhibited blood vessel formation during VEGF-induced microvessel sprouting. Furthermore, isomangiferin treatment inhibited VEGF-induced proliferation of human umbilical vein endothelial cells and the formation of capillary-like structures. Mechanistically, isomangiferin induced caspase-dependent apoptosis of breast cancer cells. Furthermore, VEGF-induced activation of the VEGFR-2 kinase pathway was down-regulated by isomangiferin. CONCLUSION: Our findings demonstrate that isomangiferin exerts anti-breast cancer effects via the functional inhibition of VEGFR-2. Pharmaceutically targeting VEGFR-2 by isomangiferin could be an effective therapeutic strategy for breast cancer.

10.Material compatibility of human albumin

【Objective】 To investigate the compatibility of human albumin and its internal packaging materials of Sinopharm Lanzhou Biopharmaceutical Co., Ltd. 【Methods】 One batch of inner packaging materials (medium borosilicate glass-molded injection bottle and halogenated butyl rubber plug for injection) was extracted with 4 extraction solvents to conduct the toxicological evaluation of potential inner packaging extracts. Through the simulated acceleration test, the trend analysis of the elements in the sample and the inner surface of the glass bottle were observed, and the routine drug inspection items during the long-term stability test process were determined. 【Results】 The detection results of the leaching elements of the internal packaging materials did not exceed the limit of 50%, and the organic matter safety threshold (margin of safety, MOS) was greater than 1.0, indicating that both the leaching elements and the organic matter had no safety risk to the user under the current exposure. The results of the simulated acceleration test show that the drug will not have the risk of peeling tablets after the long-term stability condition was placed for a period of time, and the routine inspection items of the long-term stability test drugs all meet the requirements of the pharmacopoeia. 【Conclusion】 The inner packaging material has no significant impact on the quality of drugs and has good overall compatibility, making it suitable for packaging human albumin.
