1.Experience of donation after citizens death for lung transplant: report of 19 cases

Objective To summarize the experience of harvesting and using the lungs from donation after citizens death.Method From November 2007 to December 2013,19 cases of potential donation after brain death (DBD) and donation after cardiac death (DCD) were evaluated,including 9 cases of DCD and 10 cases of DBD.All the patients received the tests of sputum culture,bedside bronchoscopes,chest X rays,and blood gas analysis.After clear evaluation,3 cases of DCD and 3 cases of DBD were discharged from the group for bilateral inflammatory infiltration and poor oxygenation index,and the rest one case of DCD was precluded due to long warm ischemic time (>60 min).The donor lungs from remaining 12 cases were harvested successfully after the declaration of brain death or cardiac death.The donors suitable for the transplant procedure were transported to our transplant center.Result Twelve lung transplants were performed successfully,including 10 cases of bilateral lung transplantation and 2 cases of right single lung transplantation.Two patients was complicated with severe infection and died of sepsis postoperatively,and the remaining 10 patients all recovered uneventfully with dramatic improvement of pulmonary function.During the follow-up period,all the patients lived an active life style with high quality of life.The mean survival time was 34.7 months (4-60 months).Conclusion Lung transplantation using DCD and DBD can be successfully performed after adequate preoperative evaluation of donor lung and abundant preparation for donor harvesting.
2.Effect of application of uridine diphosphate-glucose on self-repairment potentiality of immature cerebral white matter invivo

Objectives To explore the effect of application of uridine diphosphate-glucose (UDP-glucose) on self-repairment potentiality of immature white matter (WM) in vivo. Methods Five-day-old rats were randomly divided into sham, periventricular leukomalacia (PVL) and UDP-glucose groups. The PVL model was constructed in the PVL and UDP groups, and UDP-glucose (2000mg/kg) was induced by an intraperitoneal injection at once to the rats of UDP group. PVL in-duced proliferation and differentiation of WM-glial progenitor cells invivo were detected by using the three-label lfuorescent immunoanalysis, the apopotosis in WM cell was observed by TUNEL test, and the pathology of WM and myelination were evaluated by light and electron microscopy at day 7 and day 21 after PVL model construction. Results The numbers of new WM-progenitors (NG2+), oligodendrocytes (OLs) progenitor marker (O4+), OL precursors, cyclic nucleotide phosphodiesterase (CNPase+), immature OLs and myelin basic protein (MBP+), and mature OLs in the UDP-glucose group are signiifcantly grea-ter than those in the PVL group at each time interval after induction of PVL (P<0.05). The numbers of the apoptotic cells in UDP-glucose group are less than those in the PVL groups. Under light and electron microscopy, the white matter pathological changes and myelination were found to be better than those in the PVL group (P<0.05). Conclusion The application of UDP-glucose can induce the WM-progenitors to activate, proliferate and differentiate into immature and mature OLs. UDP-glucose can also signiifcantly reduce the apoptotic rate of the WM-new glia cells;improve the white matter pathological changes and the myelin formation.
3.Pharmacological study of Zhiyu prescription on diabetic peripheral neuritis

Objective: Zhiyu presciption is composed of Whitmania pigra (Whitman) and Cornus officinalis Sieb. et Zucc. etc. It treats mainly for diabetic peripheral neuritis. To study the pharmacologic effects of Zhiyu prescription in this paper. Methods:The analgesic effect of Zhiyu presciption was examined through writhe method in mice, and the functions of Zhiyu prescription on extrinsic thrombosia and platelet aggregation in normal rats were observed. The model of alloxan induced diabetes in mice was established, the effect of Zhiyu prescription on blood glucose level was detected. The functions of nervus electrophysiology were examined on the model rats of diabetic peripheral neuritis induced by streptozotocin. Results: Zhiyu prescription could decrease the numbers of writhes of mice caused by acetic acid, reduce the blood glucose level in alloxan induced diabetic mice, improve the nervus conduction velocity of motor nerve.Conclusion: Zhiyu prescription is effective in clinical treatment of diabetic peripheral neuritis.
4.Effect of Guilong Kechuanning Tablets Combine with Antibiotic Drugs on Pulmonary Infection after Chemotherapy in Patients with Lung Cancer

