2.Clinical Application of Chinese Decoction Pieces in General Hospital:Problems and Countermeasures

OBJECTIVE: To take feasible control measures aimed at the problems appeared in clinical application of Chinese decoction pieces in general hospital.METHODS: The problems existed in physicians' prescribing of Chinese traditional medicine,the filling of prescriptions conducted by dispensers as well as the quality of Chinese decoction pieces were analyzed.RESULTS: The problems such as physicians' nonstandard writing of drug names,no difference in the application between raw decoction pieces and processed decoction pieces,super-large dose medication,careless in checking prescriptions,dispensers' lack of sense of responsibility in filling prescriptions,unstable quality of the Chinese decoction pieces had a direct impact on clinical curative efficacy.CONCLUSION: The curative efficacy of traditional Chinese medicine can be improved by setting and improving the information database of Chinese decoction pieces with the use of hospital information management system resources,cooperating with physicians to achieve standard and rational use of Chinese decoction pieces,strengthening management on the quality of Chinese decoction pieces and promoting the professional quality of dispensers.
3.Correlation Between Spiral CT Enhancement Features and Clinicopathologiacal Characteristics of Hapatocellular Carcinoma

Objective To study the correlation between the contrast enhancement features on spiral computed tomography (SCT) and different clinicopathological characteristics in hepatocellular carcinoma (HCC).Methods Fifty cases (54 lesions) of HCC proved pathologically and examined with enhancement dual-phase SCT scanning (arterial phase and portal vein phase) were included in this study. The SCT features and the clinical details about resection and pathological differentiation were recorded. All the features of SCT were compared with some clinical and histopathological characteristics of HCC.Results The features of SCT were correlated with pathological characteristics of HCC. The pseudocapsula patterns were related with Edmondson's grade, and the enhancement patterns were associated with the diameter of tumor and intratumoral necrosis. There was a significant correlation between the high infiltration and severity (intrahepatic daughter foci, tumor-emboli in portal veins, lymphatic metastasis) on the SCT images and the diameter of tumor. The same correlation was found between the diameter of tumor on the SCT images and AFP value, and also between hepatic cirrhosis on the SCT images and HBsAg, the diameter of tumor.Conclusion This research confirms that SCT features are correlated with some clinicopathological characteristics of HCC.
4.Correlative Study on the Expression of Vascular Endothelial Growth Factor and Microvessel Density with Spiral CT Features in Lung Carcinoma

Objective To investigate the correlation of the expression of vascular endothelial growth factor(VEGF) and microvessel density(MVD) value with spiral CT(SCT) features in lung carcinoma.Methods Pre-and post-enhanced SCT scans were performed in 41 cases of lung carcinoma proved surgically and pathologically.There were 29 men and 12 women.22 cases were peripheral lung carcinoma,and 19 cases were central lung carcinoma.SCT characteristics of all cases were analyzed and recorded.SP immunohistochemical technique was experimented on each pathologic specimen.The enhanced range of lesions and the expression of VEGF,MVD value were analyzed using stepwise regression.Chi-square test was performed to determine the correlation between the expression of VEGF,MVD value and CT features in peripheral lung carcinoma.Results CT enhanced degree of lesions increased with the increased level of positive expression of VEGF and MVD value by stepwise regression analysis(standardized coefficients were 0.498 and 0.399,respectively,P3cm in diameter were significantly higher than those of the group with tumor size ≤3 cm.The MVD value increased significantly in cases with deep lubulation sign,spiculate protuberance and vessel convergence(P

Objective To evaluate the correlation between the contrast enhanced features on spiral computed tomography(SCT) and microvessel density(MVD),TP,TGF-?_1 expression.Methods Forty seven cases with hepatocellular carcinoma(HCC) proved pathologically were examined with enhanced dual-phase SCT scanning at the arterial phase and the portal vein phase.The SCT features including the size of HCC lesions,enhanced patterns,capsule patterns,the diameter of tumor,intrahepatic daughter foci and tumor-emboli in portal veins were studied.The expression of TP,TGF-?_1 and MVD were detected by immunohistochemical SP method.The enhanced features of SCT were compared with the immunohistochemical results and clinicopathological characteristics of HCC.Results TP and MVD were correlated with metastasis and capsule patterns(P0.05).Conclusion The expression of TP,TGF-?1 and angiogenesis of HCC can be evaluated by some SCT features in a certain degree.
6.Correlative Study on Spiral CT Features and Expressions of bFGF,MVD in Peripheral Lung Carcinoma

