1.Comparison of MRI and arthroscopy on defect area measurement of knee joint cartilage

Objective To explore the consistency of knee articular cartilage defect area by magnetic resonance imaging (MRI)and arthroscopy.Methods Thirty-six patients (36 knees)with knee cartilage lesions were collected in our study.The defect area meas-urement of knee articular cartilage by MRI and arthroscopy was compared.Results A total of 92 lesions were detected by both MRI and arthroscopy with an average of 2.6 defects per knee.Preoperative MRI resultes demonstrated the mean defect area per knee was (1.9±1.5)cm2 ,while arthroscopic measurement after debridement indicated the mean defect area per knee was (2.9±2.6)cm2 , which had significant difference(P <0.001).The area measured by MRI was less than that by arthroscopy,and the mean difference was (1.7 ± 1.1)cm2 .The mean area measured by MRI was 70.1% of that by arthroscopy.Conclusion Arthroscopy and MR have good consistency in the number of articular cartilage defect lesions .MRI can reflect the knee articular cartilage defect area directly or indirectly,and be capable of accurate positioning and qualitative diagnosis.
2.Epidemic analysis of brucellosis in Datong of Shanxi province between 2006 and 2009

ObjectiveTo investigate the epidemic situation of brucellosis in Datong city, and to provide scientific evidence for making appropriate prevention and control measures. MethodsSurveillance data of human brucellosis in 7 countris and 4 districts in Datong city between 2006 and 2009 were collected, throng the national network straight quote system in an infectious diseases. Excel database was established and all data were statistically analyzed. Incidence of brucellosis in local population was analyzed. The regional distribution, time distribution,occupation, age and sex distribution were analyzed. Epidemic characteristics and trend of brucellosis in Datong city were summarized. Results A total of 5195 cases of brucellosis patients in Datong were found between 2006 and 2009, the average incidence rate was 57.51/10 million. All counties had the disease, and the onset of the disease mainly in the spring and summer. Most cases were young males. Farmer case was 81.67%(4243/5195) of the total patients. ConclusionsFrom 2006 to 2009, epidemic characteristic of Datong human brucellosis ishigh-low-high(incidence). We suggests the Department concerned to strengthen the prevention and control of the disease in some counties, focusing on spreading of disease prevention and control knowledge among farmers and increase their self-protection awareness.
3.Efficacy of Zaozhu Yinchen Recipe for Treating Non-alcoholic Steatohepatitis and its Effect on Free Fatty Acid and TNF-alpha.

OBJECTIVETo observe the efficacy of Zaozhu Yinchen Recipe (ZZYCR) on non-alcoholic steatohepatitis (NASH) patients, and to explore its effect on serum free fatty acid (FFA) and tumor necrosis factor alpha (TNF-alpha).
METHODSTotally 120 patients with NASH were randomly assigned to the treatment group (60 cases, treated with ZZYCR, one dose per day) and the control group (60 cases, treated with Silibin Meglumine Tablets, 20 mg each time, thrice per day). The therapeutic course for all was 24 weeks. Serum levels of ALT and AST activities, TC and TG levels were detected before and after treatment. Peritoneal CT was performed in all patients, and CT ratios of liver and spleen calculated. NAFLD activity score (NAS) and degree of hepatic fibrosis were assessed using pathological examinations of liver tissue, and efficacy also evaluated. Serum contents of FFA and TNF-alpha were also detected.
RESULTSCompared with before treatment in the same group, activities of ALT and AST, serum levels of TC, TG, FFA, and TNF-alpha, NAS, scores of symptoms and signs all obviously decreased, degree of hepatic fibrosis was obviously improved in the two groups (P < 0.05, P < 0.01). These changes were more obviously seen in the treatment group (P < 0.05). After 24-week treatment, the total effective rate and total clinical efficacy were 80.00% (48/60 cases) and 85.00% (51/60 cases) in the treatment group, obviously higher than those in the control group [60.00% (36/60 cases) and 73.33% (44/60 cases) respectively], with significant difference (P < 0.05, P < 0.01).
CONCLUSIONZZYCR could improve the clinical efficacy of NASH patients, and its mechanism might be associated with inhibiting serum levels of FFA and TNF-alpha.