Objective:To investigate the effect ofGuilong Kechuanning tablets combine with antibiotic drugs on pulmonary infection after chemotherapy in patients with lung cancer,so as to reduce the incidence of pulmonary infection in hospital.Methods:A total of 120 patients with lung cancer complicated with pulmonary infection,who underwent chemotherapy in Baoji Hospital of Traditional Chinese Medicine from August 2013 to August 2015,were selected and randomly divided into control group (n=60) and experiment group (n=60).The control group was treated with Ceftizoxime,based on which,the experiment group was added oral Guilong Kechuanning tablets.The course oftreatment of the two groups was 2 weeks.Inflammation indicators of the two groups were compared before and after treatment,and the clinical effect,hospitalized time,antifebrile time,time significantly reduced of cough and expectoration,disappearance time of lung rales and the pulmonary bacterial clearance rate were observed between the two groups.Results:The total effective rate (91.67%) of experiment group was significantly higher than that(73.33%) of control group (x2=13.121,P=0.004).Before treatment,there were no significant differences in WBC,CRP and NEUT levels between the two groups (P>0.05);while after treatment,the levels of WBC,CRP and NEUT were all lower than those before treatment in both groups (all P<0.05),and the levels of WBC,CRP and NEUT in the experiment group were lower than those in the control group(P<0.05).After treatment,the bacterial clearance rate (99.09%) of the experiment group was significantly higher than that (86.94%) of the control group (x2=54.876,P=0.000).Conclusion:Guilong Kechuanning tablets combined with antibiotic drugs can effectively control the pulmonary infection after chemotherapy in the patients with lung cancer,and can improve the clinical symptoms and inflammatory reaction,which is worthy of clinical application.
5.Mediating effect of parental hostile attribution bias on depression and anxiety between parents and children

Objective:
To explore the mediating effect of parental hostile attribution bias on depression and anxiety between parents and children, so as to provide the reference for the promotion of children's mental health.
Methods:
Students of grades 2-6 in two public primary schools in Linping District, Hangzhou City and their parents were investigated using stratified sampling method in November 2022. Emotions of children were surveyed using Depression Self-Rating Scale for Child (DSRSC) and Screen for Child Anxiety Related Emotional Disorders (SCARED), while emotions and hostile attribution bias of parents were surveyed using Self-rating Depression Scale (SDS), Self-rating Anxiety Scale (SAS) and Social Cognitive Screening Questionnaire (SCSQ). Mediating effect of parental hostile attribution bias on depression and anxiety between parents and children was analyzed using a structural equation model.
Results:
Totally 300 questionnaires were allocated, and 263 valid questionnaires were recovered, with an effective rate of 87.67%. There were 137 boys (52.09%), and 126 girls (47.91%), with a mean age of (9.95±1.44) years. There were 69 fathers and 194 mothers investigated. The prevalence of depression among parents was 27.00%, the prevalence of anxiety among parents was 4.18%, and the median score of hostile attribution bias was 1.00 (interquartile range, 2.00). The prevalence of depression among children was 11.03%, and the prevalence of anxiety among children was 29.66%. Parents' depression and anxiety affected children's depression and anxiety directly (effect value=0.270, 95%CI: 0.131-0.436), and also indirectly affected children's depression and anxiety by increasing their hostile attribution bias (effect value=0.028, 95%CI: 0.004-0.082), with the mediating effect contributed 9.40% of the total effect.
Conclusion
Parents' depression and anxiety affect children's depression and anxiety directly or indirectly through hostile attribution bias.
6.Sclerotherapy for faciocervical macrocystic lymphatic malformations in infants

Objective To investigate the therapeutic effect of Pingyangmycin combined with Triamcinolone Acetonide on the huge faciocervical lymphatic malformations (LMs) in infants.Methods Sixty-seven infants with LMs located in head and neck from January 2009 to June 2014 were retrospectively analyzed in Kunming Children's Hospital.Thirty-five males and 32 females were enrolled,aged from 1 month to 4 years,with a median age of 1.3 years.Computed tomography and ultrasonography were used to evaluate the location,size and extent of LMs before treatment in all the patients.The size of lesion varied from 5.2 cm ×7.5 cm to 9.2 cm × 10.5 cm.All patients were given local injection of Pingyangmycin combined with Triamcinolone Acetonide after puncturing fluid with uhrasonography guiding under general anesthesia.The injection was repeated every 30 d when necessary.Results The number of injections varied from 2 to 5 times,with a median number of 3.9 times.All cases were followed up for 5 to 36 months.Thirty-two cases (62.68%) were cured,improvement in 19 cases (28.36%) and no effect in 6 cases (8.96%).The total effective rate was 91.04%.There was no severe allergic reaction or pulmonary fibrosis.Secondary operation was performed after 6 months in 12 cases.Two post-operative complications were found,1 was minor paralyses of mandibular branch of facial nerve,with mouth askew,the other was trachyphonia,who were both improved after rehabilitation treatment.Conclusions In order to avoid serious complications,the huge LMs may be given local injection of Pingyangmycin combined with Triamcinolone Acetonide after puncturing fluid with ultrasonography guiding.Graded sclerotherapy provides for a less invasive and shorter course of treatment.The complications and risk of secondary resection increase slightly if sclerotherapy has no curative effect.
7.Impact on serum 5-HT and TH1/TH2 in patients of depressive disorder at acute stage treated with acupuncture and western medication.

