1.Endoscopic treatment for gastrointestinal submucosal tumor:A clinical analysis of 94 patients

Objective To discuss the indication, technique, effect, and safety of endoscopic treatment of SMT in gastrointestinal tract. Methods Endoscopic therapy was performed in 94 patients with gastrointestinal diagnosed by endoscopy. 22 cases underwent EMR; 61 cases were treated with ESD; MBM was exercised on 7 cases; and nylon endoloop ligature were carried out on 4 cases. Results Complete resection was achieved on 86 cases, and 1 case was excised partially. 1 patient with SMT was transferred to open surgery. Lumps of 4 SMT cases treated by nylon endoloop ligature were found disappeared at subsequent visit after a month. All patients were followed up for one year, and no recurrence was found. Conclusions Gastrointestinal SMT can be completely resected by EMR, ESD and MBM. Nylon endoloop ligature can be used for the treatment of SMT in position difficult to deal with. Endoscopic therapy is relatively safe, effective, and minimal invasion for SMT.
3.CT value in diagnosing of anastomotic leak after esophagectomy

Objective To assess the diagnostic performance of CT for detecting anastomotic leak after esophagectomy.Methods A total of 50 patients who underwent both CT and fluoroscopy after esophagectomy in our hospital from September 2013 to December 2015 were enrolled in this study.The fluoroscopy features and CT findings were analyzed retrospectively.Results The sensitivity,specificity,positive and negative predictive values were 100%,86.67%,83.33%,100%,respectively,for CT,and 85%,100%,100%,90.91%,respectively,for fluoroscopy.The differences in CT features,including leak of contrast,wall discontinuity and air-fluid collection in the mediastinum were statistically significant between leak and no leak(P=0.000,P=0.000,P=0.005,retrospectively),while no statistically significant in air collection in the mediastinum,hydropneumothorax,pleural effusion and pneumatosis.The maximum diameter of air collection in leak was(1.54±0.97)cm,while the maximum diameter of air collection in no leak was (0.56±0.20)cm,the difference in the maximum diameter of air collection between leak and no leak was statistically significant(P=0.007).Conclusion CT is more sensitive but less specific than fluoroscopy for detecting anastomotic leak.CT can be a better choice for the patients who can not tolerate the fluoroscopy or the fluoroscopy results is negative but anastomotic leak is still suspected.

OBJECTIVE:To investigate the influence of TCM Granules of the detoxification and dissipation blood stasis formula Ⅱon fulminant hepatic mitochondrial lipid peroxidation in rats with hepatic failure.METHODS:The fulminant hepatic failure rat models were established by subcutaneous injection of thioacetamide (TAA).48 Wistar rats were randomly divided into 7 groups:blank control group,model group,low dose group,medium dose group,high dose group,the lactulose treatment group,and the "Bezoar pill for resurrection" treatment group.Intragastric administration was executed 3 d before model-making,twice per day with the interval of 12 hours.They were administered for 11times.12 hours after model establishment.MDA、SOD、CAT、GSH、NO and liver necrosis area in hepatic mitochondria were determined.RESULTS:The detoxification and dissipation blood stasis formula Ⅱcould notably reduce liver necrosis area and restrain produce of hepatic mitochondrial lipid peroxidatide MDA,retrieve the activity of SOD、CAT and increase the content of GSH,NO.Moreover it shows dose-effect relation.Compared with model group,there is statistical significance(P

6.Astragalus polysaccharides influence ultrastructure of islets and Th1/Th2 cvtokine gene expression of pancrease in NOD mice

The effects of astragalus polysaccharide (APS) on type 1 diabetes mellitus were investigated. The mRNA expressions of Th1/Th2 cytokines of pancrease and ultrastructure of islet were studied.Ultrastructure of islets was well protected in APS group;the mRNA expressions of Th1 cytokines were downregulated and those of Th2 cytokines were upregulated in APS group.APS may correct the imbalance of Th1/Th2 cytokines.

8.Effects of astragalus polysaccharides on metabolism of collagen in diabetic cardiomyopathy

Objective To investigate the effects of astragalus polysaccharides(APS)on the metabolism of collagen in hamsters with diabetes mellitus(DM) and explore the possible mechanism of APS on diabetic cardiomyopathy. Methods The levels of insulin,C-peptide,myocardial enzymes,glycosylated serum protein(GSP),plasma and myocardial angiotensin(AngⅡ) were tested in APS group(n=15) and DM control group(n=15).Immunohistochemistry was used to measure the expression levels of typeⅠand typeⅢ collagen,activities of matrix metalloproteinases(MMPs) were examined by using zymography,and TNF-? and TGF-? mRNA were detected by RT-PCR. Results Compared with DM control group,levels of GSP,myocardial enzymes and myocardial AngⅡ were much lower in APS group(P0.05).The level of expression of collagen Ⅰ,the ratio of collagen Ⅰ/collagen Ⅲ,and the activity of pro-MMP-2 and MMP-2 in APS group were significantly lower than those in the DM group(P

10.Value of drug resistance and homology of Acinetobacter baumannii in tracing the source of nosocomial infection in Jianyang, Sichuan

Abstract: Objective To analyze the drug sensitivity and the carrying of carbapenem resistant gene of Acinetobacter baumannii isolated from clinical patients and clinical objects, and analyze the homology of strains to provide support for the control of nosocomial infection. Methods A total of 38 strains of Acinetobacter baumannii isolated from patients and clinical objects surface were collected from January 2019 to August 2020. The antimicrobial susceptibility was tested by the minimum inhibitory concentration method. In addition, the resistance related genes were detected by polymerase chain reaction method, and homology analysis was performed by enterobacterial repetitive Enterobacterial Repetitive Intergenic Consensus Polymerase Chain Reaction (ERIC-PCR). Results All 34 strains of Acinetobacter baumannii isolated from Clinical patients and 4 strains isolated from clinical objects carried blaOXA-51 and imp resistance genes, neither of them carried blaVIM gene. 32 Acinetobacter baumannii carrying blaOXA-23 gene, 28 strains carrying blaTEM gene, 7 strains carrying blaOXA-58 gene. After cluster analysis, 38 Acinetobacter baumannii isolates were classified into 7 genotypes (expressed A, B, C, D, E, F, G), and cluster E and cluster G were the main clusters, containing 12 strains (12/38, 31.6%) and 18 strains (18/38, 47.4%), respectively, as the main prevalent clonal strains. Conclusions Acinetobacter baumannii isolated from different sources have the significant differences in drug resistance and carry different resistance genes. There is no direct correlation between patients and environmental isolates of Acinetobacter baumannii belonging to different clonal strains. Also, there aren’t significant correlation between clinical patients infected with Acinetobacter baumannii.