4.Survey report about farmers and herdsmen in a demonstration base of Datong on Brucellosis prevention and control in Shanxi province

Objective To survey knowledge of the local people's understanding of Brucellosis,and to explore the risk factors for brucellosis infection,and to determine the key issue of next comprehensive health promotion intervention.Methods Two counties,Yanggao and Guangling,which are old endemic areas with Brucellosis in history,and with epidemic rebounding in recent years,were selected.The survey was carried out by two stage stratified cluster sampling method.The questionnaires included respondents' demographic data(gender,age,education level,etc.),Brucellosis (hereinafter referred to as Brucellosis) knowledge of the investigation and behavior and attitude of people toward the measure for control of Brucellosis.Results A total of 5372 people were investigated in two counties of which 62.7%(3362/5372) of farmers.The investigated crowd had low culture level.The awareness of Brucellosis infection route of Yanggao and Guangling counties were 84.03% (2379/2831) and 333%(847/2541 ).The average awareness of Brucellosis infection route was 18.60%(6001/32 260).In the investigation of knowledge on Brucellosis prevention of the two counties,29% believed that it was necessary to wear gloves to process flow product,and 70% of people answered do not know.For farmers on how to deal with dead animals,results showed that 79.1%(664/839) in Yanggao choose to sell dead animals to the market; 61.2% (267/361) in Guangling choose to kill and bury,there were inappropriate treatment on handling of ill and dead animals in the two counties.Conclusions Spread of Brucellosis is caused mainly due to emphasis on the disease is not enough,and inappropriate handling of dead livestock.Measures like strengthening health education and behavioral intervention,increasing public awareness of the disease prevention and ability to change the incorrect way of life and cognitive concepts,can effectively reduce human infection and the spread of Brucellosis.

OBJECTIVETo establish a novel Myc gene transgenic mouse model for spontaneously forming B-lymphoma and assessing its tumorigenesis potential.
METHODSFreshly isolated hematopoietic progenitor cells served as the target for Myc gene transfer mediated by a retrovirus vector. These cells were engrafted into C57BL/6 mice with (60)Co-gamma ray radiation in advance. Tumor latency was measured and the tumor loaded mice were followed for survival time. Tumor was identified with histology and immunostaining. The exogenous Myc gene was detected by Western blot (in liver, spleen, tumor tissue) and flow cytometry (FCM) \[in bone marrow (BM)\].
RESULTSMice BM-infected with mutant Myc gene more readily gave rise to B-cell lymphomas than those infected with wild type Myc gene did Myc gene was expressed highly in BM and tumor tissues but not in liver and spleen.
CONCLUSIONOur model will be a tool in assessing the transforming potential of Myc mutants and in studying cooperation between Myc and other oncogenes. Mutant Myc is more effective than wild-type Myc in promoting B cell lymphomagenesis in mice.

6.Effect of shenluotong decoction on renal interstitial fibrosis in rats with obstructive nephropathy.

OBJECTIVETo observe the effect of Shenluotong Decoction (SD) on serum levels of aldosterone, monocyte chemoattractant protein-1 (MCP-1), α-smooth muscle protein (α-SMA), and nuclear factor-KB (NF-κB) in obstructive nephropathy rats, and to explore the initial mechanism of SD for inhibiting renal interstitial fibrosis.
METHODSTotally 48 healthy Wistar rats were randomly divided into the sham-operation group (n =12) and the model group (n =36). Renal interstitial fibrosis rat model was established by unilateral ureteral obstruction (UUO). After successful modeling, 36 rats were randomly divided into the model group, the Chinese medicine group, and the Western medicine group, 12 in each group. Eplerenone was added in the forage at the daily dose of 100 mg/kg for rats in the Western medicine group. Chinese medicine was added in the forage at the daily dose of 26 g/kg for rats in the Chinese medicine group. Equal volume of normal saline was administered to rats in the sham-operation group and the model group. All medication was performed once daily. The obstructive kidneys were extracted ten days after medication. The pathomorphological changes were observed. The contents of serum aldosterone and MCP-1, and the protein or mRNA expression of MCP-1, α-SMA, and NF-KB were detected.
RESULTSCompared with the sham-operation group, infiltration of a large amount of inflammatory cells and collagen deposition significantly increased, serum contents of aldosterone and MCP-1 obviously increased (P < 0.01), the expression of MCP-1 mRNA and protein were significantly up-regulated (P <0.01), the protein expression of α-SMA and NF-KB were significantly enhanced in the model group (P <0.01). Com- pared with the model group, infiltration of inflammatory cells and renal collagen deposition were attenua- ted in the Chinese medicine group and the Western medicine group, the serum MCP-1 level were reduced, and the mRNA and protein expression of MCP-1 were significantly down-regulated (P <0.01), the protein expression of α-SMA and NF-KB were obviously inhibited (P <0. 01). At the same time, serum aldosterone level was reduced in the Chinese medicine group (P <0.01).
CONCLUSIONSinflammatory lesions of the renal tissue could promote the progress of interstitial fibrosis in rats with obstructive nephropathy. SD could attenuate interstitial fibrosis through reducing serum contents of aldosterone and MCP-1, down-regulating MCP-1/ NF-KB, and inhibiting the expression of α-SMA.