OBJECTIVETo compare the difference in depression relief in the treatment of depressive disorder at the acute stage between the combined therapy of acupuncture and 5-HT (5-hydroxytryptamine) selective serotonini reuptake inhibitors (SSRIs) and the single application of SSRIs and explore the impact on the imbalance of 5-HT and TH1/TH2.
METHODSNinety cases of depressive disorder at the acute stage were randomized into a combined therapy group and a medication group, 45 cases in each one. In the medication group, SSRIs were prescribed forl oral administration, once or twice a day, continuously for 4 weeks. In the combined therapy group, on the basis of treatment as the medication group, acupuncture was combined. The main acupoints were Baihui (GV 20), Yintang (GV 29), Shenting (GV 24), Fengchi (GB 20), Dazhui (GV 14) and Sishencong (EX-HN 1), once every two. days, continuously for 4 weeks. Before treatment, and after the 1st, 2nd and 4th weeks of treatment, the Hamilton depression scale (HAMD) was used to evaluate the depression severity. Separately, before and after the 4 weeks of treatment, the levels of serum 5-HT, interleukin-1 beta (IL-1β), interleukin-6 (IL-6), interleukin-4 (IL-4) and interleukin-10 (IL-10) were determined and compared with those in 45 cases of the healthy group.
RESULTSHAMD score was reduced in the 1st, 2nd and 4th weeks of treatment as compared with that before treatment in the combined therapy group (all P<0 01). HAMD score was reduced in the 2nd and 4th weeks of treatment as compared with that before treatment in the medication group (all P<0. 01). HAMD scores in the combined therapy group were lower than those in the medication group in the 1st, 2nd and 4th weeks of treatment (all P< 0. 01). Before treatment, in the combined therapy group and the medication group, the levels of serum 5-HT, IL-4 and IL-10 were all lower than those in the healthy group (all P<0. 01); the levels of IL-1β and IL-6 were higher than those in the healthy group (all P<0. 01). In the combined therapy group and the medication group, the levels of 5-HT, IL-4 and IL-10 in 4 weeks of treatment were all increased as compared with those before treatment (all P<0. 01), and the levels of IL-1β and IL-6 were lower than those before treatment (all P<0. 01). In the combined therapy group, the levels of IL-1β and IL-6 in 4 weeks of treatment were lower than those in the medication group, and the levels of 5-HT, IL-4 and IL-10 were higher than those in the medication group (P<0. 01, P< 0. 05).
CONCLUSIONThe combined therapy of acupuncture and SSRIs achieves much quicker and more effective re-' sult for relieving depression in the patients of depressive disorder as compared with simple oral administration of' SSRIs, and much more contributes to adjust the imbalance of serum 5-HT and TH1/TH2.

8.Comparison of choroid thickness difference between myopia children corrected by spectacles and orthokeratology

Objective To investigate choroid thickness at horizontal meridian with optical coherence tomography (OCT) and compare the choroid thickness difference between first visit myopia children with those children who wear orthokeratology lens for more than 1 year.Methods This retrospective study enrolled 68 myopia children with low to moderate myopia (-1.00--6.00 D) who visited our hospital and took choroid images by OCT.The total subjects were divided into 2 groups.The subjects of 34 children in group 1 visited for myopia initially and wear spectacles.The other one group wear orthokeratology lens more than 1 year.Only the data of right eye were analyzed.Scans through the fovea at horizontal meridian were acquired with OCT.Choroid images were detected by custom software with 500 μm intervals up to 3.5 mm around fovea.Choroid thickness (CT) was calc~ated based on the average of the 7 zones.Statistical analysis was performed to evaluate choroid thickness at each zone.ANOVA was used to compare choroid thickness differences between various zones in each group.Paired t test was used to compare choroid thickness difference at the same zone between two groups.Results The mean age of OK lens group was (12.3 ± 1.8) years old,and the spectacles group was (11.8 ± 1.4) years old,there was no statistical difference.From temple to nasal choroid,the mean CT of the orthokeratology lens group were (296.7 ± 61.8) μm (T3),(290.7 ± 58.9) μm (T2),(285.7 ± 57.4) μm (T1),(278.5 ±57.7) μm (M),(262.2 ±57.9) μm (N1),(239.8 ±59.7) μm (N2),(214.7 ±59.0) μm (N3);And the mean CT of the spectacles group were (294.2 ± 45.4) μm (T3),(292.0±44.0) μm (T2),(283.6 ±45.5) μm (T1),(272.0 ±51.6) μm (M),(255.2 ± 56.3) μm (N1),(236.5 ±58.1)μm (N2),(212.8 ±57.7) μm (N3),respectively.The thicknesses were significantly thicker in temple zones than that in nasal zones in each group (all P < 0.05),but the CT was not significantly different between the two groups in each zone (all P > 0.05).Conclusion The choroid thickness has regional deference in myopia children,the thickest is in the temple and the thinnest in the nasal region.There is no significant difference between the children who initially corrected by spectacles and those who wear OK lens more than 1 year.
9.Clinical study on modified Shaoyao-Gancao Decoction and morphine sulfate sustained release tablets for the patients with cancer pain with qi and blood deficiency syndrome

