1.Analysis in factors influencing satisfaction degree of patients receiving introvenous infusion in outpatient department and its nursing countermeasures

Objective To investigate factors influencing satisfaction degree of patients receiving introvenous infusion in out-patient department in order to perfect nursing quality and improve the satisfaction degree of patients. Methods 900 patients receiving introvenous infusion in out-patient department were investigated with self-designed questionnaire and the results went through analysis. Results There were 886 effective responding questionnaires out of 900 with effective rate of 98.4%. The result indicated that the major factors which influenced degree of satisfaction with infusion included operation technique, service attitude and health education of nurses. Conclusions Increase of nurses' diathesis, enhancement of service quality, establishment of concept of "patient as center" are the preconditions to improve the satisfaction degree of patients receiving introvenous infusion in out-patient department.
2.Effects of recombinant BCG-Em Ⅱ/3 vaccine on apoptosis of splenocytes in mice challenged with Echinococcus multilocularis

Objective:To detect the effects of recombinant BCG-Em Ⅱ/3 vaccine on apoptosis of splenocytes in mice challenged with protoscoleces of Echinococcus multilocularis.Methods:BALB/C mice were immunized with rBCG-Em Ⅱ/3 vaccine by subcutaneous injection or intranasal inoculation.After 8 weeks of immunization,all the mice were challenged with 50 protoscoleces of Echinococcus multilocularis by intraperitoneal injection.On the 18th week after infection,mice were killed to separate splenocytes and stimulated with ConA for 16~18h,then apoptosis of splenocytes was detected by Annexin V-FITC staining methods.Results:Apoptotic rate of splenocytes in all groups with or without ConA stimulation was obviously lower than that of PBS group(P
3.Construction and expression efficiency of recombinant Bb-EmⅡ/3 vaccine of echinococcus multilocularis

Objective:To construct recombinant Bb-EmⅡ/3 vaccine of Echinococcus multilocularis and to investigate the expression efficiency of EmⅡ/3 antigen encoding gene in Escherichia coli BL21(DE3). Methods:EmⅡ/3 antigen gene was amplified by PCR. Then the gene was cloned into Escherichia coli-Bifidobacteria shuttle plasmid pGEX-1?T containing gluathione-S-transferaze(GST) gene to construct pGEX-EmⅡ/3. The recombinant plasmid was electroporated into Bifidobacteria bifidum(Bb) to construct rBb-EmⅡ/3 vaccine. The vaccine was identified with PCR and restriction-endonuclease digestion. The expression of pGEX-EmⅡ/3 was induced with isopropyl-?-D-thiogalactopyranosid(IPTG),and fusion protein EmⅡ/3/GST was examined by SDS-PAGE and Western blot techniques. Results:1 759bp gene of EmⅡ/3 was successfully amplified by PCR and cloned into pGEX-1?T by restriction analysis. rBb-EmⅡ/3 vaccine was successfully constructed according to PCR and restriction-endonuclease digestion.It was demonstrated with SDS-PAGE and Western blot that the fusion protein EmⅡ/3/GST were expressed in E.coli,BL21. Conclusions:rBb-EmⅡ/3 vaccine of Echinococcus multilocularis is successfully constructed. The plasmid pGEX-EmⅡ/3 was highly expressed in E.coli in fused form with GST and this kind of protein shows specific antigenicity according to western blot.

Diabetic cardiomyopathy is one of the major causes of congestive heart failure in patients with diabetes.The histological changes of the diabetic cardiomyopathy can be accurately characterized by MR structure and functional imaging techniques,such as CINE imaging,MRS and T1 Mapping.These cardiac MRI techniques have been used to evaluate the treatment effect of diabetic cardiomyopathy.The progresses of MRI techniques in evaluating myocardium of diabetic cardiomyopathy were reviewed in this article.
5.Clinical effect and safety of low dose aspirin in preventing lower extremity venous thrombosis after cesarean section

Objective To investigate the clinical efficacy and safety of low-dose aspirin in prevention of lower extremity deep venous thrombosis after cesarean section. Methods from January 2016 to March 2017 in our hospital obstetrics and Gynecology cesarean section in 374 cases as the research object, using the method of random number table according to the proportion of 1:1 were divided into control group and observation group, each group of the 187 cases, the control group after cesarean section treated with routine comprehensive preventive measures, the observation group in the control group received the low-dose aspirin treatment. Observation group 2 postpartum in January the incidence of deep venous thrombosis, postoperative 12 h, postoperative serum 7 d two D- dimer (D-D), fibrinogen (FIB), activated partial thromboplastin time (APTT), prothrombin time (PT) and platelet count (PLT) and blood rheology index, statistics of the 2 groups the incidence of bleeding complications. Results Deep venous thrombosis in the observation group was significantly lower than the control group (P<0.05); the 2 group after 5D-D and FIB and 12 h after operation were significantly decreased (P<0.05), and the observation group and the control group in the same period were significantly decreased (P<0.05); 2 groups were PT, APTT, PLT showed no significant difference compared with the preoperative, and the observation group and the control group in the same period. There was no statistically significant difference ; 2 group 7 d after operation and high blood viscosity, whole blood viscosity, whole blood low shear viscosity, plasma viscosity and postoperative 12 h compared were significantly decreased (P<0.05), and the observation group with the same period compared with the control group decreased significantly (P<0.05); postoperative 7 d erythrocyte deformation index and postoperative 12 h were significantly increased (P<0.05), and the observation group and the control group in the same period compared significantly increased (P<0.05); the incidence of bleeding complications was not statistically significant 2 after the operation of group. Conclusion Hypercoagulability of low-dose aspirin helps improve puerpera after cesarean section, reduce blood viscosity, reduce the incidence of venous thrombosis, does not increase the incidence of bleeding, clinical application of good safety, which is worthy of clinical application.
6.Effect of pelvic floor muscle rehabilitation training combined with Buzhong Yiqi Pill at ultra-early stage on the recovery of pelvic floor muscle function after delivery

