6.The Change of Prostate-specific Antigen and Prostate-specific Antigen Density in Patients with Benign Prostatic Hyperplasia after Dutasteride Treatment.

PURPOSE: Dihydrotestosterone(DHT) is key to the initiation and maintenance of abnormal prostatic growth in benign prostatic hyperplasia (BPH). Five alpha-reductase inhibitor reduces prostatic growth and serum prostate-specific antigen(PSA) by blocking the conversion of testosterone to DHT. Dutasteride is a dual(type 1 and 2) 5alpha-reductase inhibitor. We evaluated the effects of dutasteride on prostate volume, PSA, and PSA density in men with BPH. MATERIALS AND METHODS: A total of 83 men with a clinical diagnosis of BPH were treated with dutasteride and an alpha-blocker. We investigated the change in prostate volume, PSA, and PSA density 6 and 12 months after initiation of dutasteride therapy. RESULTS: After 6 months of dutasteride therapy, the total prostate volume was reduced from baseline by a mean of 15.46%, the PSA was reduced by a mean of 48.24%, and the PSA density was reduced by a mean of 37.97%(p<0.001). After 12 months of dutasteride therapy, the total prostate volume was reduced from baseline by a mean of 23.3%, the PSA was reduced by a mean of 52.57%, and the PSA density was reduced by a mean of 36.2%(p<0.001). There were no differences in the regression rate of PSA and PSA density, in contrast to prostate volume, between 6 and 12 months of dutasteride therapy by repeated measures ANOVA. CONCLUSIONS: The findings in this study demonstrate that the actual PSA in men receiving dutasteride would be multiplied by 2 considering the PSA regression rate after 12 months.

7.Abnormally High Blood Ethanol Concentration of Heart Blood : A Case Report and Review of Literatures.

A 48 years old man was sent to hospital through emergency room immediately after head injuries. He was arrived at hospital being dead and autopsy was done sixty hours later after insult. Multiple skull fractures and brain parenchymal contusions, subarachnoid hemorrhage, stem hemorrhage were noted, and these injuries were considering as a cause of death. Interestingly, blood alcohol concentration (BAC) using cardiac blood was very high (0.738%) in this case. Here in, we report abnormally high BAC in heart blood which is not a cause of death and we review the general aspects about alcohol concentration interpretation.

8.A Self-inflicted Pneumatic Nail Gun Head Injury.

A nail gun is a type of a tool used to drive nails into some kinds of hard materials. In this paper, an unusual case of intracranial penetrating nail gun injury is presented. The body of a badly charred man was found following a house fire. Autopsy revealed four intracranial nails in the occipital area. Also, tears of the stomach and diaphragm were found. Carbon monoxide saturation of a blood sample was a 5.2% and a small amount of soot was present in larynx and trachea. Subsequent investigation of the circumstance and autopsy findings strongly suggest that the deceased had shot himself in the head with a pneumatic nail gun and had died by self immolation.

9.Two Consecutive Deaths from Choking Asphyxia.

Choking is a type of asphyxia caused by obstruction within the air passages. Two old men died consecutively in a rural community party while enjoying raw beef and drinks. The time interval between the two deaths was about 3 hours. A lump of raw beef obstructing the laryngeal inlet was found in both cases accompanying conjunctival petechiae, heart blood liquidity and congested internal solid organs. The first victim had moderate to severe degrees of coronary atherosclerosis and fatty liver with the blood alcohol level 0.251% and the second one showed liver cirrhosis and the same coronary lesion with the blood alcohol level 0.175%. It is interesting but uncommon to see the circumstance that accidental choking asphyxias with raw beef occur within a few hours in the same place.

10.Localized Bullous Pemphigoid Following Acupuncture.

A 60-year-old man presented with a 3-week history of pruritic bullae and erosions located within an acupuncture site on the right lower leg. Ten days prior to the development of these bullae, he had been treated with acupuncture to the right calf due to Achilles tendonitis which had developed 1 year earlier. The diagnosis of bullous pemphigoid was confirmed by histologic findings and salt-split direct immunofluorescence study performed on the perilesional skin. To our knowledge, this is the first case of localized bullous pemphigoid following acupuncture.
