3.Malignant proliferating trichilemmal cyst: a case report with review of literature.

Proliferating trichilemmal cyst is a rapidly growing large cutaneous adnexal neoplasm occurring on the head and neck region of elderly women. Malignant transformation has rarely been reported in these lesions. We describe here a 85-year-old lady who presented with a large ulcerated growth over the scalp for one year duration. Incisional biopsy revealed proliferating trichilemmal cyst with malignant transformation. She underwent wide local excision of this growth. She is alive and without evidence of disease after 14 months of follow up. Because of limited number of cases reported in literature, management of malignant proliferating trichilemmal cyst is controversial. Treatment mainly entails wide local surgical excision. Many other adjuvant modalities have been tried. This paper presents the diagnosis and management of one case of malignant proliferating trichilemmal cyst followed by review of the literature.

4.Review of Surgical Cases of Colorectal Cancer

During the period of 25 years from January 1978 through December 2002, a total of 808 colorectal cancer cases were operated on in Shirakawa Kosei General Hospital. The number of sugical resection cases came to 713 (ratio : 88.25%). Curative resection was performed on 593 cases (ratio : 73.4%). The total number of cases was broken down into 446 cases of cancer of the colon and 369 cases of cancer of the rectum (7 multiple cancer cases included). By sex, male cases numbered 329 and female cases 379. Clinically or histologically, many cases were diagnosed as stage IIIa or stage II cancer. The 5-year-survival rate for the resection cases was calculated at 67.2% and that for the cure resection cases, at 79.5%.

5.Carcinosarcoma of the Urinary Bladder with Rapid Growth: A Case Report

Carcinosarcoma of the bladder is a rare malignancy characterized by an intimate admixture of malignant epithelial elements (carcinoma) and malignant soft tissue elements (sarcoma). Patients with carcinosarcoma usually present with high-stage malignancy. Cystectomy or transurethral resection of the bladder tumor (TUR-Bt) is the preferred treatment, often followed by radiation therapy, but the prognosis is very poor1-7). To our knowledge, almost 80 cases have been reported in Japan, usually as case reports or small series6, 7). In almost all cases, the chief complaint was gross hematuria. We herein report a case of carcinosarcoma of the urinary bladder in a patient whose chief complaint was abdominal pain. His pain worsened, and the occurrence of hydronephrosis indicated that extremely rapid tumor growth may have occurred. No evidence of recurrence was noted 30 months after the performance of cystectomy.

6.Histone Deacetylase Inhibitors ---Promising Agents for \lqGene-Regulating Chemoprevention\rq and \lqMolecular-Targeting Prevention\rq of Cancer---

One of the best approaches against cancer is prevention. Inactivation of the p53 or p16INK4a genes has been extensively reported in most human cancer cells. Both p53 and p16INK4a function as tumor suppressors. Therefore, functional restoration of these molecules is considered to be one of the most useful methods for cancer prevention and therapy. We have proposed a concept termed ‘gene-regulating chemoprevention and chemotherapy’ regarding the above pathway. This concept assumes that transcriptional regulation by drugs on tumor-suppressor genes, downstream target genes or functionally similar genes (for example, family genes) of the tumor-suppressor genes would contribute to the prevention of human malignancies. Histone deacetylase (HDAC) inhibitors have been shown to be potent inducers of growth arrest, differentiation and apoptotic cell death. Previously, we demonstrated that HDAC inhibitors, such as sodium butyrate and trichostatin A (TSA), transcriptionally induce the cyclin-dependent kinase inhibitor p21WAF1/Cip1, a downstream target gene of p53, in a p53-independent manner. Furthermore, we have recently shown that HDAC inhibitors activate Gadd45, another downstream target gene of p53, and p19INK4d, a gene functionally similar to p16INK4a. Our results, taken together with previous findings, suggest that HDAC inhibitors may be one of the most attractive and promising agents for ‘gene-regulating chemoprevention’ and ‘molecular-targeting prevention’ of cancer.

8.Lack of Rb2/p130 genetic alteration in Malaysian nasopharyngeal carcinoma.

The retinoblastoma-related gene Rb2/p130 has been reported to be mutated in several malignancies such as lung cancer and Burkitt's lymphoma. Nasopharyngeal carcinoma (NPC) is a common cancer in Malaysia especially amongst the ethnic Chinese. We screened for Rb2/p130 gene (exons 19 to 21) mutations in 53 archival NPC samples via PCR-SSCP-direct sequencing approach. Only one sample had a base change which involved a serine to glycine substitution at codon 995 (S995G). We conclude that Rb2/p130 genetic alterations are infrequent in NPC and may not be essential for the pathogenesis of the disease.
