1.Parental Psychological Aggression and Phubbing in Adolescents: A Moderated Mediation Model

Objective:
The present study aimed to examine the mediated moderation effect underlying the association between parental psychological aggression and phubbing, as well as the mediating role of anxiety and moderating role of sex and grade.
Methods:
Based on a cross-sectional study, a total of 758 Chinese junior high school students had completed measures on socio-demographic characteristics, parental psychological aggression, anxiety and phubbing. Structural equation modeling was adopted to examine the mediating effect of anxiety on the association between parental psychological aggression and phubbing. Multigroup analyses were conducted to explore whether the path coefficients differed by sex and grade.
Results:
Mediation analysis indicated that anxiety could mediate the association between parental psychological aggression and phubbing. The indirect effect of parental psychological aggression on phubbing via anxiety was 0.12. Multigroup analyses revealed that the higher mediation effect of anxiety was more likely to be reported by boys and students from grade eight.
Conclusion
Findings of the present study may inform prevention and intervention programs for phubbing in adolescents exposed to parental psychological aggression, by decreasing the anxiety and adopting selective strategies for different sex and grade groups.
2.Microanatomy and functional MRI study of arcuate fasciculus and superior longitudinal fasciculus

Objectives:To explore the microanatomy and functional MRI(fMRI) of arcuate fasciculus(AF) and superior longitudinal fasciculus(SLF),and to analyze their functions.Methods:Ten normal adult cadaveric head specimens (20 cerebral hemispheres) were fixed with 10% methanal at the Translational Research Institute for Neurological Disorders of the Wannan Medical College from February to December 2022.The Klingler fiber dissection technique was utilized to perform white matter fiber dissection,with a magnification ranging from 6 to 40.The study focused on the microanatomical structures of the AF and SLF,aiming to explore their relationships with deep brain fibers.Furthermore, six healthy adult volunteers who underwent fMRI of the brain were included.The collected diffusion tensor imaging (DTI) data were processed and integrated with the microanatomical findings for a comprehensive analysis.Results:After removing the gray matter of the cerebral cortex,the superficial U fibers were exposed.The long association fibers that beneath the U fibers were the AF and SLF,which were the main long association fibers in the superficial layers of the brain.The AF could be divided into dorsal and ventral parts,while the SLF could be divided into Ⅰ,Ⅱ,and Ⅲ.SLF Ⅰ lied within the upper bank of the cingulate sulcus,travels medial to the callosal sulcus.The SLF Ⅱ,Ⅲ,and the AF were located on the lateral surface of the brain.By removing the gray matter of the insular cortex and the extreme capsule,exposing the external capsule and claustrum.Subsequently,the AF and SLF Ⅱ,Ⅲ were dissected,revealing the corona radiata and sagittal stratum,along with other deep brain fibers.During the dissection,it was observed that there was a close connection between the AF,SLF Ⅱ,and the deep brain fibers.Furthermore,in the regions above the lateral fissure of the cerebral hemisphere,there was no direct connection of long association fibers between the gray matter cortex and the deep U fibers in the coronal plane.These findings were further supported by DTI studies.Conclusions:The AF and SLF are the major long association fibers that located in the superficial layers of the brain,and closely connect to the gray matter cortex and U fibers,even closely relate with deep brain fibers.In the regions above the lateral fissure of the hemisphere,only the AF and SLF Ⅱ and Ⅲ serve as superficial long association fibers in the anterior-posterior direction.These fibers are likely involved in the transmission of brain functional information between the top and bottom gray matter cortex in the coronal plane above the lateral fissure.

