1.Effects of maximal running exercise on heart rate responses after standing up in adult men.

The change in R-R interval (RRI) induced by rapid postural change from a squat posture to standing was analyzed in 8 healthy male students (20.3±1.2 years of age) before and immediately after maximal running exercise. We instructed subjects to stand up as quickly as possible, and to repeat the standing-up movement three times at intervals of 2 min. Heart rate responses and heart rate variability were analyzed by the change in RRI induced by standing up. Heart rate (HR) increased quickly at the onset of standing up. The time (T) until the maximal HR (Hmax) was reached 9.79±1.44 s after standing up, and then the HR after Hmax decreased rapidly with time. The maximal HR was 1.20 times higher while standing up than in the squat position. Maximal running exercise significantly delayed the time taken to reach Hmax after standing up, and significantly diminished the increased HR to 1.15 times. The Hmax/Hmin ratio, which expressed the magnitude of autonomic activity during standing up, was significantly lower following maximal exercise, indicating that the cardiac sympathetic nervous system seems to be in a state of hyperfunction immediately after maximal running exercise. These findings suggest that disturbance in the postural adjustment of the cardiovascular system immediately after intense exercise may be induced by the delayed response and decreased amplitude of the HR.
2.General Survey of Balneal Treatment in the Tohoku Region

The results of our general and medical surveys are summarized as follows:
1) The visitors to this spa are inhabitants of Yamagata prefecture, farmers being the largest in number.
2) Considering the fact that more than half the number of spa visitors come here with therapeutic or convalescing purpose, this spa may be said to be a spa to which visitors come with the sole object of curative treatment.
3) Visitors came for the treatment of common cold, hypertension, neuralgia and diseases of the stomach and intestines respetively from the frequency of diseases. The fact that the visitors with cold were largest in number was probably because of the prevalence of common cold at the time of our survey.
4) Half the number of spa visitors stayed for about 2 weeks, and most of them took bath four to five times a day for curative treatment.
5) 40.1% spa visitors drank hot spring water for curative treatment. Which incidence is higher than that in our previous reports for other Tohoku Area.
6) Only 6.1% of the visitors came to the spa under doctor's direction. Those who noted the bathing reaction: anorexia, feeling of weakness etc., were 23.1%
7) In the tubeless gastric analysis (Gastrotest) scarcely any change was observable for successive drinking of hot spring water.
8) In most cases the oral temperature measured at the time of bathing in this spa never returned to the value before bathing for two hours and the feeling of warmth remained for a long time. This is probably due to the chemical properties of the spring waters and proper treatment after bathing.

7.Postoperative portal vein thrombosis and gastric hemorrhage associated with late-onset hemorrhage from the common hepatic artery after pancreaticoduodenectomy.

Portal vein thrombosis (PVT) is a rare but serious postoperative complication of pancreaticoduodenectomy (PD). We reported a case of late-onset postoperative PVT with hemorrhage from the common hepatic artery (CHA) in a 73-year-old man who underwent pylorus-preserving pancreaticoduodenectomy (PPPD) for duodenum papilla cancer, followed by reconstruction using the modified Child's technique. The pancreaticojejunostomy was achieved by end-to-side, 2-layer invagination anastomosis without pancreatic duct stenting. Drain removal and hospital discharge were scheduled on postoperative day (POD) 18, but blood-stained fluid in the drain and sudden hematemesis were noted. Emergency surgery was performed because PVT and imaging findings were suggestive of necrosis of the lifted jejunum. Although no jejunal necrosis was identified during surgery, bleeding from the side of the CHA was detected and the bleeding point was suture-closed to achieve hemostasis. We suspected late-onset postoperative arterial hemorrhage and subsequent hematoma formation, which caused portal vein compression and PVT formation. We chose a conservative treatment strategy for PVT, taking into account the operation time, intraoperative vital signs and blood flow in the portal vein. Despite the complicated postoperative course, he was discharged home in a fully ambulatory state on POD 167.

8.Home Care in the Era of COVID-19 —Results from the Bereaved Families of Terminal Cancer Patients Survey—

The purpose of this study was to examine the impact of COVID-19 on home care at the end of life and the satisfaction of bereaved families. A questionnaire survey was conducted on 100 bereaved families of terminal cancer patients who were receiving home care. The effects of the COVID-19 on at-home medical treatment and the rate of satisfaction of bereaved families were examined. The response rate for this survey was 72.0%. Of the respondents, 52.8% of the bereaved families answered that the COVID-19 had an effect on their decision to choose home care. The rate of satisfaction of bereaved families was 98.6%. Even for terminal cancer patients who chose home care in the era of COVID-19 at our hospital, we were able to achieve high level of satisfaction for bereaved families.
