Objective To analyze effect of three level nursing quality control management model on the quality of the nursing service in the department of general surgery.Methods Analysis in the implementation of nurs-ing quality control before and after nursing quality and the improvement of the examination results of nurses situation in general surgery department of our hospital were compared between the 2012 -2013 without the implementation of three level quality control mode and 2013 -2014 of establishment of the three level nursing quality control manage-ment mode,which was set the nursing quality monitoring nurses,quality monitoring group leader and quality monito-ring nurses in three level nursing quality control system.Results After the implementation of quality control,the quality of basic nursing score[(94.02 ±2.01)points],ward management quality score[(94.13 ±3.04)points],first aid management quality score[(96.22 ±4.06)points]and disinfection and isolation,and the quality score[(95.13 ± 3.04)points]and quality control of basic nursing quality score[(8.121 ±3.31 )points],ward management quality score[(81.35 ±2.34)points],emergency management quality score [(91.42 ±4.36 )points]and disinfection [(81.74 ±3.44)points]degrees and isolation quality scores were significantly higher than those of before three level quality control nursing management model(t =33.07,33.32,8.06,29.17,all P <0.05).Nursing quality control of nursing personnel the technical operation of the average score [(96.35 ±1.03)points],theory test average score [(95.73 ±0.98)points],average and quality control of nursing personnel and technical operation [(89.42 ± 2.49)points],theory test average score[(90.36 ±1.79)points]were obviously higher.The results had statistical significance (t =25.72,26.32,all P <0.05).Conclusion Three level nursing quality control management model established in the general surgery department had remarkable effect on the quality of the nursing service,which is safe and reliable,and improving the quality of nursing.

Objective To investigate the effect of continuous quality improvement nursing on grasping disease knowledge and satisfaction of patients with thyroid tumors.Methods 86 cases of patients with thyroid cancer were enrolled.They were devided into the observation group(42 cases)and the control group(46 cases)according to the date of admission.Patients in control group were treated with routine care,while patients in the observation group were treated with continuous quality improvement nursing on the basis of the control group.The effects of nursing in the two groups were compared and analysized.Results After intervention,the scores of cognitive thyroid tumors,daily preventive health care,thyroid cancer surgery and surgical considerations in the observation group were higher than those in the control group,theses differences were statistically significant between the two groups (t =9.806,10.271, 9.924,12.053,all P <0.05).The rate of satisfaction in the observation group was significantly higther than that in the control group(92.86%/75.00%),theses difference was statistically significant between the two groups (χ2 =9.025,P <0.05).Conclusion Continuous quality improvement nursing can effectively improve the diseases -related knowledge in patients with thyroid benign tumor,which contributes to the smooth operation and improves surgical results.

Objective To explore the value of preoperative diagnosis for breast cancer patients in different locating ways. Methods The tissues were gotten by core needle biopsy from 146 breast cancer patients through different locating ways,and were taken for histopathological examinations and were compared with postoperative pathologic results. Results Seventy-four cases were biopsied with ultrasound-guide (ultrasound-guide group), and 72 cases with free-hand (free-hand group), without serious complications.Compared with postoperative pathologic results,the false negative rate in ultrasound-guide group was 5.41%(4/74);the false negative rate in free-hand group was 18.06% (13/72). The accuracy of the ultrasound-guided biopsy was higher than that in free-hand (U = 13.63,P ＜ 0.01 ). Conclusions Preoperative ultrasoundguided biopsy and postoperative pathologic examination has no significant difference, with high consistency.The study provides a good basis for selection for clinical work,so as to the more effective guide for the comprehensive treatment of breast cancer patients.

Objective To evaluate the effects of endogenous NO on the chemosensitivity of human breast cancer cell. Methods MCF-7 cells were cultured as monolayer, incubated with cytokine IL-1β. The pro-duction of NO was detected by NO assay. The expression of iNOS protein was measured by Western blotting. Establishing control group and experimental group, the chemosensitivity of MCF-7 cells incubated by L-NMMA and L-Arg to ADM and 5-Fu was studied by MTT assay. Results There was a positive correlation of dose-dependence between NO production and IL-1β concentration. MCF-7 cells expressed plenty of iNOS by induetion of IL-1β. There was no significant difference on iNOS whether L-NMMA and L-Arg existed or not. Incubating MCF-7 cells with 0. 5 μmol/L or 1 μmol/L ADM, the survival rate of experiment group was remarkablely decreased(P < 0.05) ; L-NMMA significantly increased survival rate of experiment group(P < O. 05) ; L-Arg decreased survival rate of experiment group(P < 0.05). Conclusion The induction of IL-113 in MCF-7 cells can increase the production of endogenous NO, which increases MCF-7 cells' sensitivity to chemotherapy.

