1) Anjo Kosei Hospital has nearly 40 years of history of the post-primary clinical training course.
2) More than 90% of the young doctors, passed through 2 years of the primary clinical training course, have chosen further training at Anjo Kosei Hospital. During this course they began to start the experience for their own career for specialist.
3) This education program is closely coordinated with Medical Colleges such as Nagoya University and Nagoya City College of medicine. After 4 or 5 years of training at Anjo Kosei Hospital, they continued their training at Nagoya University Hospital or Nagoya City College Hospital for the further career.
2.ALTERATIONS OF SALIVARY SIgA DURING TRAINING CAMP IN COLLEGIATE RUGBY FOOTBALL PLAYERS

The purpose of this study was to investigate the relation between alterations of salivary secretory immunoglobulin A (SIgA) and the occurrence of upper respiratory tract infections (URTI) in rugby football players.We examined the relationship between the onset of URTI and the daily alterations of SIgA levels in 32 male collegiate rugby football players (20.5±1.3 years) during summer training camp for 36 days.Total of 6 in 32 subjects had the appearance of URTI symptoms (18 %). SIgA secretion rate decreased significantly in the middle of training camp compared to the baseline (P<0.05). Furthermore, SIgA secretion rate during the appearance of URTI (13.7±1.1μg/min) were significantly lower than that without symptoms (19.2±1.4 μg/min, P<0.01). These results suggest that serial monitoring of SIgA may be useful to assess the risk status of URTI affection in athletes.
3.A Case of True Brachial Arterial Aneurysm

An extremely rare case of true brachial arterial aneurysm due to blunt trauma is reported. A 55-year-old male suffered from blunt trauma in his right upper arm when he was 8 years old. He had noticed a mass in his right upper arm for more than 30 years, however, it grew rapidly to 7×6×5cm within several months. Digital subtraction angiography revealed a large saccular aneurysm of the right brachial artery. The aneurysm was removed and replaced by a saphenous vein graft.
4.Application of percutaneous transesophageal gastro-tubing in palliative treatment of malignant bowel obstruction

Percutaneous transesophageal gastro-tubing (PTEG) is considered a safe and effective treatment technique for drainage or feeding in patients with malignant bowel obstruction. We report six cases with malignant bowel obstruction due to recurrent gastric cancer whose bowel symptoms were satisfactory improved with PTEG. Ileus tubes were inserted in three of the six patients for decompression of the alimentary tract, a feeding tube was inserted in one patient for enteral feeding, and double elementary diet tubes (W-ED tube) were inserted in other two for decompression and enteral feeding. The median indwelling duration was 52days (rang; 24-460days). Home enteral nutrition was possible for the three patients with feeding tube and W-ED tube. Regarding decompression of alimentary tract, PTEG relieved the symptoms of all patients. One patient suffered from dermatitis around the inserted part of ileus tube because of the leakage of gastro-intestinal juice, and the ileus tube was removed. In two cases, the inserted tubes were exchanged because of the tube obstruction. PTEG made possible to use various types of tube depending on the state of the disease. From our experience, it is suggested that PTEG technique is an effective and useful procedure of choice for long-term drainage or feeding in patients with malignant bowel obstruction. 5.Effect of Hot-Spring Hydrotherapy on Lymphocyte Subsets Expressing β2-Adrenergic Receptors