Objective To study the correlation between the features of peripheral lung carcinoma on spiral CT(SCT) and bFGF expression and angiogenesis. Methods 22 cases of peripheral lung carcinoma proved pathologically and examined with CT(SCT) both plain scan and contrast-enhanced scan were analyzed retrospectively . The expressions of bFGF and angiogenesis were detected with immunohistochemical SP method. The SCT features were compared with immunohistochemical results such as bFGF expression, microvessel density (MVD). Objective:To analyze the drug use in a patient with Crohn’ s disease and provide relevant pharmaceutical service by clinical pharmacists. Methods:Clinical pharmacists participated in the treatment of the patient. The medication was analyzed according to the check results and diagnosis and treatment process, and relevant pharmaceutical service was provided for the patient and nurses. Results: Through the participation in the patient treatment and health education for the patient and nurses performed by clinical pharmacists, the patient was recovered and discharged. Conclusion: Clinical pharmacist participating in pharmaceutical service can promote rational drug use in clinics.
8.Effects Evaluation of Antibacterial Drug Prescriptions Comment in Outpatient and Emergency Depart-ment of a Hospital

OBJECTIVE:To provide reference for rational use of antibacterial drugs in the clinic. METHODS:In retrospective study,outpatient and emergency antibacterial drugs prescriptions were randomly selected from a hospital during 2013-2014,and then analyzed statistically. DDDs of antibacterial drugs were analyzed by using DDD method. RESULTS:From 2013-2014,the ap-plication rate of antibacterial drugs in the hospital declined substantially from 13.8% to 11.9%,and the rate of irrational use dropped from 2.7% to 1.2%. The application rate of antibacterial drugs was higher in pediatric department,emergency department and urology department;drug cost per capita was higher in infectious department,nephrology department and emergency depart-ment. The application rate of antibacterial drugs in pediatric department decreased greatly in 2014,compared with 2013,maintain-ing about 40%. The application rate of antibacterial drugs in emergency department during Jan.-Sept. in 2014 was lower than corre-sponding period in 2013,and that of the hospital during Jan.-Sept. in 2014 was lower than corresponding period in 2013,but dur-ing Oct.-Dec. in 2014 was slightly higher than corresponding period in 2013. Single type of antibacterial drugs were mainly used in outpatient department,accounting for 91.50% and 90.77% of antibacterial drug prescriptions in 2013 and 2014;two-drug account-ed for 8.44% and 9.11%. The top 3 antibacterial drugs in the list of DDDs during 2013-2014 were roxithromycin,cefuroxime and cefminox. CONCLUSIONS:The use of antibacterial drugs is up to the requirements of Nation Special Rectification Actirity for Clin-ical Application of Antibacterial Drugs in 2013. The prescription comment improve the quality of antibacterial drug prescriptions and rational use index greatly in outpatient department,but the application rate of antibacterial drugs is too high in some depart-ment. There still is irrational use of antibacterial drugs.

Objective:To analyze the medication of one patient with ulcerative colitis to provide pharmaceutical care and support for rational drug use in patients with ulcerative colitis. Methods:During the treatment of the patient with severe ulcerative colitis, clin-ical pharmacists analyzed the drugs used by the patient and provided pharmaceutical care for doctors and the patient according to the ex-amination and diagnosis of the patient. Results:The compliance, therapeutic effect and medication safety of the patient were all im-proved by giving clinical drug rationalization suggestions and targeted medication monitoring and education, which fully embodied the necessity of work of clinical pharmacists in the medication of patients. Conclusion:Through case analysis, clinical thinking of clinical pharmacists can be developed to promote rational drug use, avoid adverse drug reactions and achieve optimal effect of drug treatment.