Obstructive sleep apnea (OSA)is a high incidence of potentially dangerous disease,characterized by intermittent hypoxia or hypercapnia.It is an independent risk factor for ischemic stroke.Currently a number of studies have confirmed OSA closely associated with oxidative stress.In this paper,the complex mechanisms of oxidative stress in the OSA and the occurrence of stroke will be reviewed,such as promoting atherosclerosis,damaging the mitochondria,ischemia -reperfusion injury,ischemic preconditioning.To investigate the relationship between OSA,oxidative stress and stroke from molecular mechanisms.

ObjcetiveTo study the chimerism status ot bone marrow mesenchymal stem cells (BMSC) following double-unit cord blood stem cell transplantation (DCBT).MethodsBone marrow mononuclear cells were isolated by ficoll density gradient centrifugation from the patient following DCBT and healthy donors.After cultured for about 20 days,BMSC surface makers,the ability to differentiate into adipocytes and osteoblast and the expression of hematopoietic and immune molecules were detected,moreover,chimerism status of BMSC were also evaluated by STR-PCR.ResultsMSC derived from the patient were similar to normal adult BMSC in morphology,surface and differentiation ability.Besides,MSC from the patient can express a variety of hematopoietic and immune molecules normally.The results of STR-PCR showed that the patient demonstrated complete donor chimerism,hematopoiesis in BM and PB were from a single unit cord blood (donor chimerism was 96.4％ and 95.7％,respectively).But the predominant cord blood chimerism of BMSC was 5.4％,the remaining part was from the patient himself.ConclusionSustained hematopoiesis was derived from a single unit cord blood following DCBT.The majority of BMSC were origin from host tissue,mixed chimerism donor part of MSC stemed from the engrafted unit of cord blood.

ObjectiveTo explore the procedures and methods for genetic diagnosis in one nonsyndromic variants of congenital neutropenia (NSVCN) patient and its pathogenic mutation.Methods Genomic DNA was prepared from one NSVCN patient who had progressed to chronic myelomonocytic leukemia and ELA2,HAX1,WASp and GFI1 genes were amplified and sequenced.Results A novel compound heterogeneous mutation consisting of two frame-shift mutations (c.430-1insG and c.655- 9del5bp) was found in HAX1 gene.ConclusionA practically genetic diagnosis procedure for NSVCN has been established,and the novel HAX1 gene mutation may contribute to the etiology of NSVCN.

OBJECTIVETo compare the antitumor effects of the podophyllotoxin nanoliposome and the suspensions of podophyllotoxin in bearing-tumor mice.

METHODThe experimental mice were inoculated by suspension liquid of tumor cell in axillary region near the forelimb, 0.2 mL each. The mice had been randomly divided into 8 groups 24 h latter. The podophyllotoxin nanoliposome, suspension liquid of the podophyllotoxin, CX or NS was given to each group respectively. Except CX was given by celiac injection once, the other groups were injected every 4 days for three times. The mice were killed by hauling necks on the twelfth day after treatment, the tumor was weighed and the inhibitory rate was calculated.

RESULTWhen the dosage of podophyllotoxin nanoliposome and the suspensions of podophyllotoxin reached 5.0 mg x kg(-1), the rate of inhibiting tumor were 52.37% and 38.25% to the H22 liver cancer of mice respectively.

CONCLUSIONPodophyllotoxin nanoliposome and the suspensions of podophyllotoxin have the effect in anti-liver cancer. And podophyllotoxin nanoliposome have stronger inhibitory rate compared with suspension liquid of the podophyllotoxin in same dose.

AIM: To establish a method for simultaneously determining chlorogenic acid,puerarin and paeoniflorin in Kangganning Mixture(Flos Lonicerae japonicae,Radix Puerariae Lobatae,Radix Paeoniale rubra,Fructus Forsythiae,etc.). METHODS: A multiple wavelength HPLC method was devoloped.The analysis was performed on an Agilent Zorbax SB C_(18) column(250 mm?4.6 mm,5 ?m) with acetonitrile-0.1%H_3PO_4(11∶89) as the mobile phase.The detection wavelengthes were monitored at 324 nm,254 nm and 230 nm for chlorogenic acid,puerarin and paeoniflorin,respectively.The flow rate was 1.0 mL/min and the column temperature was at 30 ℃.(RESULTS:) The calibration curves of chlorogenic acid,puerarin and paeoniflorin showed good linearity at the ranges of 0.179-2.864 ?g,0.071 55-1.144 8 ?g and 0.372 5-5.96 ?g,respectively.The average recoveries were(100.7%,) 103.3%,102.6% with RSD of 2.2%,2.4%,1.9%,respectively. CONCLUSION: The method is simple,accurate and reproducible for determining chlorogenic acid,puerarin and paeoniflorin in Kangganning Mixture.