4.Label Analysis of FDA Approved Nasal-brain Delivery Related Preparations

Nasal preparation are preparations that directly delivery drugs on the nasal cavity and exert local or systemic effects. It has the clinical advantages of rapid absorption, high bioavailability, avoid the first-pass effect of liver, and better patient compliance, et al. For nasal-brain administration, nasal preparation can be administered directly to the central nervous system(CNS) via olfactory nerve or trigeminal nerve bypassing the blood-brain barrier, which providing a sager, more effective and convenient route for brain or CNS diseases. In this paper, the label information of Food and Drug Administration(FDA) approved nasal-brain delivery related nasal spray preparations was reviewed, the similarities and differences between these nasal preparations were analyzed in order to provide reference for the development of domestic nasal preparations.
5.Progress on obesity-induced hypothalamic GnRH neuron dysfunction

The declining fertility has emerged as a significant concern impacting national welfare, with obesity identified as a major contributing factor. In the hypothalamus, gonadotropin-releasing hormone(GnRH) neurons release GnRH, which regulates the secretion of estrogen and testosterone via the hypothalamic-pituitary-gonadal(HPG) axis, thereby influencing sexual development and fertility. The proper function of GnRH neurons is crucial for maintaining normal sexual development and fertility. Research indicates that obesity can impact GnRH neuron function through several pathways, such as disrupting pulsatile secretion core kisspeptin neurons, causing imbalance in energy sensing and metabolic factor secretion, and triggering hypothalamic inflammation. These pathways directly or indirectly influence GnRH neuron function in the hypothalamus, which ultimately affects sexual development and impairs fertility. Therefore, this review consolidates recent advancements in understanding the mechanisms by which obesity induces dysfunction in hypothalamic GnRH neurons, offering new perspectives for early clinical interventions aimed at enhancing fertility outcomes in obese individuals.
6.Practice and Effect of Outpatient and Emergency Prescription Pre-audit by Pharmacists with the Help of Rational Drug Use Software

OBJECTIVE:To explore the role of pharmacists i n developi ng the outpatient and emergency prescription pre-audit with the help of rational drug use software. METHODS :With the help of rational drug use software ,outpatient and emergency prescription pre-aduit was conducted during Jan.-Apr. 2020;the intervention effects of irrational prescriptions were analyzed. The prescriptions from Sept. to Dec. in 2019 were included in the control group (only for post review ),and the prescriptions from Jan. to Apr. in 2020 were included in the intervention group (pre-audit and post review );the unreasonable prescriptions were compared before and after the intervention. The prescriptions intercepted by the software system from Nov. to Dec. in 2019 were further selected as the interception control group ,and the prescriptions intercepted from Jan. to Feb. in 2020 were selected as the interception intervention group ;the interception situation of prescriptions before and after the intervention was compared ,and the influence of pre-audit on physicians ’prescription behavior was evaluated. In addition ,108 992 prescriptions in Apr. 2020 were selected for post review using the traditional model (i.e. complete manual audit )and the information model (i.e. software-assisted manual audit )respectively;the effects of two models were compared. RESULTS :From Jan. to Apr. in 2020,a total of 2 393 prescriptions were pre-audited by pharmacists ,among which 1 387 prescriptions were reasonable (57.96%)and 1 006 prescriptions were unreasonable and intervened (42.04%). After pharmacist intervention ,983 prescriptions were modified by doctors ,and the success rate of intervention was 97.71% . Compared with control group ,unreasonable rate of outpatient and emergency prescriptions,that of each type of prescriptions were all decreased significantly (P<0.001). Compared with interception control group(1 402 pieces),the number of intercepted prescriptions (721 pieces)was significantly reduced in interception intervention group (P<0.001). Compared with traditional model , the number of prescriptions requiring manual post review and E-mail:xss306@126.com “false positive ” prescriptions were decreased significantly # under information model , and the number of discovered unreasonable prescriptions were increased significantly (P< 0.001). CONCLUSIONS :With the help of rational drug use software ,prescription pre-audit by pharmacists can significantly improve reasonable rate of outpatient and emergency prescriptions. The work intensity of pharmacists in post review of prescriptions can be reduced and the accuracy of prescription review can be improved in the information mode.
7.Risk factors of recurrence and canceration for premalignant vocal fold lesions after surgery.

