1.Clinical effect of total knee arthroplasty for severe knee valgus deformity
Journal of Regional Anatomy and Operative Surgery 2016;25(8):609-611
Objective To research the clinical effect and operation way of total knee arthroplasty for severe knee valgus deformity. Methods From March 2011 to September 2013, 17 cases (23 knees) with severe knee valgus deformity were selected as the research ob-ject. Metered the tibial angle and checked the activity of knee joint of all the patients before operation. All patients were scored with HSS knee scoring system. Through the selection of lateral parapatellar approach, targeted osteotomy, selective lateral soft tissuelax, patients were given total knee arthroplasty for the treatment of severe knee valgus deformity. The femoral and tibial angle of patients was measured again after the operation. All patients were reexamined after 6 months which included checking the activity of knee joint, evaluating the therapeutic effect of patients with HSS knee scoring system. Results All patients were reexamined in our hospital 6 months after surgery, the degree of femoral and tibial angle decreased from (28. 4 ± 4. 9)° to (5. 2 ± 2. 3)°; knee joint range of motion increased from (65. 4 ± 16. 7)° to (106. 7 ± 27. 3)°;HSS score increased from (38. 46 ± 3. 75) point to (87. 41 ± 4. 77) point. Postoperative indexes were significantly im-proved compared with preoperative indexes, and the differences were of statistical significance (P<0. 05). Conclusion The curative effect is remarkable for patients with severe knee valgus deformity using patellar medial approach, standard osteotomy combined with selective soft tissue release, and total knee arthroplasty. And the knee valgus deformity of patients was corrected and the quality of life has been signifi-cantly improved.

Object To develop a mathematical model on the relationship between the temperature used in the course of extraction of medicinal herbs and the concentration of active component obtained Methods Based on the kinetic model on medicinal herb extraction process reported in our previous article *, diffusion coefficient and concentration gradient varying with temperature were further studied Results The process which tanshinone was extracted from Salvia miltiorrhiza Bge at different temperatures showed that the model could well match the experimental data Conclusion The relation between the temperature and the concentration of active component could be described by this model

Objective To analyse the radiological features and the diagnostic method of pulmonary alveolar proteinosis(PAP).Methods X-ray and CT manifestations of PAP in 37 cases confirmed with fiberoptic bronchoscopy and bronchoalveolar lavage were studied retrospectively. Results The radiologic features of PAP could be characterized as geographic, the “crazy-paving” pattern, lobar or segmental consolidation(air-brochogram sign)and like intersititial fibrosis. The radiologic manifestations were stable and more serious than the symptoms. Conclusion PAP is of typical radiologic feature,it is not difficult for diagnosis in combination with clinical characteristics.

Objective To investigate the application of lower limb electromyography in guiding preoperative localization for multiple segment intervertebral disc herniation treated with ozone injection and in predicting the curative efficacy. Methods A total of 51 patients with MRI-proved multiple segment lumbar intervertebral disc herniation, who were admitted to authors' hospital during the period from January 2012 to December 2013 and who had complete clinical materials, were included in this study.Guided by DSA,intradiscal injection of ozone together with the injection of ozone plus compound betamethasone and bupivacaine into the space around the spinal nerve was carried out in all patients. The degree of lower lumbago was evaluated with M-JOA score, and Oswestry disability index (ODI) was adopted to assess the curative effect. Results Preoperative electromyography showed that 43 patients had nerve root injury, the positive rate was 84.3%.The positive HIZ sign was detected in 30 patients, with the positive rate being 58.8%.The positive Modic sign was observed in 24 patients, the positive rate was 47.1%.Postoperative MJOA scores indicated that one-,3-and 6-month effective rates were 90.1%,86.2% and 78.5% respectively;and postoperative ODI values showed that one-,3-and 6-month effective rates were 87.3%,79.5% and 74.6% respectively. Conclusion In treating multiple segment intervertebral disc hemiation with ozone injection, lower limb electromyography is very valuable in accurately making preoperative localization as well as in precisely predicting the curative efficacy, thus this technique is worth popularizing in clinical practice.