According to the real situation of graduates, sectional, individualized, and true practice of clinical cultivation education was conducted, which could make graduates get into the cultivation education mode of "early clinical practice, more clinical practice, true clinical practice". This could effectively enhance the cultivation effects and clinical profession post competence of integrated traditional Chinese and Western medicine graduates.
8.Sexual behavior characteristics and influencing factors of gonorrhea patients in Yunnan Province

Objective To investigate the sexual behavior types, condom use and influencing factors of gonorrhea patients in Yunnan Province, and to provide evidence for the adjustment of sexually transmitted disease (STD) prevention and control strategy. Methods A cross-sectional study was carried out to investigate gonorrhea patients in 14 STD clinics in 7 more prevalent prefectures (cities) of Yunnan Province. A questionnaire survey was conducted to investigate the socio-demographic and sexual characteristics of the patients in a one-to-one way. Multiple Logistic regression model was used to analyze the influencing factors of sexual behavior. Results A total of 179 cases of gonorrhea were investigated. The average age was (29.01±8.93) years old. 95.53% (171/179) patients were 40 years old and 88.27% were male patients (158/179). Unmarried patients accounted for 55.31% (99/179). Service and self-employed patients accounted for 34.64% (62/179) and 23.46% (42/179) respectively. The rate of sexual intercourse with opposite sex was 98.32% (176/179), the rate of men who have sex with men was 1.68% (3/179) and the rate of having more than two sexual partners was 27.93% (50/179). The rate of sexual intercourse between temporary partners, spouses/fixed partners and commercial partners were 53.07% (95/179), 37.99% (68/179) and 8.94% (16/179) respectively. Recent sexual activity had lower condom use rate, 17.89% (17/95), 19.12% (13/68) and 18.75% (3/16) respectively. The main ways for gonorrhea patients to make temporary partners were friend introductions or gatherings, accounting for 66.32% (63/95). Condom use in male patients was worse than that in female patients (OR=0.234, 95% CI: 0.084-0.656, P=0.006). Conclusions The patients with gonorrhea were mainly unmarried young adults. The main risk behaviors were unprotected behaviors between heterosexual temporary and spouse/fixed sexual partners. The condom use consciousness was generally low. The promotion of safe sex education for young adults should be further strengthened, with special attention to the use of condoms for temporary sexual behavior and spouse/fixed sexual intercourse.
9.Significance of dual-time-point 18F-FDG PET imaging in evaluation of hilar and mediastinal lymph node metastasis in non-small-cell lung cancer.

OBJECTIVETo explore the diagnostic value of dual-time-point 18F-FDG PET-CT imaging in detecting hilar and mediastinal lymph node metastasis in non-small-cell lung cancer (NSCLC).
METHODSForty-six patients with NSCLC underwent standard whole body single-time 18F-FDG PET-CT scans and a delayed imaging for the thorax alone before surgery, meanwhile, the standard uptake value (SUV) and retention index (RI) were calculated.
RESULTSA total number of 584 lymph nodes were excised in the 46 patients. Of these, 134 metastatic lymph nodes were pathologically confirmed in 31 patients. There were 189 lymph nodes detected and suspected to be metastatic by standard single-time 18 F-FDG PET-CT imaging, and 161 by dual-time-point imaging. Therefore, the sensitivity, specificity, diagnostic accuracy, positive predictive value and negative predictive value in the detection of hilar and mediastinal lymph node metastasis were 87.3%, 84.0%, 84.8%, 61.9% and 95.7% by standard single-time 18F-FDG PET-CT imaging, versus 94.8%, 92.2%, 92.8%, 78.9% and 98.1%, respectively, by dual-time-point imaging. There was a statistically significant difference in the detection of lymph node metastasis between the standard single-time imaging and dual-time-point 18F-FDG PET-CT imaging.
CONCLUSIONDual-time-point 18F-FDG PET-CT imaging is more sensitive, specific and accurate than standard single-time 18F-FDG PET-CT imaging in the detection of hilar and mediastinal lymph node metastasis, and may provide more information for diagnosis, staging and treatment of non-small cell lung cancer.

10.Living-related liver transplantation for cavernous transformation of portal vein: a clinical study of 3 cases.

OBJECTIVETo review the outcomes of living-related liver transplantation (LRLT) in treating 3 cases of cavernous transformation of portal vein (CTPV) with severe portal hypertension.
METHODSThree children (two boys and one girl) were presented to our hospital with recurring esophageal variceal bleeding, decompensating ascites, splenomegaly and refractory anemia. CTPV was confirmed by intravenous computed tomographic portography using a helical computed tomography scanner and 3-dimensional image reconstruction. LRLT were performed in these 3 patients from July 2006 to January 2007. The evaluation of the outcomes was made by referring to their clinical features and laboratory and imaging examination findings.
RESULTSAlthough one patient died from early graft thrombosis, the other two patients showed excellent prognoses. They lived and stayed well during a follow-up period of 12-14 months. Following the transplantations, there had been no esophageal variceal hemorrhage, the ascites disappeared and the portal hypertension vanished. Their hemoglobin, blood platelets count, and serum albumin reached normal values.
CONCLUSIONLRLT is an effective procedure in treating CTPV with severe portal hypertension. The reconstruction of the portal vein is the difficult part of the LRLT procedure.