Objective:To analyze the risk factors of recurrence and canceration for premalignant vocal fold lesions after surgery, and to provide a reasonable basis for preoperative evaluation and postoperative follow-up. Methods:This study retrospective analyzed the relationship between clinicopathological factors and clinical outcome(recurrence, canceration, recurrence-free survival, and canceration-free survival) in 148 patients undergoing surgical treatment in Chongqing General Hospital from 2014 to 2017. Results:The five-year overall recurrence rate was 14.86% and the overall recurrence rate was 8.78%. Univariate analysis showed that smoking index, laryngopharyngeal reflux and lesion range were significantly associated with recurrence(P<0.05), and smoking index and lesion range were significantly associated with canceration(P<0.05). Multivariate logistic regression analysis showed that smoking index ≥600 and laryngopharyngeal reflux were independent risk factors for recurrence(P<0.05), and smoking index ≥600 and lesion range ≥1/2 vocal cord were independent risk factors for canceration(P<0.05). The mean carcinogenesis interval for the postoperative smoking cessation group was significantly longer(P<0.05). Conclusion:Excessive smoking, laryngopharyngeal reflux and a wide range of lesions may be related to postoperative recurrence or malignant progression of precancerous lesions in the vocal cord, and further large-scale multi-center prospective randomized controlled studies are needed to clarify the effects of the above factors on recurrence and malignant changes in the future.

8.Case Analysis and Literature Review of a Case of Acarbose-induced Skin ADR by Clinical Pharmacists

OBJECTIVE:To summarize and analyze t he clinical characteristics of acarbose-induced skin ADR ,and to provide reference for its therapy. METHODS :Clinical pharmacists participated in the treatment of a patient with acarbose-induced skin ADR. The patient developed erythema multiforme several days after oral administration of Acarbose tablets (100 mg/d). After consultation by dermatology and clinical pharmacy ,considering that the adverse reaction was related to acarbose ,clinical pharmacists suggested to stop the drug. Based on the above cases ,clinical pharmacists searched Wanfang database ,CNKI, PubMed,Embase and other databases to collect case reports of skin ADR caused by acarbose ,summarize its general situation (gender,age,usage and dosage ,etc.),latency,ADR(diagnosis and manifestation ),intervention and outcome ,etc. RESULTS : The doctor adopted the pharmacist s’advice,stopped the use of acarbose ,and gave symptomatic treatment as Methylprednisolone sodium succinate for injection 40 mg(intravenous injection ,qd)+Medloratadine tablets 8.8 mg(oral administration ,qd)+Calamine lotion(for external use ). The patient improved and was discharged after 10 days. A total of 12 literatures involving 12 patients were retrieved. Among the 13 patients included in the analysis (including the above clinical case and 12 literature cases ),there were 8 males and 5 females,and 8 patients of them aged 50 and over;the dosage of acarb ose in most patients was within the requirements of the drug instructions. The primary diseases of 12 patients were diabetes mellitus. The latency of skin ADR in 11 patients was within 6 days of administration. Among the 13 patients,the ADR were diagnosed as rash in 4 cases,pustulosis in 3 cases, erythema multiforme in 2 cases, urticaria in 2 cases, maculopapular rash in 1 case and lip swelling in 1 case. The ADR of 1 patient improved after drug withdrawal ,and 12 patients also improved after drug withdrawal and symptomatic treatment such as glucocorticoid or antihistamine. Acarbose was re-used in 2 patients after the improvement of first skin ADR ,and skin ADR occurred again ,and the ADR were improved after drug withdrawal and symptomatic treatment. CONCLUSIONS :Skin ADR are acarbose-induced rare ADR ,mostly within 6 days of medication ,and are more likely to occur in middle-aged and older men. When the patients suffer from ADR ,the drug should be stopped in time and given glucocorticoids or antihistamines for symptomatic treatment. Clinical pharmacists should do a good job in drug publicity and education ,remind patients to closely monitor relevant indicators and ensure drug safety.