We have previously reported that hot-spring hydrotherapy for a short duration can change hormone levels in the blood and quantitatively as well as qualitatively regulate leukocytes and their subpopulations. In this study, we examined the effect of hot-spring hydrotherapy on the lymphocytes expressing β2-adrenergic receptors (β2-ARs). We obtained peripheral blood from 9 healthy volunteers (age, 49.7±11.2 years) at the same sampling time before and after (the next day) hot-spring hydrotherapy, and measured the total and differential leukocyte counts and the levels of 3 catecholamines (adrenaline, noradrenaline, and dopamine). We also analyzed the β2-AR of lymphocyte subsets by combination with fluorescent monoclonal antibodies (CD3, CD4, CD 8, CD 19, and CD56) by using flow cytometry.
The number of granulocytes significantly decreased after hot-spring hydrotherapy (p < 0.05). The ratio of β2-AR+ CD4+ cells and β2-AR+ CD3+ cells was low, but that of β2-AR+ CD56+ cells and β2-AR+ CD8+ cells was high. The CD8+ cell and CD56+ cell counts tended to increase after hot-spring hydrotherapy, but no significant variation was found in the β2-AR+ cells of each subset. Adrenaline levels also significantly decreased after hot-spring hydrotherapy (p < 0.05). The rate of change in adrenaline levels before and after hot-spring hydrotherapy was highly correlated with the rate of change in the levels of CD56+, CD8+, and β2-AR+ CD56+ cells. Thus, we found a difference in thepercentage of β2-AR-expressing cells in the lymphocyte subsets. Therefore, it is possible that a short duration of hot-spring hydrotherapy decreased adrenaline levels and quantitatively influenced the natural killer cells and CD8+ cells, which express large amounts of β2-AR.
6.Ramelteon for Delayed Sleep-wake Phase Disorder: A Case Report

Recently developed melatonin receptor agonists are expected to be effective for delayed sleep-wake phase disorder (DSWPD). To date, however, no study has described the effect of melatonin receptor agonists on DSWPD. We report the case of a 15-year-old girl with DSWPD who was successfully treated with ramelteon 4 mg at 7 PM. DSWPD symptoms were resolved; her sleep-wake and biological rhythms were normalized.

1)Continuing medical education (CME) systems were researched in 10 countries. In all countries but one CME is mandatory. Only Spain has voluntary CME, as does Japan.
2)The traditional CME systems in many countries were changed after 2000. We believe this change reflects a global revolution associated with a new wave of medical risk management.
3)To provide better medical services, we must keep improving Japan's CME system. Such improvement is an important responsibility to society.

Congenital quadricuspid aortic valve is a very rare malformation. We report two cases with severe aortic regurgitation due to isolated quadricuspid aortic valve. It consisted of three equal cusps and one smaller cusp, which was identified at the time of valve replacement surgery for severe aortic regurgitation.
9.The effects of Lactobacillus gasseri OLL2809 intake on oral immune function in college rugby athletes

The purpose of this study was to investigate the effects of Lactobacillus gasseri OLL2809 (MG2809) intake on salivary secretory immunoglobulin A (SIgA) secretion and incidence of upper-respiratory tract infection (URTI) symptoms among college rugby athletes. Sixty-seven subjects were assigned to a MG2809 group (n = 33) or a placebo group (n = 34) using a double-blind procedure. Each subject took MG2809 or placebo tablets for 9 weeks. Resting saliva samples were collected before (0 week) and after 2, 4, 6, 8, and 9 weeks of the MG2809 intakes. A log of URTI symptoms was kept every day during the study period. Our results showed that salivary SIgA level was significantly increased at 6 weeks in the MG2809 group (p < 0.05), although the placebo group did not show significant change. The duration of URTI symptoms episode in the MG2809 group was significantly shorter than that in the placebo (p < 0.05). These results suggest that regular intake of MG2809 may enhance oral immune function mediated by SIgA and reduce the risk of URTI in athletes.
10.A Case of Primary Cardiac Angiosarcoma Associated with Cardiac Tamponade

We report a rare case of primary cardiac angiosarcoma in the right atrium. A 47-year-old man was admitted to our hospital with cardiac tamponade. Echocardiography and computed tomography revealed a tumor in the right atrial cavity. We performed tumor resection to confirm the histological diagnosis, to prevent tumor embolism, and to increase the possibility of improving the prognosis. The tumor was resected with the right atrial wall and right pericardium. The right atrium was then reconstructed with a bovine pericardial patch. The pathological diagnosis was angiosarcoma. The patient survived only about 6 months after surgical resection, but there was no local recurrence. This report presents a very rare case of cardiac angiosarcoma associated with cardiac tamponade.
