Journal of Peking University(Health Sciences) 2016;48(3):418-423
Objective:To understand the relationship between child malnutrition and social determi-nants among children under the age of 5 in China, and to provide evidence and useful information to help policy makers develop social policies to improve child nutritional status. Methods:Information of 2 434 children aged 0 -5 was extracted from year 1 991 to 201 1 longitudinal survey data in the China Health and Nutrition Survey (CHNS)was extracted for analysis.Child underweight, child stunting, and child wasting were defined using World Health Organization Child Growth Standards for weight-for-age,height-for-age,and weight-for-height.Weight-for-age values,height-for-age values or weight-for-height values below 2 standard deviations were considered as underweight, stunting and wasting.World Health Organi-zation igrowup software was used to calculate the prevalence of child underweight, child stunting, and child wasting.Multivariate Logistic regression model was used to analyze the relationship between child malnutrition and social determinants (household income, parents’educational level, living regions, and communities’urbanization level). Results:The prevalence of child underweight and child stunting were decreased by 64.8% and 67.8%,respectively from 1 991 to 201 1 ,while the prevalence of child wasting had remained at a relatively low level (below 5%).The problem of child underweight and stunting had been significantly resolved in China.Female children had better outcomes than male children on impro-ving nutritional status.Among all the non-socio-economic determinants of child malnutrition, children with low height mother and children had inadequate protein intake were both risk factors of malnutrition. The social determinants significantly associated to child malnutrition included:living in the western re-gions and central regions, living in low level urbanization communities, with low household incomes, and low maternal educational levels. Conclusion:In order to further decrease the prevalence of child malnu-trition and alleviate the inequity of child health,we should pay more attention to the social determinants behind child malnutrition.Besides, social policies beneficial to child nutrition promotion need to be taken urgently, the important social policies suggested by the researchers included focusing on maternal and child dietary intake, improving household economic situation, improving maternal educational level, and balancing economic development and resource distribution between different regions in China.

Organic anion transporting polypeptides (OATP), a member of solute carrier (SLC) superfamily, is considered as an important transmembrane uptake transporters. OATP is involved in the transport of a variety of endo- and xenobiotics (bile acids, bilirubin, prostaglandin, thyroid hormones, steroid hormone conjugates), drugs and toxins in a Na+ and ATP independent manner. Multiple factors (eg. hormones, proinflammatory cytokines, drugs) can affect the distribution, expression and activity of OATPs, leading to an altered accumulation of OATP substrates and related food-drug and drug-drug interactions. Changes in the distribution and expression of OATPs in malignant tissues may be related to the pathological process of cancer, while the modulation epigenetic mechanism also contributes to its distribution patterns. This review describes the factors that can affect the expression or function of OATPs, which may provide a valuable reference for drug development and the clarification of pathogenesis.

Objective:To investigate the effects of thalidomide on vascular endothelial growth factor(VEGF) and quality of life in patients with primary hepatocellular carcinoma after transcatheter arterial chemoembotization. Methods:A total of 60 patients with HCC,who were treated with transcatheter arterial chemoembolization (TACE) in Affiliated Hospital of Chengde Medical University from January 2014 to January 2016,were randomly divided into group A (n=30)and group B (n=30).Group A was treated with Thalidomide in combination with TACE,oral 200 mg every night for 1 month;group B was treated with TACE alone. The levels of VEGF in the two groups were detected before treatment and 3 weeks after treatment;the quality of life of patients before treatment and 1 month after treatment was evaluated by quality of life scale (QOL);the weight of two groups before and after treatment and adverse reactions during treatment were compared. Results:There was no significant difference in VEGF levels between the two groups before treatment (P>0.05).After treatment, the levels of VEGF in the two groups were higher than those before treatment, and the VEGF levels in the group A was significantly lower than that in the group B,the difference was statistically significant (P<0.01).There was no significant difference in QOL score and body weight between the two groups before treatment (P>0.05).The QOL scores of the two groups after treatment were lower than those before treatment, but the QOL scores of patients in group A were significantly higher than those in group B (P<0.05).There was no significant difference in weight between the two groups before and after treatment (P>0.05).In group A,1 patients (3.33%) had severe somnolence,2 (6.67%) patients stopped taking thalidomide due to severe dry mouth, and the remaining 27 patients were able to tolerate. Conclusion:Thalidomide combined with TACE in the treatment of patients with HCC can reduce the levels of VEGF and improve the quality of life of cancer patients, which is worthy to be popularized in clinic.

Objective To explore the effects of borderline personality disorder (BPD) tendency and impulsivity on suicide ideation in counseling patients. Methods A total of 120 counseling patients were evaluated by the Personality Diagnostic Questionnaire (PDQ-4+),Barratt Impulsiveness Scale (BIS-11).All participants were also interviewed with self-made structured clinical questionnaire for suicide ideation.Re sults ①Patients with suicide ideation had significantly higher score of BPD tendency ((6.02 ± 2.15) vs (3.91±2.31),t=5.164,P<0.01),impulsivity ((72.05±8.58) vs (68.63±9.01),t=2.129,P=0.035) than patients without suicide ideation. There was still significant difference of BPD tendency between the two groups after controlling for gender(F=23.135,P>0.01) but there was no significant difference of impulsivity (F=3.536,P>0.05).②Logistic hierarchical regression analysis revealed that only BPD tendency (B=0.319;P=0.009;OR=1.375)had a significant role in the prediction of suicide ideation after controlling for gender and depressive diagnoses. Conclusion Compared with impulsivity,BPD tendency can better predict suicide ideation of counseling patients.

Objective To elucidate the effects of Ginkgo biloba extract on advanced glycation end products-induced TGF-?_1,CTGF mRNA expression and oxidative stress in cultured NRK-49F cells. Methods From Jan.to Jul.2005,NRK-49F cells were incubated with serum-free medium with or without 25 mmol/L glucose as control.Cells were incubated with medium containing AGEs-BSA with or without 25 mmol/L glucose for 24 h.Cells were pretreated with EGb(50,100mg?mL -1)for 24 h to evaluate effects of EGb.And then intracellular ROS generation was measured by flowcytometry, expression of TGF-?_1 and CTGF mRNA were performed by a semi-quantitative RT-PCR technique. Results Compared with control,AGEs significantly increased the expression of TGF-?_1 and CTGF mRNA and intercellular ROS generation in NRK-49F cells.Incubation with both AGEs and high glucose(25 mmol/L)can upregulate TGF-?_1 and CTGF mRNA expression and intercellular ROS generation, compared with AGEs and high glucose(25 mmol/L)respectively.Preincubation with EGb attenuated AGEs-induced TGF-?_1 and CTGF mRNA overexpression and intercellular ROS generation. Conclusion Attenuation of AGEs-induced TGF-?_1 and CTGF mRNA overexpression in renal fibroblasts through scavenging of free radical and inhibition of oxidative stress may be one of mechanisms underlying EGb in prevention and therapy of DN.