Objective To observe the effect of pelvic floor muscle rehabilitation training combined with Buzhong Yiqi Pill at ultra-early stage or early stage on the recovery of postpartum pelvic floor muscle function, to explore the feasibility of intervention of postpartum rehabilitation training at ultra-early stage. Methods 314 patients were randomly divided into the ultra-early stage group and the early stage group, 157 cases for each group. Patients in the ultra-early group were guided to give Kegel exercise and the biofeedback electrical stimulation therapy in the seventh week combined with Buzhong Yiqi pill in 1-2 days after delivery , patients in the early group were given Kegel exercise and biofeedback electrical stimulation therapy in the seventh week combined with Buzhong Yiqi pill. Effects of treatment two groups were observed. Results There were significant increased in pelvic floor muscle activity compared with six weeks and 2 days after delivery between two groups (P<0.05), and there were significant increased in the ultra-early group compared with the same period in the early group (P<0.05) . There were significant increased in good rate in 12 weeks after delivery compared with immediate time and 2 days after delidery (P<0.05), and more significant increased in the ultra- early group than that in the early group (P<0.05). There were significant decreased in urinary urgency, lumbosacral painand tenesmus, vaginal relaxation, and vaginal dryness rate in 12 weeks after delivery compared between two groups (P<0.05). Conclusion There were good effects in pelvic floor muscle rehabilitation training combined with Buzhong Yiqi Pill at ultra-early stage on the recovery of pelvic floor muscle function after delivery, and be worthy of clinical application.

8.Curative effects, prognostic factors, and adverse reactions of different treatment modalities of chemotherapy in patients with non-small cell lung cancer

Objective To explore the curative effects,prognostic factors,and adverse reactions of different treatment modalities of chemotherapy in patients with non-small cell lung cancer (NSCLC).Methods151 NSCLC patients were randomly assigned into 3 groups to undergo concurrent chemoradiotherapy ( n = 43 ) ,sequential chemo-radiotherapy ( n = 49 ) ,or pure chemotherapy (n = 59).The clinical data were analyzed.ResultsThe short-term effective rates of the concurrent and sequential chemo-radiotherapy groups were 81.4% and 73.5% respectively with no significantly difference between them,but both significantly higher than that of the pure chemotherapy group ( 39.0% ,both P < 0.05 ).The mortality of the concurrent cherno-radiation group was 53.5% ,significantly lower than those of the sequential chemo-radiotherapy and pure chemotherapy groups (67.3% and 67.8% respectively,both P <0.05).The median survival time of the concurrent chemo-radiotherapy group was 26 months,significantly longer than those of the sequential chemo-radiotherapy and pure chemotherapy groups ( 12 and 11 months respectively,both P < 0.05).The l-year survival rates of the 3 groups were 80.2% ,47.1%,and 45.6%.The 2-year survival rates were 58.2% ,38.5% ,and 24.4% ,and the 3-year survival rates were 32.7% ,27.5% ,and 0,respectively.The white blood cell decrease rates of the grades Ⅲ~Ⅳ of the concurrent chemo-radiotherapy group were significantly higher than those of the other 2 groups (both P <0.05 ).There were no significant differences in the rates of radiation esophagitis and radiation pneumonitis between the concurrent and sequential chemo-radiotherapy groups.Age,performance status score,clinical staging,and treatment modality were the 4 influencing factors in the prognosis of NSCLC ( all P < 0.05 ).Conclusion Relatively safe with tolerable adverse reactions,concurrent chemo-radiotherapy is superior to sequential chemo-radiotherapy,particularly to pure chemotherapy,in increase of median survival time and survival rate in treatment of NSCLC.Age,PS,clinical staging and therapy method are independent indicators influencing the prognosis.

HLA (human leukocyte antigen) is critical molecule relevant to immunoresponse, which is always involved in antigen presentation and determining killing activity of immune cells against tumor. The abnormal expression of HLA in lung cancer is believed to be a common phenomena of tumor biology. It has been demonstrated that HLA expression varies in various types of lung cancer, even in differential stages of one type of lung cancer. HLA expression is highly associated with genre, progression and prognosis of lung cancer. The thorough investigations of the relationship between HLA and lung cancer and of mechanism of HLA abnormality will provide useful information for studying of development of lung cancer and tumor immunotherapy.
