Journal of Endocrine Surgery 2010;04(3):193-195
Objective To observe and discusses the application of larger-bore autologous arteriovenous fistula as dialysis pathways in diabetes patients with chronic renal failure. Methods 86 cases of diabetic patients with chronic renal failure were divided into elderly group and non-elderly group. The patients within each group were assigned randomly into Experimental subgroup and control subgroup. Patients underwent wrist autogenous arteriovenous fistula angioplasty. The surgical method was end to side anastomosis between cephalic vein and radial artery. Vertical incision on the radial artery is about 0.7 cm-0.8 cm in the control subgroup and vertical incision on the radial artery is about 1.0 cm-1.1cm in the experimental subgroup. Results The success rate of the operation in experimental subgroup(90.9%, 95.2%) was significantly higher than that of control group(77.3%, 85.7%). The differences were significant (P<0.05). There was no heart failure in the patients underwent surgery. Conclusions In diabetes patients with chronic renal failure need autogenous arteriovenous fistula angioplasty. Properly increasing the diameter of anastomosis can meet the demand on blood flow of hemodialysis without increase complication of heart failure.

Acta Nutrimenta Sinica 1956;0(03):-
Objective: To investigate the effect of growth hormone(GH) and hypo calorie enteral nutrition(EN) on protein metabolism in patients after cardiac operations. Methods: 36 patients after cardiac operations were randomly divided into two groups. The test group received GH and EN, and the control group received EN only. The treatment was maintained 7 days postoperatively. Plasma albumin(A)?transferrin(TF)?prealbumin (PA)?retinol binding protein(RBP) on the preoperative day and day 1,4,8 postoperatively and nitrogen balance(NB) on the day 1,2,3,5,7 postoperatively were detected. Results: There was no significant difference of all plasma protein levels between two groups before operation and all decreased on the 1st postoperative day, but only RBP higher in the test group than that in the control. There was significant difference in TF,PA,RBP levels on the 4th and 8th postoperative day, the plasma A level on the 8th postoperative day, and NB on the 1st to 7th postoperative day between the two groups. Conclusion: Combined use of GH and EN can elevate plasma A,TF,PA,RBP levels and improve negative NB.

Chinese Journal of Minimally Invasive Surgery 2017;17(3):242-244,248
Objective To explore the efficacy of internal fixation in the treatment of intra-articular calcaneum fractures via the sinus tarsi approach . Methods A retrospective study was made on 55 feet with intra-articular calcaneum fractures in 51 patients treated with open reduction and intemal fixation via sinus tarsi approach from January 2010 to June 2015.According to the Sanders classification, there were 15 feet of type Ⅱfractures and 40 feet of type Ⅲfractures. Results All the patients were followed up for a mean period of 15.7 months (range, 6-33 months). The fractures were completely healed .According to the Maryland Foot Scores , the operative effect was excellent in 43 feet, good in 7 feet, fair in 4 feet, and poor in 1 foot. The excellent and good rate was 90.9%(50/55).The postoperative width of the calcaneum , B?hler angle, and Gissane angle were significantly improved than before operation (P=0.000). Conclusion Open reduction and internal fixation via sinus tarsi approach is a simple and effective method for minimally invasive treatment of Sanders type Ⅱand Ⅲintra-articular calcaneum fractures .
9.Early MRI findings and the evolution of neonatal hypoglycemic brain injury
Chinese Pediatric Emergency Medicine 2014;21(5):263-267
Objective To investigate the imaging features in neonates with hypoglycemic brain injury by early and serial MRI.Methods Neonates who were admitted to neonatal department from May 2005 to Jul 2013 received MRI and diffusion-weighted imaging(DWI) scan within 7 days after hypoglycemia.Fortynine neonates were diagnosed with hypoglycemic brain injury.Thirty-four neonates received second MRI scan between 2 ~3 weeks after hypoglycemia.Seven neonates received third MRI scan. Results All the 49 neonates showed hyperintensity on DWI in the regions of occipital and parietal lobe for the first MRI scan(mainly involved 33 cases).Six cases combined frontal lobe and temporal lobe.Ten cases presented widespread cortex and white matter involvement. All the cases presented hypointensity on DWI for the affected area,T1 and T2 weighted image signal changes were not obvious. Some cases presented deep white matter and gray matter injury.Thirty-four cases received second MRI scan between 2 ~3 weeks after hypoglycemia,20 presented hypointensity on DWI,hypointensity on T1 weighted image and hyperintensity on T2 weighted image.Eleven cases with normal signals, and the other 3 were absorbing.Seven cases received third MRI scan, appeared encephalomalacia, myelin retardation, white matter volume decreased, hypoplasia of corpus callosum. The more severe the clinical symptoms was,the more severe the degree of brain injury showed. Conclusion Cerebral occipital and parietal regions are the most vulnerable in neonatal hypoglycemic brain injury.Early DWI for the imaging diagnosis of hypoglycemic brain injury should be taken within one week after hypoglycemia.Mild injury is recoverable, but severe would chang into necrosis and encephalomalacia. Some combined deep white and gray matter injury might related to hypoxia and ischemia.

Chinese Pediatric Emergency Medicine 2015;22(9):599-602
Objective To study the clinic effect and safety of inhaled nitric oxide for the neonates with hypoxic respiratory failure. Methods A total of 21 neonates with hypoxic respiratory failure were trea-ted with nitric oxide inhalation after ineffective treatment of conventional mechanical ventilation. The values of mean airway pressure( MAP) ,oxygenation index( OI) ,mean blood pressure and saturation of blood oxy-gen percutem( TcSaO2 ) were detected before and after nitric oxide inhalation. Meanwhile, the values of coag-ulation function, nitrogen dioxide concentration and methaemoglobin were monitored. Results The lung oxy-genation of 21 cases were significantly improved in 48 hours after nitric oxide inhalation,OI declined from 26. 600 ±4. 169 to 7. 500 ±1. 716,MAP decreased from(15. 100 ±2. 132)cmH2O(1 cmH2O=0. 098 kPa) to (8. 000 ± 0. 816) cmH2O,which all had statistical differences(P <0. 01). The monitoring of coagulation function was in the normal range. The levels of methaemoglobin were less than 3% and nitrogen dioxide less than 1 × 10 -6 . In this study,18 neonates survived,2 neonates died and 1 neonate gave up treatment. Conclu-sion Inhaled nitric oxide early can improve oxygenation effectively for neonates with hypoxic respiratory failure without obvious side-effect. However,it is not yet clear for its long-term prognosis and whether there is influence development of the nervous system.