Objective:To evaluate the clinical effect of modified Shaoyao-Gancao Decoction combined with oral morphine sulfate sustained release tablets in the treatment of cancer pain with qi and blood deficiency. Methods:A total of 102 cancer pain patients with qi and blood deficiency from March 2017 to May 2018 were divided into two groups according to the random number table method, 51 in each group. The control group was treated with morphine sulfate sustained release tablets on the basis of conventional treatment, and the treatment group was treated with modified Shaoyao-Gancao Decoction on the basis of the control group. Both groups were treated for 4 weeks. Numerical rating scale (NRS) was used to assess the severity of cancer pain, and Karnofsky performance status (KPS) scale was used to assess the quality of life. The level of Endothelin-1 (ET-1) was measured by radioimmunoassay, The levels of 5-hydroxytryptamine (5-HT), Prostaglandin E2 (PGE2) and substance P (SP), TNF-α, IL-1β, IL-6, IL-2 were detected by ELISA, the clinical effects were evaluated, and the adverse reactions were recorded. Results:The total effective rate was 94.1% (48/51) in the treatment group and 78.4% (40/51) in the control group, the difference between two groups was statistically significant ( χ2=5.299, P=0.021). After the treatment, the NRS score (2.84 ± 1.32 vs. 4.22 ± 1.44, t=5.045) in the treatment group was significantly lower than that of the control group ( P<0.01), and the KPS score (73.13 ± 10.19 vs. 59.93 ± 8.81, t=6.998) was significantly higher than that of the control group ( P<0.01). After the treatment, serum 5-HT (0.34 ± 0.09 pg/ml vs. 0.53 ± 0.14 pg/ml, t=8.153), PGE2 (65.03 ± 15.79 pg/ml vs. 94.85 ± 20.88 pg/ml, t=8.135), ET-1 (99.96 ± 21.57 pg/ml vs. 176.78 ± 26.01 pg/ml, t=16.236), SP (60.27 ± 12.71 pg/ml vs. 65.63 ± 12.09 pg/ml, t=2.182) in the treatment group were significantly lower than those in the control group ( P<0.01 or P<0.05). Serum TNF-α, IL-1β, IL-6 level were significantly lower than that of the control group ( t value were 35.071, 12.472, 8.122, all Ps<0.01), and IL-2 level was significantly higher than that of the control group ( t=6.579, P<0.01). During the treatment, the incidence of adverse reactions was 13.7% (7/51) in the control group and 2.0% (1/51) in the treatment group. The difference between the two groups was statistically significant ( χ2=4.630, P=0.031). Conclusions:Modified Shaoyao-Gancao Decoction combined with morphine sulfate sustained release tablets can relieve the pain of cancer pain patients with qi and blood deficiency, reduce the level of inflammatory cytokines, and improve the clinical symptoms and quality of life of patients.
10.Efficacy of HAA regimen in the treatment of 64 patients with refractory/relapsed acute myeloid leukemia.

OBJECTIVETo evaluate the efficacy and safety of the HAA regimen (homoharringtonine,cytarabine and aclarubicin)as salvage chemotherapy in the treatment of refractory/relapsed acute myeloid leukemia (AML).
METHODSWe retrospectively analyzed 64 patients with refractory/relapsed AML who received the HAA regimen as salvage chemotherapy. The complete remission (CR)rate was analyzed. Kaplan-Meier method was used to estimate overall survival (OS) and relapse free survival (RFS), and the differences were compared by Log-rank test.
RESULTSThe overall CR rate was 70.1%, and 67.1% of the patients attained CR after the first induction course. The early death rate was 0. The median follow-up time was 61 (range:6-120) months. The estimated 3-year OS rate was 46.8% and the estimated 3-year RFS rate was 42.8%. The CR rates of patients with favorable/intermediate and unfavorable cytogenetics were 76.4% and 33.3%, respectively. The 3-year OS of favorable/intermediate and unfavorable group were 53.7% and 10.0%, respectively. The median survival time of unfavorable group was only 8 months. The side effects associated with the HAA regimen were tolerable, in which the most common toxicities were myelosuppression and infection.
CONCLUSIONHAA regimen is associated with a higher rate of CR and longer-term survival and its toxicity can be tolerated. The regimen is suitable for refractory/relapsed AML patients with favorable or intermediate risk .
